입천장성형술 시 발생한 골 노출부의 피복을 위한 협지방대 유경피판의 경험 (Clinical Experience of Buccal Fat Pad Pedicled Flap for Denuded Area in Palatoplasty)

  • Purpose: The primary goal of palatoplasty is to enable normal speech with harmonious growth of face. Some children who had palatoplasty display typical findings of transverse maxillary deficiency requiring orthodontic widening of the maxilla. Levi (2009) described a cleft palate repair coupled with pedicled buccal fat pad flaps to cover bone exposed areas of the hard palate. Hence we report clinical experiences of cleft palate repair using pedicled buccal fat pad flap. Methods: Four Veau class II and a Veau class I cleft palate patients underwent palatoplasty with buccal fat pad flap by single surgeon from April 2009 to August 2009. Two patients received 2-flap palatoplasty and three patients 1-flap palatoplasty, respectively. After the cleft palate repair, sharp mosquito scissors was placed in the superior buccal sulcus just lateral to the maxillary tuberosity and inserted directly through the mucosa resulting in buccal fat pad extrusion. The elevated flap was moved to cover mucoperiosteal defect in hard palatal area. Results: Five patients underwent primary palatoplasty using buccal fat pad flap. Flap harvest and inset took on average 9 minutes per flap. Mucosal epithelization took 18 days on average. No patients had complications related to the buccal fat pad flap. Conclusion: Buccal fat pad pedicled flap has significant potential to function as an added vascularized tissue layer in cleft palate repair and we can expect better growth of maxilla with this method although longer duration of follow-up was unavailable.

안면 비대칭 환자에서 Cone Beam Computed Tomography를 이용한 하악골 해부학적 변이의 분석 (Cone Beam Computed Tomography Analysis of Mandibular Anatomical Variation in a Patient with Facial Asymmetry)

  • Purpose: The study was performed to compare patients with anatomical variations in facial asymmetry with patients in the normal range using cone-beam computed tomography (CBCT) and to take the preoperative condition into consideration in the case of a sagittal split ramus osteotomy (SSRO). Methods: The study was conducted on 46 adult patients composed of 2 subdivided groups, an asymmetry group (n=26) and a symmetry group (n=20). The asymmetry group was divided between patients with hemimandibular hyperplasia (HH, n=8) and hemimandibular elongation (HE, n=18). Using cross-sectional computed tomography images, the thickness of cancelleous bone in the buccal area of the mandible, thickness of buccal cortex in the buccal aspect of the mandible, thickness of cancellous bone in the inferior aspect of the mandible, thickness of buccal cortex in the inferior aspect of the mandible, and cross-sectional surface area of the mandible were measured. Results: In the asymmetry group, the cross-sectional area of the mandible including the inferior alveolar nerve positioned on the affected side was significantly different from the symmetry group. Thickness of cancelleous bone in the buccal aspect of the mandible, thickness of cancelleous bone in the inferior aspect of the mandible, and cross-sectional surface area of the mandible in the affected site of hemimandibular hyperplasia was significantly smaller than in the symmetry group. Conclusion: The inferior alveolar nerve runs lower and in a more buccal direction and shows a smaller cross-sectional surface of the mandible in the hemimandibular hyperplasia patients with asymmetry.

A Study of the Electrical Properties of the Buccal Area using Facial Surface Electromyography

  • Objectives : The purpose of this study was to determine the electrical properties of the buccal area using facial surface electromyography (sEMG). Methods : This research was conducted on 44 healthy participants irrespective of their sex. Surface electrodes were attached to the midpoints of three imaginary lines connecting ST4 (Dicang) to ST6 (Jiache), ST4 to SI18 (Quanliao), and ST4 to the center point of SI18 and ST6. Then, the participants were trained in the movement that included a comprehensive action of buccal area. While the participants were performing the motion, sEMG values ($E_1$, $E_2$, $E_3$) and the distance change of the three imaginary lines ($D_1$, $D_2$, $D_3$) were measured. The data were statistically analyzed using SPSS ver. 22.0. Results : Significant differences were observed in the distance changes ($D_1$>$D_3$, $D_2$>$D_3$) and sEMG values ($E_1$<$E_2$<$E_3$). Moreover, there were positive correlations between $D_1$ and $E_1$, $D_2$ and $E_2$. Conclusion : We suggest that the measurement at ST4 to the center point of ST6 and SI18 with this motion would be adequate to check the electrical characteristics of the buccal area.

구강점막 결손 재건시 유경협지방대이식술의 임상적 적용 (THE USE OF BUCCAL FAT PAD AS A PEDICLED GRAFT FOR THE RECONSTRUCTION OF ORAL MUCOSAL DEFECT)

  • Various local flaps and distant flaps including tongue flap, palatal island flap, and buccal flap as well as skin grafts have been used for the reconstruction of oral mucosal defect. In the posterior region of oral cavity and the buccal cheek area, buccal fat pad can be used as a pedicled graft. The buccal fat pad is different from other subcutaneous fat tissue and it is easily accessible. There are many advantages in pedicled buccal fat pad graft for the closure of oral mucosal defect. The procedure is easy, there is no visible scar in the donor site, it is capable of reconstruction of various contour, and it has good viability. We had used buccal fat pad as a pedicled graft for the closure of oral mucosal defect after the excision of tumor and the oroantral fistula. From the results of these cases, we concluded that the use of the buccal fat pad flaps was worth of the consideration for the reconstruction of oral mucosal defect in the regions of the buccal cheek, and posterior oral cavity.

상악 구치 부에서 골계생술시 협지방체를 이용한 연조직 피개 : 문헌 고찰 및 증례보고 (The use of the buccal fat pad for guided bone regeneration in posterior maxilla: Review of the literature and report of 2 cases)

  • For the successful guided bone regeneration(GBR) of maxillary bony defect, proper soft tissue coverage is one of the most important things. Soft tissue dehiscence can be most common reason of osseous reconstruction failure. If a vascular supply to the graft should not develop from the host tissue, then the graft may also foil. Both of these prerequisites can be aided by judicious use of the buccal fat pad(BFP). Many methods for adequate soft tissue coverage have been proposed and the use of the BFP is one of them. BFP is useful in posterior maxillary area, can cover larger area and have higher blood flow than other methods. so the use of the BFP may offer protection and early blood supply to maxillary bone graft. This report describes the history, anatomy, blood flow, and clinical usefulness with two clinical cases.

  • The differential diagnosis of lesions in buccal area include lipoma, neurofibromas, epidermoid cyst, salivary ducts calculus, hemangioma, lymphadenopathy. Accessory parotid glands is defined as salivary gland tissue adjacent to the parotid duct, but separated from the body of parotid and it may be found in approximately 20% of human parotid glands. The appearance of an accessory parotid tumor is rare, with a reported frequency of 7.7% of all parotid neoplasm. Angiomyoma, which is also termed angioleiomyoma, is a rare solitary subcutaneous tumors arising from the vascular smooth muscle. It often occur in the extremities and is rarely found in buccal area. We present 2 cases of rare tumor in buccal mass and resected surgically without facial nerve palsy.

Maxillary Posterior Crossbite Appliance의 적용시 응력 분포에 관한 광탄성법적 연구 (Photoelastic evaluation of Maxillary Posterior Crossbite Appliance)

  • 본 연구는 협측 이소맹출된 상악 제2대구치의 협측 반대교합을 개선하기 위해 사용되는 maxillary posterior crossbite appliance에 의해 상악 제2대구치의 치근단과 그 주위의 치조골에 발생되는 응력분포를 알아보기 위해 광탄성법을 이용하여 분석하였다. 상악의 치조골을 재현하기 위해 PL-3형의 epoxy resin과 PL-3보다 경질인 레진치아를 사용하여 협측 이소맹출된 상악 제2대구치를 광탄성모형으로 재현하였다. 광탄성 모형상에 maxillary posterior crossbite appliance를 적용하고 힘을 가하기 전과후의 응력분포를 알아보기 위해 원형편광기를 사용하여 모형의 전후방에서 관찰하였다. 이상의 연구를 통해 얻어진 결과는 다음과 같다. 1. 상악 제2대구치의 구개면에 힘을 가한 경우, 협측과 구개측 치근첨에는 무늬차수 0.6차와 치경부에는 무늬차수 0.9차의 응력이 집중이 되어 나타났고 회전중심이 구개측 치근은 치근첨에, 협측 치근은 치근 1/4부위에 발생하였으며 이로 인해 제2대구치에 구개측으로의 비조절성 경사이동 및 회전력이 발생하였다. 2. 상악 제2대구치의 협측면에 힘을 가한 경우에는 구개면에 힘을 가한 경우 보다 협측과 구개측의 치근첨에 더 많은 무늬차수 2.2차의 응력이 발생하였다. 또한 치근하방의 치조골 부위에 응력의 발생과 함께 구개측 치근의 회전중심은 사라지고 협측 치근은 치근첨으로 이동하여 이로 인해 구개측으로의 조절성 경사이동 및 함입력이 발생하였다. 3. 상악 제2대구치의 협측이나 설측에 힘을 가한 경우, 고정원인 제1대구치는 치근첨부위에 소량의 응력이 발생하였고, 편측의 제2대구치에만 힘을 가한 경우보다 양측의 제2대구치에 동시에 힘을 가한 경우에서 더 많은 양의 응력이 제1대구치에 발생하였다. 이상의 결과는 협측 이소맹출된 상악 제2대구치의 구치부 협측 반대교합을 개선하기 위해 사용되는 maxillary posterior crossbite appliance를 적용할 때, 제1대구치의 고정원 상실을 최소화하기 위해서는 편측의 제2대구치를 먼저 개선시키고 그 후 반대측 제2대구치의 개선을 도모하는 것이 바람직하다는 것을 의미한다. 또한 구개측면보다는 협측면에서 견인력을 가하여 제2대구치에 함입력과 구개측으로의 조절성 경사이동을 발생시키는 것이 구치의 정출 및 회전에 의해 야기될 수 있는 교합장애를 방지할 수 있으리라 사료된다.

Anthropometric analysis of maxillary anterior buccal bone of Korean adults using cone-beam CT

  • PURPOSE. The aim of this study was to evaluate the thickness of buccal and palatal alveolar bone and buccal bony curvature below root apex in maxillary anterior teeth of Korean adults using Cone-beam CT images. MATERIALS AND METHODS. The 3D image was reconstructed with dicom file obtained through CBCT from 20 - 39 year old Korean subjects (n = 20). The thickness of buccal and palatal plate, root diameter, the buccal bony curvature angle below root apex and the distance from root apex to the deepest point of buccal bony curvature were measured on maxillary anterior teeth area using OnDemand3D program. RESULTS. Mean thickness of buccal plate 3 mm below CEJ was $0.68{\pm}0.29\;mm$ at central incisor, $0.76{\pm}0.59\;mm$ at lateral incisor, and $1.07{\pm}0.80\;mm$ at canine. Mean thickness of palatal plate 3 mm below CEJ was $1.53{\pm}0.55\;mm$ of central incisor, $1.18{\pm}0.66\;mm$ of lateral incisor, $1.42{\pm}0.77\;mm$ of canine. Bucco-lingual diameter 3 mm below CEJ was $5.13{\pm}0.37\;mm$ of central incisor, $4.58{\pm}0.46\;mm$ of lateral incisor, and $5.93{\pm}0.47\;mm$ of canine. Buccal bony curvature angle below root apex was $134.7{\pm}17.5^{\circ}$ at central incisor, $151.0{\pm}13.9^{\circ}$ at lateral incisor, $153.0{\pm}9.5^{\circ}$ at canine. Distance between root apex and the deepest point of buccal bony curvature of central incisor was $3.67{\pm}1.28\;mm$ at central incisor, $3.90{\pm}1.51\;mm$ at lateral incisor, and $5.13{\pm}1.70\;mm$ at canine. CONCLUSION. Within the limitation of this study in Korean adults, the thickness of maxillary anterior buccal plate was very thin within 1mm and the thickness of palatal plate was thick, relatively. The buccal bony curvature below root apex of maxillary central incisor was higher than that of lateral incisor and canine and it seems that the buccal bony plate below root apex of central incisor is most curved.

상악 구치부에서 즉시 임플란트 식립시 협지방대의 이용 (Use of Buccal pad on the Immediate Implant Placement in the Maxillary Posterior Area: Report of two Cases)

  • Now a days, immediate implant placement: is becoming one of the popular method. But for the success of this method in implant surgery, initial stability and establishment of sufficient blood supply to the implant placement: areas are very important. Buccal Fat Pad(BFP) has favorable characteristics for the reconstruction of maxillary hard &soft tissue defects. So it has been used for reconstruction of posterior maxillary area or closure of oro-antral area. Using BFP, we could get primary tissue closure without extensive releasing incision during implant surgery and adequate attached gingival after healing of the surgical area. So We report clinical usefulness of BFG during immediate important placement in the posterior maxillary area

치과용 콘빔CT영상에서 미니임플란트를 위한 협측피질골 두께 (Buccal cortical bone thickness on CBCT for mini-implant)

  • Purpose : Cortical bone thickness is one of the important factor in mini-implant stability. This study was performed to investigate the buccal cortical bone thickness at every interdental area as an aid in planning mini-implant placement. Materials and Methods : Two-dimensional slices at every interdental area were selected from the cone-beam computed tomography scans of 20 patients in third decade. Buccal cortical bone thickness was measured at 2, 4, and 6 mm levels from the alveolar crest in the interdental bones of posterior regions of both jaws using the plot profile function of $Ez3D2009^{TM}$ (Vatech, Yongin, Korea). The results were analyzed using by Mann-Whitney test. Results : Buccal cortical bone was thicker in the mandible than in the maxilla. The thickness increased with further distance from the alveolar crest in the maxilla and with coming from the posterior to anterior region in the mandible (p<0.01). The maximum CT value showed an increasing tendency with further distance from the alveolar crest and with coming from posterior to anterior region in both jaws. Conclusion : Interdental buccal cortical bone thickness varied in both jaws, however our study showed a distinct tendency. We expect that these results could be helpful for the selection and preparation of mini-implant sites.
