  • 맥압증폭은 대동맥과 말초기관과의 혈압기능 평가 및 심혈관 질환을 예측하는 주요 매개변수로 나이와 관련이 높다. 그러나, 기존의 맥압증폭을 구하기 위해서는 관혈적인 방법으로 대동맥압을 측정하거나 혹은 전달함수와 같은 복잡한 대동맥압 추정 알고리즘을 구현하여 측정하여야 한다. 그러나, 유헬스를 활용한 스마트케어 환경에서 복잡한 알고리즘은 오히려, 계산량을 높이고, 시스템가격을 높일 수 있으므로, 최대한 단순화된 알고리즘이 필요하다. 본 논문에서는 맥압증폭을 주파수영역의 에너지 관점으로 맥압증폭을 접근하였고, 대동맥압 추정없이 추정할 수 있는 새로운 지수를 제안한다.

Relationship between sleep duration and pulse pressure in Korean adults: Focusing on the data of the 7th National Health and Nutrition Survey (한국 성인의 수면시간과 맥압과의 관련성: 국민건강영양조사 제7기 자료를 중심으로)

  • The purpose of this study is to investigate the relationship between sleep time and pulse pressure in Korean adults, and the research method used data from the 7th period (2016-2018) of the National Health and Nutrition Examination Survey. Subjects were 5,707 persons aged 20 years or older, and for frequency and percentage continuous data, mean and standard deviation, cross analysis and independent t-test, and stepwise logistic regression analysis for high pulse pressure were performed. SPSS WIN 18.0 version statistical program was used. was used. As a result of the study, the prevalence of hypertension was higher in the overall population (OR=1.344, 95% CI=1.002-1.803) and in women (OR=1.624, 95% CI=1.112-2.371) when sleeping for more than 9 hours compared to less than 6 hours. high, but not significant in males (OR=1.012, 95% CI=0.631-1.550). Therefore, considering the effects of inappropriate sleep time on health, public health education and management are necessary. Further research on the relationship between sleep time and pulse pressure will be needed.

Fusion Research in the Association of Lung Function with Pulse Pressure and Blood Lipid in Adults (성인의 맥압과 혈청지질 및 폐기능의 관계 융합 연구)

  • The purpose of this study was to investigate whether there is a relationship between pulmonary function, serum lipid and pulse pressure, which is a risk factor of cardiovascular disease, in subjects with 109 office workers(51 males and 58 females) be at Seoul, Korea. The mean age of the subjects was $43.44{\pm}7.23$ years old (male $43.44{\pm}7.27$, fenale $49.1{\pm}11.5$). The subjects' height, weight, BMI, systolic and diastolic blood pressures were measured and there was statistical difference between men and women.The mean pulse pressure of the subjects was $47.82{\pm}9.02mmHg$, and the mean of FVC was $84.26{\pm}9.91%$. There was no difference between men and women in both. There was a positive correlation between pulse pressure and systolic blood pressure in males (p<0.01) and females (p<0.01), and FVC and HDL in males were positively correlated (p<0.05). Pulse pressure and %FVC were negatively correlated (r =-0.267, P<0.01). Pulse pressure and %FVC were negatively correlated (r =-0.267, P<0.01). Regression analysis showed that PP was the independent affected %FVC. Therefore, pulse pressure and %FVC may be useful as a predictor of cardiovascular disease in middle-aged subjects with normal, suggest a long-term convergence follow-up is warranted to elucidate the incidence of cardiovascular disease.

Association between Pulse Pressure and Impaired Pulmonary Function in Non-Smoking Adults (비흡연 성인에서 맥압과 폐기능 장애의 상관성)

  • According to previous studies, an impaired pulmonary function is associated with arterial stiffness (AS). The pulse pressure (PP) is an important predictor of AS, but the association of an impaired pulmonary function with the PP is unclear. Therefore, this study assessed the associations between the PP and the predicted forced vital capacity (predicted FVC) and predicted forced expiratory volume in one second (predicted FEV1) in Korean non-smoking adults. The data obtained from 6,857 adults during the 2013~2015 Korean National Health and Nutrition Examination Survey were analyzed. After adjusting for the related variables, the ORs of restrictive pulmonary disease (RPD, the predicted FVC<80.0% with FEV1/FVC≥70.0%) using the normal PP group (PP≤60 mmHg) as a reference group was significant for the high PP group (PP>60 mmHg; 1.337 [95% confidence interval (CI), 1.049~1.703]). In addition, the ORs of obstructive pulmonary disease (OPD, FEV1/FVC<70.0%) using the normal PP group as a reference group were significant for the high PP group (1.339 [95% CI, 1.093~1.642]). In conclusion, a high PP is positively associated with both RPD and OPD in Korean non-smoking adults.

The Study of Relationship of Obesity and Abdominal Obesity and Pulse Pressure Using Big Data (빅 데이터를 활용한 비만 및 복부비만이 맥압에 미치는 영향에 대한 연구)

  • The aim of this study was to assess the association of obesity and abdominal obesity and pulse pressure (PP) in Korean adults. The study subjects were Korean adults 20 years or older (n=5,889) who participated in the Korea National Health and Nutrition Examination Survey 2012. After adjusting for relevant variables (include age), in women, the odds ratio (OR) of High PP (PP >61 mmHg) of the obesity [1.37 (95 % CI, 1.03-1.82)] and abdominal obesity [1.38 (95% CI, 1.07-1.78)] group compared to the normal group was not significant. However, in men, the OR of High PP of the obesity and abdominal obesity group compared to the normal group was not significant. In conclusion, the obesity and abdominal obesity were associated with pulse pressure in Korean women, but not in men.

Development of Cardiovascular Simulator with Control of Pulse Pressure for Pulse Wave Study (맥압조절이 가능한 맥파 연구용 심혈관계 시뮬레이터 개발)

  • The purpose of this study is to produce a simulator that can control a pulse pressure keeping the pulse wave transfer phenomenon. For this, the elastic tube is combined with a compliance chamber for the vessel part. The simulator is comprised of four parts; a pressure generation part with slider-crank mechanism, a vessel part with resistance controller, water reservoirs and a measurement part. The changes of waveform depending on the location of a chamber is examined to determine the position of a chamber. The effects of a chamber on the pulse pressure and the pulse wave transfer phenomenon were investigated. It showed that the simulator which had the chamber in upstream of tube produces pressure wave, being more similar to the clinical waveform than in downstream of tube. Furthermore, with the chamber, the simulator generates a pulse pressure, being more similar to the normal physiological values than without one. The chamber had little effect on the pulse wave velocity.

Effects of Aerobic Exercise on Middle-aged Male Smokers' Blood Vessel Health (유산소 운동이 중년흡연남성의 혈관건강에 미치는 영향)

  • This research is aimed to prove if harmful effects of smoking, as the main reason for recently elevated blood vessel disease, could be reduced by aerobic exercise, and identify the positive effects of the aerobic exercise on smokers' blood vessel health(blood pressure, pulse pressure, and blood vessel elasticity. Experiments were performed on 40 male smokers aged between 40 and 55, which was equally divided into two groups of Aerobic Exercise Group (AEG) and Non Exercise Group (NEG). After measuring each group's blood pressure, pulse pressure, and blood vessel elasticity, AEG practiced aerobic exercise program for 50 minutes for each trial, three times a week, sustaining 12 weeks while NEG continued daily-life patterns without any special treatment. In 12 weeks, blood pressure, pulse pressure, and blood vessel elasticity of both groups were measured as the same way. As a result, the difference of systolic blood pressure between the two groups was not significant, while that of diastolic blood pressure was meaningful. The difference of pulse pressure between the two groups was meaningful, and that of blood vessel elasticity was also meaningful in all measuring points including left hand, right hand, left foot and right foot. As a consequent, it was found that aerobic exercise had positive effects on middle aged male smokers' blood vessel health.

Development of a Test Jig for Examination of Radial Pulse Waveform Variations at Different Hold-down Pressures (가압에 따른 요골동맥 맥파 변동 측정을 위한 테스트 지그 개발)

  • 한의학 진단의 중요 요소인 맥진을 자동화한 맥진기에서뿐만 아니라 비관혈적 심혈관 기능 평가를 위해서 요골동맥 맥파 측정이 널리 활용되고 있다. 요골동맥 맥파는 피험자의 생리적, 병리적 원인을 통제하더라도 가압력이 변함에 따라 형태가 달라지며, 어느 시점 맥파를 피험자의 상태를 대표하는 맥파로 해야 할지에 대한 명확한 기준이 없어 맥압이 최대가 되는 시점의 맥파를 사용하고 있다. 그러나, 맥파가 측정되는 가압력 범위뿐만 아니라 맥압이 최대가 되는 시점이 피험자마다 다르기 때문에 정확한 대표 맥파를 획득하기 위해서는 보다 가압력 단계를 늘려야 하며, 결과적으로 측정시간이 보다 길어지게 될 것이다. 본 논문에서는 최소한의 가압단계로도 보다 정확히 대표 맥파를 획득할 수 있는 방법을 개발할 수 있도록, 정밀 제어가 가능한 가압 로봇 축, 가압에 의해 눌리는 피부이동 거리 측정을 위한 레이저 센서, 가압에 따른 피부 반발력과 맥파를 측정할 수 있는 압력 센서로 구성된 테스트 지그를 개발하였다. 이를 통해 모터 이동 스텝 수에 따른 피부 이동 거리 특성, 피부 이동 거리에 따른 맥파 변화 특성 등을 관찰할 수 있었으며, 향후 효과적인 대표 맥파 획득 방법을 개발할 수 있을 것으로 기대한다.

Association of Metabolic syndrome, Metabolic syndrome score and Pulse pressure in Korean Adults: Korea National Health and Nutrition Survey, 2012 (한국 성인에서 대사증후군 및 Metabolic syndrome score와 맥압의 관련성-2012 국민건강영양조사에 근거하여)

  • The aim of this study was to assess the association of metabolic syndrome, metabolic syndrome score (MSS) and pulse pressure (PP) in Korean adults. The study subjects were Korean adults 20 years or older (n=5,889) who participated in the Korea National Health and Nutrition Examination Survey 2012. After adjusting for factors, such as year and gender and BMI, the mean PP increased with increasing MSS (MSS 0, $41.30{\pm}0.34mmHg$ and MSS 1, $42.16{\pm}0.31mmHg$ and MSS 2, $44.73{\pm}0.34mmHg$ and MSS, 3, $46.46{\pm}0.42mmHg$ and MSS 4, $48.62{\pm}0.58mmHg$ and MSS 5, $53.50{\pm}1.05mmHg$), and the mean PP for metabolic syndrome($47.25{\pm}0.34mmHg$) increased in comparison to Non-Metabolic syndrome ($42.77{\pm}0.19mmHg$). When logistic regression analysis was performed, the odds ratio (OR) of Hyper-PP (61> PP) for MSS 0 was 4.49 in MSS 1 (95% confidence interval[CI], 2.68-7.57) and 8.01 in MSS 2 (95% CI, 4.77-13.47) and 11.37 in MSS 3 (95% CI, 6.67-19.35) and 19.69 in MSS 4 (95% CI, 11.20-34.60) and 34.07 in MSS 5 (95% CI, 17.44-66.52), metabolic syndrome was associated with an increased Hyper-PP(OR 4.6, 95% CI, 2.0-10.4). Conclusion. These results suggest that an increase in MSS or metabolic syndrome might increase the pulse pressure.

Change of arterial pulse wave characteristic by measurement posture and brachial blood pressure (측정 자세 및 상완 혈압에 의한 맥파 특성 변화)

  • In this study, pulse waves were measured at radial artery using non-invasive tonometric pulse pressure measurement system, SphygmoCor(AtCor, Australia), according to subject's posture. Then it was analysed whether the pulse wave parameters, which contain heart activities, change among three different postures (upright stand, sit, and supine). And it was also verified that the pulse wave parameters change among blood pressure level groups(hypotensive, normotensive, and hypertensive). As a results, posture effects were verified in time information of pulse wave rather than amplitude. But some parameters calculated by ratio of two amplitude, such as augmented index(AI) and ratio of central aortic pulse and radial artery pulse, showed significant difference according to postures. In post hoc test, time to the $1^{st}$ and $2^{nd}$ pulse peak(P_$T_1$, and P_$T_2$), ED(ejection duration), and HR(heart rate) showed significant difference among posture groups with each other. In comparison of blood pressure groups, it was verified that the parameters related to amplitude of pulse wave showed significant difference rather than time information.
