A Study on the Serum Concentrations of Glucose, Insulin and Lipid in Elementary School Children by Body Weight

체중에 따른 일부 초등학교 아동의 혈청 중 포도당, 인슐린 및 지질농도에 관한 연구

Abstract

The purpose of this study was to investigate anthropometric values, serum biochemical levels and their correlation in elementary school children. The subjects were 98 boys and girls in the $4{\~}6$ grade and divided into three groups by body size. The mean height of under-weight group (boys: $14.9\pm7.8\;cm$, girls: $144.9\pm9.6\;cm$) was the highest of the groups. The systolic and diastolic blood pressure of all the groups ranged from $111.56\pm13.51\;mmHg/72.86\pm7.26\;mmHg\;to\;117.50\pm10.00\;mmHg/75.31\pm9.51\;mmHg$, all within the normal levels. As subject was fatter, serum concentrations of triglyceride, total cholesterol, LDL-cholesterol insulin and atherogenic index (AI) in the boys, and serum concentrations of triglyceride, insulin and AI in the girls were significantly higher The concentrations of serum glucose, insulin, triglyceride, total cholesterol and LDL- cholesterol were significantly positively correlated with BMI and WHR. No significant correlation was found with blood pressure and serum lipid concentrations. Serum insulin levels was significantly positively correlated with serum glucose and triglyceride concentrations. Thus, this study implicates that obesity in childhood may be relevant to hyperlipidemia, atherosclerosis and diabetes mellitus.

본 연구는 학동기 아동의 정상적인 성장과 건강 증진을 위한 구체적인 영양교육의 기초자료를 마련하고자 경남 사천시에 위치한 D초등학교의 특별한 질환을 갖고 있지 않는 $4{\~}6$학년 남녀 학생 98명을 대상으로 저체중군, 정상체중군, 과체중군으로 분류하여 신체계측, 혈청 중 지질, 포도당 및 인슐린 농도를 측정하고 그들의 상관관계를 살펴본 바, 다음과 같은 결론을 얻었다. 신장은 남녀 아동 모두 저체중군에서 가장 높게 나타났으며, BMI 및 WHR은 남자아동 과체중군이 $25.6{\pm}2.2 kg/m^2,\;0.90{\pm}0.05$로 가장 높았고 여자아동 저체중군이 $15.1{\pm}1.0\;kg/m^2,\;0.76{\pm}0.04$로 가장 낮았다. 남녀아동의 수축기 및 이완기 혈압은 평균 113.76mmHg, 74.34 mmHg였고 성별 및 체형별 유의적인 차이를 나타내지 않았다. 남녀 아동 모두 체중이 높은군일수록 중성지질, 총콜레스테롤 및 LDL-콜레스테롤 농도는 높게 나타난 반면 HDL-콜레스테롤 농도는 낮게 나타났는데 중성지질, 총콜레스테롤 및 LDL-콜레스테롤 농도는 남자아동인 경우 체형군별b유의적인 차이가 매우 컸으며 (p<0.001) 남자아동 과체중군이 가장 높았고, HDL-콜레스테롤 농도는 여자아동 정상체중군이 가장 높게 나타났다. 혈당 농도는 여자아동 과체중군이 $93.06\pm10.55\;mg/dL$로 가장 높았고 여자아동 저체중군이 $82.49\pm12.51\;mg/dL$로 가장 낮았다. 인슐린 농도는 성별 및 체형별 유의적인 차이가 매우 컸으며 (p<0.001), 남녀 아동 모두 과체중군이 각각 $12.06{\pm}3.17\;{\mu}U,\;16.19\pm4.80\;{\mu}U$로 가장 높았고 남자아동은 정상체중군이 $7.61{\pm}3.36\;{\mu}U$, 여자아동은 저 체중군이 $9.42{\pm}3.63\;{\mu}U$로 가장 낮았다. 남녀 아동 모두 BMI 및 WHR이 높을수록 혈당, 인슐린, 중성지질, 총콜레스테롤 및 LDL-콜레스테롤 농도는 높게 나타났으며 , BMI는 남자아동인 경우 인슐린, 중성지질, LDL-콜레스테롤 농도와, 여자아동인 경우 인슐린 농도와 매우 높은 유의적인 정상관을 보였고(p<0.001), WHR은 여자아동인 경우 인슐린 및 중성 지질농도와 유의적인 정상관을 나타내었다(p<0.01). 남녀 아동 모두 인슐린 농도가 증가할수록 혈당과 중성지질 농도는 유의적으로 높게 나타났고, 수축기 및 이완기 혈압과 혈청 지질농도와는 유의적인 상관관계를 나타내지 않았다. 이상의 결과로 미루어 볼 때 아동기 라 해도 과체중은 고지 혈증, 동맥경화 및 당뇨병을 초래할 우려가 사려되는바 평상시 올바른 식습관과 운동으로 정상체중을 유지하는 것이 무엇보다도 중요하며 과체중 아동인 경우에는 체중감소를 위한 적극적인 영양교육이 요구된다고 하겠다.
