Letter to Editor : A case of Seganmeongmok-tang for choroidal melanoma-related eye pain and headache (맥락막 흑색종으로 유발된 안구통증과 두통이 세간명목탕으로 호전된 환자 1례)

  • Objectives : choroidal melanoma is a rare solid tumor and has been known that thers is few effective drugs for the treatment. The purpose of this case report is to report a case of reduction of choroidal melanoma-related eye pain and headache treated with Seganmeongmok-tang. Methods : A patient with choroidal melanoma-related eye pain and headache was treated with Seganmeongmok-tang. The changes in clinical findings were evaluated. Results : The symptoms of patients were improved after about 1 week taking Seganmeongmok-tang. Conclusion : This case report suggests that Seganmeongmok-tang may have an effect on reduction of choroidal melanoma-related eye pain and headache. Further rigorous studies are necessary to investigate the therapeutic effects of Seganmeongmok-tang on choroidal melanoma.

Vancomycin Induced Lupus Flare in a Patient with Systemic Lupus Erythematosus: A Clinical Case Report (전신성 홍반성 루푸스 환자에서 Intravenous Vancomycin 투여로 인한 루푸스 활성기 증례 보고)

  • We report an unusual case of lupus flare induced by intravenous vancomycin in a patient with systemic lupus erythematosus. Due to methicillin resistant staphylococcus aureus in wound culture, intravenous vancomycin was administered to the patient. The patient had been on vancomycin for several days then she experienced fever, malar rash, and vomiting. Based on laboratory results, it was confirmed as lupus flare. Oral prednisolone was given to the patient for symptom control. However, when vancomycin was readministered, the patient had similar symptom to the previous one more intensively. Vancomycin was stopped then the patient became stable. This case report demonstrates that intravenous vancomycin may manifest as lupus flare in patients with systemic lupus erythematosus.

  • This case is to report the effect of renal function of chronic kidney disease(CKD) caused by IgA nephropathy. A 37-year-old man visited a Korean medicine hospital, who has been diagnosed with end stage renal disease(ESRD), 5 stage of CKD, caused by IgA nephropathy, has had no improvement of western medical treatment, and wanted to be treated using Korean medicine before renal transplantation. The decrease of creatinine value, the increase of glomerular filtration rate(GFR), and the decrease of CKD stage (5 to 4) was observed after combination treatment of Ikkigeonbiisuhwalhyeoltang and saam acupuncture was applied. This case report is suggested that combination treatment of acupuncture and herbal medicine could be effective to renal function of CKD in spite of a single case.

  • Introduction: This case report assessed the effects of Korean medicine treatments on a chronic migraine patient who did not respond to triptan. Case Presentation: The patient received Korean medicine treatments, such as acupuncture, moxibustion, cupping, and herbal medication, during a hospitalization period. The effects of treatments were assessed with the Numerical Rating Scale (NRS) and migraine attack frequency. The intensity and attack frequency of migraines were significantly reduced after the complete treatment. Conclusions: For migraines, Korean medicine such as acupuncture and herbal medication could be an alternative to conventional therapy.

  • objective The purpose of this case report is to review the possibility of utilizing Jeokseokji-uyeoryang-tang as a therapy of seborrheic dermatitis, and to consider clinical using possibility of Shanghanlun.method Patient's Diagnosis and progress of seborrheic dermatitis was evaluated based on the patient's chart and picture. And diagnosis based on Shanghanlun was organized by table.results When analyzing the patient's history and symptoms, it can be found in article No. 159 in Shanghanlun. The area and intentsiy of erythema were reduced when viewed photos findings, so diagnosis and therapies used in this case showed valid results in seborrheic dermatitis.conclusion Jeokseokji-uyeoryang-tang is efficient to patient of this case report as a treatment.

  • Objectives : This case report presents the effect of Galgunhwangryunhwanggum-tang on dysmenorrhea. The patient was diagnosed with Taeyang-byung based on Shanghanlun provisions. Methods : According to Shanghanlun seven meridian patterns diagnostic system, we diagnosed the patient as Taeyang-byung and she has been suffering from dysmenorrhea. The patient was treated with Galgunhwangryunhwanggum-tang. and it was provided for 2 months. Results : After the treatment, the degree of dysmenorrhea reduced from 10 to 2 on VAS. Conclusions : This case study show an effectiveness of using Galgunhwangryunhwanggum-tang according to Diagnostic system based on Shanghanlun provisions on hyperhidrosis.

  • Dirofilariasis is a common parasitic disease in both domestic and wild animals around the world, with canines as the principal reservoir host and mosquitoes as the vector. Human ophthalmic dirofilariasis is an uncommon condition, but there have been reports from many parts of the world, including Africa, Australia, the Americas, Europe, and Asia. Ophthalmic involvement with Dirofilaria may present itself as periorbital, subconjunctival, orbital, or intraocular infections. In this report, we present a case of orbital dirofilariasis with lateral rectus muscle involvement. To our knowledge, this is the first orbital dirofilariasis case reported in Iran. Although debulking of the tumor usually leads to resolution, our patient showed an episode of recurrence after biopsy. Complete recovery was achieved after a spontaneous discharge, without the need for systemic medication.

A Case Report of Patient with Recurrent Aphthous Stomatitis and Pompholyx Treated with Modified Palmulgunja-tang (팔물군자탕 가미방으로 호전된 재발성 아프타성 구내염 및 한포진 환자 치험 1례)

  • Introduction This study is the first case report to observe a significant improvement of clinical symptoms of both Recurrent Aphthous Stomatitis(RAS) and pompholyx treating with modified Palmulgunja-tang for Soeum-type. Case presentation A 37-year-old female patient complained of pain by ulcers on her mouth and tongue, and of itching caused by vesicles on her palms and soles was diagnosed with RAS, pompholyx, and, especially moderate pattern of 'Congestive Hyperpsychotic symptomatology' according to the theory of Sasang Constitutional Medicine(SCM). During inpatient and outpatient treatment, she took modified Palmulgunja-tang, and was treated with acupuncture, moxibustion, and cupping therapy. The effectiveness of these therapies was assessed with subjective Neumeral Rating Scale(NRS) and Dyshidotic Eczema Area and Severity Index(DASI). Conclusion After the treatment according to the theory of SCM, NRS, and DASI score about RAS and pompoholyx was decreased, and the general condition of the patient also has improved.

A Clinical Case of Tinea Pedis Treated with Korean Medicine (한방치료로 호전된 족부백선 환자 1례)

  • Objectives : This study is to report a case of tinea pedis accompanied with erythema and pruritus, treated with Korean medicine treatment. Methods : A 26-year-old female patient has suffered from erythema and pruritus on her right 3rd toe. She applied desoximetasone ointment on it for a month, but the effect didn't last long and the symptoms got worse. After disappointing application of the corticosteroid, she received Korean medicine treatment including herbal medicine and herbal external medication for 1.5 months. Results : The skin symptoms such as erythema and pruritus were remarkably improved. Also, the Visual Analogue Scale(VAS) of pruritus changed from 4 to 0. Conclusions : This case report suggests that Korean medicine may have a role in treating tinea pedis with some skin symptoms such as erythema and pruritus.

Oxaliplatin Induced Angioedema in a Rectal Cancer Patient: A Clinical Case Report (직장암 환자에서 Oxaliplatin 투여로 인한 혈관부종 발생 증례 보고)

  • We report an unusual case of angioedema induced by intravenous oxaliplatin in a patient with rectal cancer. Intravenous oxaliplatin was administered to the patient according to FOLFOX regimen (oxaliplatin/leucovorin/5-fluorouracil) for the management of his colorectal carcinoma. The patient had been on FOLFOX regimen for several months without reporting any adverse effects, but he experienced angioedema suddenly during his $9^{th}$ cycle of chemotherapy. Angioedema was observed about 2 hours after the initiation of oxaliplatin infusion. Oxaliplatin was stopped then the patient became stable. This case report demonstrates that intravenous oxaliplatin may induce angioedema several hours after the initiation of oxaliplatin infusion in later cycles of chemotherapy.
