산업간호에서 Case Management의 활용 (Applying the Case Management in the Occupational Health Nursing)

  • This study was performed to review applying the case management to the occupational health nursing as a cost-effective way for injured workers. The case management for injured workers is a comprehensive management system, which is composed of planning and implementing in cooperation with employee, employee' family, medical service provider team, employer and case manager, to provide high quality health services to employee and their family. The case management process for injured workers is similar to the nursing process. It thus appears that the occupational health nurse is best personnel to perform the role of case manager for injured workers in workplace. As a case manager in workplace. the occupational health nurse would be expected to get both knowledge and experiences, to get trusted from the employee and employer, and to expand professional domain in changing health care environment.

Case-Based Reasoning Support for ERP Pre-Planning

  • A project planning is one of the most important processes that determines success and failure of the project. A pre-project planning is also essential job for information system implementations at the early stage of project planning, especially for management information system like ERP. However, pre-project planning is very difficult, because lots of factors and their relationships should be considered. Pre-project planning of ERP implementation has been done by project manager's own knowledge and experiences. In this article, we propose a system that help project manager to make a pre-project plan of ERP project with case-based reasoning(CBR) framework. The proposed CBR system saves previous cases of ERP pre-project planning in the case base. Then, the system finds the most similar case with the current pre-project planning problem. Project manager can make a pre-project plan by adjusting the most similar case. From the interview with project managers, we collect some field cases of ERP implementation. We organized these cases by using XML(Extensible Markup Language), which is good for representing hierarchical information. XML gives us some flexibilities to correct and maintain cases. We make a prototype system, PPSS(Project Planning Support System) that help project manager to make a pre-project plan of ERP implementations. The object of the system is to support project manager to make a pre-project plan of ERP. We hope the result of the study can be applied to other information systems. Our research would be extended to cover other stages of project planning.

근로복지공단 사례관리자의 업무 효율 및 스트레스에 영향을 미치는 요인 (Factors Impacting the Work Efficiency and Stress of Case Managers with the Korea Worker's Compensation & Welfare Service)

  • Objectives: The purpose of this study is to objectify the level of case management performance and the factors influencing performance, to improve the case management performance at the Korea Worker's Compensation & Welfare Service (KWCWS) on the basis of the recognition of the objective realities of case management by job coordinators at the KWCWS, to develop a model of case management fit for the KWCWS, and to provide a basis for establishing guidelines for standardized case management. Methods: A total of 156 questionnaires were distributed to job coordinators at the KWCWS's headquarters, six regional headquarters, and 55 branches. One hundred forty-one questionnaires were collected and 126 were analyzed statistically using SPSS 21.0. Factor analysis and reliability analysis were conducted to verify the validity and reliability of the main measurement items in the research model. Frequency analysis was conducted for general characteristics of survey subjects. Frequency analysis or descriptive statistics were conducted to identify the level of independent variables (case manager's individual variables, job variables, institutional and organizational variables). Dependent variables (case management performance) and the degree of correlation were analyzed through correlation analysis between research variables. Multiple regression analysis and hierarchical regression analysis were conducted to examine the effect of independent variables on case management performance. Results: The results of the study showed that the level of overall performance in the five stages of case management was ordinary, with an average level of 3.45 on a 5-point scale. Levels of performance by step were institutional approach and intake (3.69), assessment (3.63), goal setting and intervention planning (3.46), implementation of intervention plan (3.32), and evaluation and termination (3.20), in that order. The explanatory power of case management performance (overall) by case managers with the KWCWS was case manager's institutional and organizational variables, job variables, and individual variables, in that order. At each stage of case management, the explanatory power of a case manager's institutional and organizational variables was found to be the greatest. The model changes at each stage of case management assume similar aspects statistically. In hierarchical regression analysis, it was institutional support that had a significant effect on case management performance (overall), and institutional support had the greatest effect. The results of multiple regression analysis in which all variables are input simultaneously showed that institutional support and expertise as well as self-efficacy had a positive effect. However, case management work experience, expertise (technology), and autonomy were found to have a negative effect during the stage of case management performance. Conclusions: As a result of the study, it was confirmed that raising the case manager's expertise and support from the institution and organization are important factors to improve the level of case management performance. The research also derived practical ways of reinforcement of case manager capacity, institutional and organizational support, operation of rehabilitation-case management teams, and occupational health-related aspects.

Factors Related to Treatment Refusal in Taiwanese Cancer Patients

  • Background: Incidence and mortality rates for cancer have increased dramatically in the recent 30 years in Taiwan. However, not all patients receive treatment. Treatment refusal might impair patient survival and life quality. In order to improve this situation, we proposed this study to evaluate factors that are related to refusal of treatment in cancer patients via a cancer case manager system. Materials and Methods: This study analysed data from a case management system during the period from 2010 to 2012 at a medical center in Northern Taiwan. We enrolled a total of 14,974 patients who were diagnosed with cancer. Using the PRECEDE Model as a framework, we conducted logistic regression analysis to identify independent variables that are significantly associated with refusal of therapy in cancer patients. A multivariate logistic regression model was also applied to estimate adjusted the odds ratios (ORs) with 95% confidence intervals (95%CI). Results: A total of 253 patients (1.69%) refused treatment. The multivariate logistic regression result showed that the high risk factors for refusal of treatment in cancer patient included: concerns about adverse effects (p<0.001), poor performance(p<0.001), changes in medical condition (p<0.001), timing of case manager contact (p=.026), the methods by which case manager contact patients (p<0.001) and the frequency that case managers contact patients (${\geq}10times$) (p=0.016). Conclusions: Cancer patients who refuse treatment have poor survival. The present study provides evidence of factors that are related to refusal of therapy and might be helpful for further application and improvement of cancer care.

A Benchmarking Tool to Assess the Role of the Construction Manager in terms of Project Teamwork Supports

  • Construction management services have been adopted for more than a decade and are continuously growing; however, the advantages of utilizing a construction management service are still not quite clear, regardless of many arguments can be made in their favor. The construction manager, as a coordinator, is supposed to smoothly facilitate the project teamwork in order to deliver a profitable and satisfactory project, not only for the owner's interests but also for the success of all project participants. This study has focused on the teamwork supports a construction manager can provide during project operations. This study developed a benchmarking tool to assess the construction manager's role in terms of its project teamwork supports, by utilizing the evaluation model and several case studies. In addition, this study also attempted to set the industry average and higher performance so that both the owner and the construction manager can self-assess, and more importantly, can improve project operation. The actual implementation of the benchmarking tool into on-going projects can allow the construction manager to understand the current operation and to make a better project process through teamwork.

가족사례관리사업 운영 현황 및 효율성 제고 방안에 관한 연구 : 서울지역 건강가정·다문화가족지원센터를 중심으로 (A study of current management status and ways to improve the efficiency of Family-centered case management services: Focused on the Healthy family support centers and Multi-cultural family support centers in Seoul)

  • The main purpose of this study are to investigate the current situation of family-centered case management practices by case managers working in the healthy family support centers and multi-cultural family support centers, in order to propose a practical direction for improving the efficiency of family-centered case management services. Research participants included 24 case manager working in the healthy family support centers and multi-cultural family support centers in Seoul, who have performed case management services for families for one to 7 years. The Date was collected through focus group interview(FGI) and demographic characteristics research. The results of this study are as follows: first, in the area of cognition about family-centered case management conceptualization, confused and ambiguous. Seconds, in the area of practicing as a case manager, there were limitations and biases in community resources contact and multiple intervention and limited sharing information between organizations. Third, family-centered case manager's have periodically been requested to provide diverse training and education for performance of challenging case managers roles. The Standardized manual and burnout precautions should also be prepared.

  • 정신건강 토탈케어 바우처 기관의 사례관리자와 이용자를 대상으로 사례관리 수행요소가 정신질환자의 삶의 질에 미치는 영향을 분석하였다. 클라이언트 요인, 사례관리자 요인, 서비스 전달 요인과 기관요인을 위계적 선형모형(HLM: Hierarchical Lineral Model)을 적용하여 검증하였다. 본 연구는 국내최초로 사례관리자의 사회복지경력과 사례관리 개입범위가 정신질환자의 삶의 질에 유의미한 영향력이 있다는 것을 검증하였고, 담당 사례수가 1인당 20케이스 이하일 경우 삶의 질에 영향을 미치지 않는 것을 확인하였다. 연구를 통해 사례관리자의 사회복지경력, 담당 사례수, 지역사회환경체계 개입범위의 유용성에 대한 실천적 함의를 제안하였다.

  • Purpose: The purpose of this study was to develop a curriculum for care manager based on the results of professional workshops and through the analysis of the roles and curriculums of care managers in other countries. Method: The process of this study included a review of literature, getting advice from various experts, a review of the present state of education programs at abroad. Result: This study revealed the basic content of care manager education which includes understanding health care and welfare delivery system. process of care manager, resource management, legal and ethical issues, management of health and a life style of elderly, communication, stress management. The major roles of care manager was revealed as the one who assess the needs of elders, making a care plan, evaluator, supporter, coordinator and a risk manager who coordinates, validates and reassesses various services needed by elderly. Conclusion: These results can be utilized in care manager educational program for raising the quality of care manager and care management services.

의료급여관리사의 의사소통능력과 스트레스가 직무만족에 미치는 영향 (Effects of Medicaid Case Manager's Communication Competence and Stress on Their Job Satisfaction)

  • Purpose: This study was conducted to identify effects of Medicaid Case Manager's communication competence and stress on their job satisfaction. Methods: The current work is descriptive research, and the participants were 154 medicaid case managers. Data were collected between May and July, 2017 through Embrain, a specialized research organization. The data were analyzed using independent t-test, ANOVA, and multiple regression. Results: The mean communication competence of the medicaid case managers was 3.61, stress 1.76, and their job satisfaction 2.74. Age and average monthly compensation brought significant differences in their job satisfaction. Their communication competence was correlated with stress (r=-.35, p<.001), but not with their job satisfaction. The stress was the most influential factor in job satisfaction (${\beta}=-.45$). Conclusion: The results of this work show that the medicaid case managers' communication competence was not an influence factor of their job satisfaction, which is different from implications from the previous studies and needs confirmation through future research. In addition the results of this study also suggest that stress management can be a useful approach to the improvement of medicaid case managers' job satisfaction.
