A Survey on the Status of Health Examination among Farmers in a Rural Area

This study was carried out to reveal the status of health examination among farmers and to attract more attention to the health care system for farmers. Ten pre-trained medical students interviewed the rural residents 18 years of age and older in eight villages which were randomly selected from a county near Taegu city in Korea, in August 1996. Finally 751 persons were interviewed of whom the percentages of male and female were 41.8%, 58.2% respectively. Among the subjects, 361(48.3%) were fully engaged in farming, 184(24.4%) were partly engaged, and the remaining 206(27.3%) were not engaged in farming at all. The overall prevalence of farmer's disease was 23.0% and there was no significant difference between the group of fully engaged in farming(23.3%) and the group of not-fully engaged(22.9%). But the prevalence of farmer's disease in female subjects(27.8%) was significantly higher than that in male(16.2%)(p<0.01). Among the 288 farmer engaged in spraying pesticide, 113(39.2%) had experienced one or more pesticide related symptoms during last one year, but only 18(15.9%) of them had visited medical facilities due to their symptoms. The experience of receiving education about pesticide was significantly correlated with the degree of wearing protectors during pesticide spraying(p<0.001). Among the 736 persons excluding non-respondents, 281(38.2%) received health examination during last one year ; 176(62.6%) of them received free health examination, and 105(37.4%) received charged one. Among the 533 persons 40 years age and older, only 124(23.3%) had received the 'health examination for the elderly' during last one year, which is provided for the 40 years age and older by Korea medical insurance corporation and medical insurance societies. Most of all beneficiaries of self-employed medical insurance thought the imposed contributions as very expensive(77.4%) or moderately expensive(13.2%). The great majority of farmers are exposed to various health risk factors including pesticide, high temperature, overwork etc. comparable to industrial workers. But farmers are excluded from the regular yearly worker's health examination because of not belonging to a company despite they pay relatively more medical insurance contributions compared with the industrial workers and the urban self-employed medical insureds. It is necessary to develop special health management program for farmers such as the special health examination for the industrial workers exposed harmful agents.
Survey of health status for preschool children who visit a public health center

The purpose of this study was to analyze the physical development and health status of 5 year-old children for more effective health management of preschool children. This study carried out examinations of height, weight, hemoglobin, visual acuity & dental examination for 5 years-old preschool children in Kwang-ju up, Kwang-ju gun, Kyunggi-Do from January to February, 1996. For height and weight, the data were compared with the 1985's Korean standard and the hemoglobin and visual acuity data were compared with the Korean standard. The results were as follows. The average height for 5 year-old male children was 106.3
4.7(cm) and that of 5 year-old female children was 105.3
4.3(cm). Sixty seven point eight percent of male subjects registered below the 50th percentile for the Korean standard height and 79.5% of the female subjects registered below the 50th percentile for female. The average weight for 5 year-old children was 18.6
2.6(kg) for females. Also, it is estimated that obese subjects totaled 2.7%. For the visual acuity, 18.0% of subjects had weak vision in the left eye and 16.9% in the right eye. As a result, it was advised that these children undergo further examination. It was remarkable that there were so many anemic children. Children with a hemoglobin content of less than 11.9(g/dl) totaled 47.8%. It is anticipated that the results of this study will contribute to the on-going evaluation and subsequent planning for the children health management program within the health promotion program of public health center in Kwang-ju gun.
A Study on the Serum Lipid Levels and Related Factors among Women in a Rural Community

This study was performed to offer the fundamental data for preventing and controlling the cardiovascular diseases of rural community women. The number of 168 women aged 40 to 50s who live in Kumsan-gun, Chungnam were selected for analysis after medical examination by a Kumsan-gun health center during the period of November to December in 1996. Total cholesterol(TC), triglyceride(TG), high density lipoprotein cholesterol(HDL-C), blood pressure(BP), degree of obesity(BMI) and atherosclerosis index(AI) were measured and relation between these physical measurements to serum lipid levels were studied. The results were as follows; 1. Mean TC level, TG level, BP, BMI and AI were significantly increased with advancing age. 2. Mean TC level, TG level, BMI and AI of borderline BP group and hypertension group were significantly increased than those of normal BP group. 3. Mean TC level, TG level and AI of obesity group were significantly increased than those of normal BMI group. 4. Simple correlation analysis showed that the positive correlation between TC, TG, BP, BMI and AI, but the level of HDL-C was negative correlation with TG and AI. 5. In multiple regression analysis taking HDL-C level as the dependent variable and TG, TC, BP, BMI, Age AI as the independent variable, AI, TG, BMI, TC were significantly related to HDL-C.
The Experience of Pesticide Poisoning and It's Related Factors in a Rural Area

The purpose of this study is to analyse both actual conditions and health problems of farmers in using peshcrde and to develop protective methods. This study research was carried out by means of questionnaires with members of 100 families randomly selected from 279 families residing within the managing territory of primary health post. The results are as follows: Based on 10 kinds of safety instructions, it was shown that 76% complied with more than 6 isntractrons, and less than 4 instructions were 24%, of which 25% carefully read their handling instructions, 58% did sometimes, and 17% never read such instructions. Not complying with those safety instructions, the most frequent experience was physical symptoms, of which headache accounted for 80.9%, dizziness for 73.5%, dermatitis for 64.7%, and vomiting for 41.2%. Their experiences of pesticide poisoning indicated 68.0%, of which the aged level accounted for 88.9% which was relatively high. A spraying time of peshcide was less than 2 hours accounted for 64.0%, and more than 2 hours for 36%. It was also shown that the number of farmers, who experienced poisoning after spraying such chemicals, accounted for 77.8%. The hate of poisoning experiences were 92.0% in group not wearing a protective equipment, were 62.5% in group wearing a complete set of protective equipment.
The delousing of head louse in primary schools and kindergartens established as an annex to the primary school in Namwon-shi, Jollabuk-do, Korea(1995)

To exam infestation rate and removal rate of head louse, we did delousing at 27 primary schools and 19 kindergartens established as an annex to the primary school in Namwon-shi, Jollabuk-do, Korea, from May to December 1995. Total of 3123 subjects were examined ed for the presence of head lice and nits The overall infestation rate was 9.6%, 9.5% in primary school children and 10.1% in kindergarten children. Among 299 infested children, 55 children evaluated by live nit had louse, and 79 children had dead nit. According to distractive research, Ibaek-myon and Songdong-myon revealed high infestation rate of 27.3%, and 21.7% respectively. Infestation rate of primary school was 0 to 50.9%, kindergarten was 0 to 100%. Infestation rate of female children were higher than male in all grades. When we executed one time treatment with PARA
aerosol(bioallethrin with piperonyl butoxide), treatment rate of 65.1% was examined, 82.8% was examined in three times of treatment. We concluded that 3 times of treatments interval of 14 days were more effective than one time in the case of treatment of only infested children. In order to prevent the spread of head lice, education of school hygiene and suitable delousing attempts should be executed in the present situation.
Psychiatric Factors Associated with Farmers' Syndrome

Backgrounds : There has been many studies investigating the causes of farmers' syndrome. In some studies, psychologic stress is related to farmer's syndrome. And the diagnostic criteria of farmer's syndrome is similar to those of generalized anxiety disorder. So we carried out this study to investigate the psychiatric causes of farmers' syndrome. Methods : This study was done in some rural and urban areas of Kyoungsangnam Province, July, August, October and November of 1996. Those who came to free medical service and completed interview, medical examination and laboratory examination and 20-59 years old were 150 persons. And those who came to health center for health examination and completed only interview and 20-59 years old were 94 persons. The questionnaire was composed of sociodemographic factors, health risk factors, farmer's syndrome, work load, BEPSI(inventory to measure stress), Spielberger's state-trait anxiety inventory, self-rating depression scale. To examine statistical significance, we used X2-test, Mantel-Haenszel test for linear association, t-test, ANCOVA, correlation, multiple regression, logistic regression. Results : The prevalence of farmer's syndrome, adjusted for age and sex with population of Kyungsangnam Province of 1993 was 208 per 1,000(90 per 1,000 in men and 329 per 1,000 in women). In bivariate analysis, farmer's syndrome was significantly related to age, sex, job, income, smoking, alcohol drinking, work load, BEPSI, state anxiety, trait anxiety, depression, body mass index, Hwa-Byung, hypertension, anemia. However, when age and sex were adjusted, job was not significantly related to farmer's syndrome. The score of farmer's syndrome was significantly related to age, sex, work load, BEPSI, trait anxiety by multiple regression. Farmer's syndrome was significantly related to increasing age(odd ratio 1.079, 95% C.I. 1.060 - 1.099), sex(odds ratio of male 0.434, 95% C.I. 0.349 - 0.540), and BEPSI(odds ratio 1.231, 95% C.I. 1.148 - 1.320) by logistic regression. Results of logistic regression analysis of the component symptoms of farmer's syndrome were as follows. Shoulder stiffness was significantly related to increasing age, female sex and BEPSI. Lumbago was significantly related to increasing age, female sex and trait anxiety. Numb limbs and nocturia was significantly related to increasing age and female sex. Breathlessness was significantly related to work load, sleeplessness was significantly related to depression, dizziness was significantly related to job and state anxiety, and abdominal fullness was significantly related to female sex. Conclusion : Farmers' syndrome was related to work load, but was more related to psychiatric factors such as BEPSI and trait anxiety. And the occupation was not risk factor of farmers' syndrome in this study, so further study is needed to investigate the cause of farmers' syndrome.
Determination of Antigenicity and Characterization of Proteinase from Tissue Invading Nematode Larvae

In case of tissue invading nematode, proteolytic enzyme was required at their parasitic life. Proteinases obtained from these parasites(Toxocara canis, Ansakis spp. and Trichinella spiralis) were extracted, isolated and further purified. And then the analysis for activity and inhibitory effect of proteinases were performed by appropriate substrate. Determination of protein as a circulating antigen was done in use of infected animal serum with above parasites, respectively. For above experimental objects, following procedures were performed. First, enzymatic activity was measured in use of azocasein and inhibitory effect of porteinase were studied by various inhibitors. Second, partially purified proteins containing enzymatic activity were obtained by ion exchange chromatography, ultrafiltration and electrophoretic elution. Third, role of the partially purified protein as a circulating antigen. The results obtained were as follows : 1. Enzymatic activity of each nematode proteinase was varied according to pH. Optimal pH of Toxocara canis, Ansakis spp. and Trichinella spiralis were pH 6.0, pH 5.5 and pH 6.5, respectively. The optimal molarity of buffer was 0.1M phosphate buffer. Although little difference between these proteinases was observed, temperature stability was at least maintained at
until 5 days. 2. In case of Ansakis spp. and Toxocara canis, enzymatic activity of these proteinases was considerably inhibited by Leupeptin and EDTA. For maximum enzymatic activity of above proteinases, it was required that cysteine residue of enzyme should be protected. And it was suggested that metallo type was contained in enzyme active site. Proteinase of Trichinella spiralis contained metallo type also. 3. Although partial purification was performed in Ansakis spp. and Toxocara canis, proteins maintaining enzymatic activity were identified as a circulating antigen. From SDS-PAGE and immunoblot, 25 kDa was presented in Ansakis spp.. Specific antigen of Toxocara cains was 110 kDa protein fraction. 55 and 42 kDa proteins were reacted with normal serum. Trichinella spiralis 60 kDa protein fraction was successfully purified from excretory materials in culture. As a result of immune-reaction with Trichinella spiralis infected serum, highly purified 60 kDa protein was maintained antigenicity until final purification step.
A survey of private physician's opinion on utilization of nursing manpower

The medical insurance system has been adopted in 1989. Since then, the utilization of medical care services has increased rapidly. It appears that the medical institution needs more nursing manpower such as nurses and nurse aides to meet an increased demand for medical care for the residents. However, the medical care clinics run by a medical practitioners has a more shortage of nursing manpower than hospitals. The purpose of this survey was to analyze the current employment status and to obtain an opinion on the improvement of utilization such as recruitment and retention of nursing manpower. The questionnaire sent to the private physician and the response rate was 28.7% ; 87 out of 300 physicians. 82.6% of the respondents employed nurses aides rather than nurses and an average number of employees per clinic was 2.5 persons. Most physicians had difficulties in recruiting nursing manpower. It took more than one month for replacement on average, therefore, they were suffering from giving good continual care for the patient and also high resignation of nursing personnel. The low wage, long working hours, hard work and lack of incentives are the biggest reasons for the difficulty in recruiting and retaining of the nursing personnel obtained for the clinics survey. The above mentioned problems would be solved in the near future by changing those difficulties. In the meantime, the private physicians are marking an effort to retain the nursing personnel by keeping a relationship of the private physician's cooperatives.
