Authors describe a patient who developed a myelopathy associated with Guillain-Barr$\'{e}$ syndrome and cervical myelopathy. We provide radiological evidence of non-compressive herniated cervical intervertebral disc with cord signal changes and show the clinical and electrophysiological result of coexisting Guillain-Barr$\'{e}$ syndrome and cervical myelopathy. We tried to introduce and review the case of Guillain-Barr$\'{e}$ syndrome which was combined with cervical myelopathy to let us recollect the presumptive cause.

Purpose : Cervical spondylotic myelopathy is a varied clinical syndromes and the most serious condition of cervical spondylosis. Spinal cord dysfunction is often caused by the compression on spinal cord. The purpose of this report was to describe physical examination and the process of disorders on cervical spondylotic myelopathy(CSM). Methods : Three cases with cervical myelopathy were presented with magnetic resonance image(MRI), Nurick's grade and Japanese Orthopaedic Association(JOA) score. Results : This studies showed a variable clinical course with gradual neurological deterioration in two cases. but one case was improved by operation and a regular aerobic exercise. MRI, Nurick's grade, JOA score are a useful adjunct for CSM evaluation. Conclusion : The state of CSM is variable, which may affect conservative treatments and surgery. It is needed to be a carefull approach and studies for a conservative treatment and management on CSM.

The Cervical spondylotic myelopathy (CSM) is degenerative compressive myelopathy which initiation of symptoms seems to be induced by minor cervical trauma or spontaneous event. There was a case of Claim medical examination which was requested to discriminate the cause of ambulatory quadriparesis. Patient asserted that the onset of his myelopathy was followed by minor trauma. The author considered the medical recordings, MRI scan, Claim reports by claim manager. The space available for cord was the smallest at C3-4 level. But on MRI findings, the spinal cord at C3-4 level seemed to be already damaged. There were no recent injury evidences such as hemorrhages, spinal cord contusions, edema, soft tissue hemorrhages. If the space available for cord was small enough to compress the spinal cord, the serious neurologic deficits, non-ambulatory quadriplegia, etc were commonly induced by cervical extension trauma. Patient's asserts did not correspond to his clinical course after cervical trauma. The author reports a case of medical examination for the relationship between symptom onset of cervical spondylotic myelopathy and minor trauma within author's experience.

Objectives : The purpose of this study is to report the effect of moxibustion for paitent with Myelopathy hand Methods : A 65-year-old female patient diagnosed with Cervical myelopathy was treated by moxibustion on both hand and neck, from July 17th to July 29th. The improvement of the patient's Myelopathy hand was evaluated based on JOA(Japanese orthopedic score), VAS(visual analog scale), Grips and release test, Finger escape sign, and DITI(digital infrared thermal imaging). Results : After the treatment of moxibustion, the patient's JOA score were not improved, but VAS, Grips and release test, Finger escape sign and DITI were improved. Conclusions : The result suggests that moxibustion of hand and neck is an effective treatment to Myelopathy hand on Cervical myelopathy. But more cases and researches are needed to evaluate the significancy of moxibustion for Myelopathy hand.

Objective : The purpose of this study was to investigate the surgical results and prognostic factors for patients with soft cervical disc herniation with myelopathy. Methods : During the last 7 years, 26 patients with cervical discogenic myelopathy were undertaken anterior discectomy and fusion. Clinical and radiographic features were reviewed to evaluate the surgical results and prognostic factors. The clinical outcome was judged using two grading systems (Herkowitz's scale and Nurick's grade). Results : Male were predominant (4:1), and C5-6 was the most frequently involved level. Gait disturbance, variable degree of spasticity, discomfort in chest and abdomen, hand numbness were the most obvious signs. Magnetic resonance(MR) images showed that central disc herniation was revealed in 16 cases, and accompanying cord signal changes in 4. Postoperatively, 23 patients showed favorable results (excellent, good and fair) according to Herkowitz's scale. Conclusion : Anterior cervical discectomy and fusion effectively reduced myelopathic symptoms due to soft cervical disc herniation. The authors assured that the shorter duration of clinical attention, the lesser the degree of myelopathy and better outcome in discogenic myelopathy.

Objective : Cervical myelopathy is one of the most severe disease, as well as a common diesaes. However, its clinical study and report about efficiency of oriental treatment are absolutely insufficient. Therefore, we report the treatment about 1 case of Cervical myelopathy like followings. Methods : From November 27th, 2002 to January 11th, 2003, we observed patient hospitalized because of cervical myelopathy, and for its valuation, JOA score(Japanese Orthopedic Association score), Nutric grading, VAS(visual analog scale), and D.I.T.I(Digital Infrared Thermal Imaging)were used. Results : After being hospotalized for 50 days, the patients' JOA score, Nuric grading system, and VAS were improved each from 7 to 13, from grade IV to grade II, and from 8 to 2. Conclusions : We considered that oriental treatment has an useful effect on cervical myelopathy patients' treatment and recovery. But continuous studies, such as comparative studies accompanied by more treatment case.

The degenerative spinal cord disease cervical spondylotic myelopathy (CSM), and cervical myelopathy caused by trauma, can result in debilitating symptoms affecting quality of life. This study used acupotomy and other Korean medicine treatments (acupuncture, herbal medicine, and physical therapy) to improve the symptoms of CSM and cervical myelopathy. The visual analog scale, the modified Japanese Orthopaedic Association scale (mJOA scale), the Nurick grading system, and the American Spinal Injury Association impairment scale were used as the evaluation criteria to determine the effectiveness of treatment. The functional status of both patients improved from mild to moderate, with improved gait, local sensation, and level of pain. The degree of spinal cord injury remained the same. The findings of this study suggest that combined Korean medicine treatments including acupotomy may be helpful in the treatment of CSM and cervical myelopathy.

Cervical disc arthroplasty (CDA) has become more widespread and diverges from the conventional technique used in anterior cervical fusion for cervical degenerative disc disease. As arthroplasty has become a popular treatment option, few complications have been reported in the literature. These include subsidence, expulsion, posterior avulsion fractures, heterotopic ossification, and osteolysis. One of the critical complications is osteolysis, but current studies on this subject are limited in terms of not elucidating the incidence, etiology, and consequences. The authors present two cases, who presented with clinical signs of gradually worsening myelopathy induced by posterior vertebral body osteolysis, 2 years after CDA. Subsequently, the patient underwent posterior decompression and fusion without prosthesis removal. Postoperatively, the clinical symptoms gradually resolved, with no severe deficits. The present rare cases highlight the osteolysis that occurs after CDA, which can cause cervical myelopathy, and suggest spine surgeons to be alert to this fatal complication.

Objects : This study was to report a clinical effect of Chuna Treatment for Cervical Myelopathy due to disc herniation. Methods : The patient was diagnosed as Cervical Myelopathy due to disc herniation through Cervical spine MRI and treated by cervical traction technique of Chuna Treatment with conservative management. JOA score(Japanese Orthopedic Association score), Recovery rate of Hirabayashi, VAS(Visual Analogue Scale), Dynamometer score, ROM(Range of Motion) and Motor weakness Grade were used. Results and Conclusion : JOA score, VAS, Dynamometer score, ROM and Motor weakness Grade were improved and Recovery rate was 100%.

Synovial cysts of the cervical spine, although they occur infrequently, may cause acute radiculopathy or myelopathy. Here, we report a case of a cervical synovial cyst presenting as acute myelopathy after manual stretching. A 68-year-old man presented with gait disturbance, decreased touch senses, and increased sensitivity to pain below T12 level. These symptoms developed after manual stretching 3 days prior. Computed tomography scanning and magnetic resonance imaging revealed a 1-cm, small multilocular cystic lesion in the spinal canal with cord compression at the C7-T1 level. We performed a left partial laminectomy of C7 and T1 using a posterior approach and completely removed the cystic mass. Histological examination of the resected mass revealed fibrous tissue fragments with amorphous materials and granulation tissue compatible with a synovial cyst. The patient's symptoms resolved after surgery. We describe a case of acute myelopathy caused by a cervical synovial cyst that was treated by surgical excision. Although cervical synovial cysts are often associated with degenerative facet joints, clinicians should be aware of the possibility that these cysts can cause acute neurologic symptoms.
