Peroneus Longus Dislocation associated with Trimalleolar Fracture (A Case Report) (족관절 삼과 골절과 동반된 장 비골건 탈구(1예 보고))

  • Traumatic dislocation of the peroneus longus tendon is an often unrecognized injury which has been reported to occur most commonly during sports activity. Most acute cases with early detection can be treated by simple repair of the tendon or retinaculum using one of several procedures available. Accurate treatment through a comprehensive evaluation of the ankle injury prevents suboptimal and sometimes unrecorrectable outcomes. The authors experienced concomitantly the peroneus longus tendon dislocation associated with a trimalleolar fracture of the ankle. We report this case a brief review of the literature.

Operative Treatment of Nonunion after Intra-articular Calcaneal Fracture (종골의 관절내 골절 후 발생한 불유합의 수술적 치료)

  • Purpose: Nonunion of intra-articular fractures of calcaneus is rarely reported complication. We present our experiences with 4 patients (5 cases) treated operatively for nonunion after intra-articular fracture of calcaneus. Materials and Methods: 4 patients (5 cases) with nonunion of intra-articular fracture of calcaneus after operative treatment were followed for 4 years (from 2002 to 2006). For assessment, the American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot score and Visual Analogue Scale (VAS) were assessed for clinical outcome and the union of fracture site, the talocalcaneal height and the angle of talar declination were determined for radiologic outcome. Results: The mean talocalcaneal height was 6.94 cm (range, 5.9${\sim}$7.6 cm) preoperatively and 7.34 cm (range, 6.0${\sim}$8.3 cm) at last follow-up. The mean angle of talar declination was 5.68 degrees (range, 4.6${\sim}$8 degrees) preoperatively and 13.1 degrees (range, 5.7${\sim}$21 degrees) at last follow-up. The mean preoperative AOFAS score and VAS were 20.4 (range, 14${\sim}$36) and 4 (range, 3${\sim}$6), respectively. At last follow-up, these scores improved to a mean of 59.6 (range, 54${\sim}$68) and 3 (range, 2${\sim}$4), respectively. Unions of previous nonunion site of intra-articular fracture of calcaneus were achieved in all 4 patients (5 cases). Conclusion: The reconstructive procedure for nonunion of intra-articular fracture of calcaneus showed good results in terms of bone union, radiologic results and functional improvement than preoperative state. Because the plantar pain for the inferior angular formation in nonunion site may happen, we will pay attention to reduction of fragment.

Usefulness of Computed Tomography in Patients with Acute Malleolar Fracture (급성 족근과 골절 환자에서 시행한 컴퓨터 단층촬영 영상의 유용성)

  • Purpose: We compared plain radiographs with computed tomography (CT) images to evaluate the usefulness of preoperative CT in acute ankle malleolar fracture in terms of accuracy of diagnosis and planning of operative strategy. Materials and Methods: A retrospective analysis was conducted on 210 cases of malleolar fracture treated at our institute for which plain radiograph and CT were obtained preoperatively. Observers had reviewed plain radiographs and recorded fracture classification, anatomical diagnosis, extent and configuration of fractures and then subsequently reviewed CT images. Records from each image were compared and information regarding the differences in fractures was assessed. Results: Fractures were notably changed in appearance in 88 cases (41.9%) and diagnosis changed in 30 cases (14.3%). According to the change of diagnosis and fracture appearances, the operative strategy was changed in 15 cases (7.1%) including incision, order of reduction, and target of fixation. Conclusion: CT could be a useful adjunctive imaging tool in addition to the plain radiograph in planning of operative treatment for acute malleolar fracture in terms of estimating exact configuration, extent of fractures and even newly revealed hidden fractures.

  • Fracture and dislocation of the four lateral metatarsal head and neck at the metatarsophalangeal joint, which may be associated with a hyperextension force, axial load, and additional rotating force, has rarely been reported. The patient was a 32-year-old man who sustained the injury in a motor vehicle accident. Manual reduction was easily performed but maintenance of reduction was difficult, due to the associated fractures of the metatarsal necks. Thus percutaneous internal fixation with Kirshner's wires was required.

Treatment of syndesmotic injury of ankle with bioabsorbable screw (생체흡수성 나사못을 이용한 원위경비관절 이개의 치료)

  • Purpose: Bioabsorbable screws are used to fix the syndesmotic injury of ankle because of no need for additional operation to remove it. The purpose of this study is to assess the efficacy of polylevolactic acid (PLLA) screws of in the treatment of the syndesmotic injury of ankle. Materials and Methods: Eight patients of the syndesmotic injury of ankle with malleolar fracture were evaluated in this study. They were managed with plate and screw fixation for malleolar fracture and polylevolactic acid screw fixation for syndesmotic injury, followed by plaster splinting for 6 weeks. Clinical and radiographic evaluation were done and functional scores were assessed. Results: In all cases, malleolar fractures were healed without problematic widening of syndesmosis. There were no sinus tract formation and no wound complication. Of eight patients, five had excellent results and three had good results. Conclusion: We conclude that polylevolactic acid screw is good material for fixation of the syndesmotic injury of ankle.

Postoperative Rehabilitation of Korean Medicine for Ankle Fractures Treated by Open Reduction with Internal Fixation: 2 Case Reports with 5 Months Follow-Up (개방적 내고정술을 시행한 발목 골절환자 2례에 대한 수술 후 한의 재활치료: 증례보고 및 5개월 추적관찰)

  • The purpose of this study is to report the effect of Korean medicine rehabilitation therapies in two patients with ankle fractures who underwent Open Reduction with Internal Fixation (ORIF). Two patients with fractures who received ORIF received acupuncture, electroacupuncture, herbal medicine, and physical therapy during hospitalization. Patients were evaluated for AOFAS score, NRS, ROM, and ankle circumference. Case 1 was improved from 30 points to 62 points on the AOFAS score, from 8 to 2 on the NRS, and from 33 cm to 30 cm on the ankle perimeter. ROM was improved in all directions. Case 2 showed an AOFAS score of 64 to 90 points, frome 5 to 2 on the NRS, and from 25 cm to 23.5 cm on the ankle perimeter. ROM was improved in all directions. The results of this study suggest that the treatment of Korean medicine has a meaningful effect on improvement and rehabilitation of ankle fracture patients who have received ORIF.

Clinical Evaluation of the Fracture of Talar Neck (거골 경부 골절에 대한 치료)

  • The fracture and fracture-dislocation of the neck of the talus (Hawkins' type I-IV) are uncommon injuries and represent only 0.12 to 0.32% of all fracures. Authors clinically evaluated in 12 cases Whom treated fracures of the neck of the talus, at department of orthopaedic surgery, Sun General Hospital, from 1990 to 1996, and the following results are obtained. 1. Of 12 cases, there were 11 males and 1 female, average age was 30 years. 2. Causes of fracture was fall down injury in 7 cases(58%), traffic accident in 4 cases(33%), direct trauma in 1 case(8%). 3. According to the classification by Hawkins' type I in 2 cases(17%), type II in 7cases (58%), type III in 3cases(25%). 4. Associated injuries were calcaneal fracture in 3 cases, fracture-dislocation of talus in 3 cases, subtalar dislocation in 3 cases, medial malleolar fracture in 5 cases, soft tissue injury in 3 cases, femur and tibia fracture in 1 case, and lumbar Spine compression fracture in 1 case. 5. Average time to operation after injury was 2.5 days. 6. In 2 cases were treated conservatively and 10 cases were treated open reduction and internal fixation with screw or K-wire. 7. Complications were avascular necrosis in 4 cases, post traumatic arthritis in 2 cases, skin necrosis in 4 cases, and then ankle fusion was done in 2 cases. 8. High rate of complication was seen in the talar neck fracture associated with calcaneal fracture. In the analysis of above results, evaluated by Hawkins' scoring system were excellent to fair in 75%.

Ankle Arthroscopy for Occult Injuries in the Acute Ankle Fracture (급성 족 관절 골절에서 잠재 손상에 대한 족 관절경)

  • Purpose : To identify occult intra-articular pathologies in the acute ankle fracture using arthroscopy and to investigate the factors to predict its possibility of occult injuries that could be occurred. Materials and Methods : This prospective study included fifty patients who got operation for the ankle fractures. Ankle arthroscopy was performed to document the type and anatomical location of occult lesion. We investigated the correlation between incidence of occult injuries and specific fracture type. Results : Of fifty ankles, 37 ankles had occult lesion. Twenty five loose bodies including displaced fragments were found and various cartilage lesions were found in 31 ankles. Tibial avulsion fragment by anterior inferior tibiofibular ligament was occurred in 6 cases. There was no correlation between the incidence of occult lesion and various factors including age, sex, injury mechanism and fracture type(p>0.05). Conclusion : The ankle arthroscopy had an effective role for the detection and treatment of occult injuries and it was difficult to predict the occurrence or associated accurately in all of th ankle fractures.

Arthroscope-guided Closed Reduction and Internal Fixation of the Lateral Malleolar Fracture (관절경을 이용한 족관절 외과 골절의 비관혈적 정복술 및 내고정술)

  • Purpose: In this study, we introduced an newly developed technique of operation for fracture of lateral malleolus of the ankle. We treated the fracture by close reduction and internal fixation using arthroscopy. Materials and Methods: From July 2006 to June 2007, we had treated 23 cases of lateral malleolar fracture (SER type) by closed reduction and internal fixation with arthroscopy and followed them up more six month. Operation time, union time, clinical and functional result were evaluated. Results: After the final follow-up, all the fractures were healed with satisfactory bony union. The subjective result was excellent in 15 cases (65%), good in 8 cases (35%), the objective result was excellent in 13 cases (57%), good in 10 cases (43%), and the roentgenographic result was excellent in 17 cases (74%), good in 6 cases (26%). Conclusion: Closed reduction and internal fixation with arthroscopy technique is an effective treatment method in treating lateral malleolar fracture of the ankle since it offers advantages including corrective anatomical reduction and minimizing complication associated with injury of soft tissue.
