A Survey of Characteristics and Management of Low Back Pain in Postpartum Women (산모의 요통 실태와 관리에 대한 조사 연구)

  • Many pregnant women have experienced low back pain (LBP) during pregnancy and after delivery, and it has been an important component in women health. This study was designed to investigate the characteristics and management of the LBP in postpartum women. Eighty-five postpartum women were participated in this survey. Mean age of 85 women was 28.1 years. Of 85 postpartum women, 55.3% (n=47) had LBP after pregnancy. Thirty of 47 women had pain on lumbar region, 17 postpartum women had pain on sacroilium region. Of 85 postpartum women, 74% (n=54) had LBP before pregnancy and 71.8% (n=61) had LBP during pregnancy. Of 47 postpartum women who had LBP, 83% (n=39) had not received medical management for LBP, 12.8% (n=6) took medication, and 4.3% (n=2) performed self-exercise. None of postpartum women had received physical therapy during pregnancy and after delivery for treatment low back pain. The pain in SI region was more severe than in lumbar region after pregnancy according to VAS (visual analog scale) (p<.05). However, there was no significant difference in VAS scores between SI pain and lumbar pain before and during pregnancy (p>.05). Pain region after delivery was related to pain region of pre-pregnancy and during pregnancy (p<.01). Pain level after delivery was related to the pain and night pain level during pregnancy (p<.01).

A study on the relationship between women's health status and the experience of Sanhujori, the Korean traditional non-professional postpartal care. (여성의 건강상태와 산후조리 경험과의 관계 연구)

  • This descriptive study sought to define the relationship between women's health status and the experience of Sanhujori, Korean traditional non-professional postpartal care after delivery and abortion. A convenience sample of 308 women in 7 provinces in Korea including Seoul were studied from December, 1994 to December, 1996 for two years. Mean age of respondents was 50.5 years and mean number of children was 3. The rate of abortion was 91.5% and mean frequency was 2.2 times per woman. 82% of respondents did not have Sanhujori after abortion. The period and subjective evaluation of experience of Sanhujori after delivery were decreased according to the increment of the number of childbirth. The health status implies both subjective health status women perceived and physical symptom distress women are experiencing presently, The respondents expressed the physical symptom distress as painful one. 56.7% of respondents perceived unhealthy, such as sick and 99.6% complained more than one symptom. The factors related to health status were the first and third experience of Sanhujori after delivery, such as the period and subjective evaluation whether she did Sanhujori well or not; whether or not of Sanhujori after abortion and menopause: the number of child: and age. at the level of 1% or 5% of significance statistically. The factors related to the rate of physical sumptom distress were only two: the first experience of Sanhujori after delivery, especially the subjective evaluation and whether women did Sanhujori after abortion or not. at the level of 1% or 5% of significance statistically. In conclusion. this finding reconfirmed the possible relationship between women's health status and the experience of Sanhujori after delivery & abortion. It provides a challenge to the professional care givers .to research further on the effects of Sanhujori on the health status. health recovery after abortion or delivery from the various aspects through the cross-sectional and longitudinal research for the refinement of the reality of Sanhujori not only as cultural phenomenon but as conceptual model for the appropriateness of intervention and qualty of care for desirable health outcomes. Besides, it is indispensable to refine and reestablish postpartal caring system by finding universal law through international & cross-cultural research on postpartal traditional care for women's life long health toward the 21C

Decision-making process and satisfaction of pregnant women for delivery method (임산부의 분만방법 결정과정과 만족도)

  • This study was conducted to assess the attitude of pregnant women toward delivery method, understanding of the reason for determining her own delivery method, participation in decision-making process and satisfaction with delivery method after labor. Study subjects were 693 pregnant women who had visited obstetric clinic for prenatal care in the last month of pregnancy in one general hospital and one obstetrics-gynecology specialty hospital in Taegu city from February 1 to March 31 in 1998. A questionnaire was administered before and after labor and a telephone interview was done one month after labor. Proportion of women who had health education and/or counselling about delivery method during prenatal care was 24.0% and this proportion was higher for women who had previous c-section(35.5%) than others. Women thought vaginal delivery is better than c-section for both maternal and baby's health regardless of previous delivery method. About 90% of primipara and multiparous women who had previous vaginal delivery wanted vaginal delivery for the index birth, while 85.6% of multiparous women who had previous c-section wanted repeat c-section. Reasons for choosing c-section in pregnant women who preferred vaginal delivery before labor were recommendation of doctors(81.9%), recommendation of husband (0.8%), agreement between doctor and pregnant woman(4.7%), and mother's demand (12.6%). Reasons for choosing vaginal delivery were mother's demand(30.6%) and no indication for c-section(67.2%). Reasons for choosing c-section in pregnant women who preferred c-section before labor were recommendation of doctors(76.2%), mother's demand(20.0%), recommendation of husband(1.3%), and agreement between doctor and pregnant woman(2.5%). Of the pregnant women who had c-section, by doctor's recommendation, the proportion of women who had heard detailed explanation about reason for c-section by doctor was 55.1%. Mother's statement about the reason for c-section was consistent with the medical record in 75.9% . However, over 5% points disparities were shown between mother's statement and medical record in cases of the repeat c-section and mother's demand. In primipara and multiparous women who had previous vaginal delivery, the delivery method for index birth had statistically significant association with the preference of delivery method before labor(p<0.05). All of the women who had previous c-section had delivered the index baby by c-section. Among mothers who had delivered the index baby vaginally, 84.9% of them were satisfied with their delivery method immediately after labor and 85.1% at 1 month after labor. However, mothers who had c-section stated that they are satisfied with c-section in 44.6% immediately after labor and 42.0% at 1 month after labor. Preferred delivery method for the next birth had statistically significant association with delivery method for the index birth both immediately after labor and in 1 month after labor. The proportion of mothers who prefer vaginal delivery for the next birth increased with the degree of satisfaction with the vaginal delivery for the index birth but the proportion of mothers who prefer c-section for the next birth was high and they did not change significantly with the degree of satisfaction with the c-section for the index birth. These results suggest that the current high technology-based, physician-centered prenatal and partritional cares need to be reoriented to the basic preventive and promotive technology-based, and mother-fetus-centered care. It is also suggested that active involvement of pregnant woman in decision-making process for the delivery method will increase the rate of vaginal birth after c-section and decrease c-section rate and improve the degree of maternal satisfaction after delivery.

Home Care Needs of Parturient Women and Neonates-Retrospective Study (산후 6개월동안 산모와 신생아의 가정간호 요구-후향적 방법)

  • The purpose of this study is to understand home care needs for parturient women and neonates up to the postpartum period. Methodology : The design of this study is a cross-sectional survey study. The subjects of this study are 88 postpartum mothers who delivered a first baby during the period from December 1996 to July 1997. Data collection was done with a structured questionnaire by mailing. Data were collected at the point of six months after delivery. A structured questionnaire composed of items related to home care needs. Data analysis was done with descriptive statistics. The study results are as follows : .The highest need was personal hygiene(93.2%) for the parturient women, and the peak period was the two week period after delivery. .The most frequent maternal needs regarding neonates were cord care (72.4%) during the first week, elimination(67.9%) during the first two weeks, baby crying(88.3%) and sleeping pattern(71.5%) at one month after delivery, and baby temperament(30.4%) at sixth months after delivery. .The mothers requested home care methods such as written material for self health care (35%) and counseling(34%) and direct home visits (5%) for neonate care. - Conclusion : The most important period for home health care needs was one week after delivery, and the health care needs for neonate temperament, behavior and sleeping pattern rose rapidly at the period of 6 months after delivery. Therefore it could be concluded that the postpartum home care should be done by those, written material should be enhanced for parturient women care, and counseling enhanced for neonate care.

Successful delayed-interval delivery performed 128 days after the vaginal delivery of the first fetus in a twin pregnancy

  • There has been a significant increase in the number of multiple pregnancies that are associated with a high risk of preterm delivery among Korean women. However, to date, delayed-interval delivery in women with multiple pregnancy is rare. We report a case of delayed-interval delivery performed 128 days after the vaginal delivery of the first fetus in a dichorionic diamniotic twin pregnancy. The patient presented with vaginal leakage of amniotic fluid at 16 weeks of gestation and was diagnosed with a preterm premature rupture of membranes. Three days later, the first twin was delivered, but the neonate died soon after. The second twin remained in utero, and we decided to retain the fetus in utero to reduce the morbidity and mortality associated with a preterm birth. The patient was managed with antibiotics and tocolytics. Cervical cerclage was not performed. The second twin was delivered vaginally at 34 weeks and 5 days of gestation, 128 days after the delivery of the first-born fetus. This neonate was healthy and showed normal development during the 1-year follow-up period. Based on our experience with this case, we propose that delayed-interval delivery may improve perinatal survival and decrease morbidity in the second neonate in highly selected cases.

A Longitudinal Study on Maternal Iron and Folate Status During and After Pregnancy in Korean Women (임신기간과 분만 후 모체의 철 및 엽산 영양상태의 종단적 변화)

  • Anemia in women during pregnancy and after delivery has been known to affect the mother, the fetus, and the infant's growth and health status. Studies examining, changes in iron and folate status associated with anemia during pregnancy and during pregnancy, and those supplements are stopped after postpartum. However, the effects of those have not been clearly determined in pregnant and lactating Korea women. Therefore, this study was performed to determine the changes in maternal iron and folate status during pregnancy and six months after delivery longitudinally in six pregnant women who consumed supplements from 20 wk to delivery. We concluded that the iron status deteriorated during pregnancy and especially was weak in the third trimester, but had a tendency to recovery after delivery. On the other hand, the folate status deteriorated in the first and second trimester and was good in the third trimester, but had a tendency to decrease after delivery. These results suggested that the iron status was not improved despite consuming total iron supplements of 50 mg/day through diets and supplements during the second half of the pregnancy. On the other hand, the folate status improved at the end of pregnancy by consuming folate supplements of a total of 800 mg/day through diets and supplements. However, folate status was poor in the first half of the pregnancy, and the tendency of folate status to decrease during postpartum was advanced. At the point in which iron and therefore supplementation is essential. However, the effects of supplement intake time and intake dosage need to be verified and the nutritional status changes of postpartum women should be carefully monitored.

Effects of Yoga during Pregnancy on Weight Gain, Delivery Experience and Infant's Birth Weight (임산부 요가가 임부의 체중증가, 분만경험 및 출생체중에 미치는 영향)

  • Purpose: The purpose of this study was to examine the effect of the yoga during pregnancy on the maternal weight, delivery experience and infant birth weight. Method: The nonequivalent control group pre test-post test design was used. The participants were the healthy pregnant women, whose pre-pregnancy BMI was normal, gestational period was more than 20 weeks. The final sample consisted of 21 mother-infant dyads for experimental group and 20 dyads for control group and who agreed to participate in this study. Data were collected from February I st to December 15th, 2006. The Qi exercise prenatal program was carried out for 90 minutes a day, 2 times a week for 12 weeks. The data were analyzed using SPSS 16.0 Program. Result: The degree of maternal weight gain(p<.001), labor pain(p<.001), discomfort after delivery(p<.001) and infant's birth weight(p<.00I) were significantly different between two groups. Conclusion: The yoga during pregnancy managed weight gain of mothers. Therefore, this study suggests that yoga during pregnancy to promotes stabilization can be beneficial for maintaining healthy weight, decreasing labor pain and discomfort after delivery for pregnant women and increasing infant's birth weight.

The Effects of Complex Dance Program on Body composition, BDNF & Serotonin in After Delivery Women (출산 후 여성들의 복합무용프로그램 실시가 신체구성, BDNF 및 Serotonin에 미치는 영향)

  • This study examined the effects of complex dance program(ballet, contemporary dance, yoga) on the body composition, BDNF, and serotonin of delivery women, by providing this program for 12 weeks to 14 women(within one year delivery: seven woman, 1-3 years after delivery: seven women). In the results of body composition, in the main effects in accordance with period, both groups showed significant decreases of body fat percentage. The fat-free mass was significantly increased in the group of women within one year after childbirth(t=3.821, p=.009). Regarding BDNF and serotonin, there were no interactive effects between groups and periods while the main effects did not show differences between groups and periods(p>.05). In the results of correlation analysis and regression analysis on BDNF, body composition, and serotonin, the BDNF and serotonin showed the positive correlation(p=.025). In the results of simple regression analysis, the explanatory power was 17.9%(r=.424, $r^2=.179$).

  • The purpose of this study is the fundamental study to develope basic slacks pattern for pregnant women by analysing characteristics of the lower half of their body. The results are as followers: 1. The pregnancy has changed almost all items including the waist height, lengths and etc. and changed proportion of body by accumulation of fat. 2. The changes of the waist circumference and flat figure from waist to hip could be found by analysing the body shell (pregnant women from five months pregnance to one month after delivery). 3. The postures of five month-pregnant women were compared with those of six, seven, eight, and nine month-pregnant women, and with those of one month after delivery. The waist height has been increased till the middle of pregnancy but lowed till the end of the pregnancy.

A Study on the Childbirth Experience in Primiparas (출산경험에 대한 연구)

  • This study was intended to provide women who are supposed to experience childbirth with effective nursing care exploring the childbirth experience. The purpose of this paper is to give an answer to the question of what the childbirth experience is, and phenomenological method has been used for that purpose. 17 primiparous mothers right after delivery in one university hospital of J city were served as subjects for this research. Data were collected June to October, 1993 through in depth interview with subjects using unstructured and open questions about the childbirth experience. Van Kaam's phenomenological analysis method was used for the analysis of the data. The results of the study are summarized as follows : The contents of the experience which primiparous mothers had undergone through childbirth were pain, fear, worry, relief, lightness, thankfulness, unsatisfactory, unreality, holiness for a new life, identifying the meaning of life, becoming a mother. 1. Experience before delivery Mothers experienced pain, fear, worry, unsatisfactory, relief, and thankfulness before delivery. Subjects' descriptions about the pain appeared to be various ; unbearable pain, bearable pain, anticipated pain. And their reactions were also various ours ; some endured pain, sought alternative method i.e., surgery, didn't want to reexperience it, or accepted it as women's fate. Subjects experienced fear for anticipated pain, suffering pain, and possible delayed delivery progress and were worried about delivery progress and baby wellbeing. Also, Subjects were unsatisfied with professionals' unfaithful attitude, their spouses' absence, and the ignorance of their pain. But subjects became relieved at the accustomed surroundings, good progress of delivery, support of family, care of professionals and support of other family. And they expressed their thanks to the professionals and family members for doing their best and also to their mothers who gave birth to them. 2. Experience after delivery Subjects experienced lightness, thankfulness, unsatisfactory, worry, unreality, holiness for a new life, identifying the meaning of life, becoming a mother after delivery. Mothers experienced lightness from the fact that it is over, and freshness right after delivery. They were thankful that they delivered vaginally and had an easy delivery. Subjects who delivered female baby were unsatisfied and some were worried about rearing the baby. By confessing, "I have no specific feeling, I am very confused, It is unrealistic, I feel strange", they expressed the unrealistic aspect of childbirth experience. Subjects felt holiness for a new life for novelty and birth of life. They identified the meaning of life from the fact that they felt worthy, fulfilled their duty, had their own baby, accomplished an important affair. After they experienced what it is to be a mother, they realized with it is being a mother and had a rearing expectation for the baby. The results of the study will provide basic data for caring the childbearing women.
