The Comparison between Acupotomy Therapy and Epidural Neuroplasty(Lumbar Vertebra) (침도(도침)침술과 경막외 신경성형술의 비교 연구)

  • Objectives : This study will broaden understanding of acupotomy therapy through comparison of side effects and complication which can be caused by the mechanism of treatment, surgical methods, and procedure by acupotomy therapy and epidural neuroplasty. Moreover, through an in-depth analysis of headache affected by two procedures, this research is supposed to find prospective cures for headache after acupotomy therapy. Methods : To compare acupotomy therapy with epidural neuroplasty this research was done using a comparative analysis eight theses about acupotomy therapy since 1995, as well as eleven theses about epidural neuroplasty since 2000. Other theses and data were used as references in the process of comparative analysis. Results : Acupotomy therapy and epidural neuroplasty, new treatments of damaged discs in the spine and stenosis made in 1990s, have the mechanism of treatment in common in that adhesion, a node or scar caused by the soft tissue damage is removed by putting catheter or acupuncture into the lesions. Epidural neuroplasty has additional injection into the lesions, which is different from acupotomy therapy in the process of surgical procedure. There are various reports of positive effects about curative effect in these two treatments. The two procedures may cause various complications. Headache may be a complication after surgery. The headache after acupotomy therapy is characterized as being an ache in the body, which is similar to that of post-dural puncture headache in the outbreaks and symptom. Headache after percutaneous epidural neuroplasty appears in general, which is similar to a headache as a result of the increased pressure of the brain spinal cord regardless of posture. Conclusions : Although they are alike in the mechanism of treatment, surgical methods and side effects, and complication after they are carried out as a result of analyzing theses related to acupotomy therapy and percutaneous epidural neuroplasty, there is a difference in aspects and mechanism of headaches experienced after the procedure.

Clinical Study of Acupotomy Treatment for Tarsal Tunnel Syndrome

  • Background: Compared with conventional closed therapy, acupotomy has the advantages of lower cost, convenient application, and better single and long-term effects. The aim of this study was to explore the clinical effect of acupotomy in the treatment of Tarsal Tunnel syndrome. Methods: There were 80 patients enrolled into the study who were randomly assigned to either the acupotomy or closed therapy group, with 40 patients in each group. The acupotomy group was treated once every 6 days, on 3 occasions for the duration of treatment, and the closed therapy group was treated twice a week, for 3 weeks as the course of treatment. The effects of treatment were analyzed and evaluated according to the standard of curative effect. Results: The "cure rate" of the number of patients in the acupotomy group whose symptoms had completely disappeared (13 patient out of 39) was higher than the closed therapy group (1 patient out of 39), and this difference was significant (p < 0.01). The number of patients in the acupotomy group who did not benefit from the therapy (5 patients) was significantly lower than the closed therapy group (15 patients; p < 0.05). The total number of patients in the acupotomy group who benefitted from the therapy (34 patients) resulted in an effective rate of 87.18%, which was higher than the closed therapy group (24 patients; 61.53%). The difference was statistically significant (p < 0.05). Conclusion: Acupotomy is effective in the treatment of Tarsal Tunnel syndrome, was superior to traditional closed therapy, and is worthy of clinical application.

The Clinical Study on Effects of Acupotomy Therapy in Patients with Cervical Herniated Intervertebral Disc (경추 추간판 탈출증 환자의 침도요법(鍼刀療法) 치료 효과에 대한 임상적 연구)

  • Objectives : The study on Acupotomy therapy has recently become a popular academic field as proven to be an great alternative to the limitation of Western medical treatment. However, there has been little study examining acupotomy therapy on treating Cervical Herniated Intervertebral Disc. Thus, this study aims to investigate the sense of improvement and satisfaction from the Cervical Herniated Intervertebral Disc patients treated with acupotomy therapy. Methods : A total of 23 subjects that consists of Cervical Herniated Intervertebral Disc patients were participated in this study. They were treated with acupotomy therapy and acupuncture. All the subjects were asked to answer Odom's degree, VAS(visual analogue scale), NDI(neck disability index) and JOA score before and after the treatment. Results : The results of the Odom's degree, VAS, NDI and JOA score of analysis groups, show that the improvement of neck pain in acupotomy and acupuncture treatment group is significantly effective. Conclusions : It is suggested that acupotomy therapy has development-effectiveness on Cervical Herniated Intervertebral Disc.

The Case Report of Posterior Headache Caused by Traffic Accident Treated with Musculoskeletal Ultrasound-guided Acupotomy Therapy (교통사고 후 발생한 후두통의 근골격계 초음파를 이용한 침도치료 임상증례)

  • Objectives : The objective of this case report was to observe the effect of musculoskeletal ultrasound-guided acupotomy therapy on posterior headache caused by traffic accident. Methods : Musculoskeletal ultrasound-guided acupotomy therapy was performed to two patients whose brain MRI or CT results were normal, but posterior headache did not improve with general eastern medical treatment. Results : One patient's VAS(visual analogue scale) of posterior headache was decreased for the first time after ultrasound-guided acupotomy therapy, and steadily reduced. The other patient's VAS was also decreased for the first time after musculoskeletal ultrasound-guided acupotomy therapy. Two patient's Korean HIT-6(Korean headache impact test-6) scores were decreased after one month. Conclusions : If general eastern medical treatment had little effect on traffic accident induced posterior headache, musculoskeletal ultrasound-guided acupotomy therapy can be applied.

A Proposal for Standardization of Acupotomy Terms and Definition Among Korean researchers (한국 내 도침술의 정의와 용어 표준화를 위한 제안)

  • Objectives: The aim of this study was to investigate the use of Korean and English terminology of acupotomy therapy among Korean Researchers. Methods: The search was conducted on eight medical journals (PubMed, EMBASE, Cochrane Library, OASIS, KISS, IMR, NDSL, RISS), Google Scholar and 8 Korean medine journals. The definitions of acupotomy therapy, Korean and English terminology were analyzed in the studies that met the criteria. Results: A total of 60 studies were finally included in the analysis. In Korean terminology, Chim-Do was the most used with 37 studies, Do-Chim with 23 (including duplicates). In English termology, 46 studies were used acupotomy and 10 studies were used miniscalpel acupuncture. Conclusions: As a result of analyzing the terminology use of acupotomy therapy studies in Korea, it was confirmed that Korean and English terminology are different according to the study. It will be necessary to discuss the standardization of terminology.

  • Purpose : In order to estimate clinical effects of Oriental Medicine Treatment with acupotomy therapy of Tarsal tunnel Syndrome Methods : From 5th November, 2008 to 8th November, 2008, 1 male patient diagnosed as Tarsal tunnel syndrome(clinical diagnosed) was treated with general oriental medicine therapy (acupuncture, moxibustion, cupping, physical therapy, herbal medication) and acupotomy. Results : The patient's Rt foot paresthesia, pain were remarkably improved. Conclusions : This study demonstrates that oriental medical treatment with acuputomy therapy has notable effect in improving symptoms of tarsal tunnel syndrome. as though we had not wide experience in this treatment, more research is needed.

  • Objectives : The purpose of this study is to report the effect of acupotomy for patient with avascular necrosis(AVN) of femur head. Methods : Three patient who had been diagnosed as AVN participated in this study. The patient treated by acupotomy therapy. After the treatment, the patient was measured by visual analogue scale(VAS), Harris hip score and five likert scale Results : The patient informed the researcher of reduced pain and improved range of movement after the conduct of acupotomy therapy. The efficacy of acupotomy treatment was reaffirmed by the results of the VAS, physical examination (ROM, Patrcik sign), Harris hip score. Conclusions : Acupotomy therapy on AVN of femur head shows the efficacy in reducing pain and improving movement.

A Case Report of Congestive Heart Failure Treated with Acupotomy Therapy (심부전으로 인한 호흡곤란 환자에게 시행한 침도침 시술 1례 보고)

  • Objectives : The purpose of this report is to report the effect of acupotomy for patients with congestive heart failure. Methods: We treated 1 patient who has dyspnea due to congestive heart failure with acupotomy. To check the effect and satisfaction of acupotomy we used New York Heart Association class(NYHA class), Modified Borg Scalw Dyspnea Index(Borg Index), Baseline Dyspnea Index(BDI), Five-point Likert scale. Results : After 1 month of treatment, the patient felt much better in breathing, and had better score in NYHA class, Borg Index, BDI. Conclusions : This report demonstrates that acupotomy therapy has useful effect on congestive heart failure, but the more cases and researches are needed.

  • Objectives : The study on Acupotomy therapy has recently become a popular academic field as proven to be an great alternative to the limitation of Western medical treatment. However, there has been little study examining acupotomy therapy on treating HIVD. Thus, this study aims to investigate the sense of improvement and satisfaction from the HIVD patients treated with acupotomy therapy. Methods : A total of 63 subjects that consists of HIVD patients were participated in this study. The experimental group(33subjects) was treated with acupotomy therapy and acupuncture, and the control group (30subjects) was treated with acupuncture. All the subjects were asked to answer VAS(visual analogue scale) and ODI(Oswestry disability index) before and after the treatment. Results : The results of the VAS and ODI of comparison analysis between two groups, show that the improvement of HIVD in acupotomy and acupuncture treatment group is more effective than that in acupuncture treatment group

Effect of Ultrasound-guided Acupotomy Therpy on Wrist Ganglion (완관절 다엽성 결절종의 초음파 진단을 통한 침도치료 임상증례 보고)

  • Objectives : This observation was to report the clinical effects of acupotomy in treating wrist ganglion. Methods : From 7th April, 2010 to 12th May, 2010, 1 female patient who had a wrist ganglion was treated with ultra sound-guided acupotomy therapy and self moxibustion & massage. Results : The patient's wrist ganglion was disappeared on the surface of the skin after just one time acupotomy therapy. Hypoechoic lesion was turned into hyperechoic lesion on a ultrasonographic. After 1 month, a wrist ganglion was recurred, but the size of wrist ganglion was reduced. Conclusions : Acuputomy therapy has an effect of reducing the wrist ganglion size, but patient self management is important in preventing recurrence. The ganglion not visible on the surface of the skin must be considered as a treatment target.
