An Analysis of Case Reports and Case Series on Taeeum Person in Journal of Sasang Constitutional Medicine from 2006 to 2015

Objectives This study aimed to provide a basis for treatment of taeeum person by analysing published case reports and case series in Journal of Sasang Constitutional Medicine.Methods We reviewed case reports and case series of taeeum person in the Journal of Sasang Constitutional Medicine from 2006 to 2015.Results Of the forty papers, reporting 182 cases of taeeum person. Thirty-five papers reported 6 types of Sasang Constitutional Medicine pattern. A total of 10 types of interventions were described, of which herbal medicine (100%) was most frequently used. A total of 30 types of outcome measeure were used in papers, for which symptom change (79.2%) was most frequently used. A total of 8 types of diagnosis criteria were used in 38 papers.Conclusions A sufficient number of clinical studies are warranted to provide a clinical evidence for treatment of taeeum person.
The Origin, Changes and Compositive Principles of Jeoryoungchajeonja-tang

Objectives This paper investigates the origin, the progressive changes and the constructive principles of Jeoryoungchajeonja-tang(猪苓車前子湯).Methods Jeoryoungchajeonja-tang and other related prescriptions were analyzed in terms of their pathological indications, based on previous literature including 『Sanghannon(傷寒論)』, 『Keumkuaeyoryak(金匱要略)』, 『Donguibogam(東醫寶鑑)』, 『Donguisusebowon‧Sasangchobongwon(東醫壽世保元‧四象草本卷)』, 『Donguisusebowon‧Gabogubon(東醫壽世保元‧甲午舊本)』, 『Donguisusebowon‧Sinchukbon(東醫壽世保元‧辛丑本)』Results and Conclusions 1) The origin of Jeoryoungchajeonja-tang is discovered in the prescriptions for Lee-su(利水), that is, Jeoryoung-tang(猪苓湯). 2) Ohryoung-san(五苓散) introduced in 『Donguisusebowon‧Sasangchobongwon(東醫壽世保元‧四象草本卷)』progressively transformed into Shihosaryoung-san(柴胡四苓散)(『Donguisusebowon‧Gabogubon(東醫壽世保元 ‧甲午舊本)』) and ultimately into Jeoryoungchajeonja-tang(『Donguisusebowon‧Sinchukbon(東醫壽世保元‧辛丑本)』), a prescription appropriate for usage in the Mangeum symptomatology(亡陰證). 3) The Jeoryoungchajeonja-tang is composed of 10 herbs. Of these, Notopterygium incisum Ting(羌活), Aralia cordata Thunb.(獨活), Schizonepeta tenuifolia Briq.(荊芥), Saposhnikovia divaricata Schischk.(防風) make the Pyoeum(表陰) drop, and Anemarrhena asphodeloides Bge.(知母), Gypsum fibrosum(石膏) scatter the uihwa(胃火), and Poria cocos Wolf.(茯苓), Alisma canaliculatum(澤瀉), Polyporus umbellatus Fr.(猪苓), Plantago asiatica L.(車前子) facilitate urination.
The Reliability and Validity Test of Sasangin Diagnostic Questionnaire for Specialist(SDQ_S)

Objectives The purpose of this study was to develop the Sasangin Diagnostic Questionnaire for Specialist(SDQ_S) and to evaluate the reliability and validity of SDQ_S for standardization of sasang constitutional diagnosis by korean medicine Specialist in clinical practice.Methods SDQ_S was developed through discussions of three sasang constitutional medical specialist. We recruited 210 subjects aged from 20 to 70, and conducted the evaluation of SDQ_S and sasang constitutional diagnosis. For the reliability test, internal consistency was examined by calculating the cronbach`s α coefficient on 88 items excepted measurements of trunk, and independence and agreement was examined by chi-square test and McNemar test for 84 subjects which retested SDQ_S. For the validity test, discriminant functions was derived from the sample group(105 subjects), and they were applied to the test group(105 subjects). We identified the hit-ratio of SDQ_S in the test group.Results and Conclusions In the reliability test, the cronbach`s α coefficient was 0.664 and 87 items(98.9%) have dependent relationships between 1st test and 2nd test. And 39 items(44.3%) have high agreements(0.8≤McNemar), 26 items(29.5%) have common agreements(0.4≤McNemar<0.8). In the validity test, four discriminant variables(Soeum Index, Taeeum Index, Torso Ratio I, Taeyang Index) were selected, and two discriminant functions were derived. Total hit-ratio was 74.2%, and hit-ratio of Taeeumin(76.7%) was higher than that of Soeumin(75.9%) and Soyangin(69.7%). SDQ_S has good reliability and validity for sasang constitutional diagnosis.
The Effects of Paced Breathing in Specific Respiration Rate on Heart Rate Variability

Objectives We observed what effects of Paced Breathing(PB) in specific respiration rate have on heart rate variability (HRV) according to Sasang Constitution.Methods HRV of 72 healthy participants in sitting position was recorded while they carried out usual breathing, 0.2Hz, 0.1Hz, and 0.05Hz PB each 5 minutes in consecutive order. Five minute of relaxation was permitted between each breathing. Finally, HRV indices were statictically analyzed of 32 participants (SOEUM: 11, SOYANG: 10, TAEEUM: 11) after data out of accord with respiration rate or outliers were excluded.Results and Conclusions According to respiration rates, there was no statistical significance of HRV among Sasang Constitution. Regardless of Sasang Constituion, 0.2Hz PB increased mean heart rate and decreased natural logarithmic low frequency(lnLF) oscillation of HRV without the change of natural logarithmic high frequency(lnHF), while 0.1Hz PB increased lnLF and standard deviation of N-N interval(SDNN), and slightly decreased lnHF without the change of mean heart rate. 0.05 Hz PB also showed the same effect as 0.1Hz PB, but was impracticable.
Comparison of Effects of Yangkyuksanhwa-tang, Palmulgunja-tang and Cheongpyesagan-tang on the Rat Hyperthyroidism Induced by Levothyroxine

Objectives This study`s object was to observe the comparative effects of Yangkyuksanhwa-tang, Palmulgunja-tang and Cheongpyesagan-tang on the chronic LT4(levothyroxine) induced hyperthyroidism in rats.Methods Six groups, each of 8 rats in group, were used in this study. Saline and distilled water treated rats are intact control group. Hyperthyroidism was induced by daily subcutaneous LT4 300 μg/kg treatment for 27 days(LT4 control). Since 12th LT4 treatment PTU(propylthiouracil) 10 mg/kg was intraperitoneal injected(PTU group) and aqueous extracts of Yangkyuksanhwa-tang, Palmulgunja-tang and Cheongpyesagan-tang(YS, PG and CS) 500 mg/kg were orally administrated(YS, PG, CS group), once a day for 15 days. The differences in the body, thyroid gland and epididymal fat pad weights, serum T3(tri-iodothyronine), T4(thyroxine), TSH(thyroid-stimulating hormone), thyroid gland and epididymal fat pad histopathology, liver weight, AST(asparte aminotransferase), ALT(alanine aminotransferase) concentrations, hepatic lipid peroxidation, GSH(glutathione), SOD(superoxide dismutase), CAT(catalase) activities, liver histopathology were observed to evaluate effects on hyperthyroidism, liver damages and antioxidant effects.Results As results of LT4 treatment, hyperthyroidism and related liver damages such as lower body, thyroid weights, higher serum T3, T4, AST, ALT levels, thinner follicular lining epithelium in thyroid glands were observed. However, these symptoms were inhibited by oral treatment of YS, PG and CS. As compared with PTU treatment, these herbal prescriptions showed lower overall efficacy on the hyperthyroidism, but YS showed more favorable effects on the hepatic antioxidant defense systems.Conclusions This results suggest that YS, PG and CS favorably control the LT4 induced hyperthyroidism and related liver damages in rats through modulation of the hepatic antioxidative defense systems.

A Case Report of a Soyangin Patient with Aftereffects of the Tuberculosis Aggravated after a Pleural Effusion and a High Fever of Unknown Origin

Objectives It is important to care aftereffects of the tuberculosis such as cough and sputum especially for the old. The purpose of this study is to report a case which showed symptoms improvement after treatment with Hyungbangjihwang-tang.Methods To evaluate the results of this treatment, Decrease of cough and sputum was assessed by Visual Analogue Scale(VAS). The patient`s oral intake and body weight were measured.Results The patient who suffered with cough and sputum after tuberculosis developed high fever and pleural effusion of unknown origin. After treatment with western and oriental medicine, high fever and pleural effusion were subsided but cough and sputum got worse and body weight was decreased after high fever and pleural effusion. So we prescribed Hyungbangjihwang-tang and then the symptoms of the patient were improved.Conclusions This study suggests that using Sasang constitutuional medical treatment is effective for Soyangin patient with afftereffects of the tuberculosis such as cough and sputum.
A case Report of Tinnitus after Subarachnoid Hemorrhage(SAH) Operation Diagnosed as Soyangin Hyunggyeok yeoljeung

Objectives This case study reports patient diagnosed as Soyangin Hyunggyeok yeoljeung with tinnitus, headache and sleep discomfort after Subarachnoid Hemorrhage(SAH) had treated with HyungBangSabaek-San.Methods The patient suffering from tinnitus, sleep discomfort had taken HyungBangSabaek-San from the first to the end three times a day. We measure tinnitus by Tinnitus Handicap Inventory(THI) and observe symptoms change using Visual Anlaogue Scale(VAS).Results The symptoms had been steadily decreased and quality of life had increased also. Headache and constipation had been improved in a week and tinnitus and chest discomfort had decreased next week. At last, patient complained symptoms slightly.Conclusions HyungBangSabaek-San a is treatment of Soyangin Hyunggyeok yeoljeung and may be considered as a treatment of tinnitus.
A Case report of Soyangin patient with early-stage Parkinson`s disease treated with Hyeongbangjihwhang-tang

Objectives The purpose of this case study is to report significant improvement of headache, constipation and a slight improvement of tremor after treatment with Hyeongbangjihwhang-tang in a Soyangin patient with Parkinson`s disease.Methods This Soyangin patient is treated by Hyeongbangjihwhang-tang and acupuncture therapy. The progress was evaluated using the Unified Parkinson Disease Rating Scale before and after treatment. Secondary results assessments include Global Assessment Scale, change of patient`s omniloquilism, Xerostomia and Cold hands & Feet Syndrome.Results and Conclusions The patient`s symptoms of headache, constipation and tremor were improved after using Hyeongbangjihwhang-tang. and UDPRS score decreased in Part Ⅰ, Part Ⅱ, and Part Ⅲ. In conclusion, this study shows that Hyeongbangjihwhang-tang can be effective treatment for Soyangin patient with early-stage Parkinson``s disease. Overall, Sasang medicine can be a solution of the symptoms caused by Parkinson`s disease. Through Sasang medicine we can contribute Parkinson`s disease Patient`s quality of life get better.
A Case Study of a Taeeumin Patient with Advanced Parkinson`s Disease Having Orthostatic Hypotension

Objectives This case study is about a Taeeumin patient with advanced Parkinson`s disease identified as Dry-heat (Joyeol) pattern. In this study, we report significant improvement of non-motor and motor symptoms after treatment with Cheongsimyeonja-tang.Methods The patient was identified as Taeeumin Dry-heat (Joyeol) pattern and treated with Cheongsimyeonja-tang and acupuncture. The Unified Parkinson Disease Rating Scale (UPDRS) was used to assess the overall functions of the patient. And the global assessment scale (GAS) was used to assess the improvements of dizziness, bradykinesia and tremor after the treatment.Result and Conclusion The UPDRS total score decreased from 138 points to 86 points after 5 weeks treatment. And symptoms of dizziness caused by hypotension, bradykinesia, and tremor showed significant improvement in GAS after the treatment. Furthermore, sleep disturbance and constipation were reported to be improved after the treatment. In conclusion, this study shows that Sasang constitutional medicine can be effective treatment for Taeeumin patient with advanced Parkinson`s disease having orthostatic hypotension.
