T2 Relaxographic Mapping using 8-echo CPMG MRI Pulse Sequence

  • The mapping of the spin-spin relaxation time T2 in pixed-by-pixel was suggested as a quantitative diagnostic tool in medicine. Although the CPMG pulse sequence has been known to be the best pulse sequence for T2 measurement in physics NMR, the supplied pulse sequence by the manufacture of MRI system was able to obtain the maximum of 4 CPMG images. Eight or more images with different echo time TEs are required to construct a reliable T2 map, so that two or more acquisitions were required, which easily took more than 10 minutes. 4-echo CPMG imaging pulse sequence was modified to generate the maximum of 8 MR images with evenly spaced echo time TEs. In human MR imaging, since patients tend to move at least several pixels between the different acquisitions, 8-echo CPMG imaging sequence reduces the acquisition time and may remove any misregistration of each pixel's signal for the fitting T2. The resultant T2 maps using the theoretically simulated images and using the MR images of the human brain suggested that 8 echo CPMG sequence with short echo spacing such as 17∼20 msec can give the reliable T2 map.

조기발병 정신분열병 환아에서 SPECT로 측정한 국소 뇌혈류량 이상 : SPM(Statistical Parametric Mapping) 분석 (Regional Cerebral Perfusion Abnormalities Assessed with Brain SPECT in Early-onset Schizophrenia : Statistical Parametric Mapping Analysis)

  • Objectives : A voxel based investigation of cerebral blood flow was conducted to identify functional differences during resting state between children with early-onset schizophrenia and normal controls. Methods : 19 children and adolescents with early-onset schizophrenia(8 boys and 11 girls, mean age $14.0{\pm}1.7$ years old) and 17 comparison children(13 boys and 4 girls, mean age $11.0{\pm}1.9$ years old) were examined by HMPAO-SPECT. The SPECT images were compared using statistical parametric mapping analyses, controlling for age and sex. Results : Increased cerebral blood flow in left medial and inferior frontal gyrus, right superior and middle frontal gyrus, both inferior temporal gyrus, and right cerebellar tonsil was found in children and adolescents with early-onset schizophrenia compared to control subjects. In addition, decreased cerebral blood flow in right thalamus, left posterior cingulate gyrus, right anterior cingulate gyrus and relatively wide areas from left medical frontal gyrus to superior parietal lobule were found in children and adolescents with early-onset schizophrenia compared to control subjects. Conclusion : The results of the current study provide additional evidences for brain areas involving the onset of schizophrenia in early age.

뇌열 1예의 기능적 자기공명영상과 경두부 자기자극 (Functional-Magnetic Resonance Imaging and Transcranial Magnetic Stimulation in a Case of Schizencephaly)

  • 목적 : 뇌열환자에서 기능적 자기공명영상과 경두부 자기자극을 이용하여 뇌기능을 지도화하고자하였다. 대상 및 방법: 자기공명영상에서 우측 대뇌반구에 뇌열 소견이 있으며 좌측 편측부전마비를 보인 28세 남자환자를 대상으로 기능적 자기공명영상과 경두부 자기자극을 시행하였다. 임상적으로 좌측 손의 운동기능은 감소되어 있었고, 우측 손의 운동기능은 정상범주에 속하였다. 뇌기능적 자기공명영상은 EPI 기법을 이용하였고 운동자극은 1-2 Hz의 주기로 손가락을 아래위로 구부리게 하는 운동을 시행하였고 15초의 휴식기와 15초의 운동기를 반복하여 절편당 60 개의 영상을 획득하였다. 두부자기자극은 지름 90mm의 원형 자성자극기를 이용하여 maximal out-put의 80%로 자극하여 양측 단무지 외 전근에서 유발된 운동유발전위의 잠시와 진폭을 구하였다. 결과: 기능적 자기공명영상을 시행한 결과 정상적인 우측손의 운동자극시에 좌측 운동피질이 활성화되었고 좌측손의 운동자극시에는 좌측운동피질, 좌측 부가운동영역, 그리고 자측 전운동영역에 활성화소견이 나타났다. 두부자기자극에서는 우측 대뇌반구에서는 한군데에서도 운동유발 전위가 발생되지 않았다. 좌측 대뇌반구에서는 5군데에서 운동유발전위가 유발되었으며 모두 양측 단무지 외 전근에서 운동유발전위가 유발되었다. 양손에서 운동유발전위의 잠시 , 진폭 모양이 유사하였다. 결론: 뇌열환자의 손운동기능의 피지도화는 기능적 자기공명영상과 두부자기자극을 이용하여 성공적으로 시행할 수 있었다. 뇌열환자의 동측 운동경로는 동일한 운동피질로부터 기원한 동측피질척수로에 의한 것으로 추정된다.

  • Objectives : Acupuncture has been widely applied to rehabilitation after stroke by Oriental medical doctors in Korea. It has been reported that acupuncture increased cerebral blood supply and stimulated the functional activity of brain nerve cells. In addition, a correlation between activation of specific areas of brain cortices and corresponding acupuncture stimulation at the therapeutic points had been well illustrated. rill now, however, there were few studies which evaluated a correlation between activation of specific areas of brain and frequently-used acupuncture therapy for stroke, such as LI4-LI11 electro-acupuncture (EA) for paresis after stroke. This study was undertaken to evaluate the effect of LI4-LI11 EA on regional cerebral blood flow (rCBF) in normal volunteers using Single Photon Emission Computed Tomography (SPECT). Methods : In the resting state, $^{99m}Tc-ECD$ brain SPECT scans were performed on 10 normal volunteers (8 males, 2 females, mean age $25.6{\pm}2.3$years; age range from 24 to 31 years). 7 days after the resting examination, 15 minutes of electro-acupuncture were applied at LI 4 and LI 11 on the right side of the subjects. Immediately after LI4-LI11 EA, the second SPECT images were obtained in the same manner as the resting state. Significant increases and decreases of regional cerebral blood flow after LI4-LI11 EA were estimated by comparing their SPECT images with those of the resting state using paired t statistics at every voxel, which were analyzed by Statistical parametric mapping with a threshold of p=0.001, uncorrected (extent threshold: k=100 voxels). Results : EA applied at right LI4-LI11 increased rCBF in right frontopolar area (Brodmann area 10) and left middle frontal area (Brodmann area 46). Interestingly, all the areas showing increased rCBF corresponded to the territories of both anterior cerebral arteries. However, LI4-11 EA decreased rCBF in the left occipital lobe (peristriate area, Brodmann area 19). Conclusions : The results demonstrated a correlation between LI4-11 EA and rCBF increase in the frontal lobes. It is also suggested that there may be a correlation between LI meridian and the territory of the anterior cerebral arterties.

  • Objectives: Acupuncture has been applied in Asia for thousands of years, especially to rehabilitation after stroke. It has been reported that acupuncture increased cerebral blood supply and stimulated the functional activity of brain nerve cells shown by using brain imaging techniques. This study was to evaluate the effect of GB 34-GB 39 electro-acupuncture (EA) on regional cerebral blood flow (rCBF) in stroke patients and normal volunteers using single photon emission computed tomography (SPECT). Methods: The study procedure was divided into two parts: patients and volunteers studies. For the patients study, ten ischemic stroke patients (3 males, 7 females, mean age $68.5{\pm}8.9$ years old) were selected. Baseline brain SPECT was done with triple head gamma camera (MultiSPECT3, Siemens, USA) after intravenous administration of 1,110 MBq of $^{99m}Tc-ECD$. Fifteen-minute EA at GB 34 and GB 39 were applied on the affected limb. The same dose of $^{99m}Tc-ECD$ was injected during the EA, and the second set of SPECT images wasobtained. Using the computer software (ICON 7.1, Siemens, USA), 3 SPECT slices (upper, middle, lower) surrounding the brain lesion were selected and each slice was divided into 10-16 brain regions. Asymmetry indexes (AI) were analyzed in each brain region. We regarded over 10% changes of AI between before and after EA as significance. For the volunteers study, 10 healthy human volunteers (5 males, 5 females, mean age $28.1{\pm}6$ years old) were selected. In the resting state, $^{99m}Tc-ECD$ brain SPECT scans were performed. On the 7th day after the resting examination, 15 minute EA was applied at GB 34 and GB 39 on the right side of the subjects. Immediately after EA, the second SPECT images were obtained inthe same manner as the resting state. Significant increases and decreases of rCBF after EA were estimated by comparing their SPECT images with those of the resting state using paired t statistics at every voxel, which were analyzed by statistical parametric mapping with a threshold of p = 0.01, uncorrected (extent threshold: k=100 voxels). Results: In stroke patients, six of the eight (75%) had significantly increased perfusion in post-acupuncture scans compared to their baseline state. In normal volunteers, GB 34-GB GB EA increased rCBF in both hemispheres including right ventral posterior cingulate (Brodmann area (BA) 23), left superior temporal, anterior transverse temporal (BA 22, 41), left parastriate, peristriate (BA 18, 19), right occipitotemporal, angular (BA 37, 39), left rostral postcentral, caudal postcentral and preparietal (BA 2, 3, 5). However GB 34-GB 39 EA decreased rCBF in the right hemisphere including triangular and middle frontal lobes. Conclusions: The results demonstrated that OB 34-GB 39 EA increased cerebral perfusion in ischemic stroke patients and increased rCBF grossly in temporal lobes of normal volunteers. It is also suggested that there may be a correlation between the GB meridian and the territory of the middle cerebral artery.

  • EEG signal is analyzed by two methods, analysis by visual inspection of EEG recording sheets and analysis by quantative method. Generally visual inspection method is used in the clinical field. But this method has its limitation because EEG signal is random signal. Therefore it is necessary to analyze EEG signals quantatively to obtain more precise and objective information of neural and brain. In this paper, power spectrum of EEG signal was estimated by AR(AutoRegressive) model in the frequency domain. This process is useful as a preprocessing stage for tomographic brain mapping (TBM) at each frequency, band. As a method for estimating power spectral density of EEG signals, periodogram method, autocorrelation method. covariance method, modified covariance method, and Burg method are tested in this paper.

  • 증감뇌유발전위와 성격의 상호관계의 영상적 분포를 조사하기 위해 38명의 대학생에게 증감뇌유발전위를 검사하였다. 그리고 Zuckerman의 추구척도와 Eysenck 성격설문지로 그들의 성격을 조사하였다. 외향 및 내향성 척도와 증감뇌유발 전위 사이에 후 전두엽 부위에서는 유의한 정상관관계가 있었으나 우측 후반구에서는 유의한 부상관 관계를 보였다. 스릴과 모험추구 척도와는 우측 후 측두엽에서 유의한 부상관 관계를 보였다. 모든 검사자의 평균 증감뇌유발전위는 전두엽 부위에서는 증가 경사도를 보였으나 후반구 부위에서는 감소경사도를 보였다.

  • Sensory substitution involves mapping the characteristics of one sensory modality to the stimuli of another sensory modality. In general, when a person is visually impaired or deaf, they do not actually lose their ability to see or hear completely; however, they only lose their ability to transmit sensory signals from the periphery to the brain. It has been experimentally proven that a person who has lost the ability to retrieve data from the retina can still visualize subjective images by using data transferred from other sensory modalities such as tactile or auditory modalities. This is because vision processing pathways are still intact in most cases. Therefore, sensory substitution uses human perception and the plasticity of the human brain to transmit sensory signals through pathways that have not been lost. In this study, we analyze the characteristics and problems of various devices used for sensory substitution and summarize the recent technological trends in these devices.

건강한 성인의 양측상지기능의 상상훈련과 신체적 수행의 대뇌 연결성 비교: 사례 연구 (Comparison of Brain Connectivity in Mental Practice and Physical Performance of Bilateral Upper Extremity Function in a Healthy Adult: A Case Study)

  • 목적: 본 연구의 목적은 건강한 성인을 대상으로 양측상지기능 훈련을 통해서 활성화 되는 대뇌 연결성에서 상상 훈련과 신체적 수행 간에 차이가 있는지를 알아보고자 한다. 연구방법: 연구 대상자는 건강한 성인 1명으로 상상훈련과 신체적 수행 시 EEG 측정이 이루어졌다. 양측상지기능 훈련은 대칭 과제와 비대칭 과제로 구성되었다. 대칭 과제는 양손으로 박스를 잡고 동시에 위의 선반으로 올렸다가 다시 내려놓는 과제이고, 비대칭 과제는 한 손으로 병을 잡고 다른 한 손으로 뚜껑을 여는 과제였다. EEG 전극은 Fp1, Fp2, F3, F4, T3, T4, P3 및 P4에 부착되었다. 데이터 분석은 EEG 전극 쌍 간의 상관 분석을 위해 Cross-Line Mapping을 사용하였다. 결론: 본 연구 결과 대칭 및 비대칭의 양측 상지 과제에서 대뇌 연결성 패턴은 운동과 감각 영역에서 유사한 패턴을 가지는 것으로 나타났다. 또한 본 연구를 통해 양측상지기능 훈련 시 상상훈련보다 신체적 수행에서 대뇌 연결성이 더 높은 상호상관을 갖는다는 것을 확인하였다.

3차원 영상의 깊이 인식에 대한 매핑 알고리즘 구현 (A New Mapping Algorithm for Depth Perception in 3D Screen and Its Implementation)

  • 본 연구에서는 입체영상을 위한 실시간 스테레오 화면을 생성하는 기법 중에서 특별히 깊이 감도를 충실하게 구현할 수 있는 새로운 알고리즘에 대하여 다루어 본다. 제안한 내용 중에는 깊이 질감에 간접적 영향을 주는 빛의 강도를 물리학에서 이용되는 라플라시안 방정식에 근거를 두어 영상처리를 하는 개념도 다루어 본다. 본 논문에서 다루어 진 내용들은 N.Holliman 교수의 연구 내용 중 다중영역 알고리즘과 유사하나, 큰 차이점은 본 연구에서는 관찰자와 피사체 사이의 거리를 고려하고, 여기에 라플라시안 방정식의 개념을 도입한 부분이다. 제안된 알고리즘은 원형편광 처리가 된 LCD 화면에 OpenGL 라이브러리 함수를 이용하여 구현되었으며, 실제 인체의 시각의 생체적인 기능 하에서 입체적인 느낌이 잘 발휘되도록 처리를 하였다 비록 현재는 가상적으로 생성한 입체화된 물체에 대하여 컴퓨터 시뮬레이션을 시도하였지만, 본 논문에서 다루어진 알고리즘은 개인 컴퓨터의 편광모니터는 물론 공중파용 입체 단말기에도 적용가능하리라 사료된다.
