How to Effects of Manual Therapy : Chronic Low Back Pain Patients (도수치료가 만성요통환자의 기능회복에 미치는 영향)

  • Purpose : To evaluate effects of joint mobilization on the range of motion of chronic low back pain patients. Methods : The subjects were consisted of sixty patients who had non specific chronic low back pain(10 females. 10 males; mean aged 36.5). All subjects received modalities treatment with therapeutic massage for 10minutes and joint mobilization or manipulation for 10minutes per day and three times a week during 4 weeks period. The Multilevel Roland-Morris Disability Questionnaire(MR-MDQ) was used to measure functional disability level. Visual Analogue Scale(VAS) was used to measure subjective pain level. Remodified Schober test(RST) was used to measure forward flexion range of motion of lumbar segment. Finger-to-Floor test(F-T-FT) was used to measure forward flexion range of motion of full spine of low back pain patients. All measurements of each patients were measured at pre-treatment and 4 week post-treatment. Results : The MR-MDQ, VAS, RCT and F-T-FT were significantly different within-subjects(p<.05), Conclusion : The manual therapy included therapeutic massage and joint mobilization found that improved chronic low back pain patients. Further studies are needed to including more subjects on long-term outcomes.

Degrees of Low Back Pain, Knowledge of and Educational Needs for Low Back Pain in Patients with Chronic Low Back Pain (만성 요통 환자의 통증, 지식 및 교육 요구)

  • Purpose: The purposes of this study were to identify degrees of low back pain, knowledge of and educational needs for low back pain of patients with chronic low back pain and to investigate their relationships. Methods: Data were collected from questionnaires distributed to 83 patients with chronic low back pain at a hospital. Results: The low back pain score was $4.70{\pm}2.22$ out of 10. The degree of low back pain was a statistically significant difference according to gender, smoking, radiating pain and frequency and duration, daily life disturbance degree, sleep disturbance and depression. The knowledge score was 8.29 out of 13. The knowledge was a statistically significant difference according to smoking and degree of sleep disturbance. The educational needs score was 39.83 out of 50. The educational needs was a statistically significant difference according to age, duration of disease, radiating pain, standing time, depression, pain treatment experience, and treatment institutions. As the low back pain increased, the educational needs increased (r=.254, p=.021). There were no correlations between low back pain and knowledge (r=-.040, p=.720) and knowledge and educational needs (r=.061, p=.581). Conclusion: It is important to focus on items with statistically significant differences in pain, knowledge, and educational needs, and to select low knowledge and high educational needs items to develop a systematic education plan.

Effect of Proprioceptive Neuromuscular Facilitation Stretching on Pain, Hip Joint Range of Motion, and Functional Disability in Patients with Chronic Low Back Pain

  • Objective: We aimed to identify the effects of proprioceptive neuromuscular facilitation (PNF) stretching on pain, hip range of motion, and functional disability in patients with chronic low back pain. Design: Randomized controlled trial Methods: In total, 45 patients with chronic low back pain were randomly divided into a conventional stretching group (n=22) and a PNF stretching group (n=23). Both interventions were performed three times per week for 6 weeks. Assessments were made using the visual analog scale, Flexion-Abduction-External Rotation test, modified Thomas test, prone hip extension test, and Oswestry disability index before and after the 6-week intervention period. We conducted a paired t-test to compare the within-group findings before and after the intervention. An independent t-test was used to compare the between-group differences. The statistical significance level was set at α=0.05, for all variables. Results: Both groups showed significant improvements in pain, hip range of motion, and functional disability after the intervention (p<0.05). A significant difference was observed in pain, hip range of motion, and functional disability in patients belonging to the PNF stretching group (p<0.05). Conclusions: This study provides evidence that the application of PNF stretching can effectively reduce pain and improve hip range of motion and functional disability in patients with chronic low back pain.

Lumbar Combined Rehabilitation Exercise for Lumbar Reposition Sense, Static Balance and Pain of the Patient's with Chronic Low Back Pain (만성요통환자의 복합재활운동이 통증과 정적균형, 재위치감각인지에 미치는 영향)

  • The purpose of this study is to identify the effects of the apply of lumbar stabilization exercises and lumbar strengthening exercises using Medx machine on back functions such as static balance, lumbar reposition sense, and back pain(VAS) in chronic low back pain. This study divided 30 chronic low back pain patients who experienced only lumbar back pain(male: 5, female: 10) into a lumbar stabilization exercise group and a complex exercise group, and then performed their respective exercise programs for three times a week over a 8-week period. The lumbar static balance, lumbar reposition sense, and lumbar back pain were measured using a pair t-test within each group, and were also compared between two groups using an independent t-test. The results of the present study were as follows: Both groups exhibited statisti cally significant increases after performing their own exercise program in the static balance, lumbar reposition sense, and lumbar back pain(p<.05). However, the comparison of two groups confirmed that the complex exercise group resulted in greater effects than the lumbar stabilization exercise group(p<.05). In conclusion, muscle strengthening and stabilization exercises in chronic low back pain patients are considered to not only relieve back pain, but also improve various back functions.

  • Low back pain is a common clinical condition with heterogeneous causes and challenges to manage. High prevalence and numerous assessments result in an enormous socioeconomic burden. Clinician must conduct efficient and stepwise evaluation process to rule out serious spinal pathology, neurologic involvement, and identify risk factors for chronicity. The process can be achieved through the focused history taking and physical examination. Certain factors related to serious spinal pathology include age (>50 years), trauma, unexplained fever, recent urinary or skin infection, unrelenting night or rest pain, unexplained weight loss, osteoporosis, immunosuppression, steroid use, and widespread neurological symptoms. In non-specific low back pain, diagnostic imaging and laboratory studies are often unnecessary and can disturb an appropriate management. For the management of acute low back pain, patient education and medication such as acetaminophen, non-steroidal anti-inflammatory drugs, and muscle relaxants are recommended. For chronic low back pain, behavior therapy, back exercise, and spinal manipulation are beneficial. The evidence based approach could improve success rate of management, result in prevention of acute low back pain from being chronic intractable pain.

A Study on multifidus muscle activation by Needle EMG during shoulder flexion in Chronic Low Back Pain Patients (침 근전도로 측정한 만성 요통 환자의 어깨 굴곡시 나타나는 다열근 활성도 비교)

  • Purpose : The purpose of study is activation of lumbar multifidus muscle by needle EMG during shoulder flexion in chronic low back pain patients. The subject were consisted of 10 women patients with chronic low back pain and healthy asymtomatic subject 10 women. Methods : 10 women patients with chronic low back pain and healthy asymptomatic subject 10 women is voluntary participated for the research. Subjects were positioned in standing. The needle EMG were measured activation of multifidus. Needle electrode was used to 28 gauge. The shoulder flexion movement used to activate the multifidus was then measured. Results : Results of the analysis showed that asymptomatic subjects had significantly larger multifidus muscle activation compared with CLBP subjects during shoulder flexion. Conclusion : This study will be used as multifidus measurement method of patient with chronic LBP. The multifidus muscle in chronic LBP patient clinical significance. Most of chronic LBP patients have multifidus contraction pattern. Therefore chronic LBP patients necessary multifidus activation measurement with needle EMG.

Influence of the Application to Hyperbaric Oxygen Therapy and Physical Therapy on Chronic Low Back Pain in Elderly (고압산소와 물리치료의 복합 적용이 노인의 만성 요통 완화에 미치는 영향)

  • Purpose: To investigate the effects of hyperbaric oxygen therapy (HBOT) on chronic low back pain in elderly patients with impaired functioning in daily life. Methods: 35 subjects with chronic low back pain were randomly allocated into two groups, HBOT group and Placebo group. We applied TENS and mechanical massage to the lower back in both groups. Then 35% oxygen at 1.4 ATA was given to the HBOT group and atmospheric oxygen at 1 ATA to the placebo group. Results: Comparing the HBOT group with the placebo group after performing interventions six times in 2 weeks, a statistically significant difference in the Visual Analogue Scale (VAS) and Oswestry Disability Index (ODI) scores was observed between the two groups. There was a statistically significant difference in VAS and ODI scores (56% and 34.97% respectively) between the HBOT group and the placebo group. A statistically significant difference was ascertained in the degree of variables VAS and ODI in the HBOT group. Conclusion: Application of HBOT in chronic low back pain could be beneficial for reducing pain and could help in functional recovery.

A comparative study on the cardiovascular function response to maximal exercise of chronic low back pain patients and normal group (최대운동부하시 정상인과 만성요통환자의 심폐계 반응에 대한 비교연구)

  • This study is aimed to determine the cardiovascular function response to maximal exercise of chronic low back pain patients(N=13) and normal group(N=13). by using BRUCE PROTOCOL, subjects underwent tredmill exercise test. Their cardiovascular function responses during rest and after maximal exercise were compared The responses were analyzed using t-test for SPSS 7.0 program. The Cardiovascular function variables employed at rest time(Vo2, HR. Vo2/kg, VE, Vco2.) and all out time(Vo2peak. HRpeak, Vo2peak/kg, VEpeak. Vco2peak). Result show that : 1 There was no significant difference in Vo2 between chronic low back pain patients and normal group at rest time. However significant difference in Volpeak was observed after maximal exercise( p<.05). 2. There was no significant difference in HR between chronic low back pain patients and normal group at rest time. No significant difference in HRpeak likewise observed. 3. There was no significant difference in Vo2/kg between chronic low back pain patients and normal group at rest time. However significant difference in Vo2peak/kg was observed after maximal exercise load(p<.05). 4. There was no significance in VE between chronic low bark pain patients and normal group at rest time. However significant difference in VEpeak observed after maximal exercise load(p<.05).

Development of Questionnaire for Pattern Identification of Chronic Low Back Pain by Delphi Method (만성 요통 변증 설문지 개발을 위한 전문가 Delphi 조사)

  • Objectives The aim of this study is to contribute to developing questionnaire for pattern identification of chronic low back pain using delphi method. Methods The questionnaire which includes symptoms and signs of chronic low back pain is studied by delphi method. Seven experts of Korean medicine, especially of rehabilitation medicine participated in delphi examination. Delphi examination was carried out through evaluating and correcting the questionnaire by e-mail. Results By the Delphi method and score evaluation, 20 items of the initial copy of questionnaire are chosen for pattern identification of chronic low back pain. Conclusions By the delphic method among experts, a reference questionnaire for pattern identification of chronic low back pain was suggested. Further research is necessary for modification of questionnaire by statistics and certification by clinical trial research.

Comparison of the Effects of Joint Mobilization, Gym Ball Exercises, and Breathing Exercises on Flexion Relaxation Phenomenon and Pain in Patients with Chronic Low Back Pain

  • Background: Although various exercises have been performed for patients with chronic low back pain (CLBP), the effects of these exercises including joint mobilization, gym ball exercises, and breathing exercises on flexion relaxation ratio (FRR) have not been compared. Objective: To compare the effects of joint mobilization, gym ball exercises, and breathing exercises on the flexion relaxation phenomenon (FRP) and pain in patients with chronic low back pain. Design: Randomized pretest-posttest control group design. Methods: Thirty-six patients with chronic low back pain who were undergoing rehabilitation at a rehabilitation center were included. The patients were randomly divided into three groups: joint mobilization group (JMG; n=12), gym ball exercise group (GBG; n=12), and breathing exercise group (BEG; n=12). The exercises were performed for 40 minutes a day, twice a week, for a total of 12 weeks. Results: There were no significant differences in FRR between the three groups (P>.05). Significant decreases in the modified visual analog scale (MVAS) scores after intervention between the groups were found (P<.05). The GBG was significantly decreases from the JMG in the MVAS (P<.05). However, there were significant improvements between the pre- and post-interventional findings on FRR and MVAS in the three groups (P<.05). Conclusion: We demonstrated that intervention using joint mobilization, gym ball exercises, and breathing exercises improve FRP and pain in patients with CLBP.
