개심술후 유미심낭 (Chylopericardium) 합병증의 수술치험 1예 (One case of surgical treatment for chylopericardium following cardiac surgery)

  • Only five instances of chylopericardium following cardiac surgery have been reported in the literature previously. We encounted this complication in a patient who was operated on for secundum atrial septal defect. The patient readmitted one month after discharge because of large amount of chylous pericardial effusion. Conservative treatment of pericardiostomy drainage and parenteral hyperalimentation was continued for 3 weeks without improvement. Partial pericardiectomy and pericardiopleural window was done with success and no recurrence of chylopericardium was observed upto 3 months after surgery. We think this is the first case report of chylopericardium after open heart surgery in Korea.

유미흉 및 유미심낭의 치료 (Management of Chylothorax and Chylopericardium)

  • The development of chylothorax and chylopericardium are serious and often life-threatening clinical entity. The ideal treatment of these problems is not well established to date. We reviewed our experiences with chylothorax or chylopericardium in 16 patients [17 cases] from July 1979 to May 1989. Ages ranged from 20 days to 41 years. The etiologies were traumatic in 10, congenital or idiopathic in 5, and tuberculous lymphadenopathy in one. In 8 patients, the chylothorax or chylopericardium occurred as a complication of cardiothoracic surgery. Eleven patients were treated nonoperatively with either repeated thoracenteses or chest tube drainage. Five patients underwent operative treatment: transthoracic thoracic duct ligation [three patients], thoracic duct ligation combined with decortication [one], and oversewn the defect of mediastinal pleura[one]. Duration of preoperative therapy ranged from 18 to 38 days. One of eleven [9.1 %] patients treated nonoperatively died. Of the surgically treated group, there were no deaths. All patients except one dead cured or improved either treated nonoperatively or operatively. Our experience suggests that surgical management of the chylothorax or chylopericardium is not always required, and each patient must be judged individually according to disease process.

  • Chylopericardium is a rare disease entity characterized by the accumulation of chylous fluid in the pericardial sac. It usually arises from mediastinal neoplasms, thrombosis of the subclavian vein, tuberculosis, nonsurgical trauma, thoracic or cardiac surgery. The spectrum of symptoms for chylopericardium varies from an incidental finding of cardiomegaly to dyspnea, upper abdominal discomfort, cough, chest pain, palpitation, fatigue. However, most of the patients are asymptomatic. The main purpose of treatment of chylopericardium is the prevention of cardiac tamponade and prevention of metabolic, nutritional, and immunological compromise due to chyle leak. Here, we report a case of chylopercardium secondary to lymphangiomyoma with review of the literature.

  • A 17-year-old man was admitted for chronic anterior chest pain and dyspnea. He was undergone pericardiocentesis for chylopericardium and thoracostomy tube drainage for right sided chylothorax. Approximately 2000ml per day from right chest tube was drained during 20 days Supradiaphragmatic ligation of thoracic duct was performed and there was no drainage postoperatively and immediately antituberculous medication was done.

관통성 손상에 의한 심장내 이물 - 수술 치험 1례 - (Intracardiac Foreign Body by Penetrating Cardiac Injury)

  • Violence in our society, combined with improving transport system, resulted in increased numbers of patients with cardiac wounds reaching the hospital alive. Most patients with penetrating cardiac injury, rather than blunt injury, present with a syndrome of either hemorrhagic shock or cardiac tamponade. And they should be operated upon as soon as possible. Often the atrioventricular valves and other important cardiac structures are also damaged by the penetrating instruments or missile. Both intracardiac communications and atrioventricular fistulas may result in significant left-to-right shunts accompanied by congestive heart failure, necessitating surgical correction. Usually, retained cardiac foreign bodies, which are almost always bullets or fragments of missiles, may lie within a cardiac chamber or in the myocardium. Emboli of bullets or other missiles from distant sites to the right side of the heart are numerous enough to require attention. Recently we experienced a case with intracardiac foreign body due to penetrating cardiac injury. A 19 year-old man was admitted to our hospital due to penetrating anterior chest wound by iron segment. The roentgenogram of the chest revealed a radio-opaque metallic shadow in left lower chest around the cardiac apex, mild blunting of left costophrenic space, but no cardiomegaly. During operation the foreign body was noted to be present in the cardiac chamber by the portable C-arm fluoroscopy. But during the manipulation it moved into left inferior pulmonary vein from left ventricle by way of left atrium. So we could manage to remove it from left inferior pulmonary vein by direct approach to the vein. It was iron segment, sized 0.lcm x0.6cmx0.5cm, with sharp margins. The patient had an uneventful postoperative recovery except for chylopericardium and was discharged.

영아기 심실중격결손 봉합술의 임상적 고찰 (Clinical Analysis of Patch Repair of Ventricular Septal Defect in Infant)

  • 단순 심실중격결손증은 선천성 심질환중 흔한 질환이지만 어린 나이 혹은 저체중환아에서 시행하는 경우 경험에 따라 만족스럽지 못한 경우도 있어 영아기에 수술을 시행한 단순 심실중격결손증 환자들의 수술 결과를 조사하였다. 1996년부터 2000년까지 첩포봉합술을 시행한 45명을 대상으로 체중 5 kg을 기준으로 수술의 결과를 분석하였다. 수술사망은 없었으며 두 군간의 합병증 발생에 있어 유의한 차이는 없었다. 수술 수기로 사용된 단순 연속봉합법은 전례에서 적용할 수 있었으며 증상이 있는 심실 중격결손증은 체중이 5kg이하의 환아에서도 시기에 관계 없이 안전하게 시행할 수 있었다.
