Comparison of Expectation-Perception between Patient and Nurse on Nursing Care Service in Comprehensive Nursing Care Wards (간호간병통합서비스 병동의 간호서비스에 대한 환자-간호사 인식도 비교)

  • The aim of this study was to compare perception between patient and nurse on comprehensive nursing care service using SERVQUAL model. To accomplish this goal, the 5 gaps of expectation-perception on comprehensive nursing care service was analyzed. The subjects were 192 nurses and 321 patients who admitted in comprehensive nursing care service wards. As results, patient's satisfaction was higher than patient's expectation and also nurse's performance. Therefore, we found that comprehensive nursing care service would contribute to perform holistic nursing care services in the future. To develop comprehensive nursing care service system sustainably, it needs continuous measurement of patient's expectation and satisfaction using conceptual model of this study.

A Job Analysis of Acute Care Hospitals' Formal Caregiver (급성기병원 간병인의 직무분석)

  • This study aims to improve caregiver's labor conditions, to provide a high-quality care service and to offer basic research information for developing a standardized job content of caregiver. The study establishes basic principles for a standardized job content with a literature review and then collects basic information of caregiver's task with an observation technique. The study based on the observation technique develops 11 job categories and 72 duty categories expected to do by the caregiver. The study examines acute care hospitals that provide joint care services in Seoul and Gyeonggi province. Sixty caregivers who work at these hospitals are carried out a survey regarding a specific task of caregiver, an importance of task and a frequency of task. Collected data is analyzed using statistical package SPSS 18.0, and descriptive statistics (frequency, percentage, average, standard deviation) is reported. An analysis of frequency is used to analyze target for research. From the collected information, tasks of caregiver are sorted into 10 job categories and 60 duty categories. As a result of the study, tasks of caregiver are sorted into 10 job categories and 68 duty categories based on the observations technique. One job category and four duty categories are added to the final survey based on an interview with professionals and officials in the field of care service but, one additional job category and four duty categories based on the interview are excepted from the result of the analysis. The interview with professionals and officials is nothing meaningful on the task analysis of caregiver.

Factors of Bodily Pain Among Stroke Patient's Female Caregivers in Their Middle to Older Age (뇌졸중 환자를 돌보는 중·고령층 여성 간병인의 신체통증 영향 요인)

  • Objective: This study aimed to examine the factors affecting bodily pain in women in their middle to older age who are caregivers of stroke patients. Methods: This study selected 75 stroke inpatients and 75 female caregivers in their middle to older age from five hospitals in Daegu and Gyeongbuk. Measurements included general characteristics, cognitive function, upper limb function, and activities of daily living of stroke patients, and general characteristics and bodily pain for the caregivers. We used stepwise multiple regression analysis to determine the factors affecting the bodily pain and Pearson's correlation analysis to confirm the correlation. Results: The factors affecting bodily pain in women in their middle to older age caregivers were the activities of daily living(${\beta}=-.489$, p<.001) and caregiver duration(${\beta}=-.309$, p=.003)($R^2=.276$). Correlation analysis showed that the activities of daily living, upper limb function, and cognitive function of stroke patients were significantly correlated (r=.434~.751, p<.001). Conclusions: The results of this study demonstrate that the functional level of stroke patients being cared for and the caregiver duration are important variables for reducing bodily pain in women in their middle to older age.

A Study on the Relationship of Infection Control Performance in a Long Term Care Hospital Caregivers (일개 지역 요양병원 간병인의 감염관리수행도 관련성 연구)

  • The purpose of the study is to investigate infection management status of long term care hospitals and infection prevention knowledge of caregivers. This is descriptive study to investigate the factors affecting caregivers' infection control performance in long term care hospitals. The data collection period was from August, 2018, and a total of 197 caregivers from 8 long term care hospitals. The data analysis were done Win SPSS 20.0 with t-test, ANOVA, pearson's correlation coefficient, and hierarchical multiple regression analysis. Infection prevention knowledge was positively correlated with infection control performance (p=<.001). The factors influencing performance in a long term care hospital caregivers were over seventy years old (t=2.50, p=.013), eight-hour working time (t=-2.62, p=.010), nursing staffing First grades (t=2.48, p=.014), and infection prevention knowledge (t=2.96 p=.003), which explained 12.9% (F=6.70 p<.001). In summary, we recommend to develop infection education interventions to improve caregivers' infection control performance in long term care hospitals and to improve of the function in long term care hospitals.

The Survey of Perception, Performance and Importance in Nursing Practice of Comprehensive Nursing Care Service (간호·간병통합서비스의 간호업무 인식, 수행도, 중요도에 대한 조사연구)

  • The purpose of this study was to identify a survey on total care services, to verify the perception, performance and importance of nursing practices between nursing students and nurses. The sample of the study were 93 nursing students and 38 nurses, A structured questionnaire used in this study composed of 235 items. The collected data were analyzed using the SPSS WIN 20.0 to calculate frequencies, percentage, mean, standard deviation and t-test. The Major findings of this study were as follows: The amount and responsibility of nursing practices in total service showed that 'very much increase'was the most. There were statistically significant differences in performance of total care service by nutritional care and elimination care. There were statistically significant differences in importance of total care service by respiratory Care.

The Factors Related to Musculoskeletal Symptoms of Family Care-Givers who Have a Patient with Brain Damage (뇌손상 가족 간병인의 근골격계 자각증상과 관련요인)

  • This study was conducted to identify factors related to the musculoskeletal symptoms of 340 family caregivers who have a patient with brain damage based on self-administered questionnaires. The questionnaires included general characteristics, characteristics of care activities and caregivers' musculoskeletal symptoms. Data were collected from many long-term care hospitals and rehabilitation centers during March 17 to March 21, 2014. The results indicated that complaint rates of work-related musculoskeletal systems of the body sites differed. Factor analysis revealed that neck complaints were related to education (under mid), while shoulder complaints were related to sex (female), age (50-59), education (mid) and duration of care (< 2 years). Arm/elbow complaints were related to age (40-49), education (high) and duration of care (12-24). Complaints regarding the hand/wrist/finger were related to age (50-59) and duration of care (12-24), while those associated with the legs/feet were related to age (50-59) and duration of care (< 6[ED highlight - years? Please specify.]). Back problems were related to education (under mid, mid stage, high) and gait. The complaint rate regarding musculoskeletal symptoms during daily life was not statistically significant. Logistic regression analysis of significant factors related to subjective musculoskeletal symptoms identified ambulation and gait as having the greatest influence and complaint factor among family caregivers. The complaint rate of family caregivers differed among body parts. Being a caregiver for less than one year was found to have a significant impact on pain. Overall, long term family caregivers could be faced with risk factors for musculoskeletal problems, but there are many different factors that affect musculoskeletal symptoms with regards to their activities. Accordingly, comprehensive and systematic prevention plans for family caregivers who have patients with brain damage should be developed.

Job Satisfaction, Job Stress, Burnout, and Turnover Intention of Comprehensive Nursing Care Service Ward Nurses and General Ward Nurses (간호·간병통합서비스병동 간호사와 일반병동 간호사의 직무만족, 직무스트레스, 직무소진 및 이직의도)

  • This study was carried out to analyze and compare job satisfaction, job stress, burnout, and turnover intention of comprehensive nursing care service ward nurses and general ward nurses. The participants were 181 nurses from six hospitals providing comprehensive nursing care service in G metropolitan city. The data were collected using structured questionnaires in May 16-June 30, 2017. The data were analyzed with SPSS program, which was used for t-test, one-way ANOVA, and Pearson's correlation. As a result, there were no significant differences in the variables between comprehensive nursing care service ward nurses and general ward nurses. However, comprehensive nursing care service ward nurses had higher job satisfaction, lower job stress, lower burnout, and lower turnover intention than general ward nurses. There was a significant relationship between job stress (r=.39, p=<.001), burnout (r=.43, p=<.001), job satisfaction (r=-.66, p=<.001), and turnover intention. Comprehensive nursing care service ward nurses were more positive about their nursing work than general ward nurses. The findings of this study can be used as basic data for effective nursing resource management in a comprehensive nursing care service ward.

Smart Elderly-care System using Smart-phone (스마트폰을 이용한 고령자용 스마트 간병 시스템)

  • In this paper, we propose a system to monitor the condition of elderly people who are uncomfortable by using smart-phone and biometric sensor at any time and for care-givers to provide with the best medical service anytime and anywhere. The proposed system monitors the status of the elderly through various bionic sensors installed in the hospital ward based on the Arduino system and not only provides the physiological and medical services required by the elderly, but also informs the guardian so that he can cope with the emergency. In conclusion, this paper suggests that a reading light used by elderly people can operate as a home server with a biosensor using Arduino and Android applications (Apps.), and the smart-phone of care-givers is configured as a remote management and a emergency call system. Therefore, this study suggests important ways to improve the satisfaction of medical service for the both elderly people and care-givers of chronic diseases in the future.

The effects of Emotional Labor, Resilience, and Nursing Work Environment on Turnover Intentions of Comprehensive Nursing Care Medical Service Nurses (간호간병통합서비스병동 간호사의 감정노동, 회복탄력성, 간호근무환경이 이직의도에 미치는 영향)

  • The purpose of this study was to examine the effect of emotional labor, resilience, nursing work environment on turnover intentions of comprehensive nursing care medical service ward nurses. The data were collected from 186 subjects using a structured self-report questionnaires and analyzed using SPSS WIN Version 22.0. The mean score of emotional labor, resilience, nursing work environment, and turnover intentions were 3.66, 3.52, 2.62, and 3.82. Turnover intentions had significant correlation with emotional labor, resilience and nursing work environment. The influencing factors on turnover intentions were sex, emotional labor, resilience and nursing work environment of comprehensive nursing care medical service ward nurses. These result indicates that in order to reduce the turnover intentions of comprehensive nursing care medical service ward nurses, it is necessary to develop and apply an intervention programs considering sex, emotional labor, resilience, and nursing work environment.
