A Clinical Report Seborrheic Dermatitis (지루성피부염 치험1례)

  • Seborrheic dermatitis is acute or choronic inflammatory diseasa frequently caused by herediary factor, endocine abnormality, alchol, etc. It is similar to Myunyoupung in Oriental Medicine. Under the assumption that Sopung-san and Cheongsangbangpung-tang may be effective for treatment seborrheic dermatitis. The symptoms including erythematous plaque, scale, itching were resolved clearly by Sopung-san and Cheongsangbangpung-tang. Sopung-san and Cheongsangbangpung-tang are effective in the treatment of seborrheic dermatitis. if more clinical trials like this are proved to be effective in seborrheic dermatitis, we can expect that Sopung-san and Cheongsangbangpung-tang are used to treatment of seborrheic dermatitis as good method.

Skin Regeneration, Anti-wrinkle, Whitening and Moisturizing Effects of Cheongsangbangpung-tang Aqueous Extracts with Cytotoxicity (청상방풍탕 열수 추출물의 피부재생, 주름개선, 미백, 보습 효과 및 세포독성 평가)

  • Objectives: The present study is to observe the skin-regeneration, anti-wrinkle, whitening and skin moisturizing effects of Cheongsangbangpung-tang (CSBPT) with cytotoxicity. Methods: In the present study, cytotoxicity of CSBPT lyophilized aqueous extracts (yield=18.71%) was experimented against human normal fibroblast cells and B16F10 murine melanoma cells by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium Bromide (MTT) assay, and skin regeneration and anti-wrinkle effects were also showed through the assay of collagen type I synthesis by an enzyme immunoassay (EIA) kit as comparing with transforming growth factor (TGF)-${\beta}1$, hyaluronidase, collagenase and matrix metalloproteinase (MMP)-1 inhibitory assays as comparing with oleanolic acid (OA), and elastase inhibitory effects as comparing with phosphoramidon disodium salt (PP). In addition, whitening effects of CSBPT were observed by tyrosinase inhibitory assay and melanin formation test in B16/F10 melanoma cells as comparing with arbutin, and skin moisturizing effects were measured through mouse skin water contents test, respectively. Results: No CSBPT treatment related cytotoxic effects were demonstrated against human normal fibroblast cells and B16/F10 murine melanoma cells. CSBPT concentration-dependent increased collagen type I synthesis at human normal fibroblast cells. It also effectively suspreessed hyaluronidase, collagenase, elastase and MMP-1 activities, which were enzymes that related to declining of ECM and formation of wrinkle. CSBPT supressed B16/F10 melanoma cells's melanin productions with tyrosinase activity, which was an enzyme connected with melanin formation, and dose-dependent and significant increases of skin water contents were detected in CSBPT treated mouse skin as compared with vehicle control skins. Conclusions: CSBPT showed favorable and enough skin regeneration, anti-wrinkle, whitening and skin moisturizing effects at least in a condition of this experiment. However, more detail mechanism and in vivo skin protective efficacy studies should be conducted in future with the screening of the biological active compounds in individual herbs of Cheongsangbangpung-tang.

Cardiotoxicity assessment of 31 herbal formulae by activity of hERG potassium channel in HEK 293 cells (hERG 칼륨채널 활성도 변화에 따른 31종 한약처방의 심장독성 평가)

  • Objectives: Drug-induced blockade of the human ether-à-go-go related gene (hERG) potassium ion channel causes acquired long QT syndrome, which is known to cause cardiac arrhythmias and be fatal. To establish safety evidence of herbal formulae, we evaluated the effects of 31 herbal formulae on hERG channel activity. Methods: The current through hERG channel was measured by changing the membrane voltage before and after treatment with 31 herbal formulae in HEK 293 cell overexpressing hERG channel using a whole-cell patch clamp system. The current-voltage curves and the activity curves were fitted, and the hERG activity and 50% inhibitory concentration (IC50) according to each herbal formula were calculated. Results: Chokyungjongok-tang, Oncheong-eum, and Cheongsangbangpung-tang strongly inhibited the hERG activity, with IC50 values of 67.67, 141.2, and 296.3 ㎍/mL, respectively. Yeonkyopaedok-san, Eunkyo-san, Ukgan-san gajinphibanha, Daegunjoong-tang (except Oryzae gluten), Insamyangyoung-tang, Banhahubak-tang, SokyungHwalhyul-tang, Jodeung-san, Hyeonggaeyeongyo-tang, and Bangkeehwangkee-tang weakly inhibited hERG activity, with IC50 values ranging from 400 to 1000 ㎍/mL. The other 18 herbal formulae showed very weak hERG activity inhibition of less than 50% at the highest concentration (1000 ㎍/mL). Conclusion: This study provided safety information on cardiotoxicity by cardiac arrhythmia risk assessment of herbal formulae, and is expected to be a reference data for predicting the safety and risk of herbal formulae.

A Case Report of Acne Vulgaris Patient Treated with Cheongsangbangpung-tang (청상방풍탕(淸上防風湯)으로 호전된 성인 여드름 환자 치험 1례)

  • Purpose: This study aims to explain the effect of Chungsangbangpoong-Tang(as followed CBT) on an adulthoods' acne vulgaris. Methods: This is a case report on a female patient who has been suffering from acne vulgaris since she was 19 years old. To make her acne condition better, we designed to provide a specific combined treatment, mainly drinking CBT 2 or 3 times per a day with taking a herbal, acupuncture therapy and acupuncture once per every week through whole session. Results: After a series of we prescribed CBT treatments, we could find that CBT treatments are effective and available to ameliorate for her symptoms of acne vulgaris within its period. Conclusion: This clinical case study shows us that CBT might have not only a potentiality as a new acne treatment but also an effect as a new available prescription for adulthoods' acne. Hence more specific studies about CBT should be demanded in a treatment fields of adulthoods' acne vulgaris as soon as possible.

A Review on Acne Treatment in Korean Medicine by Analyzing Case Studies with Herbal Medicine Treatment (국내 한약 치험례 분석을 통한 한의학적 여드름 치료법에 대한 고찰)

  • Objectives : The purpose of this study is 1) to explore the general tendency of the herbs or herbal formula for acne treatment in clinical practice 2) and to provide a basis for explaining the principles of acne treatment in Korean Medicine in order to make patients understand more easily based on pharmacological mechanism by analyzing the herbs or herbal formula using for acne treatment in clinical practice. Methods : In the domestic databases (Oriental medicine Advanced Searching Integrated System-OASIS, Korean Traditional Knowledge Portal-KTKP, National Discovery for Science Leader-NDSL, Research Information Sharing Service-RISS), we selected among the papers published using search terms related to "acne". Reports related to sasang constitutional medicine were excluded and a total of 23 papers were finally selected. Results : 29 prescriptions were retrieved from 23 papers. Chungan-tang (淸顔湯) notified formulas ranked first by Cheongsangbangpung- tang (淸上防風湯), Hwangnyeonhaedok-tang (黃連解毒湯), Dangguijakyaksan (當歸芍藥散) and Bipachungpe-eum (枇杷淸肺飮). Commonly used herbs are Forsythiae Fructus (連翹), Scutellariae Radix (黃芩), Gardeniae Fructus (梔子), Ligustici Rhizoma (川芎), and Angelicae Gigantis Radix (當歸). In most patients with inflammatory lesions or excessive flushing, exterior-releasing medicinal (解表藥) and heat-clearing medicinal (淸熱藥) were mainly prescribed for acne treatment, while in patients with acne accompanied hyperkeratinization, Excess sebum secretion, Dysmenorrhea or indigestion, qi-tonifying medicinal (補氣藥) or blood-tonifying medicinal (補血藥) were mainly used. KAGS (Korean Acne Grading System) was the most commonly used method for acne severity assessment. Conclusions : These findings suggest that the herbs or herbal formula for acne treatment in Korean Medicine is prescribed differently according to the cause and symptoms of acne in a unique way for control the whole body balance. Also, our findings could provide a basis for explaining the principles of acne treatment in Korean Medicine in order to make patients understand more easily based on pharmacological mechanism in clinical practice.
