Improvement of Reliability in Cause Analysis of Industrial Accidents

Safety certification and inspection of dangerous machines and equipments used in industries are to save lives of workers and properties involved. Cause analysis of industrial accidents is essential to prove the effectiveness of such certification and inspection. This study focuses on suggesting systematic method for cause analysis of industrial accidents associated with dangerous machines and devices. Incorporating transition from the current user-oriented indirect regulations to more manufacturer and user balanced direct regulations, suggested method coupled with safety certification, safety inspection, safety management and safety education will guarantee more effective prevention of industrial accidents.
The Effect of the CFRP/GFRP Composite Thickness on AE Characteristics and Mixed Mode Crack Behavior

Recently many efforts and researches have been done to cope with industrial facilities that require a low energy machines due to the gradual depletion of the natural resources. The fiber-reinforced composite materials in general have good properties and have the proper mechanical properties according to the change of the ply sequences and fiber distribution types. However, in the fiber-reinforced composite material, there are several problems, including fiber breaking, peeling, layer lamination, fiber cracking that can not be seen from the metallic material. Particularly, the fracture and delamination are likely to be affected by the thickness of the stacking laminates when the bi-material laminated structure is subjected to a load of the mixed mode. In this study, we investigated the effect of the thickness ratio of the difference in the CFRP/GFRP bi-material laminate composites by measuring the cracking behavior and the AE characteristics in a mixed mode loading, which may be generated in the actual structure. The results show that the thickness of the CFRP becomes more thick, the mode I energy release rate becomes a larger, and also the influence of mode I is greater than that of mode II. In addition, AE amplitude which shows the level of the damage in the structure was obtained the more damage in the CFRP with the thin thickness.
The Lightweight and the Self-escape Function Development of the SRL

Many kinds of falling prevention systems with a safety block have been supplied in order to prevent falling accidents and acquire the long life and cost down for the maintenance. However, there are not the reliable and domestic the falling prevention system until now. Almost systems were imported from U.S.A, Japan, U.K and Germany. The structural safety of the imported safety block is satisfied sufficiently, but it has heavy weight due to the cover with the aluminum and thickness. Especially, the falling prevention system as the safety block is very expensive. It brings about flow the enormous money out of country. Furthermore it has a heavy weight when workers climbed the ladder with a falling prevention system and moved, many workers are not feeling themselves. Thus, the aim of this work is to develop a commercial self-escape SRL(Self Retracting Lifeline) with the safety block function that has a light weight and an advanced strength. The cost efficiency and convenience of the system and safety for workers also will be improved remarkably even though this system has a light weight. The results show that the maximum stress is obtained in each part by the lower more than yield strength and has sufficient safety in the developed new safety block.
A Study on the Safety and Improvement of Small Theatres in Korea

The market of performance arts industry such as concert, musical and opera etc is now expanding in Korea. However, the inadequate safety management in theatres often causes some accidents. The guarantee of safety in theatres is very important since the accidents in theaters can lead to many casualties and serious property damages. In particular, the small theatres which have no obligation of safety inspection by law are very vulnerable to safety mattes. This study has done the research into the improvement of safety in small theaters through the inspection and analysis of their safety status. For the purpose, this study has inspected and analyzed the safety status of small theaters over 120 in the field of theater management and operation, fire protection, electrical facilities, ceiling structure and etc. Moreover, this study had investigated the law and technical standards related to theatre safety. This study shows that the essentials to make sure of the safety in small theaters are (1) the education to improve the sense of security of people who work in theaters (2) the training to strengthen the operating skills of the facilities in theaters (3) the cooperation and support of the related organization such as technical research center, central and local government.
Fatigue Characteristics on Welded Joint of Gear Box-Shank in Vibro Ripper for Rock Crash

Vibro ripper worked by high frequency vibration is developed to do rock fragmentation and work of ripper is the different concept with other existing breakers. The gear box-shank welded joint of vibro ripper is very important part to deliver vibromotive force to tooth, so this part should endure high frequency vibration environments. The purposes of this study are to choose the optimal welding conditions for fatigue strength. The conditions were made using three kind of shank materials and two kind of filler metals. Shank materials are Hadox-hituf, Posten80 and AR400, and filler metals are CSF-71T and CSF-81T. The fatigue test was conducted each condition. Fracture surface was observed to estimate fracture characteristics of welded joint using SEM.
Analysis of Resistive and Capacitive Leakage Current according to Wiring Type and Length at Cattle Barn

This paper is aimed to prevent danger of electrical fire at cattle barn to detect resistive and capacitive leakage current component for wiring type and length. In order to analysis for electric leakage component for cattle barn sizes and normal buildings, this paper was studied field state investigation which are at cattle barn companies(10 companies) in Cheong-won location and normal buildings at Nam-bu market in Jeon-ju location. Market to deduce the problems of electric leakage component is analyzed. The resistive and capacitive leakage current component for wiring type and length is analyzed at Beon-young cattle barn. Results show that electric leakage component suggested in this paper are valuable and usable to electrical fire in leakage current based on environment factor, which will prevent severe damage to human beings and properties and reduce the electrical fires in cattle barn. It is acceptable for electrical equipment use in an cattle barn.
A Study on Improvement of the Performance of Pulsed AC Ion Bar (2)

In display such as LCD, LED, and AMOLED or semiconductor related industries are required to have static ionizer in order to produce reliable goods. The most general type of ionization is called, "corona discharge" that has a slight chances to generate unequal and unstable amount of each +/- ion to the target object. Development and Its Characterization of Earth Plate for Early Streamer Discharge

This paper compared and demonstrated the performance of the earth plate for conventional type and early streamer discharging type developed a new plate in order to improve the performance of the ground plate for use in grounding. The results obtained are shown below; early streamer discharge earth plate was high 5 kV on DC flashover voltage and corona discharge current was higher than about 2 times under the same test conditions compared to the conventional earth plate. From the experiential result, streamer discharge earth plate had a function of rapid inflow/out flow than the general ground plane because of by corona discharge current to be generated in the sharp serrated in formed the ground plane when strike current occurred. In the future, the newly developed early streamer discharge ground plate would be beneficial for grounding work to be widely used to promote the convenience and economy of construction.
A Study on the Development of an Electrostatic Eliminator and Evaluating Method of Explosion-Protection Construction

Electrostatic eliminators are essential in various areas of manufacturing industries to protect electrostatic hazards and to reduce inferior products. For ion sources used in the charge neutralizers, there are corona discharge, soft X-ray, and ultraviolet and glow discharge. Among them, corona discharge is generally used, because the corona discharge can easily and economically produce positive and negative ions including electrons in air at atmospheric pressure. But it is necessary to equip explosion-protection electrostatic eliminators wherever hazardous atmosphere. The electrostatic eliminators and their testing method of explosion-protection type have been developed in this research. The contents and scope of the research as follows; developing the type 'Ex s IIB T4' electrostatic eliminator of explosion-protection; developing the type 'Ex s d IIB T4' electrostatic eliminator of explosion-protection; developing the explosion-protection performance testing method of electrostatic eliminator for using AC power source.
Measurement and Prediction of Autoignition Temperature of n-Propanol+n-Decane Mixture

The autoignition temperature (AIT) of a material is the lowest temperature at which the substance will spontaneously ignite in the absence of an external ignition source such as a spark or flame. The AIT may be used as combustion property to specify operating, storage, and materials handling procedures for processs safety. This study measured the AITs of n-Propanol+n-Decane system from ignition delay time(time lag) by using ASTM E659 apparatus. The AITs of n-Propanol and n-Decane which constituted binary system were
, respectively. The experimental AITs of n-Propanol+n-Decane system were a good agreement with the calculated AITs by the proposed equations with a few A.A.D(average absolute deviation).
Consideration on Flash Fire of Fuel Tank by Plate and Projectile Impacts

This study was performed to analyze the vulnerability of the situation in which combat system is shot by external projectile impacts. In developing combat system, it is vital to consider the survivability as well as its mission capability because it is directly connected with loss of lives. Especially, when the parts which are susceptible to fire are shot under battle situation, the system is exposed to the dangerousness and the situation when the parts such as fuel tanks are impacted by external projectile impacts can lead to flash fire as a result of the leakage of fuel. Therefore, in this study the possibility of flash fire was calculated by analyzing a variety of variables supposing that fuel tank in the combat system is shot. The aim of this study is to suggest effective methods in the basic steps when combat system is designed.
Validity Review on Classification of Explosion Hazardous Area using Hypothetic Volume

It is very important to classify explosion hazardous area (EHA) suitably and to use proper explosion-proof electric installations for facilities using flammable gases and liquids. In the past, various examples in the Notification of Ministry of Employment and Labor were referred to in classifying EHA. But, at present, many companies use the hypothetical volume in Korean Standards (KS). This study reviews the validity of EHA classification based on the hypothetical volume by comparing the calculated radii of EHA with those obtained by a consequence analysis program called PHAST and a mathematical approach in British Standards (BS). The radii of EHA by the hypothetical volume were found to be slightly larger than those by the other two methods. This was attributed to rather conservative uses of a safety factor(k) and a correction factor(f) for availability of ventilation in calculating the hypothetical volume. Since the differences are not so conspicuous, however, it is concluded that the hypothetical volume in KS is a valid means for the classification of EHA. This study also presents a table of the radii of EHA for easy reference by small-scale companies using city gas, C3-LPG and flammable liquid(toluene), respectively. The table consists of 25 leakage scenarios corresponding to combinations of 5 pipe(nozzle) sizes and 5 operating conditions for each flammable gas and liquid.
Measurement of Flash Points for n-hexanol+n-butyric acid and n-butanol+propionic acid by Seta-flash Closed Cup Method

The flash point is one of the most important physical properties used to determine the fire hazard of flammable liquid mixture and defined as the lowest temperature at which a liquid produces sufficient vapor to form a combustible mixture with air. The main purpose of this paper is to measure and predict the flash point of binary flammable miscible mixtures. The flash points for n-hexanol+n-butyric acid and n-butanol+propionic acid, were measured by using Seta-flash closed cup method. The experimentally derived data were correlated with the binary interaction parameters of the van Laar and NRTL equations through the optimization method. The flash points estimated by these correlations were compared with those calculated by the method based on Raoult's law. The optimization method were found to be better than the method based on the Raoult's law.
Fatigue Analysis for Fiber Right Angle Direction of FRP Deck

Composite materials have high specific stiffness, specific strength than existing concrete or steel materials. It has superior dynamic properties when utilizing advantages of material such as Non-corrosive, light weight, non-conducting and it has superior mold ability which can make variable shapes. Thus, in the construction, for using composite materials as construction materials, the study carried out static strength of fiber right angle direction and fatigue performance of FRP deck member. The study is going to deduct S-N curve by analyzing the results comparatively and estimate long-term durability. From now on, the study is going to provide interpretation of FRP member and basic data of design basis, furthermore providing foundation technique of composite materials' application of structural frame is the goal of this study.
A Case Study on Chloride Corrosion for the End Zone of Concrete Deck Subjected to De-icing Salts Added Calcium Chloride

In this study, the reinforced concrete rahmen bridge damaged by the chloride attack was investigated. According to the investigation, the degraded concretes on cantilever kerb and end part were intensively observed. Thus, the chloride content test and half-cell method were performed to evaluate the degraded parts. As a result, the contents of chloride on degraded parts were C and D grade. On the other hand, the half-cell potential values of rebar in degraded concrete were measured with the minor corrosion. This rebar corrosion is expected to progressing. Chloride content D grade is due to expansion pressure by corrosion of rebar and freeze-thaw by permeate water, could see progresses rapidly degradation. In order to prevent chloride attack to concrete deck caused by deicing salts, corresponding to the chloride critical concentration must maintain grade b or at least grade c. Chloride condition evaluation standard apply to evaluation of marine structure chloride attack with chloride attack by deicing salts.
A Study on the Correlation between Optimal Safety of Structures and Minimization of Life Cycle Cost(LCC)

This study was intend to develop the optimal design method of suspension bridge by the reliability analysis based on minimization of life cycle cost(LCC). The reliability analysis was performed considering aleatory uncertainties included in the result of numerical analysis. The optimal design was estimated based on life-cycle cost analysis depending on the result of reliability analysis. As the effect of epistemic uncertainty, the safety index (beta), failure probability (pf) and minimum life cycle cost were random variables. The high-level distributions were generated, from which the critical percentile values were obtained for a conservative bridge design through sensitivity assessment.
A Study on the Safety Consciousness Survey of Old Workers

Recently unemployed construction workers are increasing because of construction recession. But the lack of manpower occurs at the same time, in construction sites. So, the excess of workforce supply and demand exists in construction sites. Construction companies have a difficulty in seeking young skilled workers due to this problem. In this study, construction accidents reported by Korean Occupational Safety and Health Agency from 2007 to 2010 were analyzed. It has shown that 50-54, 45-49 and 55-59 year old workers formed 22%, 18% and 17% respectively. This means that old workers becomes a real problem in construction. This research has shown that old workers didn't have much more accidents than young workers, construction accidents related to the employment period of workers.
Analysis of Flow Reversal by Tidal Elevation and Discharge Conditions in a Tidal River

The Han River is the only waterway in Korea where estuary is not blocked by dykes so that tidal water is flowing in and out through the tidal reach. The extreme tidal range in the Yellow Sea causes an intense flood current, stretching over horizontal extents of tens of kilometers into the rivers. To elucidate the flow reversal by discharge conditions and transient tidal level in the Han river, numerical simulations were conducted under 7 boundary conditions for two days with 10 minute time step. As the flow conditions changed from low discharge and high tidal difference to high discharge and low tidal difference, the flow reversals became weaker and the velocity of forward flow direction became higher due to the increased flow momentums and decreased tidal differences. In the case of normal flow, the maximum reverse velocity was 0.4 m/s, which was equivalent to the maximum forward velocity. In addition, the pattern of the development and decay of forward and reverse flow was presented.
The Reinforcement Plan of Safety Management for the Small-sized Facility

While large-sized facilities (type I II) have been managed systematically after the establishment of the Special Law for Safety Management on Facility, the management of small-sized facilities is relatively poor. The small-sized facilities have been managed by The Basic Law for Disaster and Safety Management, however, it is hard to manage them systematically as related standards are not established. Therefore, this study proposed the management plans for including the facilities such as some road tunnels and utility tunnels, which have the definite manager and a high possibility to harm the public, into type I and II facilities. In addition, it proposed the reinforcement plans of safety management for small-sized and vulnerable facilities such as breast wall and cut slopes, traditional markets and pedestrian bridges, which are fundamental facilities closely related to people's life, although a budget and a man-power are not enough.
Assessment of Fire Evacuation Safety for Building Construction

Recently buildings are constructed in larger and higher scales and becoming more complex. However from small to large scale buildings and on construction sites there still are fire safety accidents that occur continuously. Therefore this study is aimed to assess of evacuation safety for building construction by use of simulation. On this study, the evacuation times required were estimated for the permissible evacuation times in cases. Fire safety management plans were suggested for building construction.

The purpose of this study is to investigate the effect of high quality CPR using the 4 types of rescue device equipment and chest compressions energy measurement in pre-hospital settings. So, we used the mode to insert load cell in ALS Skill master Manikin to develop CFMM(Compression Force Measurement Manikin) on main stretcher, CPR board, long spine board, scoop stretcher and floor. And, our research team could know that the main stretcher needed average force of 32.55 (
) kg, CPR board of 27.23 (
) kg, long spine board of 27.13 (
) kg, Scoop Stretcher of 27.38 (
) kg and Floor of 27.24 (
) kg. CPR board must be necessary in the case of CPR on main stretcher in a moving ambulance. But if the condition of patient's back surface is the removable stretcher and the long spine plate, the patient doesn't have to be spent time to use a CPR board. Furthermore, this research suggests to consider that how to take advantage of the education to students for the equipment to check in real time the energy(kg) requirement of chest compressions.
A Study on the Industrial Injury Characteristics in the Agricultural Workers for Five Years(2008~2012)

Recently supporting policy of government for agricultural is changing from individual to workplace. For this reason, the number of industrial injury is increasing due to increase workplace in the agriculture. However, because the studies on the characteristics and causes of injury have not been performed, the effective measure for the prevention of industrial injury is difficult to establish. The aim of this study is to analyze the industrial injury characteristic and prepare the effective counterplan to prevent industrial injury of the workplace in the agriculture. This study analyzed 2,970 cases of industrial injury during the last five years (2008-2012) in the agriculture sector. The data was offered by Korean Occupational Safety and Health Agency. The results of analysis the industrial injury characteristic of the workplace in the agriculture, number of industrial injury for source of industrial injury was 12.3% in the outdoor floor and injury type was 23.1% in the fall. This study analyzed industrial injury characteristic which size and type of workplace, injury type and source of industrial injury through the case of industrial injury in agriculture. The result of this study will be helpful to identify the industrial injury characteristic in the agriculture and prepares the effective industrial injury prevention method.
A Study on the Process Analysis and the Risk Assessment for Removal Work of the Asbestos Cement Slate

Asbestos is given to a variety of six naturally occurring silicate minerals. These minerals possess high tensile strength, flexibility, resistance to chemical and thermal degradation, and electrical resistance. These minerals have been used for decades in thousands of commercial products, such as insulation and fireproofing materials, automotive brakes, textile products, cement and wallboard materials. When handled, asbestos can separate into microscopic-size particles that remain in the air and are easily inhaled. It is now known that prolonged inhalation of asbestos fibers can cause serious and fatal illnesses including malignant lung cancer, mesothelioma, and asbestosis. Therefore the use of asbestos and asbestos products has dramatically decreased in recent years. Also all constructions including asbestos should be removed under strictly controlled conditions and very tightly implemented health & safety management systems. In this study, the process of the removal work of the asbestos cement slate was analyzed by IDEF-0 modeling and evaluated by 4M risk assessment method. The results show that removal work of the asbestos cement slate was classified five process and eighteen detail process. The risk of safety side the higher than the risk of health side in 4M risk assessment.
Comparative Analysis of Work Stress Assessment Tools for Estimating Human Work Performance

A man is a component of a large system how complex it may be so that human performance which can influence on the system safety should be included in system safety assessment. Meanwhile, human performance can vary over time due to lots of factors. Among them, stress is an indirect factor that may cause human error which can result in industrial accidents. To assess stress level of human workers, not a few assessment tools have been developed. However, it can be questionable to utilize them for human performance anticipation because they were mainly developed with the viewpoint of mental health, though stress assessment can be also required in the safety aspect. Therefore, this study aimed to survey the possibility of their application with safety purpose. About 10 kinds of work stress tools were collected and analyzed with reference to assessment items, assessment and analysis methods, and follow-up measures. The results showed that most tools focused their weights on Demands, Supports, and Relationships, in sequence. However, they, except only one tool developed by the Japanese researchers, merely advised to set up counterplans in PDCA cycle or risk management activities. In consequence, application of stress assessment tools mainly developed for mental health seemed impractical for safety purpose with respect to human performance anticipation so that it was concluded that development of a new assessment tools aimed to human performance variation and accident prevention would be inevitable.
Developing Rear-End Collision Models of Roundabouts in Korea

This study deals with the rear-end collision at roundabouts. The purpose of this study is to develop the accident models of rear-end collision in Korea. In pursuing the above, this study gives particular attention to developing the appropriate models using Poisson, negative binomial model, ZAM, multiple linear and nonlinear regression models, and statistical analysis tools. The main results are as follows. First, the Vuong statistics and overdispersion parameters indicate that ZIP is the most appropriate model among count data models. Second, RMSE, MPB, MAD and correlation coefficient tests show that the multiple nonlinear model is the most suitable to the rear-end collision data. Finally, such the independent variables as traffic volume, ratio of heavy vehicle, number of circulatory roadway lane, number of crosswalk and stop line are adopted in the optimal model.
The Gender Sensitive Analysis of Occupational Injuries and Illnesses of Employees

The statistics of occupational injuries and illnesses is one of the most important indicators of worker's safety and health in the field of occupational safety and health. The conventional statistics was produced without consideration of gender difference. This study was planned to investigate the difference of occupational injuries and illnesses by gender because that recently female workers was increased through the change of industrial structure such as from manufacturing dominated to service dominated structure. Statistics of occupational injuries and illnesses collected by workmen's compensation insurance system that have no information about gender composition of population. Economically active population survey was used to calculated the rate of occupational injuries and illnesses by gender. The distribution of estimated rate of occupational injuries and illnesses by industry, occupation and the type of accident were different between male and female employees, but those by the size of enterprise, age of the injured, the duration of the first recuperation, and job tenure were similar between male and female employees. This study has shown that gender sensitive approach may be applied to policy of occupational safety and health because that gender neutral perspective may not discover the fact of gender specific problems.
Operating Criteria of Core Exit Temperature in Nuclear Power Plant with using Channel Statistical Allowance

Nuclear power plants are equipped with the reactor trip system (RTS) and the engineered safety features actuation system (ESFAS) to improve safety on the normal operation. In the event of the design basis accident (DBA), a various of post accident monitor(PAM)systems support to provide important details (e.g. Containment pressure, temperature and pressure of reactor cooling system and core exit temperature) to determine action of main control room (MCR). Operator should be immediately activated for the accident mitigation with the information. Especially, core exit temperature is a critical parameter because the operating mode converts from normal mode to emergency mode when the temperature of core exit reaches
. In this study, uncertainty which was caused by exterior environment, characteristic of thermocouple/connector and accuracy of calibrator/indicator was evaluated in accordance with ANSI-ISA 67.04. The square root of the sum of square (SRSS) methodology for combining uncertainty terms that are random and independent was used in the synthesis. Every uncertainty that may exist in the hardware which is used to measure the core exit temperature was conservatively applied and the associative relation between the elements of uncertainty was considered simultaneously. As a result of uncertainty evaluation, the channel statistical allowance (CSA) of single channel of core exit temperature was +1.042%Span. The range of uncertainty, -0.35%Span (

), was obtained as the operating criteria of core exit temperature.
A Study on Severe Accident Management Scheme using LOCA Sequence Database System

In terms of an accident management, the cases causing severe core damage need to be analyzed and arranged systematically for an easy access to the results since the Three Mile Island (TMI) accident. The objectives of this paper are to explain how to identify the plant response and cope with its vulnerabilities using the probabilistic safety assessment (PSA) quantified results and severe accident database SARDB(Severe Accident Risk Data Bank) based on sequences analysis results. Although PSA has been performed for the Korean Standard Power Plants (KSNPs), and that it considered the necessary sequences for an assessment of the containment integrity. The developed Database (DB) system includes a graphical display for a plant and equipment status, previous research results by a knowledge-based technique, and the expected plant behaviour. The plant model used in this paper is oriented to the cases of loss of coolant accident (LOCA) is be used as a training simulator for a severe accident management.
