Curriculum Development for Patient Support Teams in Long-term Care Hospitals

Abstract

Purpose: This study aimed to develop an educational program designed to strengthen the competencies of patient support teams that facilitate hospital-to-community transitions for patients in long-term care hospitals. Methods: The program was developed through a literature review, analysis of existing curricula, and needs assessments involving patient support team members, family caregivers, and experts. Educational needs were analyzed using the Borich Needs Assessment Model and the Locus for Focus Model. Results: The final curriculum included six core competencies and 27 subjects, consisting of eight common courses and 19 elective courses tailored to each professional category. Conclusion: Patient support teams require education across medical, nursing and social care domains to accurately identify complex patient needs and support safe transitions to community living. Nurses play a key role in addressing addressing the health needs of older adults. Further studies are needed to evaluate educational outcomes and competency improvement following program implementation.
