Pathogenesis of allergic conjunctivitis and trends in its treatment (알레르기결막염의 병리와 치료)

  • Allergic conjunctivitis is characterized by specific immunologic responses known as type 1 hypersensitivity, resulting in corneal and conjunctival inflammation. Histamine plays an important role in the pathophysiologic mechanism of allergic conjunctivitis. Five subtypes of allergic conjunctivitis have been defined according to specific signs and symptoms: seasonal allergic conjunctivitis, perennial allergic conjunctivitis, vernal keratoconjunctivitis, atopic keratoconjunctivitis and giant papillary conjunctivitis. Above all, avoidance of the offending antigen is the primary behavioral modification used to treat all types of allergic conjunctivitis. However, this may be difficult for practical reasons; therefore, a range of medical treatment options, such as topical antihistamines, mast cell stabilizers, non-steroidal anti-inflammatory drugs, and corticosteroids are prescribed in clinical practice.

The Fluctuation Patterns of Conjunctivitis Cases Caused by Asian Dust Storm (ADS) : Focused on the ADS Density and the Accuracy of ADS Forecast (황사예보 및 황사농도에 따른 결막염 질환의 발생 패턴 분석)

  • This study has an aim to analyze the effects of ADS on conjunctivitis patients among the residents of Seoul, Korea, between 2005 and 2008. For this purpose, the number of medical services provided to conjunctivitis patients on the days of windblown dust storms and the days without any windblown dust storms were analyzed by conducting paired t-test. The interactive effects of the ADS density and the accuracy of ADS forecast on the fluctuation of conjunctivitis cases were also investigated. The results showed that, even with an accurate forecast issued 24 hours prior to the event, the average number of medical services provided for conjunctivitis was higher on the index days than the comparison days. On the other hand, in cases of failure to provide an accurate forecast 24 hours prior to the ADS event, the number of conjunctivitis attacks reported was statistically significantly higher on the index days for 3~5 days after the occurrence of a dust storm in relation to the comparison days. We also found that the rate of increase in asthma treatments on the index days with low level of $PM_{10}$ concentration rather than high $PM_{10}$ level was more significant for all lag days. This study provides evidence that ADS events are significantly associated with conjunctivitis symptoms and the failure to forecast ADS events with low $PM_{10}$ level might aggravate conjunctivitis disease.

Incidence of the chlamydia conjunctivitis in the newborn period (신생아에서의 Chlamydia결막염의 빈도에 관한 관찰)

  • After application of silver nitrate on every newborn, the incidence of gonococcal conjunctivitis was markedly decreased. But recently neonatal conjunctivitis due to chlamydia infection is increasing, so clinical observation was made on 26 newborn infants who showed eye discharge from June 1st to August 31st 1989. The results were as follows. 1. The incidence of chlamydia infection among neonatal Conjunctivitis was 34.6%. 2. The most common age at diagnosis was 6-15days of life and there was no sexual prepondrance. 3. There was no significant difference on clinical symptoms between chlamydia conjunctivitis and other Conjunctivitis. 4. Of 26 infants examined. 16 cases revealed no growth on routine bacterial culture. Of the organism cultured, P. aeruginosa was the most common agent(19.2%) and followed by S. aureus(11.5%) and S. epidermidis(7.6%). In one case of chlamydia I conjunctivitis, there was concurrent S. aureus infections. 5. On this study, Giemsa stain did not give significant diagnostic aid of chlamydia conjunctivitis.

  • Allergic conjunctivitis is frequent 90% of allergic reaction of eyes. It is difficult to know the cause, so it is impossible to keep away from the antigens. The purpose of the treatment is just subsiding the symptoms without side effects. We experienced one case of Allergic conjunctivitis treated with oriental medical treatment, acupuncture and herbal medication. 27 year-old male patient had diagnosed Allergic conjunctivitis by western medical doctor. He suffered from eye itching, so he couldn't sleep well. After treatment his itching was subsided and he could sleep well. Therefore, oriental medical treatments could be safe and effective methods.

  • Our study investigated the prevalence of allergic conjunctivitis and skin prick test reactivity and type of allergen from 1352 individuals living in Gwangyang city. The prevalence of allergic conjunctivitis symptom were 40,2% of study subjects and diagnosised subjects with allergic conjunctivitis of them were 15.8%. In skin prick test, 20.2% of total subjects had at least one positive skin prick reaction and positive skin prick reaction with allergic conjunctivitis of subjects was 21.3%(n=103). Their major allergens were indoor allergen like dermatophagoides pteronyssinus, Dermatophagoides farinae.

Effect of Ophthalmic Fluoroquinolones on Bacterial Conjunctivitis: Systematic Review (세균성결막염에 대한 안과용 플루오로퀴놀론계 항균제의 효과: 체계적문헌고찰)

  • This systematic review was conducted to assess the clinical effect of ocular fluoroquinolones used for the treatment of bacterial conjunctivitis. A literature search for randomized controlled clinical trials registered up to January 2010 based on PubMed database, using the following search terms: conjunctivitis and fluoroquinolones (besifloxacin, moxifloxacin, gatifloxacin, levofloxacin, lomefloxacin, ciprofloxacin and ofloxacin) were performed. Pooled data on the clinical resolution and bacterial eradication rates derived from selected 16 studies were reported as the relative risk (RR) and 95% confidence interval (95% CI) compared with placebo. Early clinical resolution and microbiological eradication rates in placebo were 28% and 62% respectively. Fluoroquinolones were significantly effective comparing to placebo: early RR 1.94 (95% CI 1.60~2.34) and late RR 1.30 (1.19~1.43) in clinical resolution rates, and early RR 1.75 (1.58~1.94) and late RR 1.28 (1.18~1.39) in microbiological eradication rates. Besifloxacin, ciprofloaxain and moxifloxacin in clinical resolution, and besifloxacin and levofloxacin in microbiological eradication showed higher RRs than pooled overall fluoroquinolones' RRs. New quinolones had higher antibacterial potencies for all pathogens isolated from bacterial conjunctivitis and resistant isolates than old generation quinolones. In conclusion, ocular 7 fluoroquinolones were all effective than placebo for bacterial conjunctivitis and there were differences between quinolones in early and late clinical resolutions and microbiological eradications, and no differences in safety comparing to placebo.

Inhibitory effects of Tinospora cordifolia and Rubia cordifolia Linn. on egg albumin-induced experimental allergic conjunctivitis in rats

  • Allergic conjunctivitis is the most common allergic disease. These diseases are severe & frequent which requires search of new treatments. The aim of the study was to investigate the effects of Tinospora cordifolia (TC), Rubia cordifolia Linn. (RC) on experimentally induced allergic conjunctivitis in rats. In this study, dried water soluble extracts of TC and RC. (250 and 500 mg/kg, p.o. for 7 days) were evaluated for their antiallergic activity in Wistar rats. They were tested for inhibition of egg albumin-induced vascular permeability, inhibition of histamine release from the rat conjunctiva as well as in histamine content in tears. TC and RC showed significant (P < 0.05) inhibition in vascular permeability, inhibition in histamine release from the rat conjunctiva which is reflected by reduced level of histamine content in tears. The activities were found to be comparable to azelastine hydrochloride. These results suggest that the inhibitory effect on egg albumin-induced experimental allergic conjunctivitis in rat may be due to the antihistaminic activity of TC and RC. Our studies provide evidence that TC and RC may be beneficial in the treatment of allergic conjunctivitis.

A Case Report on the Idiopathic Conjunctivitis Treated by CHUNA Manual Therapy (경추 추나를 이용하여 치료한 특발성 결막염 치험 1례)

  • Objective : There is no case report on the Idiopathic Conjunctive treated by CHUNA manual therapy in oriental medicine. We experienced a patient with the Idiopathic Conhunctivitis improved by CHUNA manual therapy in X-ray imagine and visual analogue scale(VAS), and we report It. Methods : During the patient with the Idiopathic Conjunctivitis was treated CHUNA manual therapy, we examined cervial X-ray and evaluated the patient's symptoms by VAS and counting eye drop. And we observed whether the change exist or not in X-ray. Result : Symptoms(Red eye, eye xerosis) was improved significally, and the X -ray image was improved. Conclusion : Conjunctivitis was generally treated steroid eye drop and conservative treatment. But CHUNA manual therapy may be more effective in the Idiopathic Conjunctivitis, if more clinical studies and researchs are accomplished.

A study on factors which affect Dry Eye Syndrome and Eye Health in Filipino and Korean male students (필리핀과 한국 남학생들의 건성안 증후군 및 안건강에 영향을 미치는 요인에 관한 연구)

  • Purpose: The purpose of this study is that research eye health in Filipino and Korean male students for comparative analysis of prevalence and risk factors. Method: The subjects of this study were 196 male students, aged 17, from Korea and the Philippines and it was investigated whether they have dry eye syndrome, subjective symptoms, conjunctivitis, and pterygium or not. The McMonnies questionnaire was used for measuring dry eye syndrome. Subjective symptoms, video display time and sun exposure time were added to the questionnaire items; while conjunctivitis and pterygium were confirmed by visual examination. Results: The average sunlight exposure time per day excluding school hours were 2 hours and 12 minutes for the Filipino students and 1 hour and 1 minute for the Korean students. Sunlight exposure time had a significant effect on the Filipino students with dry eye syndrome, subjective symptoms, conjunctivitis, and pterygium, but multimedia viewing time did not. In contrast, the sunlight exposure time and video display time didn't effect to Korean students, excluding the scores on the McMonnies questionnaire. The Filipino students had a statistical difference in the occurring frequency of subjective symptoms, conjunctivitis, and pterygium in dry eye syndrome. Also they had the higher the scores on the McMonnies questionnaire and high prevalence of conjunctivitis and pterygium. Conclusion: It was identified that the Filipino students had their sunlight exposure time affecting their eye health, whereas the Korean students didn't have. And it related to occurring frequency of conjunctivitis and pterygium in case of dry eye syndrome patients. Another analysis needs to be made of other risk factors besides sunlight exposure time analyzed by this study, which may affect the eye health of the Filipino students.

Acupuncture Treatment in a Case with Equine Conjunctivitis (말 결막염에 대한 침 치료 1례)

  • One male Arabian horse, 5-year-old, reared in Gort, Co Galway, Ireland was diagnosed as conjunctivitis. Acupuncture and aquapuncture with dexamethasone were done at BL01, BL02 and ST01, twice a week, and oculo-acupuncture was done into Shangjiao area using with oculo-acupuncture needle for human use. At session 3, there were no lacrimation, ocular discharge and hyperemia in the eye. The present patient was a case with equine conjunctivitis which showed favorable therapeutic response by acupuncture treatment.
