A Study about the current infra-structural status of the aged care worker to improve the quality of long-term care in Germany (독일 노인장기요양보험의 서비스 질 향상을 위한 인프라 구축 현황에 대한 연구)

  • Currently in Germany, there is talk of 'state of emergency care', which is addressed to the problems of quality assurance in aged care and shortage of aged care workers. In order to solve this problem in the aging German society, the federal government has set itself the goal of providing high qualified care givers through a systematic and on a high level of professional training opportunities. Various projects and measures have been carried out to improve the reputation of the primarily care profession in society and for the purpose of attracting especially young trainees for the aged care professions. The present work considers training and qualifications in the long-term care sector in Germany; it points out both the characteristics of aged care education and the learning content in the aged care education and attempts to highlight what roles and perception of tasks contained therein.

Definition and Strategy of Community Involvement in Health (보건의료 주민참여의 정의와 전략에 대한 연구)

  • Community involvement is to be the keystone of primary health care, Unfortunately, efforts to promote it failed in many countries. Author inquired into the reasons why such efforts fail, and the alternative strategies for effectively promoting it in Korean situation today. In doing so, the concept of community involvement is clarified as an amalgamation of two different concepts; one is that of the community development, and the other is that of approaches for overcoming the alienation of the people from the health care process. Stategies of community involvement in Korea so far followed the line of the community development, e.g. village health worker. It loses, however, the feasibility as Korea has experinced so cial and economic developpment. The strategies of the second line of thought is now more feasible; empowering the people by providing more information and enhancing comitment to health, improving acceptabilty of community involvemt on the part of the health professionals and the bureaucrats, and building the mechanism of community involvement into the health policy-making process.

DACUM Job Analysis on Elementary Health Teachers' Roles (초등학교 보건교사의 역할에 대한 DACUM 직무분석)

  • Purpose: The aim of this study was to identify the roles, duties and tasks of elementary health teachers. Required knowledge, skills, and worker behaviors were also examined. Methods: Elementary health teachers' jobs were analyzed by DACUM workshop. First, the health teachers' roles and related jobs were described, and then the jobs were divided into duties and subordinate tasks. The identified roles, duties and tasks were reviewed and refined, and then were organized in a DACUM chart. DACUM committee members discussed not only general knowledge, skills and work behaviors but also future trends and concerns. Results: The DACUM chart for elementary health teachers consisted of 8 duties and 52 tasks. Required knowledge, skills and worker behaviors were also listed. Conclusion: Elementary health teachers play roles as health manager, health care provider, and teacher in school. Their roles, duties and tasks are being changed. Thus, their jobs need to be redefined legally, politically, and institutionally.

Quality Assessment on Social Services in General and Teaching Hospitals in Korea (의료사회사업서비스의 질에 관한 연구)

  • Background : As an empirical study, current research about the quality of social services carried out in general and teaching hospitals across the country. In the flux of health care reforms and market transformation, the quality of social services in the hospital becomes increasingly significant. Methods : A sample of total 80 hospitals, including general hospitals with one social worker at least and single-department hospital with two social workers or more, were identified nationwide through the registry of Korean Association of Medical Social Workers and Korean Association of Hospitals. The subjects of this survey were 80 leaders of social service units. The survey data from each subjects were measured to evaluate level of quality that service provider perceived of sample hospitals. Under the method of one-way ANOVA and multiple regression, the level of quality in social work service was analyzed. Results : The major findings were as following ; First, the level of quality perceived showed less score, especially the lowest was the score of quality of outcome. Second, the key variables of each hospital which turned out significantly different in quality of social work service were the departmental form of social work unit, unit leader's age, educational level, field experience, and job rank. Third, the level of quality of social work service correlated positively with the field experience of unit leader, the size of social work unit, the job rank of the unit leader. Conclusion : The most influential variables to the quality of social work service proved departmental form of social work unit, leader of social work unit. Therefore, to assure the proper level of quality, social work unit in hospital must be structured single, independent department in which entitled social worker is supposed to supervise and manage. And a leadership-development program for leaders in social work unit are strongly recommended.

Effect of Long-term Care Worker's Emotional Labor on Service Quality of long term care facility - Mediating Effect of Professional Quality of Life - (요양보호사의 감정노동과 장기요양기관의 서비스 질의 관계 - 직무관련 삶의 질 매개효과 -)

  • In this study, we analyze the relationship between long-term care workers' emotional labor, professional quality of life, and service quality. The subjects of this study were 211 long-term care workers working in long-term care facilities. Self-administered questionnaires were used. In the research model, emotional labor(surface behavior, internal behavior) was verified using independent variables, with professional quality of life (compassion satisfaction, compassion fatigue) as the parameters and service quality as the dependent variables. The results of this study are as follows. First, the fit of the model was good. Second, the direct effects of emotional labor on service quality were not significant. However, there was a positive correlation between internal behavior and service quality, and a negative correlation between surface behavior and service quality. Third, professional quality of life(empathy satisfaction) was the most influential variable in terms of service quality. The results of this study demonstrate the necessity to payclinical and academic attention to long-term care workers' emotional labor and quality of life.

  • This study was designed to search for the direction to develop housing service which assists the older people to live independently in their community and provides the systematization of community care. The subjects of the research include 533 older people between the age of 60 to 79 residing in Seoul and 84 welfare officials at a dong(in)-office and 67 social workers working at comprehensive social welfare institutions. The results were as follows: Both demanders and deliverers recognized that housing service for older people was necessary for community care, and, in particular, the deliverers had higher degree of necessity as people charged in welfare service. The most needed service was community alarm service which currently is proceeded partially for low-income and living-alone olders. It needed to expand the scope of the people to be served. It appeared that the need for inside safety measures rather than for housing space or structural problems. It requires less effort and cost for those who need simple repair, such as safety grips or slippery protection, and safety checks. Among financial support services, the need for tax benefits appeared the most necessary service for the older people. It suggested that more 'direct' financial support service is needed for the elderly people.

The relationship between precarious work and unmet dental care needs in South Korea: focus on job and income insecurity (한국 노동시장 불안정성과 미충족 치과의료의 관련성: 고용과 소득 불안정성을 중심으로)

  • Objectives: The aim of this study was to analyze the accessibility of dental care services among individuals with precarious employment in South Korea. Methods: We used the $9^{th}$ wave of the Korean Health Panel data (2015) and included 7,736 wage and non-wage earners in our study. We determined precariousness in the labor market as a combination of employment relationship and job income, and categorized individuals based on this into the following four groups: Group A comprising those who report job and income security, Group B comprising those who experience job insecurity alone, Group C comprising those who report a stable job but low income, and Group D comprising those who experience both job and income insecurity. Accessibility to dental care services was determined by experience of unmet dental care needs and unmet dental care needs caused primarily by financial burden. Logistic regression analyses were used to assess the effect of precarious work on access to dental care services. Results: Individuals with job insecurity (Group B; OR=1.445; 95% CI=1.22-1.70) and both job and income insecurity (Group D; OR=1.899; 95% CI=1.61-2.24) were more likely to have unmet needs than the comparison group. Both groups B and D were also 2.048 (95% CI=1.57-2.66) times and 4.435 (95% CI =3.46-5.68) times more likely, respectively, to have unmet dental care needs caused by financial burden. Education status, health insurance, and health status were all also effective factors influencing unmet dental care needs. Conclusions: Unstable employment and low income resulted in diminished access to dental care services. Therefore, governments should consider health policy solutions to reduce barriers preventing individuals with employment and income instability from accessing adequate dental care.

The mediating effect between the degree to provide emotional labor and personal relationship in the intent to stay for Care worker (돌봄 여성노동자의 감정노동수행정도와 직무지속의사와의 관계에 있어 대인관계의 매개효과)

  • The purpose of this study is to find out the relationship among the degree to provide emotional labor, personal relationship and the intent to stay of female care service workers. Specifically, this study focuses on the mediating effect of the personal relationship (client and agency relation). The path analysis and structural equation modeling analysis were performed on the collected data using SPSS18.0. and AMOS 8.0. And Sobel test conducted for examine the mediating effect. The results are the followings. First, the result of the analysis showed that agency relationship was an indirect factor as the partial mediating effect on relationship between the degree to provide emotional labor and the intent to stay for the care women workers. The result suggests that education utilizing various techniques and strategies to overcome the difficulty accompanied by emotional labor such as communication education and the government try to effort managing and controlling the agency which impact on the labor condition of care workers. Second, the government also tries to provide the trust and collaboration network system which construct a good relationship between the care worker and the agency.

A Case Study on Care Workers' Experiences of Coping with Falls among Community-Dwelling Elders (재가노인 방문요양보호사의 낙상관련 대처 경험에 관한 연구)

  • Purpose: The purpose of this study was to understand the experience and meaning of falls among care workers caring for the elderly. Methods: The participants comprised 10 care workers in Seoul and Gyeonggi Province, South Korea. The data collection period was from September to November 2020, and the data were obtained through individual in-depth interviews. The collected data were analyzed using the content analysis method of qualitative research. The main interview questions were as follows: First, in the event of a fall, "can you tell us about your experience and how you dealt with the fall?; second, "can you tell us about your role and the role of family members or the center that manages falls that occur outside of nursing visits.", third, "can you tell us about the difficulties in managing falls based on your experience?", and fourth, "how are falls that occur during nursing visits managed?" Results: As a result of analyzing the experiences of nursing care workers of falls while caring for elders in their homes, five main themes were derived. These were understanding the characteristics of the elderly at home, understanding possible emergency situations, providing caring services, minimizing physical harm, and delivering clear information. Conclusion: A significant finding of this study was that care workers who visit and provide care to elders at home confirmed the need for standard guidelines on appropriate responses in the event of a fall at home and the importance of managing emergencies arising from falls among elders at home. These results highlight the need for the development of emergency response education programs, such as programs on fall care among nurses and care workers who provide care to the elderly, and for enhanced understanding of the fall risk of elders who live at home.

Performance of Occupational Health Services by Type of Service : Cost Benefit Analysis (사업장 보건관리 사업의 형태별 수행성과 분석 -비용편익 분석을 중심으로-)

  • Occupational health services in Korea have been operated as dual types : one is operated by occupational health care manager and the other is health care agency without their own personnel. The performance of occupational health service should be different due to the variety of characteristics of health care manager and workplace, qualification of health care manager. This study is to analyze performance of occupational health care services with a particular consideration of job performance shape and efficiency, based on comparing those two types of health care management to show on the basic data for the settlement of more qualitative health care management system at workplace. For this study, total 391 places in Seoul and Inchon city area ; 154 places (39.4%) managed by designated health care manager and 237 places (60.6%) by the agency with their commission are selected as research samples. Tools for data collection are questionnares that have been investigated during the period of 20 September 1993-20 December 1993. Those data are compared with percentiles, mean, standard deviation and B/C ratio using SPSS PC program. Conclusions observed from the tests and each comparison could be summerized as follows : 1. Occupational health care have been accomplished at workplaces with designated people than with agencies people, and coverage rate of the occupational health care services has differences, due to management types. The reason of these results is due to visit only one or two times monthly by the agencies, while their own health care manager obsess, at the workplaces all the times. 2. Most of the expense for environmental control of all health care services expenditures shows that there is almost no fundamental improvement because more expenses are needed for procuring personal protective equipment and measuring work environment instead of environmental improvement. 3. It is investigated how much the cost of occupational health care services needs per worker, and calculated how much the cost needs per service hour per worker. The results from this show that the cost of occupational health services at workplaces with their own managers used less than the cost of health care agencies, eventually the former gives better services with less cost than the latter. 4. Benefit/Cost ratio is also produced by total benefit/total cost. The result from the above way reads 4.57 as a whole, while their own manager having workplaces reads 4.82 and the agencies do l.56. Even if their own manager performing workplaces spent more cost, this system produces more benefit than the agencies management. 5. The B/C ratio for medical organization such as local clinic, health care center and pharmacy shows more than or equal to at the workplaces controlled by the agencies. It is inferred that benefit would be much less than the cost used, with so being inefficient. 6. It is assumed that the efficiency ratio of health education is equal to reduction rate of workers medical organization visit. Estimated reduction rate 5%, 10%, 15%, show that the efficiency ratio of health education have an effect on producing benefits. It is estimated that more benefit can be produced if more qualitative education will be provided for enhancing health care efficiency. 7. Results of this study cannot be generalized because there are large scale of deviation in case of workplaces with less than 300 full time workers, but B/C ratio reads 2.69 as a whole and 3.25 at workplaces with their own health care manager are higher than 1.63 at the workplaces manged by the agencies. Finally, all the benefit concerning health care services could not be quantified, measured and shown on the value of money. This is a reason that a considerable part of benefits are so underestimated. This is also thought that measurement tools should be developed for measuring benefits of health care services with a comprehensive quantification. in the future. It is also expected that efficiency of occupational health care services should be investigated using cost-effectiveness analysis.
