A Philological Research on the Way of Medical Study of ShangHanLun

Objectives : ShangHanLun is a clinical guideline, and its core is in the part of Six-meridian Diseases. In recent philological sutdies, independent textual analysis of Shanghanlun is essential to define the scope of research. Methods : By the textual study, I defined the Shanghanlun clinic model. And I researched about medical range of Shanghanlun, and relations between Shanghanlun and JinGuiYaoLue, and between Shanghanlun and Huangdineijing. Results : In six topic sentences, the word '-之爲' means 'a process'. In provisions following topic sentences, the word '者' means 'a person'. So Shanghanlun is describing processing factors of Six-meridian Diseases and related human changes with clinical therapy. In the philological studies, meaning of '傷寒' was possibly just the 3rd provision of the greater yang disease part. Practically Shanghanlun's study range is over the concept of Cold damage today. Additionally Jinguiyaolue deals diseases, but Shanghanlun deals human. Gangpyeong-Shanghanlun's phrases can be separated into five part, and oldest part is independent of Huangdineijing. Conclusions : So we need to set a new and independent clinic model of Shanghanlun to verify it. The Shanghanlun's clinic model has a 3 steps. 1st step is to find 6 type defined symptoms, which act as processing factors of Six-meridian Diseases. 2nd step is to confirm a human changes after a Disease appeared. 3rd step is to treat a patient as a provision suggests. Philologically Shanghanlun's clinic range is not limited by the word '傷寒'. And many concepts in Shanghanlun should be independent of Huangdineijing.
Investigation of difference of Gwakhyangjeonggi-san decoctions produced by different pressure levels and various extraction times

Objectives : Gwakhyangjeonggi-san (GJS) which consists of 13 herbal medicines has been used to treat gastrointestinal disorders caused by common cold. This study was performed to compare GJS decoctions produced using different pressure levels for various extraction times. Methods : Decoctions were prepared by the pressure levels of
(pressurized) for 30-180 min. The extraction yield, total soluble solid content (TSSC), and hydrogen ion concentration (pH) were measured, and the contents of the nine marker compounds were determined using high performance liquid chromatography. Results : The higher pressure and longer extraction time significantly increased TSSC value, while decreased the pH value. However, only extraction time affected the extraction yield of pressurized decoction. Variation of the amounts of chemical compounds was shown in pressurized and non-pressurized decoctions during extraction time. The result of regression analysis showed that pressure and extraction time can influence to extraction yield, TSSC, pH, and the content of chemical compounds. Conclusions : This study suggests that the pressure and extraction time can significantly affect the extraction efficiency of components from GJS decoctions.
A review on pharmacological properties of Bangpungtongsung-san in obesity - Focused on Japanese researches -

Objectives : Excessive nutrition intake and lack of physical activity induce obesity through accumulation of fat in human body. The obesity have been categorized as a disease and known to cause several chronic diseases and cancers. In Japan, Bangpungtongsung-san has been applied as one of the therapies of the obesity and it's pharmacological uses have been studied for a long time. The purpose of this study is reviewing researches of Bangpungtongsung-san related to the obesity. Methods : By searching with the keyword of "Bangpungtongsung-san" or related words such as "Bofutsushosan" and 'TJ-62" from PubMed and off-line libraries, many reports were gathered and reviewed from the perspective of the obesity. Results and Conclusions : It has become evident by reviewing the reports that Bangpungtongsung-san is useful as a therapy of the obesity though some case reports showed side effects. Also, the mechanisms of Bangpungtongsung-san on obesity were reviewed and suggested.
Antiproliferative Effect of the Salviae miltiorrhizae Radix Extracts on the Cancer Cell Lines

Objectives : The purpose of this study was to identify antiproliferative effects of Salviae miltiorrhizae Radix(SM) extracts against cancer cell lines. Methods : We used 2 kinds of cancer cell lines such as colon cancer cells(HT-29), human oral epitheloid carcinoma cells(KB). MTT assay was performed to examine the efficacy of SM extracts on the cytostaticity of cancer cells in proportion to time and doses. Apoptosis was evaluated by DNA laddering and DAPI nuclei staining. Results : The MTT absorbances against HT-29 and KB of SM extracts were significantly decresed. DNA ladders could be identified in KB of SM extracts. The morphological change were observed and number of cells were decreased by SM extracts. Conclusions : SM extracts is considered to be effective to induce apoptosis and inhibit cancer cell proliferation.
A study on the comparative method of prescription using gunsinjwasa theory

Objectives : The main objective of this study is to classify herbal components to 4 groups which are similar to Gunsinjwasa grades by using herbal composition ratio of prescription. Another objective is to design the searching system which compares prescriptions and improves efficiency with 4 groups like Gunsinjwasa grades. Methods : This study was proceeded with Acess 2007 on Microsoft Windows 7 and we created composition ratio based on weight by using prescriptions of Donguibogam, Uihagipmun and Banghakhabpyun. We could make comparison and searching method of prescriptions. Results : We could search using composition ratio degree of herbs which composes prescription. And the similarity comparison of prescription was possible with value from 0 to 10. Conclusions : We could increase the accuracy of the searching prescriptions and comparison with putting into the information about composition degree and composition ratio of herbs which compose a prescription.
Study on a Prescriptional Discrimination and Usage of Cinnamomi Cortex

Objectives : The purpose of this study is to improve the fomula efficiency and prevent the resource abuse by clarifying the origin of kinds of Cinnamomum cassia Presl. Methods : 1. We researched "Shennongbencaojing", "Bencaogangmu", "Shanghanlun", "Jinguiyaolue", "Dongeuibogam", and other books. 2. Choosing a related words such as Cinnamomi ramulus(Gye-ji), Cassiae Cortex Interior(Gye-sim), Cinamomi Cortex(Yuk-gye). 3. Searching prescriptions: We searched prescriptions containing the Cinnamon in above books, and compared the differency among them. Conclusions : 1. Cinnamomi ramulus(Gye-ji) used for Shanghanlun contains Cinamomi Cortex(Yuk-gye), Cinnamomi ramulus(Gye-ji) and Cassiae Cortex Interior(Gye-sim) written in the Korean Pharmacopoeia. 2. Cinamomi Cortex(Yuk-gye) is the thick cortex of trunk of Cinnamomum cassia. It has functions like downwarding and warming-tonifying, and it is mainly used for Palmi-won(Bawei-yuan). 3. Cassiae Cortex Interior(Gye-sim) is about 30% of Cinamomi Cortex(Yuk-gye) middle layer, removing outer and inner cortex. Its main functions are regulating heart, coordinating the heart and kidney, and dispelling stastic blood and it is mainly used for Gyejibokryoung-hwan(Guizhifuling-wan) and Dangguisoo-san(Dangguixu-san). 4. Current Cinnamomi ramulus(Gye-ji) is a twig. thus, it means a small branch. Its main functions are upwarding, transversing, and exterior-effusing and it is mainly used for Gyeji-tang(Guizhijiagui-tang) and Gyejigagye-tang(Guizhi-tang).
Comparison of Pakistani Ephedra Herba and Chinese Ephedra Herba Containing Gangjihwan in the Improvement Effects of Weight Loss in a High Fat Diet-Fed Obese Mice

Objectives : This study investigated the improvement effects of Pakistani (DF-a) and Chinese Ephedra herba-containing Gangjihwan (DF-b) on obesity in a high fat diet-fed obese mouse model. Methods : Eight-week-old C57BL/6N mice were divided into four groups: a normal lean group given a standard diet, an obese control group given a high fat diet, and DF-a and DF-b groups given a high fat diet with DF-a (80 mg/kg), and DF-b (80 mg/kg), respectively. After 8 weeks of treatment, body weight gain, feeding efficiency ratio, blood lipid markers, fat weight and histology were examined. Results : 1. Body weight gain and fat mass were significantly decreased in DF-a and DF-b groups compared with control. The extent of decreases was eminent in DF-a group. 2. Feeding efficiency ratio and circulating leptin concentration were significantly decreased in DF-a and DF-b groups compared with control, whereas circulating adiponectin concentration was increased in DF-a and DF-b groups compared with control. 3. Consistent with their effects on body weight gain and fat mass, circulating concentrations of triglyceride, glucose and insulin were decreased in DF-a and DF-b groups compared with control. 4. The size of adipocytes were decreased by DF-a and DF-b compared with control, whereas the adipocyte number per unit area was increased by them, suggesting that DF-a and DF-b decreased the number of large adipocytes. 5. Consistent with their effects on body weight gain, liver fibrosis was reduced in DF-a and DF-b groups compared with control. Conclusions : In conclusion, these results suggest that DF-a and DF-b not only decrease feeding efficiency ratio, plasma leptin concentration, and blood anti-obesity biomarkers, but also reduce fat mass, contributing to the improvement of obesity. DF-a and DF-b also inhibit liver fibrosis. In addition, these effects were similar between Pakistani Ephedra herba and Chinese Ephedra herba-containing Gangjihwan.
Measuring a Dosage of Pills Type Medicine in Bangyakhappyun

Objectives : To determine a dose of medicine precisely and conveniently, this study was tried to specify the weight of Bangyakhappyun pills by measuring in terms of grams. Methods : 1. Selected typical prescriptions in order of size and formula written down in Bangyakhappyun. 2. Pulverized each medicine in prescription and weighed one Bangchonbi(方寸匕). 3. Weighed the 10 seeds of Firmiana simplex sized one(梧子大) which were mixed with auxiliary materials such as water, honey, starch paste, etc. 4. The dosages of other type forms were measured based on the weigh of 10 seeds of Firmiana simplex sized one(梧子大). Results : 1. A well-used size of pills is Firmiana simplex sized one(梧子大) 2. Honey and starch paste are typical auxiliary materials for formulating pills. 3. A weigh of starch paste based 10 seeds of Firmiana simplex sized one(梧子大) ranged between 1.09g and 2.55g. 4. A weigh of honey based 10 seeds of Firmiana simplex sized one(梧子大) ranged between 1.18g and 2.77g. Conclusions : A dosage of each prescription can be calculated in terms of grams. 1. In case of Firminia simplex sized one(梧子大) with paste, a daily dose is 5.28g to 17.8g. 2. In case of Firminia simplex sized one(梧子大) with honey, a daily dose is 11.8g to 17.3g. 3. The dosage of pills with toxic herb and mineral medicinal material for baby is the least amount of 0.017g. 4. The dosage of most tonifying medicinals are over 10g. Table 3. shows more details.
Anti-oxidative and Anti-inflammatory Effect of 7 Herbal Extracts and Methods of Herbal Formula Compositioning

Objectives : Herbal formulas are complicated to analyze and difficult to compose because of its mixed, complex features. So we discussed about how to compose herbal formulas effective and efficient by analyzing various effectiveness of each herbal extracts. Methods : To evaluate the effectiveness of herbal formula, anti-oxidative and anti-inflammatory effectiveness were mathematically analyzed. DPPH, superoxide anions, Nitric Oxide scavenging activity was measured to evaluate the effectiveness of 7 herbal extracts. And next, cytotoxic activity of extracts on RAW 264.7 cells were measured using MTS assay. To asses anti-inflammatory effect, nitric oxide and
production were measured. Based on these anti-oxidative and anti-inflammatory experiment result, mathematical analyzation were carried out with constOptm function, and determined efficacy-maximizing ratio. Results : This mathematical analysis based formula showed significantly outstanding effectiveness than other formulae. And estimated tendency of anti-inflammatory effectiveness was matched with real effectiveness. Conclusions : So, mathematical analysis can be available to evaluate and estimate the effectiveness of herbal formulae.

Conclusions : In conclusion, these results suggest that DF and DF+GSH groups decrease feeding efficiency ratio, plasma leptin concentration, blood anti-obesity biomarkers and fat mass, improves body weight gain contributing to the inhibition of liver fibrosis. In addition, these effects were more effective in DF+GSH combination group than in DF-only group.
A Case Study of a Serious Antibiotics-associated diarrhea Patient Treated with Modified Hyangsayukgunja-tang

Objectives : The purpose of this study is to report a therapeutic effects of a Korean herbal prescription for antibiotics-associated diarrhea. Methods : Modified Hyangsayukgunja-tang was prescribed once or twice a day to a antibiotics-associated diarrhea patient. To observe the therapeutic effects, the frequency of defecation was counted and Bristol Stool Form Index was used. Laboratory tests were conducted periodically. Results : The frequency of defecation was decreased from 9 times a day to once a day. According to Bristol Stool Form Index, stool form was improved from Type 7 to Type 5. The level of red blood cell, hemoglobin and albumin increased slightly from the day of admission to discharge. Conclusions : Hyangsayukgunja-tang can be a choosable treatment option for a serious antibiotics-associated diarrhea.
A Case Report of Tinnitus Occurred in the Sudden Sensory Neural Hearing Loss Treated with Korean Medical Treatments

Objectives : The purpose of this study is to show the clinical effect of Korean Medical Treatments for Tinnitus with Sudden Sensory Neural Hearing Loss. Methods : The patient was treated by Korean Medical Treatments for 10weeks. The effect of treatments on tinnitus was measured with Visual Analogue Scale(VAS) and Korean Tinnitus Handicap Inventory(K-THI). And the effect of treatments on sudden hearing loss was measured by VAS. Results : VAS of Tinnitus was decreased from 10 to 3 points and K-THI score was decreased from 70 to 26 points. VAS of hearing loss was decreased from 9 to 8 points. Conclusions : Korean Medical Treatments are effective on Tinnitus.
