A Development of a Tailored Follow up Management Model Using the Data Mining Technique on Hypertension (데이터마이닝 기법을 활용한 맞춤형 고혈압 사후관리 모형 개발)

  • This study used the characteristics of the knowledge discovery and data mining algorithms to develop tailored hypertension follow up management model - hypertension care predictive model and hypertension care compliance segmentation model - for hypertension management using the Korea National Health Insurance Corporation database(the insureds’ screening and health care benefit data). This study validated the predictive power of data mining algorithms by comparing the performance of logistic regression, decision tree, and ensemble technique. On the basis of internal and external validation, it was found that the model performance of logistic regression method was the best among the above three techniques on hypertension care predictive model and hypertension care compliance segmentation model was developed by Decision tree analysis. This study produced several factors affecting the outbreak of hypertension using screening. It is considered to be a contributing factor towards the nation’s building of a Hypertension follow up Management System in the near future by bringing forth representative results on the rise and care of hypertension.

  • 본 연구에서는 보건기관이 효율적으로 고혈압 관리 대상자를 탐색하고, 고혈압 관련 요인에 대한 지식을 효과적으로 관리할 수 있도록 하는 고혈압 고위험군 추정 모형 및 우선 사업 대상자 탐색 모형을 제안한다. 특히, 대용량 데이터 처리 및 실시간 시스템 운영, 외부 환경 변화를 고려한 자동 학습과 같은 현실적인 제약 조건을 해결하는 모형을 개발하는 것을 주 목표로 한다. 지역 보건소에서 수집된 의료 데이터를 이용하여 최적의 파라미터 값을 설정한 고혈압 고위험군 탐색 모형을 도출하였으며, 모형의 검증을 위하여 고혈압 환자정보로 구성된 평가용 데이터를 사용하여 고혈압 자연 발병률 대비 약 2배 수준으로 향상된 고혈압 환자 예측 정확도가 얻어지는 것을 확인하였다. 시스템 운영과 유비보수 측면에서 현실적으로 중요한 문제인 대용량 데이터 처리 및 외부 환경 변화에 강인한 자동학습 이슈를 해결하기 위한 방안에 대해서도 설명하였다.

Prevalence of Hypertension and Related Risk Factors of the Older Residents in Andong Rural Area (안동 농촌지역 중년 및 노인 주민의 고혈압 유병율과 위험요인 분석)

  • This study was performed to assess the risk factors associated with hypertension from Jan/2003 to Feb/2003. The subjects were 1,296 people (496 males, 800 females) aged 40 years and over living in Andong rural area. The hypertensive group was composed of 602 people (272 males, 330 females), who were diagnosed as hypertension ($SBP{\geq}140\;mmHg$ or $DBP{\geq}90\;mmHg$) for the first time at this health examination. The mean anthropometric values of body weight, body fat (%), body mass index (BMI) and waist circumference were significantly higher in hypertensive group than those in normal group. However, the biochemical measurements such as total-cholesterol (TC), triglyceride (TG), HDL-C, LDL-C and fasting blood glucose (FBG) levels did not show any difference between two groups except TG in female. The risk factors of interest in the development of hypertension were analyzed using the multiple logistic regression and expressed as odds ratio (OR) and 95% confidential interval (CI). The results showed that age, sex, obesity, waist circumference, alcohol drinking and meat intakes were risk factors for hypertension. In contrast, cigarette smoking, exercise and the increased fish, fruit and vegetable (except Kimchi) consumption, blood lipid levels and FBG were not linked with the development of hypertension. Nutrient intakes were not associated with hypertension, either. In conclusion, we cannot assert that this study established the existence of the cause-and-effect relationship between nutrient intakes and risk of hypertension in the subjects, but it does suggest that this is a question worth investigating further using a larger scale of case-control study to determine how the past exposure to some nutrient or dietary component relates to the development of the disease.

  • 고혈압이 있으면 신장병 발생률이 높아지고, 만성신장병으로 진행될수록 고혈압의 빈도가 증가하는 것으로 나타났다. 대한신장학회 조사에 의하면 건강한 사람의 경우 만성신장병의 발생률이 9.3%인 반면 고혈압환자에서는 21.6%가 신장에 이상이 있는 것으로 밝혀졌다. 또한 고혈압이 심할수록 만성신장병의 발생가능성이 급격히 증가했다. 수축기 혈압이 120mmHg 미만인 정상인은 8.2%에서 만성신장병이 나타났으나 140mmHg(수축기혈압) 이상인 고혈압환자들은 23.1%가 만성신장병을 앓고 있는 것으로 드러났다.

Altered Vascular Expression of Nitric Oxide Synthase Isozymes in Hypertension (고혈압에서 혈관 산화질소 합성 동위 효소 발현 변화)

  • Background: The endothelium-dependent vasorelaxation has been largely accounted for by the release of nitric oxide (NO). Three distinct isoforms of NO synthases (NOS) have been characterized, i.e., brain(bNOS), inducible (iNOS), and endothelial constitutive (ecNOS). Although hypertension hasbeen associated with a vascular endothelial dysfunction, changes in the vascular expression of NOS isoforms have not been established. The present study was aimed at exploring the vascular expression of NOS isozymes in hypertension. Material and Method: Two-kidney, one clip (2K1C) and deoxycorticosterone acetate (DOCA)-salt hypertension were induced in rats. The expression of different NOS isozymes in the thoracic aorta was determined by Western blot analysis. The vascular tissue contents of nitrites were measured by colorimetric assay. Result: Arterial blood pressure was significantly higher in experimental groups of 2K1C and DOCA-salt rats compared with their corresponding control rats. The vascular expression of bNOS as well as that of ecNOS was decreased in both models of hypertension. iNOS was not changed in DOCA-salt hypertension, but was also decreased in 2K1C hypertension. The vascular contents of nitrites were significantly decreased in DOCA-salt as well as in 2K1C hypertension. Conclusion: These results suggest that 2K1C and DOCA-salt hypertension are associated with decreases in the vascular expression of NOS isozymes and nitrite contents.

  • 고혈압은 다양한 유전적 요인과 환경적 요인이 상호작용하는 다인자성 질환으로 알려져 있으며, 최근의 연구에 의하면 사립체 DNA에 존재하는 유전적 다형성이 고혈압과 유의한 관련성을 나타낸다는 보고가 있다. 이에 본 연구에서는 한국인 집단을 대상으로 하여 사립체 DNA의 5178번째 위치의 염기서열에 존재하는 A/C 다형성이 고혈압과 관련성을 나타내는 지를 분석하였다. 환자-대조군 연구를 수행한 결과, 사립체 DNA의 5178번째 위치에 존재하는 다형성의 대립 유전자 빈도는 한국인에서 고혈압군과 정상 혈압군 사이에 유의한 차이를 나타내지 않았다. 따라서, 이 다형성은 적어도 한국인에 대해서는 고혈압에 유의하게 영향을 미치는 유전적 소인은 아닌 것으로 사료된다.

An Efficient Management by Analyzing Control Level of Hypertension (고혈압 관리수준에 따른 효율적인 관리 방안)

  • The purpose of this study is to contribute basic data toward the efficient management of Hypertension by comprehensive analysis of several factors which make influences on indicators of Hypertension management. To achieve this, using the data of 2007, 2008 Korea National Health and Nutrition Examination Survey, 2,580 adults aged over 19 who suffered from hypertension have been analyzed comprehensively. The major results can be summarized as follows: First, in regard to the influential factors for awareness and treatment of hypertension, the older the patients were, the poorer their self-rated health status were. And when they had experienced a health examination, the more likely the hypertensive patients were aware of their hypertension and took the treatment for it. Second, as for the influential factors for control of hypertension, those who were older and those who had a health examination were significantly more likely to control their hypertension irrespective of gender. However, regular drinkers fell significantly behind in that aspect. The findings of this study show that systematic hypertension management, focusing on health behavior factors that affect the level of hypertension management, in accordance with the patients' sex and age will provide a more efficient way.
