A case of Idiopathic Bilateral Brachial Neuritis Involving the Bilateral Phrenic Nerves (양측 횡격막신경을 침범한 원인불명의 양측 상완신경염 1예)

  • Bilateral brachial neuritis is clinically uncommon and accidentally involvement of bilateral phrenic nerves is rarely reported. We experienced a 26 year old man who developed subacute onset of asymmetric bilateral shoulder and arm weakness. The weakness slowly aggravated and finally suffered from dyspnea due to bilateral phrenic nerve palsy. Cervical spine MRI and CSF study showed no abnormality. Viral markers and other serological test showed no specific finding. Electromyographic study showed bilateral brachial axonal polyneuropathy with cervical and upper thoracic polyradiculopathy. And bilateral phrenic nerve conduction study showed no resopnse. He showed no improvement for 10 months after treatment and managed with continuous artificial ventilation. We report a case of idiopathic bilateral brachial neuritis accidentally involving bilateral phrenic nerves.

Surgical Treatment of Bilateral Bronchiectasis (양측성 기관지 환장증의 수술적 치료에 관한 검토)

  • We reviewed medical records of 179 patients with bronchiectasis admitted in our department in our department in Pusan Paik Hospital, Inje University from Dec. 1979 to Jun. 1989. Surgical resections were performed in 167 patients [bilateral disease 29, unilateral 138]. To evaluate the results of surgical treatment of bilateral bronchiectasis, it was compared with that of unilateral one. The results were as follows: 1.Bilateral disease was 20.1% of all patients with bronchiectasis. The average age of patients with bilateral disease treated surgically was 28.8 2. There was no significant difference in age, clinical symptoms and disease distribution between bilateral and unilateral bronchiectasis. 3. The numbers of diseased segments in bronchography were 6.86$\pm$2.00 and 4.33$\pm$2.01 in bilateral and unilateral diseases respectively. 4. Six patients underwent bilateral resections, and 25 patients had some of diseased segment [average 2.24$\pm$1.51] after resections, 5. After resections, 48.3% of patients with bilateral disease were free of symptoms and it was less than that in the unilateral one [87%]. But the remaining 51.7% of bilateral disease presented much improved clinical symptoms without aggravation.

  • Simultaneous bilateral bleb resection was done through bilateral transaxillary thoracotomy in 10 patients with spontaneous pneumothorax during the period from May 1991 to Novemver 1992 in whom bilateral bulla or bleb was detected with using simple chest X-ray and chest CT scanning. To compare the effectiveness of bilateral transaxillary thoracotomy, we investigated 10 unilateral transaxillary thoracotomy patients with spontaneous pneumothorax and two clinical reports from other institutes which dealt the results of bilateral bleb or bulla resection through median sternotomy also. In bilateral transaxillary thoracotomy group,mean operation time was 115 minute,mean intraoperative bleeding was 329 cc, mean postoperative hospital stay was 7.5 days. Postoperative ABGA[Arterial Blood Gas Analysis] was in normal range and postoperative recovery rates of FVC[Forced Vital Capacity], FEV1[Forced Expiratory Volume at 1 second], TV[Tidal Volume] were 84.3%, 93.4%, 88.7%,respectively. In median sternotomy group,mean operation time was 129 minute,mean intraoperative bleeding was 490 cc, mean postoperative hospital stay was 12.4 days. Postoperative ABGA was in normal range and postoperative recovery rates of FVC, FEV1 were 97.3%, 97.4%, respectively. In unilateral transaxillary thoracotomy group, postoperative ABGA was in normal range also and postoperative recovery rates of FVC, FEV1, TV were 91.6%, 99.0%, 96.0%,respectively. In conclusion, simultaneous bilateral bleb resection through bilateral transaxillary thoracotomy should be considered in pneumothorax patients with bilateral bleb or bulla because of cost-effectiveness[reducing hospital days] and better cosmetic result without any impairment in recovery of respiratory function.

  • Objective : The purpose of this study were to investigate perception of occupational therapist and the current conditions about bilateral upper extremity training for patients with stroke. Methods : A survey was conducted on 10 occupational therapists for a month from June, 2016. The questionnaire was consisted total 17 items including 5 area related to characteristics of occupational therapist and patient, bilateral activities with difficulty to perform, bilateral activities used in bilateral training, and bilateral activities which are wanted for therapist to apply in bilateral training. Results : Drive a nail was bilateral activity with difficulty to perform for stroke patient, and unscrew lid of jar was bilateral activity used in bilateral training by occupational therapists. Also, wash one's face was bilateral activity wanted for therapists to use for bilateral training. Conclusions : This study will be used as basic data to select the bilateral upper extremity activities for bilateral upper extremity training.

A Study on the Determinants of Bilateral Trade : Evidence from China and US

  • Purpose - Recently, the trade war between China and US has been escalating, which has also attracted worldwide attention. Based on this background, this paper sets China and US as an example to explore the determinants of bilateral trade between China and US. Research design, date, and methodology - A quarterly data from the 2000-Q1 to the 2017-Q4 will be used to perform an empirical analysis under some econometric approaches such as the fully modified least squares and the vector error correction estimates. Result - The results illustrate that the two economic entities of China and US have the greatest positive effect on bilateral trade between China and US. The real exchange rate has a positive effect on bilateral trade between China and US. The nominal exchange rate has a negative effect on bilateral trade between China and US in the short run. US's average price has a positive effect on bilateral trade between China and US in the short run. China's average price has a negative effect on bilateral trade between China and US in the short run. Meanwhile, the bilateral trade between China and US also suffers from the economic crisis happened in 2008. Even through the bilateral trade between China and US in the short run is deviate from the long-run equilibrium, there exist an error correction mechanism back to the long-run equilibrium. Conclusion - This paper provides some empirical evidences for both governments. Based on the results of this paper, both governments should take corresponding measures to promote the development of bilateral trade between China and US.

Bilateral Shoulder Involvement with Mirror Image Lesion -An Arthroscopic Study in Overhead Workers (양측 견관절을 침범한 거울 병변 -머리 위의 작업을 하는 근로자 대상 관절경적 연구)

  • Purpose: To demonstrate arthroscopically the presence of bilateral shoulder "mirror lesions" due to overhead work. Materials and Methods: A cross-sectional study was performed in a historical cohort of 10 male tire plant workers who underwent MRI studies of both shoulders. Bilateral intraarticular shoulder pathology was diagnosed by magnetic resonance imaging and confirmed by arthroscopy. "Mirror lesions" were defined as similar pathology between an individual patient's two shoulders. Results: We report on ten patients who had bilateral "mirror lesions." The most common mirror lesions were tears of the rotator cuff. Most patients (7 of 10) had more than one mirror lesion in their shoulders. There were two bilateral full thickness tears involving the supraspinatus and infraspinatus, four bilateral partial thickness supraspinatus tears, six bilateral partial or upper corner lesion complete subscapularis tears, and one bilateral complete supraspinatus tear. Conclusion: Occupational overhead work with simultaneous use of both arms is associated with bilateral shoulder lesions verified by arthroscopy. By demonstrating the work-related bilateral shoulder involvement, this study suggests an etiopathogenesis for these lesions and provides rational for developing worksite prevention strategies.

Bilateral Pulmonary Sequestrations - A Case Report - (양측성 폐 격리증;1례보고)

  • Bilateral intralobar and extralobar pulmonary sequestration is an extremely rare anomaly. At present, four pathologically proven cases and two possible cases have been reported in the literature insofar as we know. We have found no previous reports in the literature in which simultaneous bilateral intralobar and extralobar pulmonary sequestration were present. We report a first case presenting as bilateral intralobar and extralobar pulmonary sequestration, recently performed staged bilateral resection, and emphasize the importance of computed tomography and angiography in diagnosis and operation.

  • Air space disorders are usually considered medical diseases, although some patients with air space disorders can benefit from surgical intervention. Recently we experienced two cases of bullous emphysema. One case is large bilateral apical bullae and the other is infected large bulls of RUL with bullous emphysema. The patient with large bilateral apical bullae underwent simultaneous operation via bilateral thoracotomy and other patient underwent simultaneous bilateral operation via median sternotomy. Postoperatively, the patient with large bilateral apical bullae showed subjective as well as objective improvement and other patient is resulted subjective improvement.

Bilateral Breast Reconstruction with Free TRAM Flaps (횡복직근 유리피판술에 의한 양측 유방 재건)

  • Free TRAM flap is now increasingly suggested to patients requiring breast reconstruction after the mastectomy. This study is to introduce the experiences of bilateral free TRAM flaps for reconstruction of bilateral breasts and to suggest the way of getting the more satisfactory results. A total of 6 breasts were reconstructed in 3 patients using bilateral free TRAM flaps immediately following the mastectomy. Average operative time for bilateral breast reconstruction was 8 hours comparing to 6.5 hours for unilateral breast reconstruction. Partial or total flap loss did not occur in 6 flaps. Abdomen was repaired directly with muscle and fascia sparing technique without necessity of mesh graft. There was no complication in donor site like abdominal hernia. Bilateral breast reconstruction can achieve exceptionally good aesthetic result with low complication if it is performed with skillful technique and experience. The reason for this is that fairly good symmetry usually is obtained in the initial surgery and in most cases only minimal additional surgery is required to achieve a satisfactory aesthetic result. The one disadvantage of bilateral reconstruction with autologous tissue is the length of the surgical procedure. Although the initial bilateral breast reconstruction can be a long, tedious procedure if free flaps are used, it must be a valuable treatment option for bilaterally mastectomized patients.

Bilateral Superior Cerebellar Artery Infarction after Stent-Angioplasty for Internal Carotid Artery Stenosis

  • Spontaneous bilateral cerebellar infarction in the territory of the superior cerebellar arteries is extremely rare. Occasionally there have been reports of bilateral cerebellar infarction due to vertebrobasilar atherosclerotic occlusion or stenosis, whereas no report of bilateral cerebellar infarction due to complicated hemodynamic changes. In this report, we present a patient with bilateral cerebral infarctions related to stenoses of bilateral internal carotid arteries, in whom vertebrobasilar system was supplied by multiple collaterals from both posterior communicating arteries and right external carotid artery. We performed stent-angioplasty of bilateral internal cerebral arterial stenosis, and then acute infarction developed on bilateral superior cerebellar artery territories. The authors assumed that the infarction occurred due to hemodynamic change between internal carotid artery and external carotid artery after stent-angioplasty for stenosis of right internal carotid artery.
