Metabolic Syndrome and Sasang Constitution in Cohort Study

Objectives This study is to investigate the related factors to contribute the metabolic syndrome according to Sasang Constitution. Methods Nine hundred twenty six persons out of 1774 persons, over 40 years old, participated in community-based cohort in Wonju City of South Korea from June 2006 to August 2009. The diagnosis of metabolic syndrome was carried out by NCEP-ATP III(National Cholesterol Education Program in Adult Treatment Panel III) and Asian Pacific Criteria for abdominal obesity. The related factors were checked using questionnaire and blood samples. Sasang Constitution was verified by a Sasang Constitution specialist using the results of PSSC(Phonetic System for Sasang Constitution), facial pictures and simplified Sasang Constitutional questionnaires. Metabolic syndrome incidence rate according to Sasang Constitution and binary logistic regression analysis were performed with SPSS 19.0. Results Metabolic syndrome incidence rate was 30.3% and the majority of newly categorized as metabolic syndrome was Taeeumin(40.7%). There were significant risk factors like systolic blood pressure, fasting blood sugar, triglyceride and female and a significant defense factor like HDL-cholesterol. In terms of constitutional view, there were significant risk factors like waist circumference, systolic blood pressure, triglyceride in Soyangin, female, waist circumference, systolic blood pressure, fasting blood sugar, triglyceride in Taeeumin, female, waist circumference, systolic blood pressure, triglyceride in Soeumin. And there was a significant defense factor like HDL-cholesterol in only Taeeumin. Conclusions Regimens on metabolic syndrome were considered to be changed according to Sasang Constitution. Taeeumin female and Soeumin female should be cautious of body weight and metabolic syndrome when elderly. There are more cautious risk factors in each constitution; systolic blood pressure and triglyceride in Soyangin and fasting blood sugar and serum lipids levels in Soeumin and Taeeumin.
Hematologic Characteristics of Stroke Inpatients according to Sasang Constitution in One Korean Medicine Hospital

Objectives The purpose of this study is to know hematologic characteristics of patients with stroke in Sasang Constitutional Medicine which will be helpful for the prevention and treatment of stroke. Methods We collected data from 111 patients who received treatment as a stroke inpatient at one Korean medicine hospital in Busan, Korea. A Sasang Constitutional Medicine(SCM) specialist diagnosed their Sasang Constitutional types based on the four diagnosis standard of SCM and herbal medicine reaction. Results of hematologic tests were classified according to gender, age, stroke types and Sasang Constitutional types and analysed by SAS system. Results 1. Male's mean value of RBC count, Hemoglobin and Hematocrit was lower than normal range. 2. The mean value of hsCRP of above age 65 group was 3.19 times significantly higher than under age 65 group. 3. The mean value of platelet of cerebral hemorrhage group was significantly higher than cerebral infarction group. 4. The mean value of triglyceride of Taeeumin was significantly higher than Soyangin and Soeumin. Conclusions For the prevention of stroke, appropriate treatment of stress, trauma and infections is needed and male require a regular and balanced meals. Especially Taeeumin need to eat small meals to avoid excessive accumulation of energy.
Development of Clinical Practice Guidelines for Sasang Constitutional Medicine Symptomatology - Purpose, Development Strategy and Procedure -

Objectives This study was aimed to develop the clinical practice guidelines, which focuses on Sasang Constitutional Medicine symptomology. By developing clinical practice guidelines, we should present prevention, diagnosis, treatment and assessment of Sasang Constitutional Medicine. Methods We used the method of University of Michigan Health System Guidelines model. Electronic databases including English, Korean databases were systematically searched for clinical articles investigating Sasang Constitutional Medicine symptomology up to March 2010. And "Donguisusebowon-ShinChukbon" was basically used to develop clinical guidelines. Results and Conclusions 12,694 Articles were reviewed. Only 25 articles described adequate methods of this study. Using clinical practice guidelines, we can manage th quality of medicine, emphasis treatment appropriacy and educate patients and doctors. Finally We can get the Sasang Constitutional Medicine standardization and socioeconomic benefit.
Comparison of Keywords of the Journal of Sasang Constitutional Medicine with MeSH Terms

Objectives The purpose of this study was analyzing the equality between the MeSH terms and the keyword used in the papers published in Journal of Sasang Constitutional Medicine and investigating how to use an appropriate MeSH terms as keyword in the papers. Methods A total of 704 keyword used in 177 papers published from 2009 to 2012 in Journal of Sasang Constitutional Medicine were analyzed to investigate the equality between the keyword and the MeSH terms. The collected data was analyzed using SPSS 17.0 software for frequency analysis. Results Among the 704 keyword, 107 keyword(15.2%) was perfectly matched with the MeSH terms. 64 keyword(9.1%) showed partial difference was with the MeSH terms, and 11 keyword(1.7%) showed partial difference was with the Entry terms. 127 keyword(18.0%) were included in the exception item due to the nature of journal, and 395 keyword(56.1%) were not perfectly matched with the MeSH terms. In the yearly analysis result, the number of papers that keyword and MeSH terms perfectly matched was not significant changed, however the number of papers that keyword and MeSH terms did not matched was continuously increased, which clearly indicate use of MeSH terms as the keyword of the papers published in the journal of Sasang constitution medicine is insufficient. Conclusions The papers published in journal of Sasang constitutional medicine need to be cited in various fields and the paper's finding need to affect in other studies for the development of Korean medicine and Sasang constitutional medicine. The use of proper keyword aligned with the international standards is necessary to accomplish the globalization of them.
The Survey on the State and Scale of Constitutional Medical Service Market in Korea

Objectives The aim of this study is to survey the present scales of constitutional medical service market on the base of Korean medical service market following 2006, 2008 year survey. Methods This survey was conducted from Nov. 1 to Dec. 15, 2011 by offline mail. A questionnaire including personal information, general information, and management related to constitutional medicine of Korean medical institution was sent to each subject with a returned envelope. The subjects were asked to complete the questionnaire and 108 data were collected and analyzed. Results This study revealed that the 62% facilities of all Korean medical clinics in Korea are using constitutional medicine for patients. The proportion of constitutional medical service was 13.7% in Korean medical care insurance and 23.5% in total Korean medical service market. Conclusion It is assumed that the scales of total Korean medical service market seemed to be 3,974,425 million won and the scales of total constitutional medical service market seemed to be 929,404 million won.
Simultaneous Analysis and Anti-obesity Effect of Taeeumin Cheongsimyeonja-tang

. 3T3-L1 preadipocytes were differentiated into adipocytes by adding insulin, dexamethasone and 3-isobutyl-1-methylxanthine (IBMX) for 8 days in the absence or presence of CSYJT. Anti-obesity effects of CSYJT were evaluated by Oil Red O staining, glycerol-3-phosphate dehydrogenase (GPDH) activity, triglyceride contents, and leptin production. Results Calibration curves were acquired with
>0.9999. The contents of baicalin, wogonoside and baicalein in CSYJT were 14.54-14.65 mg/g, 5.24-5.27 mg/g and 0.01-0.02 mg/g, respectively. CSYJT showed inhibitory effect on lipid accumulation and GPDH activity in the differentiated 3T3-L1 cells. Furthermore, CSYJT significantly decreased contents of triglyceride and leptin production in 3T3-L1 adipocytes. Conclusions These results will be helpful to improve quality control and anti-obesity effect of Taeeumin CSYJT.
A Case Study of Rectal Cancer Patient Treated with Dokhwaljihwang-tang

Objectives This study reports a case of rectal cancer patient treated with Dokhwaljihwang-tang. Methods The patient's subjective symptoms such as pain, sleeping difficulty, frequent defecation and dysuria were observed and the change of the quality of life(QOL) was evaluated using Functional Assessment of Cancer-Colon (FACT-C). Results After the treatment, symptoms such as pain, sleep disorder, defecation, and urination difficulty ameliorated and the score of the FACT-C improved. Conclusions A rectal cancer patient suffering from adverse consequences from the low anterior resection and the pain induced by the metastases showed the improvement of general condition and the quality of life after treated with Dokhwaljihwang-tang.
A Case Study of Taeeumin Patient Who Experienced Refusing Food Intake after Stroke

Objectives The purpose of this case thesis paper is to report the improvement of symptoms for a 58-year-old male patient who experienced refusing food intake 1 month after onset of stroke by treating the patient with Chungpyesagan-tang. Methods We have determined the patient to have the Taeeumin type physique with Dry Heat Syndrome and provided Chungpyesagan-tang to alleviate the patient's symptoms. The level of improvement was measured based on changes of food intake amount. Results The patient was hospitalized to receive treatment for roughly 2 weeks. The symptom of refusing food intake as well as other symptoms that simultaneously appeared including insomnia, headache, dizziness, and general weakness have improved. Conclusions In this case, the symptom of refusing food intake appeared one month after experiencing stroke, and the patient refused not only meals, but the intake of all other forms of food including water and medicine. The cause of such symptoms were deemed to be emotional rather than the types of food which generally provide the causes for Oh Shik Jeung. As the patient was deemed to have the Taeeumin type physique with Dry Heat Syndrome. Chungpyesagan-tang was provided for treatment. Acupuncture and oriental cupping therapy were also provided concurrently, and the main symptom and other accompanying conditions have largely improved after 2 weeks of hospitalized treatment.
