Circulatory Disease Surveillance System in Korea (순환기질환 감시체계)

  • The purpose of establishing the circulatory disease surveillance system in Korea is to ensure that the problems of circulatory disease importance are being monitored efficiently and effectively. The goals of circulatory disease surveillance system are to monitor the epidemiological trends of circulatory disease and to evaluate the outcome of health activity for controlling circulatory diseases. Surveillance system are being updated to achieve the needs for the integration of the surveillance and information system, the establishment of data standards, the electronic exchange of data, and changes in the goals of circulatory disease surveillance system to facilitate the response of this system to manage the national health problem effectively. This article provides the target diseases and determinant indicators to be monitored, structure of circulatory disease surveillance system, and many tasks and related activities that should be applied to this system.

Lipid-lowering and Antioxidant Effects of Curcuma Radix in Poloxamer 407-induced Hyperlipidemia Model Rat Models

  • Objectives : This study investigated the hypolipidemic and antioxidant effects of Curcuma radix using a rat model induced by poloxamer 407 injection. Methods : Serum lipid parameters and oxidative stress-associated biomarkers were determined. Additionally, hepatic cholesterol and triglyceride as well as lipid metabolism-associated gene expressions were observed in hepatic tissue. Results : 1. Curcuma radix ameliorated elevation of serum cholesterol, triglyceride, LDL-cholesterol, MDA, hepatic cholesterol level, and reduction of serum TAC, SOD, GSH, GSH-reductase level. 2. Curcuma radix augmented up-regulated ACAT gene expression. 3. Curcuma radix almost completely ameliorated down-regulated CYP-7A1 but up-regulated HMG-CoA gene expression. Conclusions : The hypolipidemic and antioxidant properties of Curcuma radix were evidenced. This study provides a scientific basis for the clinical application of Curcuma radix and development of hypolipidemics using this herb in the future.

A Study on the Cause of Death of School Teachers in Korea (한국 교원의 사인에 관한 연구)

  • Mortality rate and causes of death are regarded as an index of strength as well as level of development of a country. However, there is no accurate data for the causes of death in Korea due to lack of systematic vital data collection system. The objective of this study was to define the causes of death of the school teachers, its changing pattern, cause-specific mortality rate, and geographic variation. The study population included all of the teachers in primary school, middle and high schools, and college who joined in Korean Teachers' Union between 1968 and 1985 that provided a total of 1,972, 069 person-years to observe (1,384,911 man-years, 587,158 woman-years). There were 3,678 deaths in this period (3,377 males, 301 females). The most common cause of death was neoplasm which was followed by the diseases of circulatory system. The proportion of death of neoplasm was 1.5 times higher than that of the general population. Causes of death were classified into 5 major groups (neoplasm, diseases of circulatory system, accidents and poisoning, diseases of liver, and all others). The mortality rates of diseases of circulatory system and all others for general population were 4 to S times higher than those for the teachers. However, mortality rates of neoplasm and diseases of liver were only about 2 times higher than those for teachers. Mortality rate of liver cancer for teachers was higher than gastric cancer mortality rate which is the reverse in general population. The crude death rate was 2.12 per 1,000 person-years for male and 1.00 for female which is one-third of the crude death rate of general population. Crude death rate of study population was higher in rural area than in urban area. However, mortality rate of neoplasm for male was higher in urban area than in rural area while mortality rates of all other causes were higher in rural area. For female, mortality rates of neoplasm and diseases of circulatory system were higher in urban area and the rates for all other causes were higher in rural area. Crude death rate was lowest in Gyeongin area and highest in Yeongnam area. The mortality of neoplasm for male accounted the highest proportion of all death in Gyeongin, Chungcheong and Yeoungnam areas while the mortality of neoplasm and mortality of circulatory system accounted the same proportion in Jeonra area. For female, the mortality of disease of circulatory system accounted the highest proportion in Gyeongin and Yeoungnam and Jeonra areas. Proportion of death due to accidents and poisoning was high in Chungcheong area and death due to all other causes was high in Yeoungnam area. The most common cause of death for male by city and province was neoplasm in Seoul, Busan, Daegu, Gyeonggi, Chungnam, Chungbuk, Gyeongnam and Gyeongbuk. Diseases of circulatory system was the leading cause of death in the rest of city and provinces. The leading cause of death for female was diseases of circulatory system in Seoul, Incheon, Chungbuk, Chungnam, and Gyeongbuk, neoplasm in Busan, and accident and poisons in all other cities and provinces. The mortality rates of male were above 2 per 1,000 person-years in Jeju, Gyeongbuk, Gyeongnam, Daegu, and Chungbuk, and it was below 1.5/l,000 in Seoul, Incheon and Gyeonggi. The mortality rate of female was above 1.2/1,000 person-years in Gyeongnam and Incheon while it was below 0.5/l,000 in Daegu, Geonggi Chungbuk and Jeju. The leading cause for male by school of employment was neoplasm in all levels of school with a remarkably higher rate in the professors of college. Leading cause of death for female was disease of circulatory system in primary schools, high schools and college but neoplasm in middle schools. There was no death due to liver diseases in middle and high school teachers and college professors and no death due to all other category in high school teachers and college professors, in females. High school teachers and the highest mortality rate and college professors showed the lowest mortality rate. Temporal trend of mortality was examined in three periods; period I ($1968{\sim}1974$), period II ($1975{\sim}1979$), and period III ($1980{\sim}1985$). The leading cause of death for male was diseases of circulatory system in period I and II but neoplasm in period III. Such trend of decreasing diseases of circulatory system and increasing neoplasm was observed in female. Overall mortality rate was decreased over the 3 periods. The mortality rates of diseases of circulatory system, liver disease and all others were decreased in male but the mortality rates of neoplasm and accident and posions was increased. Female showed a similar trend to male but the mortality rate of liver diseases was increased. Mortality rates of diseases of circulatory system, neoplasm and liver diseases increased with age of teachers up to 50 years of age but decreased in 60 years of age. Mean age at death due to each cause was higher in male than female by $4{\sim}10$ years. However, the mean age at death of the teachers was $2{\sim}5$ years lower than that of the general population in all causes of death and the sex difference in the mean a2e at death was smaller ($2{\sim}3$ years) in general population. In sex ratio of mortality, male was higher than female in almost all diseases except suicide and maintained a high ratio. The general population showed universally high ratio in male like teachers, and more or less did regular patterns in mortality with ratio smaller.

A Review of Clinical Studies for Treatment of Multiple System Atrophy Using the CNKI Database (다계통 위축증의 치료에 대한 임상연구 동향 - CNKI검색을 중심으로)

  • Objectives: The aim of this research was to investigate and review the trends in recent clinical studies of multiple system atrophy (MSA) in China. Methods: We searched for clinical studies discussing Oriental medicine-based treatments for MSA in the China National Knowledge Infrastructure (CNKI) database. The search focused on the authors; publication year; type of study; method and duration of treatment; evaluation criteria; and results of the selected articles. Results: Among the articles retrieved from the database, eight case series, two randomized controlled trials (RCTs), and one non-randomized controlled trial (nRCT) were analyzed. Eight articles used acupuncture; ten used herbal medicine. The most frequently used acupoints were GV20, GV24, GB20, PC6, Hyeopcheok Acupoint, CV4, EX-HN3, and GV17. Jihwangeumja was the most commonly used herbal prescription. All 13 studies confirmed the efficacy of Oriental medicine treatments. Conclusions: More varied and scientifically designed clinical studies are required to develop treatments for MSA. The results of this study may provide guidance for further studies of MSA.

The Effects of Gamijihwang-tang(GJT) on Hyperlipidemia in Rats (가미지황탕(加味地黃湯)이 고지혈증(高脂血症) 관련(關聯) 인자(因子)에 미치는 영향)

  • Objective : Gamijihwang-tang(GJT) has been used as a therapeutic agent for hyperlipidemia in oriental medicine for several years. This study was performed to investigate the effects of GJT on hyperlipidemia in rats using diverse biological methods. Method : Hyperlipidemia was induced by a hyper-lipidemic diet fed for 4 weeks. Total cholesterol, LDL-cholesterol, HDL-cholesterol, triglyceride, and glucose were measured in the serum after an oral administration of GJT. Lipid peroxidase, SOD, catalase, ACAT, and HMG-CoA were measured in liver after oral administration of GJT. Result: 1. GJT showed safety against cytotoxicity and toxicity in liver. 2. GJT significantly decreased rat's body and liver weight. 3. GJT significantly decreased serum total cholesterol and LDL-cholesterol, but increased serum HDL-cholesterol. 4. GJT significantly decreased serum triglyceride and glucose. 5. GJT significantly decreased lipid peroxidation and increased SOD and catalase in liver. 6. GJT significantly decreased ACAT and HMG-CoA reductase of cholesterol manifestation in liver. Conclusions : These results suggest that GJT might be effective in treatment and prevention of hyperlipidemia.

Comparative Analysis of the Characterastics of Oriental Medicine Pattern Identification according to the Plasma Fibrinogen Levels and Platelet Counts in Stroke Patients (뇌졸중 환자의 Plasma Fibrinogen Level 및 Platelet Counts에 따른 한방 변증 비교 분석)

  • The aim of this study was to evaluate oriental medicine pattern identification in patients with stroke on the basis of plasma fibrinogen levels and platelet counts. 555 patients diagnosed with stroke between November 2006 and February 2010 were divided in several ways according to the plasma fibrinogen levels and platelet counts on admission. And comparative analysis of the distribution ratio of oriental medicine pattern identification was done between the groups. The mean value of the plasma fibrinogen levels of whole population was 449.18 mg/dL, and it was higher than normal range. The mean value of the platelet counts of whole population was 244.29 /mL, and it was lower but in normal range. Oriental medicine pattern identifications were not characteristic between groups divided according to the serum levels of fibrinogen and platelet counts. In this study, there was not significant correlation between Oriental medicine pattern identifications and the thrombotic factor like plasma fibrinogen levels and platelet counts. This study could be the steppingstone for the next study to develope the objective indicator for the Oriental medicine pattern identifications.

Impact of Complex Hemodynamics to the Management of ArterioVenous(AV) Fistula (동정맥루의 복합성 혈류학 소견이 그 관리에 미치는 영향)

  • Human circulatory system between heart and tissue is not directly connected in normal condition but mandatory to go through the capillary system in order to fulfill its physiologic aim to deliver oxygen and nutrients, etc. to the tissue and retrieve used blood together with waste products from the tissue properly. When abnormal connection between arterial and venous system (AV fistula), these two circulatory systems respond differently to the hemodynamic impact of this abnormal connection between high pressure (artery) and low pressure (vein) system. Depending upon the location and/or degree (e.g. size and flow) of fistulous condition, each circulatory system exerts different compensatory hemodynamic response to this newly developed abnormal inter-relationship between two systems in order to minimize its hemodynamic impact to own system of different hemodynamic characteristics. Pump action of the heart can assist the failing arterial system directly to maintain arterial circulation against newly established low peripheral resistance by the AV fistula during the compensation period, while it affects venous system in negative way with increased venous loading. However, the negative impact of increased heart action to the venous system is partly compensated by the lymphatic system which is the third circulatory system to assist venous system independently with different hemodynamics. The lymphatic system with own unique Iymphodynamics based on peristaltic circulation from low resistance to high resistance condition, also increases its circulation to assist the compensation of overloaded venous system. Once these compensation mechanisms should fail to fight to newly established hemodynamic condition due to this abnormal AV connection, each system start to show different physiologic ${\underline{de}compensation}$ including heart and lymphatic system. The vicious cycle of decompensation between arterial and vein, two circulatory system affecting each other by mutually negative way steadily progresses to show series of hemodynamic change throughout entire circulation system altogether including heart. Clinical outcome of AV fistula from the compensated status to decompensated status is closely affected by various biological and mechanical factors to make the hemodynmic status more complicated. Proper understanding of these crucial biomechanical factors iii particular on hemodyanmic point of view is mandatory for the advanced assessment of biomechanical impact of AV fistula, since this new advanced concept of AY fistula based on blomechanical information will be able to improve clinical control of the complicated AV fistula, either congenital or acquired.

Hydrodynamic Design of Thrust Ring Pump for Large Hydro Turbine Generator Units

  • Thrust-ring-pump is a kind of extreme-low specific speed centrifugal pump with special structure as numerous restrictions from thrust bearing and operation conditions of hydro-generator units. Because the oil circulatory and cooling system with thrust-ring-pump has a lot of advantages in maintenance and compactness in structure, it has widely been used in large and medium-sized hydro-generator units. Since the diameter and the speed of the thrust ring is limited by the generator set, the matching relationship between the flow passage inside the thrust ring (equivalent to impeller) and oil bath (equivalent to volute) has great influence on hydrodynamic performance of thrust-ring-pump. On another hand, the head and flow rate are varying with the operation conditions of hydro-generator units and the oil circulatory and cooling system. As so far, the empirical calculation method is employed during the actual engineering design, in order to guarantee the operating performance of the oil circulatory and cooling system with thrust-ring-pump at different conditions, a collaborative hydrodynamic design and optimization is purposed in this paper. Firstly, the head and flow rate at different conditions are decided by 1D flow numerical simulation of the oil circulatory and cooling system. Secondly, the flow passages of thrust-ring-pump are empirically designed under the restrictions of diameter and the speed of the thrust ring according to the head and flow rate from the simulation. Thirdly, the flow passage geometry matching optimization between thrust ring and oil bath is implemented by means of 3D flow simulation and performance prediction. Then, the pumps and the oil circulatory and cooling system are collaborative hydrodynamic optimized with predicted head-flow rate curve and the efficiency-flow rate curve of thrust-ring-pump. The presented methodology has been adopted by DFEM in design process of thrust-ring-pump and it shown can effectively improve the performance of whole system.

  • This study was performed on the bases of clinical consideration about patient who had fever. The patient had a high fever, headache, diarrhea, sore throat, general aching. The symptoms of the patient was regarded as syndrome of Ki(energy) system in the differentiation of epidermic febrile disease and syndrome of the middle energizer in the differentiation of syndrome in accordance with the therory of triple energizer. According to the oriental medicine principle, oriental medicine was taken such as Dalwonum(達原飮) and the patient had taken a turn for the better.

Development and Application Effect of STEAM Program Using Technology Based on TPACK - Focused on the Circulatory System - (TPACK 기반 테크놀로지 활용 STEAM 프로그램 개발 및 적용 효과 - 순환 기관을 중심으로 -)

  • The purpose of this study is to develop a STEAM program using technological pedagogical and content knowledge (TPACK) model to effectively utilize technology to solve the difficulties in the teaching of 'structure and function of our body' unit in the primary science curriculum and to confirm the effect on the academic achievement, creative problem solving ability and scientific interests of elementary students. The program was developed as the STEAM program of the 8th class by utilizing the construction knowledge of the TPACK model. The developed program was applied to 29 experimental group students in 5th grade. And the textbook-oriented circulatory system learning program was applied to 29 5th graders in the same school. As a result of the application of the program, the experimental group showed significant improvement over the comparison group in its creative problem solving ability and scientific interests, and the satisfaction of the class was also high. This caused a positive effect on students because the process of self-directing information about the circulatory system using smart devices, making outputs creatively using 3D printers, and presenting them through role play using produced outputs.
