Experimental Study of Hwangginaetak-san on Anti-inflammatory Effect

Objective: Chronic otitis media is an inflammation and infection of the middle ear which is persistent. Chronic otitis media occurs when the eustachian tube becomes blocked repeatedly, multiple infections, ear trauma, or swelling of the adenoids. A chronic ear may be the result of an acute ear infection that does not clear completely, or the result of recurrent ear infections. A chronic ear infection may be more destructive than an acute ear infection because its effects are prolonged, and it may cause permanent damage to the ear. Methods : Experimental animals made use of 4-5 weeks age(weight 20-25g) ICR(male)mouse. The extracts which were extracted from Hwangginaetak-san devided low dose group( 1.0g/kg-HN) and high dose group(3.0g/kg-HN), they were intragastrically administered to the mouse of sample A and sample B prior to LPS I.P injection. Compared with inflammation induced group which were induced by LPS, we measured the WBC count, IL-6 level in plasma and TNF-
level in plasma. Results: 1. Hwangginaetak-san decreased WBC count in inflammatory reaction induced by LPS 2. Hwangginaetak-san decreased IL-6 level in inflammatory reaction induced by LPS. 3. Hwangginaetak-san didn't decreased TNF-
level in inflammatory reaction induced by LPS Conclusion: According to above results, Hwangginaetak-san was improved its suppression effect to the inflammatory reaction through WBC count and IL-6 level. So Hwangginaetak-san is considered to be used for treatment of chronic otitis media by controlling the WBC count and IL-6 level in plasma.
Study on the Anti-inflammatory Effects of Seungmagalgeuntanggamibang(SMG)

This study was carried out to investigate the effects of SMG on the in vitro and in vivo inflammatory reactions. In experiment I, in vitro tests, ethanol extract of SMG showed potent radical scavenging activity tested by DPPH(I,1-diphenyl-2-picryl-hyrazyl) method and inhibited the lipopolysaccharide-induced gene expressions of interleukin-1
, interleukin-6 and tumor necrosis factor-
at a concentration of 50㎍／㎖) by the macrophage RAW 246.7 cells. Among the herbal ingredients of SMG, ethanol extracts of Scutellaria baicalensis, Paeonia lactiflora, Glycyrrhiza glabra showed potent radical scavenging activity. And Glycyrrhiza glabra and Scutellaria baicalensis showed potent inhibitory activity of nitric oxide production. Especially, ethanol extract of Scutellaria baicalensis inhibited the gene expression of IL-1
, IL-6 and TNF-
. In cyclooxygenase-2 assay, Scutellaria baicalensis and Glycyrrhiza glabra showed the potent inhibition of prostaglandin E2 generation. In experiment 2, in vivo tests, SMG showed inhibitory effects on vascular permeability (28.7

). These results mean that SMG has a anti-inflammatory effects by it's inhibitory effects of leukocyte migration and vascular permeability as well as it's inhibitory effects of lipopolysaccharide-induced gene expression of IL-1

, and radical scavenging activity. Therefore, I expect that SMG may be used as a effective drug for treatment on inflammation.
Effects of Herbal Extracts on the Inflammatory Reactions

Taken together, I expect that herbal extracts may be used as a drug for treatment on skin inflammation.

The Study on Depigmentation of Kamikwibi-Tang

Objective: This study was performed to investigate the depigmentation effects of the extracts of Kamikwibi-Tang. Methods Inhibition of tyrosinase activity, melanin production & cell viability in cultured B16 melanoma cells, UV screen and cytoprotective effects on PC12 cells injured by hydrogen peroxide were measured. Results: The extracts of Kamikwibi-Tang did not have any inhibitory effects on tyrosinase activity and did not show any inhibitory effects on melanin production in melanoma cells and also did not have any inhibitory effects on UV screen. But the extracts showed high cytoprotective effects on PC12 cells injured by hydrogen peroxide. Conclusion : These results suggest that Kamikwibi-Tang indrectly inhibits melanin biosynthesis which is involved in hyperpigmentation and could be used as a whitening agent for the skin.
The Effect of Various Partition Extracts of Radix Codonopsis Lanceolatae on the Melanogenesis

Melanin pigmentation has an essential role in protecting human skin against the damaging effects of ultra violet radiation, infection, environmental factor, etc. However abnormal melanin pigmentation can directly cause a number of congenital and acquired pigmentary skin disease. This study conducted to evaluate the effects of various partition extracts of Codonopsis lanceolata on the melanogenesis in the B 16 melanoma cells. The cells were treated for 3 days with various partition extracts of Codonopsis lanceolata. While treatment with the BuOH partition extract of Codonopsis lanceolata increased significantly the tyrosinase activity and melanin content, the n-Hexane or ethyl acetate partition extract dose-dependently suppressed them compared with untreated control. But ethyl acetate partition significantly decreased the cell viability. The H2O partiton extract at various concentrations did not exhibit any significant change on the melanogenesis. These results indicate that treatment with the n-Hexane partition extract inhibits the melanogenesis in B16 melanoma cells, and raise the possibility that this extract may be effective in the abnormal melanin pigmentation as well as the whitening agent for the skin.
Therapeutic Effects of Lizhongtang plus Baidusan Extract in Rats with Allergic Rhinitis

Background and Objectives: Recently the incidence of allergic rhinitis has increased but treatment in most cases has only dealt with the symptoms. Medicine has been developed that shows fewer side effects. However, some side effects and the psychological stress over taking medicine have remained. There have been no studies so far performed on the effect of Lizhongtang plus Baidusan Extract. This study aimed to find out the therapeutic effects of its exclusive use in rats with Allergic Rhinitis. Materials and Methods : Thirty Sprague-Dawley rats were divided into three groups: the control group, the cetirizine HCI group and the sample group. To induce allergic rhinitis in the control group, the cetirizine HCI group and the sample group, rats were sensitized intraperitoneally with 0.1
ovalumin solution 3 times at an interval of I week. Then intranasal sensitization was performed by diffusing 0.1
ovalumin solution 3 times at an interval of 2 days. After that time, rats of the cetirizine HCI group were orally administered with cetirizine HCI. Rats of the sample group were treated with Lizhongtang plus Baidusan Ex. for 28 days. We observed changes in nasal mucosa and submucosa. Also we found changes in the segment of neutrophil and lympocyte in Leukocyte. We used the statistical methods of ANOVA test(p 〈0.05). Results: The loss of the cilium and the secretion of mucus in the treated group was rare when compared to control group. Effects of Lizhongtang plus Baidusan Ex. on the liver function were also studied in rats. Treatment of Lizhongtang plus Baidusan Ex. did not affect AST and ALT. The segment of neutrophil was significantly increased in the treated group when compared with the control group and the cetirizine HCI group(p 〈0.05). The segment of lympocyte was significantly decreased in the treated group when compared with the control group and the cetirizine HCI group(p 〈0.05). Conclusion: This study shows that Lizhongtang plus Baidusan Ex. decreases the inflammatory response in rats with Allergic Rhinitis.
A Clinical Study of Chronic Urticaria

Objective: This study was designed to investigate clinical features of chronic urticaria and assess Korean traditional medical therapy on that. Methods: The ratio of male and female, duration of symptoms, personal history and family history of allergic diseases, serum IgE level, and suspected causes and provocation factors were investigated in 41 patients with chronic urticaria. And frequency of wheals, duration of wheals, number of wheals, itch severity, and antihistamine counts of 6-point(0-5) score was assessed in 26 patients. The change of total scores between baseline and follow up were analyzed using Wilcoxon signed-rank test(p〈0.05). Results: 1. The ratio of male and female was 1:1.28 and the mean duration of symptoms was 26.0 months. 2. The frequency of personal history and family history of allergic diseases was each 53.7

A Clinical Analysis of the Correlation between Acne Distribution in the Jaw and Emmeniopathy

Objective: We would like to find out the correlation between acne distribution in the jaw and the emmeniopathy. Methods: The clinical studies were made on 43 cases of acne vulgaris who visited to Department of Dermatology, Dongguk University Gangnam Oriental Medical Hospital during from August, 2002 to July, 2004. We divided the patients into two groups. A group had the emmeniopathy. B group didn't have the emmeniopathy. In the acne grade and the number of acne, we investigated the difference between A and B group statistically. We used the statical methods of student's t-test on the number of acne and wilcoxon rank sum test on acne grade. Results: The mean age was 25.8
4.80 in A group and 27.6
5.68 in B group. There were painful menstruation, quality of thickness, menstrual cycle disorder, menstrual amount disorder, dark purple color, premenstrual acne in A group with the emmeniopathy. The acne grade was 1.69

4.07 in B group. P-value was 0.52 in acne grade and 0.25 in the number of acne. We investigated no difference between A and B group statistically on acne grade and the number of acne. Conclusions: We needed the further study about the factor which affect on acne distribution and emmeniopathy.
A Case of Contact Dermatitis

A female sixty years old visited the Dept. of Oriental Medical Ophthalmology & Otolaryngology & Dermatology Hospital, Dong-eui University with contact dermatitis. she was suffering from the topical itching, empalement, ruler, edema and vesicle etc in skin of the face and two dorsum manus. We treated a patient with only Oriental Medicine.(internal and external use of the herbal medicine and acupuncture) After two weeks or so, the symptom of the skin disease recovered remarkably, we think that this is good case to treat with Oriental Medicine.
Two Cases of Temporomandibular Joint Disorder

Temporomandibular joint disorder is quite common dental problem. Research has shown that 5 to 15
of the population have a TMJ disorder requiring some type of treatment. TMJ disorder's symptoms are pains in the jaw and muscles of your face, limitation of opening or closing jaw and noises in jaw joint such as clicking or popping sounds. Two patients with TMJ symptoms visited our clinic. They complained pain, limitation of jaw opening and clicking sound. We treated them by stimulating trigger points on the TMJ muscle with acupuncture and had a good effects. The symptoms is reduced or disappeared.
A Case of Vertigo with Heterophoria and Nystagmus

Vertigo is a symptom, not a disease. The tenn vertigo refers to the sensation of spinning or whirling that occurs as a result of a disturbance in balance. It's sometimes referred to as a hallucination of motion. The cause of vertigo is very various and generally divided into peripheral and central. A patient presented with severe vertigo, nystagmus and heteophoria. She has taken the BanHaBaekChulChunMa-tang and treated by acupuncture. After 2 days, her symptoms become disappeared and about 1 week, all symptoms except heterophoria were disapperaed. And after 2 weeks, heterophoria was disappeared. In conclusion, we diagnosed her case as vestbular neuronitis by her clinical course, but we also considered her illness as vertebrobasilar insufficiency, TIA etc.
Clinical Study on one Patient with Vitreous Hemorrhage Caused by Diabetic Retinopathy

Objective: To carry out the oriental medical treatment on a patient with vitreous hemorrhage in the left eye caused by diabetic retinopathy and record the results of the treatment. Methods: 1. Diagnosis: Fundus photography, Colored paper, Dr. Hahn's standard test chart for 5M, Blood sugar measurement. 2. Treatment: Acupuncture, Electro-Acupuncture, Indirect moxibustion, Western medicines, Oryoungsan(Crude drug preparations) Results: Oriental treatment using Ohaeng-acupuncture, Electro-Acupuncture, Indirect moxibustion resulted in the Unaided visual acuity of 0.1 while it used to be the left eye visual acuity with only light sense I month ago. Looking from Fundus photography result, progress was achieved and diabetic retinopathy was found to be in progress in fluorescein fundus angiography to right eye also by revisiting the patient after treatment. Conclusions: 1. Vitrectomy has many advantages but there are instances where patients do not recover their visual acuity due to complications. Therefore it is necessary to prove the effect oriental medical treatment through more cases in future. 2 For diabetic retinopathy patients, diabetes must be treated together with visual acuity.
Two Cases of Herpes Zoster and Post-herpetic Neuralgia Improved by Oriental Medicine with Bee Venom Acupuncture

After administration of Oriental medicine with Bee venom acupuncture to two cases of Herpes zoster and Post-herpetic neuralgia. We can observe remarkable efficacy on relief from pain. This results suggest that Oriental medicine with Bee venom acupuncture can be one of a good remedy for Herpes zoster and Post-herpetic neuralgia.
A Case of Pompholyx Treated with Wet Dressing by Distilled Solution of Gamihwangryunhaedok-tang(Jiaweihuanglianjiedu-tang)

Pompholyx is a special vesicular type of hand and foot dermatitis. It is an acute, chronic or recurrent dermatosis of the fingers, palms and soles, characterized by a sudden onset of many deep-seated pruritic clear vesicles. We diagnosed a patient with pompholyx as damp heat and treated with wet dressing by gauze soaked in distilled solution of Gamihwangryunhaedok-tang(Jiaweihuanglianjiedu-tang). After treatment with wet dressing, we observed the improvement. Based on this study, it is considered that wet dressing with Gamihwangryunhaedok-tang(Jiaweihuanglianjiedu-tang) can be applied to the treatment of pompholyx.
Three Cases of Atopic Dermatitis

Background: Infantile atopic dermatitis involves the face, with erythematous weepy patches. This weeping eczema has severe itching and causes eczema secondarily. Also it has a profound effect on the quality of life. Many infantile AD patient and parent suffer troubles because of weeping eczema. Objective: The purpose of this study was to evaluate the efficacy of Gyejigahwanggitang(桂枝加黃？湯) in infantile AD patient with weeping eczema. Method: I treated them with Gyejigahwanggitang(桂枝加黃？湯) for balancing YoungWi(調和榮衛). I also used acupuncture therapy.
An Analogous Case of Systemic Lupus Erythematosus

Systemic lupus erythematosus is a disease of unknown etiology that affects many organ system and is characterized by the presence of multiple autoantibodies that participated in immunology mediated tissue injury. A 36 years-old female patient was admitted to ward due to high fever and erythematous rash on face and hole body. She exhibited itching sense, joint pain, nausea, fatigue, sensitivity to light. The homatologic finding revealed anemia, decrease of lymphocyte, low platelet count, but LE cell, Anti nuclear antibody(ANA) were negative. In the point of Differentiation of Syndrome(辨證), SLE can be thought to be a category of Seasonal febril disease(溫病). This subject diagnosed as Domination of intense evil heat(熱毒熾盛), and was administrated with Chungonpadocyem-gamibang(淸瘟敗毒飮加味方). The clinical and laboratory findings of our patient were improved by herb medication, acupuncture therapy and general supportive measure.
A Clinical Review of Peripheral Dizziness Patient-one case

Introduction: Vertigo is one of the common symptoms that we can see often clinically. It is hallucination to motion of oneself or surroundings. Vertigo include not only simple whirling sensation but also leaning or falling down sensation. Particularly in vestibular neuritis, the principal symptoms is dizziness and accompanied by nystagmus, gait imbalance, nausea, vomiting. Cause of vestibular neuritis is inflammatory disease such as common cold. Objective: The aim of this study was to estimate the efficacy of oriental-treatment on vestibular neuritis patient Subjects: We diagnosed one patient who had severe vertigo and gait imbalance as "dizziness retention of phlegm"(담훈) and treat orientally. Conclusion: After oriental-treatment for 15 days, walk balance was improved and no more vertigo was appeared. We could assume that in the vertigo and gait inbalance due to vestibular neuritis, the acupuncture and herbal medicine can be used.
A Case of Erythema Multiforme(EM)

Background and Objective: Erythema multiforme(EM) is related disorders of skin and mucous membranes which is typically associated with antecedent medications or infections. etc. EM is an erythematous maculopapular cutaneous eruption of variable form. However, the pathophysiology of the EM remains obscure. Treatment at present is symptomatic and supportive. There is no report on EM treated with oriental medical treatment. Methods: We conducted one patient with EM seen at Won-Kwang University Kwang-Ju Oriental Medical Hospital in 2004. We treated EM in a twenty-two years old man by a herbal medicine(Joa-Chang-Bang), a herbal medicine for external use, acupunture, moxa, COTDE-made cosmetics, and P-Tx(carbon). Conclusions. We had a significant results. So we report this case to estimate the efficiency of oriental medical treatment and management on EM.
A Case of Treatment of Blepharoptosis

Blepharoptosis is the state that upper eyelid sagges, palpebal fissure becomes narrow due to disorder of a levator palpebrae superioris or innervation. It affects not only the physical vision but also mental shrink. In the view of oriental medicine, there are various causes, among them mainly cause is the deficiency of spleen Gi. The other hand, we treated a patient who suffered from blepharoptosis(with headache, exotropia, dizziness) differentiate from stagnation of Gi(energy) and blood stasis with Sungihwalhyeoltang(順氣活血湯) mostly. After about 40days of Sungihwalhyeoltang(順氣活血湯) centered treatment, acupunture, we observed an improvement Based on this experience, it is considered that stagnation of Gi(energy) and blood stasis can bring about blepharoptosis.
A Case Report of Ptosis After Midbrain Infarction

Objective: The purpose of this case study is to show a case of severe neurogenic ptosis due to midbrain infarction improved by acupuncture and Herb-med. Method: This case study has been carried out for a case of ptosis due to midbrain infarction which had been hospitalized at the Bundang CHA Oriental Medical Hospital. We evaluate through Levator function test and measurement of MRD and the distance between upper and lower eyelid. Results: Levator function increase 3.5mm and distance between upper and lower eyelid increase 4mm more than before. The degree of ptosis turned 'fair' state from 'poor' state. Conclusions: We suggest to treat ptosis due to midbrain infarction with acupuncture and Herb-med meaning conservative therapy and would like to research oriental medical treatment plan for the further treatment.
A Review on the Follow-up Study of the Refractive Surgeries

Refractive surgeries for myopia are rapidly spreading in our daily lives. In this study Korean and foreign follow-up studies, mainly focused on long-term outcomes of LASIK and LASEK, were analyzed. Examining the evidence of the efficacy, accuracy, stability, complications and patient satisfaction will help refine our approach to refractive surgeries.
