거울치료프로그램이 뇌졸중 환자의 상지기능과 균형, 보행 및 일상생활활동에 미치는 효과에 관한 국내 연구 메타분석

Effects of Mirror Therapy Program on Upper Body Function, Balance, Gait and Activities of Daily Living in Stroke Patients : Meta-analysis of studies in Korea

본 연구에서는 뇌졸중 환자를 대상으로 한 거울치료에 대한 지식을 축적하는데 의미 있는 정보를 제공하고자 한다. 또한 거울치료 연구들의 현 상태를 알아보고 개발 및 적용을 위한 근거자료로 거울치료의 효과크기를 알아보고자 한다. 본 연구는 뇌졸중 환자들을 대상으로 거울치료프로그램을 중재로 한 논문을 수집하였다. 체계적으로 메타분석을 실시하기 위해서 PICO 기준에 의거하여 검색한 후 최종 24편이 분석 대상으로 선정되었다. 개별 연구들의 효과크기를 산출하기 위해 메타분석 CMA 프로그램을 이용하여 시행하였다. 거울치료프로그램에 대한 메타분석을 실시한 결과, 전체 효과크기는 .500으로 나타나 뇌졸중 환자에게 적용된 거울치료프로그램이 중간크기의 효과를 나타내는 것으로 확인되었다(p<.05). 또한 거울치료가 뇌졸중 환자에게 미치는 종속변인에 대하여 운동회복(.472), 상지기능(.631), 일상생활활동(.533), 균형(.245), 보행(.274)으로 나누어 효과크기를 알아본 결과, 상지기능의 효과크기가 가장 크게 나타났다. 거울치료프로그램은 뇌졸중 환자의 상지기능과 일상생활활동 능력을 증가시키고 균형 및 보행능력을 향상시킨다. 향후 뇌졸중 환자를 대상으로 거울치료에 대한 장기적인 효과에 대한 연구가 지속적으로 이루어져야 할 것이다.

This study aimed to provide meaningful information for the accumulation of knowledge on mirror therapy in patients with stroke. It also aimed to evaluate the current status of research on mirror therapy, and to the effect size of mirror therapy as evidence for its development and application. This study collected articles in which the mirror therapy program was investigated as an intervention for patients with stroke. For systematic meta-analysis, 24 articles were finally selected after a literature search based on the PICO (population, intervention, comparisons, outcomes) criteria. To extract the effect size of each study, the Comprehensive Meta-Analysis program was used. The meta-analysis showed that mirror therapy for patients with stroke had a moderate effect size, with a total effect size of .500 (p<.05). Furthermore, analysis of the effect sizes of dependent variables showed that upper limb function (.631) had the greatest effect size compared with motor recovery (.472), activities of daily living (ADL) (.533), balance (.245) and walking (.274) in patients with stroke who underwent mirror therapy. Mirror therapy not only increases the upper limb function and ADL of patients with stroke but also improves their balance and walking. Studies on the long-term effect of mirror therapy in patients with stroke are necessary.
