Cryotherapy for Treatment of a Mucocele on Lower Lip (냉동요법(Cryotherapy)을 이용한 점액낭종(Mucocele)의 치료)

  • Mucoceles of the minor salivary glands are the most common cystic lesions affected the oral mucosa. They are believed to be the result of trauma to the salivary duct caused, for example, by biting the lip, cheek, or tongue. Surgical excision has been the most common treatment for these lesions, but occasional recurrences develop after excision because surgical trauma may damage the surrounding minor salivary glands Although various alternative nonsurgical approaches, such as steroid infection, application of gamma-linolenic acid, have also been reported, they are not used routinely, Lasers, particularly the carbon dioxide laser, have been used in the management of mucoceles. Although this treatment requires specialized equipment. Cryotherapy is another effective nonsurgical method for treating mucoceles. Clinically, cryotherapy has primarily been applied to the treatment of leukoplakia and hyperplastic, granulomatous, vascular, and pigmented lesions. Limited information, however, is available on the application of cryotherapy in salivary gland lesions, including mucoceles. A simple and easy cryotherapy to treat a mucocele on the lower lip is described. A 25 years old female patient with a mucocele on the lower lip was treated by direct application of liquid nitrogen with a cotton swab. The lesion was exposed to 4 or 5 cycles composed of freezings of 10-30 s and thawings of double the freezing times. No anesthesia was required. The lesion nearly disappeared without scar 10 days after the cryotherapy. Cryotherapy has become an established nonsurgical method, characterized by its simple application, therapeutic effectiveness, painless during the procedure and low incidence of secondary infection and hemorrhage.

A Literatural Study on the Evidence of Using Thermotherapy and Cryotherapy of Meridian Muscle in Korean Medical Physiotherapy - Focusing on 『Hwangjenaegyeong』 - (경근(經筋)에 대한 온열(溫熱)·한냉요법(寒冷療法)의 적용에 대한 고찰(考察) - 『황제내경(黃帝內經)』을 중심으로 -)

  • Objectives : The purpose of this paper is to establish theoretical bases upon which to complement the clinical evidence in representative literature (Hwangjenaegyeong;黃帝內經)of Korean medicine. Methods : We searched applicable paragraphs about thermotherapy and cryotherapy in Hwangjenaegyeong(黃帝內經) and defined them as historical and theoretical bases of thermotherapy and cryotherapy in korean medicine. Results : The meridian muscle treatment originated from Young-Chu(靈樞) was developed for many centuries. Five paragraphs about thermotherapy and one paragraph about cryotherapy are in Hwangjenaegyeong(黃帝內經). These records to treat the disease of the muscle meridian are enough to explain historical and theoretical evidences. Conclusions : Thermotherapy and cryotherapy of meridian muscle are treatments which are used widely in Korean medicine. They mean that thermotherapy and cryotherapy has been explained under the system of Korean medicine. It is necessary for more literatural study and clinical trials to be carried out to secure the evidence of physical therapies in Korean medicine.

Effect of whole body cryotherapy with spinal decompression on lumbar disc herniation by functional assessment measures (기능적 평가 측정을 통하여 전신냉기치료와 척추 감압기 적용이 요추 추간판 탈출증 환자에게 미치는 효과)

  • This study was conducted to investigate whether whole body cryotherapy and spinal decompression have effectiveness on the pain, lumbar flexion range of motion, and global perceived effect scale in patients with lumbar disc herniation. Thirty subjects were participated in this study. And they were all randomly divided into whole body cryotherapy and spinal decompression group. To evaluate the effects of whole body cryotherapy and spinal decompression, subjects were evaluated by using visual analogue scale, lumbar flexion range of motion, and global perceived effect scale. The assessment parameters were evaluated before, after 2 weeks treatments. And we received a consent form whole body cryotherapy subjects. The results of paired t-test, independent t-test, and ANCOVA analysis of variance showed that pain, lumbar flexion range of motion, global perceived effect scale were significantly improved after than before therapy in whole body cryotherapy group. So we conclude that whole body cryotherapy has effectiveness on the pain, lumbar flexion range of motion, and global perceived effect scale in patients with lumbar disc herniation.

A Literatural Study on the Evidence of Using Thermotherapy and Cryotherapy of Cutaneous and Muscle Meridian in Korean Medical Physiotherapy (한방이학요법 중 경피경근온냉요법에 대한 문헌적 고찰)

  • Objectives : The purpose of this study is to establish literatural evidence about thermotherapy and cryotherapy for Korean medicine through literatural review. Methods : Applicable paragraphs which were related to the thermotherapy and cryotherapy of cutaneous and muscle meridian were phrased from in "Yibujicheng(醫部集成) and "Dongyibaojian(東醫寶鑑)" where were archiving of Oriental or Korean medicine literatures. Searched paragraphs were analysed for establishing historical and theoretical bases of thermotherapy and cryotherapy in Korean medicine. Results : Thermotherapy of cutaneous and muscle meridian(經皮經筋溫熱療法) such as hot pack, warm water therapy, paraffin bath, ultrasound is originated from yu(慰) warm water(溫水) hot water(熱水). Matching indications are various pain conditions(caused by coldness(寒), hard-work(僗若), extravasated blood(瘀血), inflammatory skin disease, frostbite and several internal diseases. It also treats gynecological diseases and facial palsy. Diathermic therapy on acupuncture points(穴位照射溫熱療法) such as infra-red, microwave, shortwave is originated from huolu(火爐), wenlu(溫爐), xianglu(香爐), lamp light(燈火). Its objective is to improve the effects of herb medicine by aiding sweating or to treat the residual symptoms of fever disease or to care skin disease and pain from bone fracture, contusion. Cryotherapy of cutaneous and muscle meridian(經皮經筋溫寒冷療法) such as ice pack, ice spray, iced whirpool, cool water bath is originated from lengfu(冷敷), lengtie(冷貼), lengshiyu(冷石熨). Matching indications are contusions, animal bite injury, corn(肉刺) and (淋病), eye disease, nasal bleeding, hemorrhoid, inflammatory skin disease and chicken pox. Conclusions : Thermotherapy and cryotherapy of cutaneous and muscle meridian(經皮經筋溫冷療法) are the treatments which were used in Korean medicine from the ancient Korean medicine. As scientific equipments were originated from yu(慰), huolu(火爐), wenlu(溫爐), xianglu(香爐), lamp light(燈火). lengfu(冷敷), lengtie(冷貼), lengshiyu(冷石熨). It can be said that these are elements of Korean medicine. More rigorous studies are needed to establish clinical evidence about not only thermotherapy and cryotherapy but also the other physiotherapy of Korean medicine.

Nursing Protocols of Cold Application in Different Medical Institutions in Korea (전국 의료기관의 냉 요법 실무지침 및 임상간호사의 냉 요법 실무 현황)

  • Purpose: The purpose of this study was to evaluate nursing protocols for superficial cryotherapy in different medical institutions. Methods: The study was conducted with a cross-sectional descriptive design. The medical institutions including general hospitals, hospitals, and geriatric hospitals were randomly selected. A total of 435 nurses from 126 institutions completed a questionnaire. Data were collected from December 2014 to June 2015. Results: Forty-two institutions (39.5%) had nursing protocol for cryotherapy. The nurses reported that durations of cold application were 2 minutes to 5 hours. Frequently used cold therapy devices in order of frequency were frozen gel packs, ice packs, and frozen IV fluid bags. There were variances in the duration of cold application according to the types of institution ($x^2=7.78$, p=.020) and nursing units ($x^2=26.42$, p<.001). In addition, intervals of cold application were different according to the nursing units (x=12.23, p=.032). There were differences in cold application instruments by regional groups (x=70.38, p<.001). Most of the nurses (95.6%) responded that national nursing protocol for superficial cryotherapy were needed. Conclusion: There were difficulties in providing consistent nursing interventions because of the practical differences and absence of evidence-based guidelines for cryotherapy. The researchers recommend that basic studies with various instruments be conducted and proper nursing protocols be developed for cryotherapy.

Effect of intracanal cryotherapy on postendodontic pain: a systematic review and meta-analysis of randomized controlled trials

  • This systematic review aimed to qualitatively and quantitatively evaluate the effectiveness of cryotherapy in the reduction of postendodontic pain. The review question was, "What will be the success rate of cryotherapy technique among human patients with postendodontic pain?". The review protocol was framed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) checklist. Six studies were included in the review, and quantification of five studies was performed through a meta-analysis. In the forest plot representation of the studies comparing the control and cryotherapy groups in terms of the success rate in the management of postendodontic pain, the combined risk ratio (RR) was 0.80 (95% CI: 0.56 to 1.13) with a P value of 0.20. Based on the quantitative analysis, it can be suggested that intracanal cryotherapy does not play a significant role in reducing postendodontic pain.

The Effects of Cryotherapy Treatment with Leg Elevation on Swelling of Patient Who had an TKA (슬관절 전치환술(TKA) 환자를 대상으로 다리거상과 냉치료 적용이 부종에 미치는 영향)

  • Background: The purpose of this study was to investigate the effect of structured cryotherapy on edema of patients who had a total knee arthroplasty (TKA). Methods: The subjects were 58 patients who were diagnosed with osteoarthritis and had total knee arthroplasty in D hospital. In total, 29 people were in the leg elevation cryotherapy group (LECTG), 29 people were in the control group (CTG). After TKA, the patients' swelling on post operation days (POD) 3 were checked. After the post OP checking, LECTG was treated with cryotherapy with leg elevation for 12 days but CTG was cryo therapy with sit on chair. Results: Swelling show significant difference within the group (p<.05), but did not show signigicant difference between groups (p>.05). Conclusions: Base on the results, it was found the cryo therapy of $-78^{\circ}C$ improved swelling after TKA. Accordingly, it is thought that cryo therapy of $-78^{\circ}C$ has positive effect on swelling in the process of recovery after operation. but It did not approve that the leg elevation was even more effective. For this study, the reserch developed to enhance the effectiveness of the cryotherapy, would make it possible to apply to an effective cryotherapy posture.

Therapeutic Effect of Superficial Cryotherapy on Alopecia Areata: A Prospective, Split-scalp Study in Patients with Multiple Alopecia Patches

  • Background: Alopecia areata (AA) affects anagen hair follicles resulting in non-scarring hair loss. Since its introduction, superficial cryotherapy has been used as one of the meaningful treatment modalities for AA. Objective: The purpose of this study is to clarify the therapeutic efficacy and safety of superficial cryotherapy for treatment of AA. Methods: In 19 patients with multiple bilateral AA patches on their scalp, superficial cryotherapy was performed on the right side, every 2 weeks. Prednicarbate 0.25% solution was applied twice a day to both the treated and the control sides. Clinical improvement was estimated using the Severity of Alopecia Tool (SALT) score by 3 different dermatologists, along with the changes in terminal and vellus hair count and hair thickness analyzed by phototrichogram. Results: After 4 months of treatment, mean terminal hair count on the right scalp was 1.6-fold increased (p=0.005), while mean terminal hair count on the left scalp showed no significant change (110.1% increase, p=0.285). Mean SALT score decrease on the right was 4-fold greater than that on the left (40.7% and 9.6%), without statistical significance (p=0.282). Mean vellus hair count and mean hair thickness did not show significant changes after superficial cryotherapy. No remarkable adverse event was observed. Conclusion: Superficial cryotherapy is an efficacious treatment modality with advantages of simplicity and noninvasiveness. It can be considered as a meaningful therapeutic modality for AA, especially when the disease status is limited to minimal to mild, or the conventional AA treatment is not applicable.

  • A cryotherapy system using the low temperature air was developed. The developed cryotherapy system had superior low temperature characteristics and many safety devices. For a clinical evaluation, skin and intra-articular temperatures of the knee joint were measured during and after the cryotherapy. After 5-minute therapy, skin and intra-articular temperature decreased by $23.3{\pm}4.7(^{\circ}C)$ and $4.1{\pm}1.0(^{\circ}C)$ respectively. A 5-minute cryotherapy therapy is good enough to maintain low intra-articular temperatures or 2-3 hours.

Successful Removal of Endobronchial Blood Clots Using Bronchoscopic Cryotherapy at Bedside in the Intensive Care Unit

  • Acute airway obstruction after hemoptysis occurs due to the presence of blood clots. These conditions may result in lifethreatening ventilation impairment. We report a case of obstruction of the large airway by endobronchial blood clots which were removed using bronchoscopic cryotherapy at the bedside of intensive care unit. A 66-year-old female with endometrial cancer who had undergone chemotherapy, was admitted to the intensive care unit due to neutropenic fever. During mechanical ventilation, the minute ventilation dropped to inadequately low levels and chest radiography showed complete opacification of the left hemithorax. Flexible bronchoscopy revealed large blood clots obstructing the proximal left main bronchus. After unsuccessful attempts to remove the clots with bronchial lavage and forceps extraction, blood clots were removed using bronchoscopic cryotherapy. This report shows that cryotherapy via flexible bronchoscopy at the bedside in the intensive of intensive care unit is a simple and effective alternative for the removal of endobronchial blood clots.
