ANTIMICROBIAL EFFECT OF ESSENTIAL OILS ON ORAL BACTERIA

Essential oils are mixture of volatile, lipophilic compounds originating from plants. Essential oils have potential biological effects, i.e., antibacterial, antifungal, spasmolytic and antiplasmodial activities and insect-repellent property. In this study, five essential oils, namely R, LG, FR, O, and NM, extracted from various aromatic plants were used to test their antimicrobial activity against the oral microorganisms. The effects of essential oils were investigated against eight important bacteria, Streptococcus mutans (S. mutans), Staphylococcus aureus (S. aureus), Streptococcus sanguis (S. sanguis), Streptococcus anginosus (S. anginosus), Actinobacillus actinomycetemcomitans (A. actinomycetemcomitans), Streptococcus sobrinus (S. sobrinus), Staphylococcus epidermidis (S. epidermidis), and Escherichia coli (E. coli). Essential oils, except NM, effectively inhibited the growth of tested oral pathogenic microorganisms dose-dependently. However, the essential oils didn't show a significant inhibitory effect against E. coli and S. epidermidis. Consequently, these results represented that essential oil-mediated anti-microbial activity was prominent against the oral pathogenic bacteria. For example, minimum bactericidal concentration(MBC) of R, LG, FR oil against A. actinomycetemcomitans was very low as 0.078 mg/mL. In addition, minimum inhibitory concentration (MIC) of R, LG, FR, O oil against S. mutans was low as 0.156 mg/mL in vitro.
THE ANTIBIOTIC ACTIVITY OF ACTINOMYCES ISOLATED FROM PLAQUE OF BLACK STAINED PRIMARY TEETH TO STREPTOCOCCUS MUTANS

The aim of this study is to assess the antibiotic activity of Actinomyces in plaque from black stained primary teeth to Streptococcus mutans. Samples were obtained from four children, 2-6 years of age, who had black stains on all erupted primary teeth. 16 different Actinomyces spp. were isolated, and antibiotic activity test with paper disc method was done. The results were as follows, 1. No.1 and No.5 Actinomyces spp. showed the antibiotic activity to Streptococcus mutans and the activity of No.5 Actinomyces spp. could compete with that of Oxacillin. 2. No.1 and No.5 Actinomyces spp. also exhibited the antibiotic activity to Bacillus cereus, Bacillus subtilis commonly used as experimental bacteria for testing antibiotic activity. 3. For identification of No.1 and No.5 spp., PCR analysis was done. No.5 spp. matched Actinomyces viscosus at 97% level but No.1 spp. didn't match.
A STUDY ON THE EFFECT OF POLYMERIZATION SHRINKAGE OF SEVERAL COMPOSITE RESIN USING STRAIN GAUGE

Cavities were prepared on the permanent molars with width 3 mm, height 3 mm and depth 1.5 mm and the filling materials were filled with 1 step, 2 step layering technique and 3 step oblique filling methods. HISTOLOGIC FEATURE AND INFILTRATION OF ADHESIVE RESIN ACCORDING TO PRETREATMENT ON PROXIMAL EARLY CARIES LESION

Early enamel caries is commonly remineralized by the patient‘s improved oral hygiene or fluoridation, however the result is clinically unreliable. As an alternative, we tried to seal the lesions with low-viscosity light-curing resin. The aim of the present study was to search the proper methods of the adequate pretreatment prior to applying adhesive resin on natural proximal caries lesions. Thirty nine extracted deciduous molar teeth showing proximal early caries lesion were used for this study. They were divided into 5 groups : Group 1; only carefully cleaned with water, group 2; etched with 15% HCl for 15s, group 3; etched with 35% phosphoric acid for 15s, group 4; etched with 35% phosphoric acid for 30s, and group 5; cleaned with 0.5% NaOCl. Following results were obtained by evaluating with SEM and CLSM after applied with adhesive resin. 1. As a result of SEM evaluation, group 2 showed clearly removed surface layer, group 3,4 showed partially removed surface layer irregularly, group 5 showed slightly removed surface layer. 2. Group 2 showed the deepest infiltration depth, followed by group 4, group 3, group 5, group 1 and besides group 5, other groups showed significantly deep infiltration depth. (p < 0.01) In conclusion, the best methods of the adequate pretreatment on natural proximal caries lesion for deep infiltration of adhesive resin was to etch with 15% HCl for 15s.
COMPARATIVE STUDY OF THE REMINERALIZATION EFFECTS USING CPP-ACP AND FLUORIDE ON THE ARTIFICIAL ENAMEL LESION

The aim of this study was to examine efficacy of the commercially available 10% CPP-ACP cream (Tooth mousse, GC Co., Japan) and/or 0.05% NaF solution on the remineralization of artifical caries-like lesion in the bovine teeth enamel. Sixty bovine teeth were embedded in orthodontic resin and flattened. The enamel surface in 3 mm diameter was exposed with nail varnish. Specimens were stored in demineralizing solution and divided 5 groups; Group 1 (No treatment), 2 (0.05% NaF solution 1 min), 3 (Tooth mousse 3 min), 4 (After 0.05% NaF solution, Tooth mousse treatment), and 5 (After Tooth mousse treatment, soaking in 0.05% NaF solution during 1 min). After treatment by groups, all specimens was stored in artificial saliva for 30 min. After the process described as above was performed during 10 days without pH cycling, surface hardness (Vickers Hardness Number, VHN) was tested and analyzed by paired t-test and one-way ANOVA test with SPSS 14.0. In intragroup comparison between surface hardness of pre and post-treatment, group 3, 4, 5 showed statistically significant increase (P < 0.05). In intergroup comparison among surface hardness increase of all groups, difference of group 5 between pre and post-treatment (

). Tooth mousse can be a good alternative agent for the fluoride, and the combination use with fluoride might have the additional anticariogenic effect.
THE DIFFERENCE OF THE SKELETAL MATURATION OF CERVICAL VERTEBRAE BETWEEN VERTICAL AND HORIZONTAL SKELETAL PATTERNS IN THE ADOLESCENTS

The purpose of this study was to compare the skeletal maturation of cervical vertebrae according to vertical skeletal patterns in the adolescents. Lateral cephalograms of 198 subjects(10 years old), 216 subjects(12 years old), and 138 subjects(14 years old) were reviewed, and subjects were classified according to vertical skeletal patterns. The 30 subjects were selected with the greatest predominance of vertical growth, and the other 30 subjects, with the greatest predominance of horizontal growth; these subjects comprised the vertical and horizontal groups, respectively. For evaluating of skeletal maturation, the concavity of inferior border and verticohorizontal ratio of cervical vertebrae were observed and measured according to age. The measurements were analyzed statistically by SPSS computerized program. The results were as follows : In general, the concavity of the inferior border of the cervical vertebrae was greater in the horizontal group than the vertical group in all of the 10, 12, 14 year olds, but was not statistically significant. The vertico-horizontal ratio of the 3rd and 4th vertebra of the horizontal groups was a significantly larger than the vertical groups in 12 year olds but did not show any statistical significance in 10 and 14 year olds. There were a weak negative correlations between SUM, FMA and the vertico-horizontal ratio for each groups at 12 ages.
A SURVEY ON THE USING STATUS AND PERCEPTION OF PIT AND FISSURE SEALANT

The property of pit and fissure sealant has been improved and many studies on the bond strength, penetration, microleakage have been published. But there are few studies on the using status and perception of pit and fissure sealant within the country. Therefore, this study made a survey on it. Pedodontists and non-pedodontists were surveyed by interview. The Results were as follows; 1. On caries prevention effect, 96.7% of the pedodontists replied that sealants were effective on both permanent teeth and primary teeth. On the other hand, 13.5% of the non-pedodontists replied that sealants weren't effective on both. 2. All of the pedodontists and 27% of the non-pedodontists used rubber dams. 83.3% of the pedodontists and 40.5% of the non-pedodontists used bonding agents. 3. Non-pedodontists used enameloplasty more frequently than Pedodontists but the pattern was not significantly different. 4. The causes of sealant failures included salivary contamination, caries under sealant, low strength, low flowability, overfilling. 5. In the pedodontists, 90% replied that PRR application was desirable and PRR applications were more frequent than sealant application.
FILLER LEACHING FROM NANOFILLER-CONTAINED COMPOSITE RESIN IN VARIOUS MEDIA

The objective of this study was to measure the leaching of filler (Si, Ba) from nanofiller-contained composites (Palfique Estelite
(Tokuyama Dental Corp., Tokyo, Japan),
(3M ESPE, USA), Ceram X duo
(Dentsply, Konstanz, Germany)) under different conditions. The samples used for the study of leachable components were made by insertion of the material into a circular mold, 10 mm in diameter and 3.0 mm high. Each specimen was placed in a disposable polystyrene vial containing 5 mL of distilled water, artificial saliva or 0.1N NaOH and kept in an oven at
. ; water and artificial saliva - 150 days, 0.1N NaOH - 15days. Inductively coupled plasma atomic emission spectroscopy (ICP-AES) was used to determine the amount of Si and Ba in the test solutions. 1. Filler leaching was significantly great in 0.1N NaOH among all samples(p<.0.001). 2. When samples were stored in the distilled water, Estelite showed the lowest amount of Si leaching. When samples were stored in the artificial saliva, Z-350 showed the lowest amount of Si leaching. 3. There were significant differences in filler leaching between 3 storage medias and composite resins(p<.0.001). 4. Si and Ba leaching occurred in greater proportion when samples were stored in the artificial saliva than distilled water. 5. There were significant interactions in monthly filler leaching between leaching in artificial saliva and in distilled water, as well as the interaction between storage medium and filler(p<.0001). These results indicate that a continuous filler leaching of nanofiller-contained composite resins was in storing aqueous solutions under over time.
THE STUDY OF CHANGES IN ORAL HEALTH CARE OF PRESCHOOLERS IN TAEBAEK CITY THROUGH ORAL HYGIENE EDUCATION

The objective of this study is to find the improvement in the dental health care of the children through oral hygiene education and TBI. In Taebaek public health care center, an oral hygiene education, TBI and fluoride gel application was conducted 2 times for about 631 preschool children living in Taebaek City. The ages of preschool children were from 4 to 7. To examine the changes, a questionnaire was written and distributed for the collecting the results after the education of the second half for children, parents and teachers. As a result, the children showed better understanding of the caries through oral hygiene education and reduction of fright and fears for dental treatment. Many children learned how to brush and there was a significant increase in the number of brushing. Children learned to be able to classify good food and bad food for teeth and showed the change in eating habits although in small part through the oral hygiene education. Through this study, it is judged that the oral hygiene education of the preschool children has effectiveness and it is in need for the expansion of oral hygiene education in homes, kindergartens and dental offices.
ESTHETIC RESTORATION WITH FIBER-REINFORCED POST FOR CHILDHOOD AND ADOLESCENT PATIENTS

In case of endodontic treatment and extensive restoration of severe dental caries, trauma, and developmental defect, esthetic restoration of primary incisors and permanent anterior teeth for children and adolescents period is delicate matter for pediatric dentists. Existing restorative methods for anterior teeth have retentive and esthetic limitations for badly damaged teeth, especially for the adolescent anterior teeth. Therefore, the preparative stage for setting the permanent prosthesis as well as the retention and esthetics have to be considered. ORTHODONTIC TRACTION OF THE LOWER DECIDUOUS SECOND MOLAR IMPACTED BY AN ODONTOMA

Tooth impaction refers to situations where a tooth fails to erupt due to mechanical blocking and remains unerupted beyond the normal time of eruption. It is commonly found in permanent dentition, but rarely in deciduous dentition. Tooth impaction is caused by systemic or local factors, and most common etiology is odontoma. Odontoma is also commonly found in permanent dentition, and rarely in deciduous dentition. We are presenting case of 4 year and 4 month year old girl, who had impacted and disto-angulated lower left deciduous second molar due to odontoma. We removed odontoma and performed orthodontic traction of impacted lower left deciduous second molar. Then, the impacted primary molar was erupted and acceptable occlusion was obtained.
MOLAR RESTORATION WITH AN ORTHODONTIC BAND

The use of stainless steel crowns are indicated for restoration of primary or permanent molars with proximal dental caries, extensive dental caries, or previous pulp treatment with increased danger of tooth fracture. Stainless steel crowns were introduced by Humphrey in 1950. For their improved durability, longevity, and success rate, they have been strongly considered for restoring extensive and multi-surfaced dental caries of molars in pediatric dentistry. However, they also have shortcomings, such as possibility of pulpal exposure or damaging proximal surface of adjacent teeth. In addition, when oversized stainless steel crowns are used, eruption of the adjacent permanent teeth may be disturbed by their prominent margin. As a means to compensate the shortcomings of stainless steel crowns, use of orthodontics bands may be considered. It is an alternative restoration method, where an orthodontic band is placed on a tooth first and cavity is restored with filling material, such as composite resin, glass ionomer, or amalgam. The use of an orthodontic band is indicated for molar restoration with cervical dental caries, extensive dental caries, enamel hypoplasia, or previous pulp treatment. Because it requires shorter chair time compared to stainless steel crown, its application is very useful for children with poor behavior. However, restoration using an orthodontic band requires good oral hygiene after its application. This case report illustrates the conservative restoration of primary molars and permanent molars with extensive dental caries using orthodontic bands.
REGIONAL ODONTODYSPLASIA : CASE REPORT

Regional odontodysplasia(ROD) is relatively rare localized developmental anomaly of tooth formation in which hard tissue is affected. The maxilla is typically affected than the mandible, and especially the maxillary left quadrant is the most commonly involved. Females are affected twice as often as males, and there is no association with race. Its etiology remains undetermined, but local circulatory disorders, somatic mutations, virus infections, local trauma, hyperpyrexia, irradiation, metabolic disturbances, and hereditary transmission are considered as possible etiologic factors. The affected teeth are likely to be small, hypoplastic, brown, and grooved. Eruption failure or delay is frequently seen as well as abscess or fistulae formation in absence of caries. Radiographically, there is a lack of contrast between the enamel and dentin, both of which are less radiopaque than unaffected counterparts. Moreover, enamel and dentin layers are thin, giving the teeth a “ghost-like appearance”. The pulp chambers and canals are large, the roots seem like to be short and indistinct. A 2-year-3-month old boy came to the department of pediatric dentistry, Yonsei University, with the chief complaint of delayed eruption and abnormal tooth shape on the lower left quadrant. He was diagnosed as regional odontodysplasia based on the clinical and radiographic findings.
THE PREVALENCE OF WHITE SPOT LESIONS ON THE MESIAL SURFACES OF THE 1ST MOLARS IN CHILDREN AND MINIMAL INVASIVE APPROACH - A PILOT STUDY

In clinical pediatric dentistry, we have many chances to encounter the white spot like incipient enamel lesions on the mesial surfaces of the 1st molars with direct vision, especially just after the 2nd primary molars were exfoliated. But it was thought highly desirable to assess if these lesions are properly and effectively managed yet. This study aims at surveying the prevalence of incipient lesions on the mesial surfaces of the 1st molars in children through direct observation and examining the suitability of adhesive sealing on them as a pilot trial in searching for their proper management. 1. Among the 124 mesial surfaces of the 1st molars examined, 34% were sound, 53% had incipient carious lesions and 13% had cavitated lesions. 2. In the sectional views of the specimens, 20% showed microleakage after thermo-cycling and it was thought not recommendable as a permanent method. Therefore in order to effectively fight against the incipient caries lesions in children‘s permanent teeth, it was thought proper not to rely on any one method, but to perform reinforcing oral hygiene and promotion of remineralization in combination with therapeutic sealing which is stronger in short-term sealing effect. Although therapeutic sealing has been considered as the core in minimally invasive concept to treat the white spot lesions, its long-term clinical trials have not been suggested. Continuous research is strongly required for making this approach to acquire permanent nature, especially in regards of proper pretreatment and high molecular materials deeply penetrable into enamel.
TREATMENT OF CLASS II MALOCCLUSION WITH FR-II : A CASE REPORT

The functional regulator(FR) appliances act to remove the restrictive forces that prevent the normal maturation of the maxilla and mandible. FR appliances are different from other functional jaw orthopedic appliances(e.g., the twin block, bionator, and activator).
has based his treatment philosophy on the concept that the capacity to regulate growth residues in the soft tissue environment, and that adequate space must be available for the proper development of the hard tissue. In class II malocclusion with mandibular retrusion, FR-II treatment is not only the change in the postural position of mandible, but also expansion of the dental arches. By balancing the neuromuscular environment, not only can severe malocclusions be treated successfully, but also the tendency toward relapse is minimized because the neural and soft tissue factors associated with the skeletal malocclusion have been addressed as well. We report cases using by FR-II that is applicated in cases of Class II malocclusion without fixed appliance, only FR-II and space supervision.
SIALOLITHIASIS ON THE MOUTH FLOOR IN A CHILD

Sialolithiasis is the formation of calcific concretions within the ductal system of major or minor salivary glands. The submandibular gland is most involved because of its high viscosity of the saliva and the long, curved duct. It may occur at any age but, it is most common in middle-aged adults and rare in childhood. Clinical symptoms in sialolithiasis are variable but, swelling is the most common, followed by the pain. Clinical examination and radiographic examination(panoramic and mandibular occlusal radiographs, sialography, intraoral-, extraoral- ultrasound, CT scan, MRI and sialoendoscopy) can help to confirm a diagnosis and localize a stone. The treatment is surgical intervention, either removal of the sialolith or sialoadenectomy. However, non-invasive techniques including shock-wave lithotripsy,
laser and endoscopic treatment used in selected cases. A 5-Year-old girl referred from private practice for evaluation of a yellowish mass on the floor of the mouth. She complained that it had became three times bigger than four months ago when it was found for the first time and she had some pain on submandibular gland area occasionally. On physical examination, a firm and yellowish mass could be seen at the orifice of the submandibular duct. Diagnosis is the submandibular sialolithiasis in the anterior Wharton`s duct. Under local anesthesia, stone was removed.
DOUBLE TEETH IN MAXILLARY PERMANENT INCISORS : CASE REPORTS

Fusion and gemination are irregularities in tooth development. It is often difficult to differentiate between gemination and fusion and it is common to refer to these anomalies as 'double teeth'. The deciduous teeth are most commonly involved, but in 0.1% of cases permanent teeth are affected, in which case aesthetic, funtional and periodontal problems can result. Double teeth present great difficulties in management and required a multidisciplinary approach. The central groove on the labial and palatal surfaces of a double tooth is extremely prone to caries, therefore early 'fissure sealing' is essential. In permanent dentition, surgical separation of fused teeth may be possible with subsequent orthodontic alignment and restorative treatment as needed to reshape the crown. Reshaping or reduction of a double tooth with a single canal may be attempted by modifying the appearance of the labial groove and the use of composite tints but is often impossible and extraction may be the only alternative. Orthodontic treatment and prosthetic replacement is then required. Implants may be an option for adolescents. The present study describes three clinical cases of double teeth in the position of the maxillary permanent incisors. The first case demonstrates an example of multidisciplinary care including surgical intraoral hemisection, root canal therapy, restorative and orthodontic treatment. The second and third cases describe the external and internal morphology of the two fused teeth by means of three dimensional dental computer tomography.
ESTHETIC RESTORATION OF FRACTURED IMMATURE PERMANENT INCISORS

Injuries of permanent teeth by trauma occur frequently in childhood and adolescence. Crown fractures are as frequent as 25-76% and especially maxillary incisors are prone to fractures. There have been numerous efforts to achieve both aesthetically and functionally satisfying restoration. When a mature tooth is fractured, porcelain crown or laminate veneer could be a choice of prosthodontic treatment. However, in a case of immature permanent incisor fracture, prosthodontic treatment is more complicated due to the immaturity of the tooth. Moreover, if endodontic treatment is accompanied with the prosthodontic treatment, the treatment period is prolonged. In the past, restoration using an orthodontic band, a ready-made crown, or glass ionomer cement did not exhibit esthetically satisfying result. As restorational materials have been improved, now more esthetic restoration is possible by reattaching fractured fragments or light-curing composite resin restoration. We reports cases of patients with fractured maxillary incisors and their successful treatment results through reattachment of fractured fragments and composite resin restoration.
CROUZON SYNDROME : CASE REPORT

Crouzon syndrome is a rare disease, first decribed by Crouzon in 1912. This syndrome is cuased by mutations in the FGFR2 gene, which is mapped to chromosome locus 10q25-10q26. The condition occurs in about 1 of every 25,000 birth and is inherited as an autosomal dominant trait. We experienced a case of Crouzon's disease in a 9-year-old-female child. Physical examination revealed craniosynostosis, hypertelorism, exophthalmos, hypoplastic maxilla and a relative mandibular prognathism. The purpose of this study is to report the dental and medical characteristics of the patient and review the literatures of Crouzon syndrome.
ORAL AND MAXILLOFACIAL MANIFESTATIONS OF LARON SYNDROME

Laron syndrome was first described by Dr. Laron. Administration of exogenous growth hormone failed to stimulate insulin-like growth factor-I(IGF-I) production which was related to postnatal growth, because these patients lacked receptors in the liver for this hormone. The diagnosis of this syndrome is based on the typical features of GH resistance such as normal or elevated serum GH, low serum IGF-I, and impaired IGF-I response to hGH. Laron syndrome patients showed characteristically severe postnatal growth failure and markedly reduced adult height. This report describes the oral and maxillofacial manifestations of children associated with Laron syndrome. Children with Laron syndrome have several dental and skeletal irregularities. Relatively little is known of the direct effect of Laron syndrome on dental development. Further research should be needed.
STURGE-WEBER SYNDROME : A CASE REPORT

Sturge-Weber Syndrome is a rare congenital disorder and is characterized by port wine nevus following one or more divisions of trigeminal nerve, ocular involvement(eg, glaucoma) and neurologic involvement(eg, epilepsy, mental retardation). Oral menifestations include unilateral blood vessel expansion of the oral mucosa and gingiva, gingival hyperplasia, giant tooth, ipsilateral large tongue, blood vessel anomaly of maxilla or mandible and abnormal eruption sequence. This case report is about 8-year-old Sturge-Weber Syndrome patient presented violet discoloration on upper gingiva and buccal mucosa, gingival hyperplasia and abnormal eruption sequence. In this case, we performed lingual frenectomy and periodic oral hygiene management, and obtained satisfactory result.
REMOVABLE FLEXIBLE DENTURE FOR CHILDREN WITH OLIGODONTIA : A CASE REPORT

Objectives : The conventional removable appliance, composed of wires and acrylic resin, had unaesthetic results and poor retention. The flexible denture, as an alternative, presents improved aesthetics with the thin and strong resin retentive area. In addition, it also enhances patients' sensory function as a result of decreased volume of denture base. The flexibility of the flexible denture reduces the possibility of fracture and distributes the masticatory forces transmitted to the abutments and residual bone tissue. This report describes a 10-year-old girl and a 6-year-old boy with oligodontia treated with the flexible dentures as an alternative to conventional removable appliances. Methods : Impression was taken using alginate material and sent to a laboratory with the bite for fabrication of the flexible denture. Prior to try-in, the flexible denture was immersed in water at
for one minute and cooled. Impinging area of the denture was checked by
and removed and the denture was delivered to the patient. Results : Both patients were satisfied with the flexible dentures, which presented improved retention and aesthetics. Conclusion : For patients with oligodontia, flexible dentures can be considered as a treatment of choice, which may replace the conventional denture.
MANAGEMENT OF ORAL COMPLICATIONS IN THE PEDIATRIC POPULATION WITH CANCER

The incidence of childhood cancer is greatest in the first year of life. Early diagnosis and advances in medicine have significantly improved outcomes of treatment resulting in higher survival rate; however, this progress comes at the expense of a higher incidence of adverse side effects because of more aggressive antineoplastic treatment strategies. The oral cavity, a trauma-prone environment, is extremely sensitive to toxicities from antineoplastic agents. Oral health care specialists, including pediatric and hospital dentists can support the oncology team by providing basic oral care, implementing oral care protocols, delivering emergency dental treatment, and assisting and/or managing oral complications from cancer therapy. This article covers the considerations in the dental management of pediatric patients undergoing cancer treatment, specifically chemotherapy, radiotherapy, and hematopoietic stem cell transplantation.
