A Comparison of Safety Management System between Accident-occurred Companies and Accident-free Companies in which Press Machine are Used

Notorious press machine bothers employers by causing so many industrial accidents which are mostly of caught-in or between type. In 2008, press machines caused 1665 cases of such type of accidents, which reached about 11% of all those. Even though various approaches has been adopted to prevent those press machine accidents, up to now, no one was found to be satisfactory. In this study, two kinds of companies, accident-occurred and accident-free by press machine, were surveyed to find out positive and negative factors for accidents prevention. 400 companies which experienced accidents in 2008 and 200 companies which did not experience accident at all during the same period were selected for this survey. Most of these companies are small companies that employ less than 50 employees. Even though small companies are exempted from legal obligations to have a safety management system for accident prevention, we surveyed about safety management systems of those companies because of its crucial importance for accident prevention including whether they appointed safety managers and various supervisors. Awareness and efforts for safety of employers and employees were also measured. Results were analysed by the method of cross tabulation analysis. Main findings of this study are as follows. Most accident-occurred companies did not appoint any safety manager or supervisor. While it is recommended that the workplace of high accident rate have a safety manager. In addition, as less employers' effort for safety was made in accident-occurred companies in order to prevent accidents during press processing, the author proposes an amendment to the relevant law. Finally, the most referred reason for the negligence of hazard is the cumbersomeness in installing safeguards.
A Study of Numerical Reproducibility for the Backdraft Phenomena in a Compartment using the FDS

A numerical reproducibility of the backdraft phenomena in a compartment was investigated. The prediction performance of two combustion models, the mixture fraction and finite chemistry models, were tested for the backdraft phenomena using the FDS code developed by the NIST. The mixture fraction model could not predict the flame propagation in a fuel-air mixture as well as the backdraft phenomena. However, the finite chemistry model predicted the flame propagation in the mixture inside a tube reasonably. In addition, the finite chemistry model predicted well the backdraft phenomena in a compartment qualitatively. The flame propagation inside the compartment, fuel and oxygen distribution and explosive fire ball behavior were well simulated with the finite chemistry model. It showed that the FDS adopted with the finite chemistry model can be an effective simulation tool for the investigation of backdraft in a compartment.
A Study on the Reliability Analysis for Smoke Detector using Dust

This paper presents a study on the analyzing reliability of smoke fire detector using accelerated life test. In general, the smoke fire detector is broken by dust which flow in smoke detection chamber. In order to conduct accelerated life test of smoke fire detector dust is set accelerated factor in this paper. The dust is fly-ash which is test particle 5th regulated by KS A 0090. The dust accelerated level is 60 g, 180 g and 360 g and failure time is measured by smoke sensitivity testing. It is considered to failure of detector if detector don't operate within 30 secconds when subjected to an air stream having a velocity of 20 cm/s~40 cm/s containing smoke with a concentration of 15% of rate of light-response of 1 m. The goodness of fit test and mean life prediction conduct using the failure time. The result show that life distribution fits the weibull distribution for failure time data and the mean lifes calculate 22.5 year in domestic product and 14.7 years in overseas product applied dust stress only.
A Study on the Fire Safety of Polypropylene Powder Coatings with Flame Retardant

The production of flame-retardant polypropylene powder coating with the addition of flame retardant (PP) controlled rheology (CR) and polypropylene, were discussed comparing the mechanical properties, such as safety and topical coating properties. Mechanical properties measurements showed almost the same as AA-1 where the flame retardant is not added. These results appear to, and because it is excellent in compatibility with polypropylene and flame retardant. Physical properties of the coating film, a test piece flame retardant organic is added, although it shows the physical properties of the coating film was stable, did not show the physical properties satisfactory is the test piece flame retardant of weapons has been added. In the safety of the topic, AA-4, AA-5 the results of thermal analysis but it is excellent, the LOI is excellent in 27.8 vol% 27.0 vol% in AA-4 and AA-2, AA-3, and 27.4 vol%, did not show many obstacles flame AA-5 result UL94.
Comparison of Fire Extinguishing Effects for Water Mist Additives

In order to improve extinguishing performance of water mist, many studies of additives have been conducted. In this study, viscosity agent which has the ability to improve extinguishing performance by adhering to the surface on fire was used and fluorine-free surfactant was also added to water to enhance water's wetting ability. This study aimed to verify optimal concentration of extinguishing of additives according to fire source and extinguishing performance by comparison with pure water. In case of wood crib fire, the results show that flame suppression and extinguishing time of sodium alginate 0.4 wt.% are 3.4 times and 2.2 times shorter than those of pure water in 0.2 MPa respectively. It seems that large amount of water adhere to surface on fire, thus cooling effect on surface was enhanced. Also water consumption of sodium alginate 0.4wt.% is up to 65% lower than that of pure water. In case of heptane fire, extinguishing time of cocobetaine 0.1 wt.% is 9.7 times shorter than that of pure water in 0.2 MPa. It is thought that because cocobetaine can block oxygen and suppress oil mist by making emulsion film on fire surface due to a low surface tension. On the other hand, water consumption of cocobetaine 0.1 wt.% is 92% lower than that of pure water.
An Experimental Study on Steel Bar Corrosion of Reinforced Concrete Structure

The purpose of this study is to investigate the steel bar corrosion and degree of reinforced concrete bridge, and analyze the cause of corrosion occurrence. Therefore they could ensure the durability and stability as to suggest the corrosion prevention of reinforced concrete structure. To study the corrosion state reinforced concrete structure, We investigate the cover of concrete, the compressive strength by schmidt hammer, the neutralization test of site, the compressive strength of core and the measurement of neutralized depth. As the results of test, the corrosion-grade of reinforced concrete structure which the degree of corrosion is 3, 4 degree get to 18% in the used time of 40 years and the time elapsed of 25 years. Therefore the corrosion of steel bar give rise to public discussion. The degree of corrosion is serious, and the neutralization come to the cover of concrete.
Visualization of Delamination Region in Concrete Structures using Mode Shapes of Delaminated Concrete Section (II) : Impact-Echo Test

Previous study showed that delamination region in concrete structures can be successfully visualized using mode shapes of delaminated concrete section. However, modal tests for this purpose to obtain mode shapes of the delaminated concrete section may not be applicable in practice since, to correctly obtain the mode shapes of the section, the location and the shape of the delamination region in a structure should be known in advance. Unfortunately those are normally unknown in a real structure. Therefore, a moving forward test method may be useful to obtain the mode shapes of the concrete section when the location and the shape of the delamination region are not known. In this study, impact-echo testing based mode shape estimation technique is proposed and experimentally validated for visualization of delamination region.
Forensic Engineering Study on Structural Integrity Evaluation of Scaffolding System Tower using ANSYS

Forensic engineering is the application of engineering principles covering the investigation of constructed facilities and systems that fail to perform as intended, causing personal injury or damage to property, environmental, economy etc. In the year 2012, two collapsed accidents of the large scaffolding system in national thermal power station occurred one after another, causing many casualties. In this study, we had performed to investigate the collapsed accident of scaffolding system occurred in the a thermal power station of two accidents. First, the investigation about the collapsed accidents site had performed to understand collapsed state and structures of the scaffolding system. Second, reviewing the materials concerning about the applied weight on the scaffolding system had performed. The applied weight is sum of the weights of the 15 workers, additional materials for coating work and dispersed and loaded shot ball on the foothold etc. the applied weight that calculated exceed more three times than the safe working load. Third, we had confirmed the install state of the materials of the scaffolding system by reviewing the quantity of the materials on the manual and the real system. Last, structural analysis had performed to evaluate structural integrity of the scaffolding system using Ansys. Through a series of this processes, the definite accidents causes of the collapsed scaffolding system revealed. Through these studies, the collapse accident that may occur in the scaffolding system in thermal power station can be minimized by performing specialized and systematic investigation on the accidents in terms of Forensic engineering.
Behavioral Characteristics Investigation of Rack Structure Depending on Forklift Impact Scenarios and Storage Distributions

The statistics of recent accidents in warehouses show that a heavy toll of lives were produced by various accidents, e.g. collision, overturn, fall, slip, exposure to harmful substances or environments, etc. Of significant concern amongst them is the collision, especially the collision between forklift and storage rack structure. Accordingly, this study focuses on behavioral characteristics of rack structure subjected to dynamic impact loading of a forklift. For this purpose, time-domain response analysis has been performed on a standard 2-bay six-story rack structure consisting of columns, beams and bracing members with perforated open section. In order to investigate the most critical scenario, the impact loads are applied in both down-aisle and cross-aisle directions, and the impact locations are also varied along the shelves of the palettes. In order to deal with storage distributions, three types of rack structures are further taken into account: original empty rack structure with no storage, half-loaded rack structure and fully-loaded rack structure. The numerical simulation results demonstrate that the dynamic characteristics of the rack structure are significantly dependent on the distribution of the storage goods and its natural period varies from 0.24sec to 1.06sec, approximately 4.4 times. Further, the parametric studies show that the forklift impact is most critical to the safety of the rack structure when it collides either at the base or at the top of the rack structure.
An analysis method Flexural Crack Propagation Behavior of Concrete with Aggregate Distribution of Section

This paper discusses 2D models of beams for simulating the fracture of brittle materials. A simulation of an experiment on a concrete beam subjected to bending, in which two overlapping cracks occur, is used to study the effect of individual beam characteristics and different arrangements of the beams in the overall network mesh. It was found that any regular orientation of the beams influences the resulting crack patterns. Methods to implement a wide range of poisson's ratios are also developed, the use of the mesh to study arbitrary micro-structures is outlined. The crack pattern that are obtained with mesh are in good agreement with the experimental results. Also, numerical simulations of the tests were performed by means of a model, and non-integer dimensions were measured on the predicted mesh damage patterns.
Risk Assessment for Reducing Safety Accidents caused by Construction Machinery

Construction machinery has been utilized to carry out construction works effectively. Using construction machinery enables a builder to reduce the time and the cost needed for the construction, but the fatal accident caused by it has been increased. This study is intended to identify risk magnitude of accident by kind of construction machinery through interviews with experts. Construction machineries surveyed in this research are excavator, tower crane, lift, mobile crane and forklift, those are the machinery which accident occurs more often than the other machinery. To evaluate the risk of the accidents identified, risk category was determined according to the US DOD system safety program (MIL-STD-882B) considering the risk degree and intensity. As a result, accident occurred in the process of material transport & handling was found to be the most dangerous. On the other hand, the accidents caused by the defective machinery and the poor safety gear were considered to be less dangerous relatively. The risk category by type of construction machinery suggested in this study is expected to provide the basic data in developing the safety guidance for construction project.
A Study on the Improvement of Validation and Application for Slipmeters using Reference Surfaces

The purpose of this study was to evaluate three kinds of slipmeters (BOT, BPT, English XL) used on-site floor with ASTM F2508 which is comprised of four different standard surfaces(polished granite, glazed porcelain, vinyl composite tile ;VCT, and ceramic tile). ASTM F2508 has two criteria that decide which slipmeter is appropriate or not. The evaluated slipmeters were dreg sled, articulated sturt, and pendulum strike type. The test results revealed that two kinds of slipmeters(BOT, BPT) successfully ranked all four standard surfaces and differentiated among standard surfaces with varying degrees of slipperiness. Nevertheless, the measured value with BOT on the VCT, which was reported as slippery floor in previous study, was higher than its threshold(0.6). Although some slipmeter satisfy two criteria of ASTM F2508, they can underestimate the slip potential. So, another criteria is needed so as to reduce this problem. English XL couldn't properly measure slipperiness under the two kind of floors(glazed porcelain, VCT). So the slider of English XL was modified in order to meet two criteria of ASTM F2508.
A Study on the Safety Consciousness between Permanent and Temporary Workers and the Improvement of Safety Management System about Railroad Facilities Maintenance's Outsourcing Operation

The advent of private railroad corporations presented many problems on administration's effectiveness. These brought an effect on safety management too. Most public railroad corporations maintain their rolling stocks and facilities by their own workers. On the other hand, private railroad corporations depend on outsourcing to cut down the cost of maintenance. But they have similar safety management systems. Therefore we investigated the gap between the two types of corporations, focusing on workers' safety attitude and proposed solutions to improve safety management systems in private railroad corporations.
Analysis on Factors Affecting Traffic Accident Severity - Case Study : Arterial Included Curve Section -

The main causes of traffic accidents can be classified by 3 factors - human error, vehicle deficiency and road environmental problem and most accidents occurs not only 1 factor but combination of 2 or 3-factors. Among these factors, road environmental factor is the most important factor due to influence the behavior of cars and road users and road environmental factor affects 30% of total accidents approximately. The 5 years traffic accidents data analyzed to verify the accidents severity on Korea National Highways. In order to analyze the severity, Ordered Probit Model was used. As a independent variables of this model the number of lane, neighbor road environments, sight distance, vertical grade, lane width, shoulder width and traffic volume were used and as a dependent variables the minor injuries, serious injuries and fatalities were used. Research results shows that sight distance and lane width are identified as significant factors for the traffic accident severity and lesser sight distance and lane width shows greater traffic accident severity.
A Study on the Review of Risk Concepts

Since the last 50 years, the terminology 'risk' has been widely used in general and industrial applications, with different meanings. The different definitions of risk can lead to the different risk assessments and risk management process elements, and the misunderstandings. For the reasons, the ISO(International Standard Organization) has recently defined a new risk concept through the publication of IS0 31000:2009. Although the new concept of risk was globally accepted, it seem to be argued in its definition debate. Thus, this paper reviews the definition and meaning of the concept of risk in terms of risk origin, general and industrial applications, related domestic laws and risk management standards published to improve an understanding of risk concept.
