A Study of Accident Mode and Effects Analysis for Using FMEA
Journal of the Korea Safety Management and Science, volume 6, issue 1, 2004, Pages 11~23
The modem industrial society is developing while growing more diverse and gigantic. Accordingly, occupational injuries or accidents can be caused in various situations, not just in the limited range of workplaces but also in the surroundings, and interest has increased in the prevention of occupational accidents with respect to occupational health and safety, and environment. Thus, this thesis will consider 4MlE (Man, Machine, Method, Material, Environment) as the fundamental causes of accidents and introduce a model of system in which the output of the process control system is replaced by accidents with its input by 4M1E. Furthermore, it will demonstrate how occupational hazardousness can be measured, whereby it can also be rated, by examining the relationship between 4M1E and types of accident in terms of the categories of severity, frequency, and detectability, based on the application of the model to the framework of FMEA.
A Study of Safety Function Deployment for Using QFD
Journal of the Korea Safety Management and Science, volume 6, issue 1, 2004, Pages 25~35
We achieved a Quantitative economic growth through rapid developed in 1970' and 1980'. It was confronted after IMF crisis that we needed to improve the past economic policy and the industrial structure. In line of that, the problem of the industrial structure like huge accidents of 1990' converted the recognition of safety. And to improve the conversion, the research of a safety management was needed. In this paper, we thought 4M1E(Man, Machine, Method, Material, Environment) as the cause of accident using the principal of process, assuming that output is accident. Applying 4M1E to the structure of QFD(Quality Function Deployment), we propose the safety function deployment, which has the flow line as followed; demand safety, safety characteristic, direct cause, 4M1E and safety management.

The system consists of 3 components: the FRP body rotation jig, the automatic manhole cutting machine, the automatic sanding processing machine. The safety of workers and working environment are greatly improved, because the dangerous jobs are automatized and the toxic fiberglass dust is automatically collected. Also, the productivity is greatly improved and the cost is reduced.
Development of Program for Qualitative Risk Based Inspection (RBI) using API-581
Journal of the Korea Safety Management and Science, volume 6, issue 1, 2004, Pages 49~60
Industrial facilities are becoming more complex and various with rapidly developing technologies. A number of installations and equipments are increased to improve productivity, and they are operated under the condition of high temperature and pressure. They are continuously repaired, replaced or abolished because of the deterioration with time, but their repair and replacement are not easy for economical and environmental concerns. If possible, the deteriorating equipments should be inspected and repaired effectively considering the state of maintenance and preservation, economic efficiency, and safety. That's where RBI(Risk Based Inspection) fits in. Even if RBI technology has been known among specialists, there were no tools readily available and it was hard to practice and apply RBI in industrial inspection and safety management. In the development of the qualitative RBI, the overall algorithm adopted API-581 BRD. The qualitative RBI program was developed to search the appling process.
Optimal Number of Failures before Group Replacement under Minimal Repair
Young Kwan, Yoo ;
Journal of the Korea Safety Management and Science, volume 6, issue 1, 2004, Pages 61~70
In this paper, a group replacement policy based on a failure count is analysed. For a group of identical repairable units, a maintenance policy is performed with two phase considerations: a repair interval phase and a waiting interval phase. Each unit undergoes minimal repair at failure during the repair interval. Beyond the interval, no repair is made until a number of failures. The expected cost rate expressions under the policy is derived. A method to obtain the optimal values of decision variables are explored. Numerical examples are given to demonstrate the results.

On the Selection of Demand Used in Planning for the Distribution Networks
Jun Geol, Baek ;
Journal of the Korea Safety Management and Science, volume 6, issue 1, 2004, Pages 135~146
This paper first addresses a distribution planning method on centrally controlled supply chain. The distribution channels are assumed to be network of arborescence form. For such distribution networks, this study proposes a distribution planning scheme when the demands for retail sites are provided for a given planning horizon. As the planning horizon rolls forward, for a new horizon, forecasted demand distributions of periods in the horizon are updated. An idea of controlling customer service level by the selection of demand to be used in the planning (Demand Used in Planning, DUP) from the forecasted values is also discussed.

On the Development of NC Simulator System for Wire Cut EDM
Journal of the Korea Safety Management and Science, volume 6, issue 1, 2004, Pages 283~300
This paper describes the NC simulator system for Wire-cut electrodischarge machining. Electrodischarge Wire-cut machining is applicable to all materials including metals, alloys, and most carbides. Although CAM system generate the NC code considering electrodischarge conditions, incorrect Wire-cut tool path bring about fatal results. The simplest way of simulating a EDM process is to display the trajectory of Wire-cutter location by line segments. With this kind of simulation, the programmer can get a general idea about whether the wire is moving as planned but cannot locate gouging or excess material because only the wire location will be seen, not the changes in the workpiece as it is machined. The ideal way of simulating the EDM process is to display the solid model of the workpiece as it is machined. Therefore we propose the ideal NC simulator system for Wire-cut EDM.
Electric Properties of Silicone Rubber on Reinforcing Agent Dependence
Journal of the Korea Safety Management and Science, volume 6, issue 1, 2004, Pages 301~309
An inorganic filling agent, ATH (alumina trihydrate) was used to induce inorganic-organic coupling by mixing with stearic acid, acryl silane, vinyl silane as surface treatment agents in order to apply as a high voltage insulating material. Volumetric resistivity was shown to vary with surface treatment agents, and the highest value was obtained in case of the mixture with vinyl silane. The dielectric breakdown intensity was shown to decrease gradually and saturate to a stable value, possibly due to the increase of cross link density in the vinyl radicals introduced to silica surface, resulting in stable dielectric breakdown intensity in the final stages. Tracking and flame retardant properties were also shown to be the best among the samples investigated in this study.
Impact Characteristics on the Laminated Shell for CF/Epoxy Composite
Journal of the Korea Safety Management and Science, volume 6, issue 1, 2004, Pages 311~323
This paper is to study the energy absorption characteristics of CF/Epoxy(Carbon Fiber/Epoxy Resin) laminated shell with the various curvatures subjected to transverse impact loadings under the low impact velocity in consideration of design of structural members for use of transportation machine, which are consisted of the characteristics of high stiffness, strength and lightweight. The curvature radius are associated with the energy absorption characteristics of CF/Epoxy laminated shell which is brittleness material. In all tests, maximum load of CF/Epoxy laminated plate is higher than that of laminated shell with curvature, but maximum deflection is lower. And then absorbed energy of laminated shell with curvature is higher than laminated plate(curvature radius is unlimited), As curvature radius is increased, the absorbed energy is increased in laminated shell with curvature.
