A Study on Clinical Classification and Characteristic of Children with Recurrent Abdominal Pain

Purpose : This Study was conducted to evaluate clinical characteristic of children with recurrent abdominal pain (RAP) and to be classified by its six subtype in the Oriental Pediatric Text Book and to find out relationship of western classification. Methods : Patients who visited Dong-Eui Oriental Medical hospital from August, 2001 to October, 2002 due to RAP were included. According to questionnaire and history taking, RAP was classified by its six subtype based on Oriental medical theory. Results : 1. Patients with RAP were more internalized, have a close relation with their parents, and have strong desires of success, but social intercourse is low. 2. 76% of Patients have a less desire to eat and 67% of Patients have a diarrhea or constipation. 3. hours after eating and no characteristic most, site of AP was the umbilicus most, shape of AP was impotent pain most, cause of reduce pain was abdominal massage and defection most. 4. frequency of RAP's type, AP caused by diet(食積腹痛) is 45.5%, AP caused by cold(寒腹痛) is 29.1%, AP caused by cold in internal organs of deficiency(臟腑虛冷腹痛) is 12.7%, stagnation of qi and stasis of blood(氣滯血瘀腹痛) is 10.9%, AP caused by internal diet and external cold(內食外寒腹痛) is 1.8%. There is no AP caused by parasites(蟲腹痛). 5. During clinical classifications of RAP, cause of occurrence was most important cause of reduce pian, defection practice was helpful for diagnosis, but shape of AP, site of AP was not helpful. 6. With relationship of Oriental classification and western classification, AP caused by diet is similar to dysmotilitylike dyspepsia and irriTable bowel syndrome. AP caused by cold is similar to irriTable bowel syndrome. AP caused by cold in internal organs of deficiency is similar to unspecified dyspepsia. stagnation of qi and stasis of blood and AP caused by internal diet and external cold is not like to western classification. Conclusion : RAP in Childhood is most occurred by food and cold. there is few AP caused by stagnation of qi and stasis of blood and internal diet and external cold. So the study on subclassification and clinical Manifestations of RAP in Childhood is more performed.
Clinical study for inspection of face color and tongue state on Weak Children

We have plenty of study of adults diseases, but not much has been said about children. 79 children who had visited in the weak children of Dongeui Oriental Medical Hospital from March, 2002 to October, 2002, were the respondents of the Weak Child Questionnaire. The results obtained were as follows: 1. Their symptoms have been observed with sequences of respiratory diseases, Digestive diseases, Psycho-neurological diseases. 2. Distribution of sex and ages : male 38 cases, female 41 cases.

The texture of tongue was ended in tongue state : tongue form-not plump not haggard patten(57 cases), a pricky patten(12), a map patten(7), a fissured patten(3). tongue color- pink (57 cases), red(16), light white(6). coating color- thin white(61 cases), thin yellow(l0), lack(6), deep and white(2). tongue coating substance-thin(72 cases), moisten(5), dry(1), deep(1).

Crohn's disease is an indolent, chronic inflammatory bowel disease capable of involving the entire alimentary tract. The exact etiology and pathogenesis remain unknown despite a long and intensive research, but the finding of various abnormalities of the immune response in patients with Crohn's disease has led to the concepts that immune mechanism is involved in the pathogenesis of this disease. Recently, we have experienced a case of Crohn's disease. So the purpose of this study is to examine the efficacy of Oriental medicine for Crohn's disease.
A Clinical study on the Effects of Ryoumijihawangtang gamibang on 4 Constipation Children

Introduction : constipation is the condition of being unable to empty the bowels frequently enough and effectively. It happens many times to children. however it is usually tempoary and not a serious disease. therefore, the parents don't need to worry. Purpose : The purpose of this study is to find out weather Ryoumijihawangtang gamibang has effects on children's constipation. Methods The subjects are severe constipation children who visited Dong-sand oriental Medicine hospital from March 2002 to July 2002. The clinical period is three months and one week. Results and Conciusion : 1)After on medication, three children showed positive a result, being empty the bowels frequently and enough and effectively, but one child stated the same, the children's parents showed diverse satisfaction regardless of their children's result. 2)This study is not satisfactically correlational. however, the effect of the medicine is clinically positive to alleviate the constipation. 3)After on medication, the subject tried to alleviate constipation by diet and the change of lifestyle(eating more fiber and getting more exercise)
A Study of the Chief Complaints of Pediatric Outpatients

The Purpose of this study was to collect statistics of pediatric problems, and so to investigate the new effective oriental medicine treatments in pediatric disease and its clinical applicability. The Study was composed of 698 new patient who had been treated at the pediatric unit in th Dong Sea Oriental Medical Hospital for 1 year, from 1 September 2001 to 31 October 2002, and aged between 0 and 18 years. The Chief complaints and their care-givers. Result: 1. In the study, male children are 403 and female children are 295. So the sex ratio between male and female is 1.37 : 1. 2. In age distribution, 0-3 years : 317(45.4%), 4-6 years : 148(21.2%). 7-9 years 98(14.0%), 10-12 years: 66(9.5%), 13-15 years : 40(5.7%), 16-18 years 29(4.2%). 3. The chief complaint according to systemic division was Respiratory diseases covers 30.1%, Digestive diseases covers 20.8%, Physique diseases covers 10.6%. 4. The Respiratory diseases tended to increase in autumn and appeared mostly in the age group between 0 and 9 years. The Digestive diseases increase in summer and appeared mostly in the age group between 10 and 12 years. The physique diseases increase in winter and appeared mostly in he age group between 13 and 18 years. 5. Most of patient came to the hospital from diseases such as common cold, weakness, dyspepsia, atopic dermatitis, rhinitis, take a easy cold, short stature, sweating, Bell's palsy, abdominal pain, etc. Conclusion : 1. The chief complaint In pediatric diseases that needed an oriental medical treatment was mainly the disease that tends to take long time and the weakness, and appeared frequency in respiratory and digestive disease. 2. The oriental medical treatment was still preferred as a way to improve the weakness by patients, rather than a way to overcome their disease. in particular, the study shows that the oriental medical treatment should be emphasized in terms of preventing the disease. 3. The new disease, which were developed with change of human life and environment, should be investigated as a new fie of oriental medical treatment.
A Study on Chronic or Recurrent Respiratory Symptoms

Respiratory tract symptoms such as cough, wheeze, and strider may occur frequently or persist for long periods in a substantial number of children, others may have persistent or recurring lung infiltrates with or without symptoms. It is important to study on clinical significance of this symptoms for avoiding unnecessary test or treatment. Chronic or Recurrent Respiratory Symptoms are similar to cold(感冒) and cough(咳嗽) in Oriental Medicine. Diagnostic criteria is followed : in case of children less than three years old, upper respiratory infections are more than seven per year or lower respiratory infections are more than three ; in case of children three to five years old, upper respiratory infections are more than six per year or lower respiratory infections are more than two ; in case of children six to twelve years old, upper respiratory infections are more than five per year or lower respiratory infections are more than two. In oriental medical treatment is classified into the treatment of internal use, external use, internal and external use, acupuncture, cupping therapy, and combination treatment of chinese and western medicine.
The Clinical Study in Children with Common Cold (in summer & winter)

The common cold is the most common pediatric infectious disease and occupies a great number of outpatients in oriental pediatrics. Because children's common cold has characteristic symptoms, we studied 276 children suffering from common cold Oriental Medicine Hospital in Kyungsan University and analyzed sex, age, chief complaints, characteristic symptoms.(period : summer 2001. 6.1.-2001.8.31, winter 2001.12.1-2002. 2. 28) The results were as follows : 1. Male to female ratios were 17: 1 in summer, 1.2: 1 in winter, 1.4: 1 in total 2. Age distribution of children from 0 to 1 was 20.29%, from 1 to 3 was 39.49%, from 3 to 6 was 32.97%, from 6 to 10 was 7.27%, over 10 was 0%, from 0 to 6 was 92.75%. 3. Frequency of chief complaints In summer : cough was 22.28%, nasal discharge was 18.39%, fever was 16.58%, secretion and sputum were 14.50%, sweating was 6.99%, nausea or vomitting were 4.92% etc. In winter : cough was 24.20%, nasal discharge was 18.03%, secretion and sputum were 16.43%, occlusion of nares was 11.64%, fever was 9.58%, nausea or vomitting were 3.88% etc. Frequency of chief complaints has seasonal difference. 4. Children's common cold has characteristic symptoms and seasonal difference. 5. Children's common cold has characteristic clinical subtype.
A Literature Study of Allergic Rhinitis for Children

Objectives : The aim of this study was to investigate the classification methods of the cause of Allergic Rhinitis for Children. Methods : We surveyed the oriental & western medical book concerning the Allergic Rhinitis for Children. Results : 1. The Oriental medicine, Allergic Rhinitis is belong to the BiGu, BunChe and the symptoms are watery rhinorrhea, sneezing and nasal obstruction. 2. The cause of disease is the weak of lung, spleen and kidney, and invasion in to nasal cavity of Poong Han etc a wrong air. 3. In children, the cause of disease is the weak of lung and spleen. and the aim of the treatment is helping the vital energy and expelling the vice.
A clinical study on infancy patients who had visited Oriental Medicine Hospital

The Purpose of this study was to investigate infancy patients who had visited Oriental Medicine Hospital, and so to consider a counterplan by oriental medicine. The study was composed of 101 new infancy patients who had visited Dongguk Kyeongju Oriental Medicine Hospital during 1 year from January 2001 to December 2001. The results were as follows : 1. Male children are 65(64.3%), female children are 36(35.6%), male to female ratio is 1.8: 1. 2. In age distribution, 1 month 5.9% ; 2 month 10.9%, 3 month 4.0%, 4 month 11.9%, 5 month 5.9%, 6 month 9.9%, 7 month 10.9%, 8 month 10.9%, 9 month 10.9%, 10 month 14.8%, 11 month 4.0%. 3. According to systematic division of the chief complaint, respiratory diseases are 37.6%, digestive diseases are 25.7%, nervous diseases are 21.8%, urogenital diseases are 1.0%, musculoskeletal diseases are 1.0%, dermatologic diseases are 7.9%, infirmity diseases are 3.0%. 4. In treatment, herb-medication is 86.1%, consultation is 7.9%, acupuncture is 17.8%, moxibution is 2.0%, venesection is 14.8%, aromatherapy is 4.9%, chimsband is 16.8%.
The type of disease and symptom according to Sasang constitution of children

We proceeded the judgement of Sasang constitution of 79 children(the age 101 between 15) who have visited Dongguk Bundang Oriental Medical Hospital during 3 months from August 2002 to October 2002 by Questionnaire of Sasang Constitution Classification and conducted questionnaire survey about the type of disease and symptom occurred frequently. The conclusion is following. 1. The distribution of Sasang constitution: Soeum group(少陰人) 32, Taeeuml group(太陰人) 29, Soyang group(少陽人) 18. 2. The distribution of systemic disease occurred frequently : in digestive system, Soeum (46.9%) & Soyang(44.4%) group more than Taeeum group(34.5%) and in genitourinary system and others, Soeum(28.1%) more than Taeeum group(17.2%). 3. The distribution of symptom occurred frequently : in rhinitis and sinusitis, Taeeum(60.0%) & Soeum(60.0%) more than Soyang(45.5%), in abdominal pain, Soeum(58.8%) & Soyang(62.5%) more than Taeeum(36.4%), and in diarrhea, Taeeum(45.5%) more than Soeum(123.5%) & Soyang group(25.0%). Headache & vexation(心煩) are almost found in Soeum group with abdominal pain and excessive sweating is only found in Taeeum group. 4. The efficacy of herbal medication combined with Cervix Cornu Pranum (鹿茸加味劑) : after the medication, change of condition have showed in Taeeum group at the most high rate(31.0%) and showed in Soeum(15.6%) and Soyang(11.1%) group. 5. The distribution of allergic disease: allergic rhinitis and atopic dermatitis took up the majority of all allergic diseases. In morbidity of allergic rhinitis, Taeeum(62.1%) & Soyang(55.6%) more than Soeum(37.5%). In morbidity of other allergic diseases and degree of severity, it has no difference among three Sasang constitution groups.
A Study on Growth of Children with Atopic Dermatitis

Background: Owing to stress of external appearance and bad sleeping by itching, children with atopic dermatitis grow slowly. Object: The purpose of this study is estimation of growth degree on children with atopic dermatitis Method: During I year from 2001 till 2002, it became the object of studing that 45 children with atopic dermatitis and random sample 45 children without atopic dermatitis, without another disease related to growth from 2 years to 10 years in Dep. of Pediatrics, Dongguk university Bundang Oriental Hospital. Result: Height percent of children with atopic dermatitis is more lower than Height percent of children without atopic dermatitis out of considering for the distinction of sex and age. The more atopic dermatitis is severe, the more height percent of children with atopic dermatitis is lower Conclusion: Atopic dermatitis is related to the growth on children
A Clinical Study of Spine Distortion and Self-conscious Pain in High School Students

Objectives : The purpose of this study was to investigate the spine distortion and self-conscious pain in high school students. Method : Using the phase-shifting moire developed by KAIST(Korea Advanced Institute of Science and Technology) we measured the back surface distortion, and using questionnaire investigation, we investigated the self-conscious pain in 30 high school students(17 boy students and 13 girl students). Results : This study show that low back pain was most cases, and next headache, upper back pain, abdominal pain, neck pain, shoulder pain, leg pain, chest pain and gluteal pain. The ratio of each self-conscious pain was 90.0%, 86.7%, 83.3%, 83.3%, 80.0%, 76.7%, 66.7%, 33.3%, 33.3%, 30.0%. In remainder value of left and right shoulder, above of 3.0 were three students., above 2.0 to bellow 3.0 were 10 students, bellow 2.0 were 17 students. In remainder value of left and right gluteal, above of 2.0 was one students, above 1.0 to bellow 2.0 were 18 students, bellow 1.0 were 10 students. In remainder value of cervical vertical line to left and right axillae, above 4.0 was 1 student, above 3.0 to bellow 2.0 were 2 students, above 2.0 to bellow 3.0 were 3 students, above 1.0 to bellow 2.0 were 16 students, bellow 1.0 were 7 students. In remainder value of gluteal vertical line to left and right axillae, above 6.0 was 1 student, above 5.0 to bellow 6.0 were 7 students, above 4.0 to bellow 5.0 were 3 students, above 3.0 to bellow 4.0 were 2 students, above 2.0 to bellow 3.0 were 5 students, above 1.0 to bellow 2.0 were 7 students, bellow 1.0 were 5 students. In remainder value of cervical vertical line to gluteal vertical line, above 40 was 1 student, above 3.0 to bellow 4.0 were 4 students, above 3.0 to bellow 3.0 were 6 students, above 1.0 to bellow 2.0 were 8 students, bellow 1.0 were 11 students. Conclusion : It is suggested that the significant correlation exist between the spinal distortion and self-conscious pain. And we can find many cases as spinal distortion in high school students also. We have to study further more about juvenile postural distortion and it's prevention.
Comparision of Decoction and Distillation drug prescribed the Apricot Seed and Other Herbs on the Proliferation and Differentiation of Preadipocyte from the C57BL/6 Mouse

Objective: This study was proceeded to determine to what extent the decoction recipes and distillation recipes extracted from the infusion of the apricot seed and other herbs respectively would produce an effect on the proliferation and the differentiation of Preadipocyte 3T3-L1 Model separated from C57BL/6 mouse. Method: Preadipocyte 3T3-L1 Model was formed from the fat tissue of the epididymis organ, which was removed from 4-week-old C57BL/6 mouse and then was treated with collagenase. Results: 1. Concerning the restraint effect of proliferation on Preadipocyte 3T3-L1 Model, the decoction recipes, in the state of low consistency, were proved to be more effective than the distillation recipes. 2. Concerning the restraint effect of differentiation on Preadipocyte 3T3-L1 Model, the decoction recipes were shown to be more effective.
Effect of Naesowhajung-tang on Electrogastrography in children with functional dyspepsia

We have surveyed the epidemiology of functional dyspepsia in 7 cases of Children and investigated the therapeutic effect on Functional Dyspepsia following 4 weeks medication of Naesowhajung-tang using Electrogastrogram. . 2. Most of subjects have a normal growth state. But one case have severe growth disturbance. Familial tendency for this disease is noted in 85.8%. 3. Abdominal pain was seen in 100% of subjects. The indicated symptom was lowed in order of abdominal pain(100%), nausea vomiting(71.43%), general fatigue(65.34%), vertigo headache(57.14%), belching(55.38%), etc. 4. The EGG finding after treatment was improved in 53%, aggrevated in 43. In cases, postprandial improvement was more remarkable than fasting improvement. Common cold and antibiotics seems to have functioned as an aggrevating factor on gastric motility. From the above results, Naesowhajung-tang seems to have been used in the treatment of functional dyspepsia for children.
