The Effect of the Dietary Approaches to Stop Hypertension (DASH) Diet Education Program on Bone Mineral Density in the Middle-aged Women: A Pilot Study (DASH 식이 교육 프로그램이 중년 여성의 골밀도에 미치는 효과: 예비 조사 결과)

  • Purpose: The purpose of this study was to develop the dietary approaches to stop hypertension (DASH) diet education program tailored to Korean adults and to examine the effect of the DASH diet education program on bone mineral density in middle aged women. Methods: The subjects of this study were 26 middle aged women, pre and post menopause, who attend a church located in Kyung-In district. The DASH group I (n=13) was provided with the DASH diet education program only, while the DASH group II (n=13) was provided with DASH diet education program along with calcium supplements (1 g/day). The DASH diet education program included one lecture on DASH diet, one face to face counseling, and five telephone counseling sessions during the 8 weeks period. Results: The knowledge and subjective compliance level of DASH diet increased significantly in both groups, at 4 and 8 weeks after the intervention. The level of objective compliance of DASH diet in both groups increased over the study period, but it was not significant as compared to those of the baseline. Bone mineral density of both groups decreased at 8weeks as compared to the baseline after the intervention. Conclusion: Although 8weeks of DASH diet education program increased the knowledge and compliance of DASH diet, it was not effective to bring the changes in bone mineral density among the middle aged women.

The Effects of Diet Health Education Program for Prehypertension Group on the Diet Habit and Blood Pressure (고혈압 전단계 집단 식이보건교육이 식이습관과 혈압에 미치는 효과)

  • Objectives: The aim of this study was to investigate the effects of DASH diet and law salt diet health education program on the blood pressure and the diet habit in the prehypertension group in Korea. Methods: The participants in this study were 141 people with prehypertension who were seen at a public health center. Data collection was done from March 11 to November 11, 2005 using interview with questionnaires. The participants took part in the diet health education program for 8 weeks. In order to evaluate the effectiveness of health education program, the variables of the diet such as hypertension, law salt diet and DASH diet were measured before and after the health education. The blood pressure was measured three times during the education. The collected data were analyzed with paired t-test and repeated ANOVA using SPSS 12.0 for Windows. Results: The diet habit variables after the education showed that their diet habit tend to the low salt diet and the DASH diet. The participants improved in their knowledge of hypertension and law salt diet, and they improved in the behavioral level of law salt diet and DASH diet after the health education for 8 weeks. Systolic and diastolic pressure were significantly decreased in prehypertension group after 8 weeks [10mmHg/6mmHg(p<0.001)]. Conclusion: These result suggest that the improvement in the level of behavior might be related with the change in blood pressure among the people having prehypertension in Korea.

  • Purpose: The purpose of this study was to investigate the effects of the Korean Dietary Approaches to Stop Hypertension (DASH) education program on oxidative stress, antioxidant capacity, and serum homocysteine (Hcy) level in elderly Korean women. Methods: This was a quasi-experimental study employing non-equivalent control group pretest-posttest design. The DASH group (n= 27) underwent the DASH education program for 12 weeks, whereas the control group (n= 21) was provided one education session. The survey was conducted before and after the intervention period. Results: In DASH group, monounsaturated fatty acid (MUFA), eicosapentaenoic acid (EPA), and docosahexaenoic acid (DHA) intakes significantly increased after the intervention as compared to prior to the intervention. No significant differences were found in the control group. After the intervention, thiobarbituric acid reactive substance (TBARS) and Hcy levels significantly decreased in the DASH group but not in the control group. Conclusion: The DASH education program was effective in reducing oxidative stress and Hcy levels in elderly Korean women.

  • Purpose: This study was aimed to investigate the effects of the nutritional intervention program including DASH diet education and calcium/vitamin D supplements in Korean elderly women. Methods: This was a quasi-experimental study employing non-equivalent control group pretest-posttest design. The experimental group (n=26) was given DASH diet education and supplements (calcium 1200mg and vitamin 800 IU) while the control group (n=16) was given only general health consults. After the interventions, differences were analyzed in nutrient intake, bone turnover markers and bone mineral density between the two groups. Results: After one year, bone mineral density was found reduced in both groups, but showed higher levels (p=.003) in the experimental group than the control group. After research, nutrient intakes of participants improved generally, yet there was no significant difference between the two groups. The experimental group was divided into subgroups after interventions according to the level of calcium intake, and bone density and bone markers were compared between the subgroups. In a subgroup whose calcium intake was in the normal range, bone mineral density was significantly high (p=.002) while CTx and osteocalcin were significantly low (p=.003, p=.006, respectively). Conclusion: This study is significant in that it provided a nutritional intervention program for one year to elderly women who are susceptible to osteoporosis and severely low in dietary calcium intake and it proved to be effective.

  • The purpose of this study was to conduct a systematic review of the current published research related to improvement in cognitive function. A systematic search was performed in three bibliographic databases (PubMed, Cochrane Library, and EMBASE) using "dementia", "memory", "food", "diet", and "nutrition" as keywords. Meal management intervention, including Dietary Approaches to Stop Hypertension (DASH) diet, Mediterranean (Med) diet, Diet Mediterranean-DASH Intervention for Neurodegenerative Delay (MIND) diet, and other studies, was also included in the analysis. Through extensive screening, 21 articles, out of 2101 papers retrieved, were used for the final systematic review. The methodological quality of the randomized controlled clinical trials (RCTs) was assessed using the Cochrane Risk of Bias tool. These articles recommended vegetables, fruits, whole grains, olive oil, fish, berries, nuts, and beans. In conclusion, this study suggests the potential use of meal management to improve cognitive function.

Effects of Dietary Approaches to Stop Hypertension Program on Blood Pressure among the Pre-hypertension Group in the Community (지역사회 주민의 고혈압 예방 식이프로그램의 효과 평가)

  • Objectives: DASH (dietary approaches to stop hypertension) diet are recommended as first-line therapy for prevention of hypertension individuals with high normal blood pressure. We studied the effects of DASH program on blood pressure among the pre-hypertension group. Methods: To determine the impact of dietary patterns on the control of hypertension we studied the subgroup of 141 participants with systolic blood pressure of 120 to 139 mm Hg or diastolic BP of 80 to 89mmHg, body mass index(BMI) ${\geq}25kg/m^3$ or waist to hip ratio(WHR) ${\geq}0.95$(for man). 0.85(for woman) enrolled in DASH program. Participants were received of education and consulting about DASH every week for 8 weeks. Results: The level of diet to prevention of hypertension and compliance of DASH increased after education. Also, The DASH program significantly reduced systolic BP (from $136.03{\pm}12.40mmHg$ to $126.09{\pm}11.25mmHg$, p< .01) and diastolic BP (from $81.80{\pm}6.32mmHg$ to $76.44{\pm}10.61mmHg$, p< .01). Conclusion: The DASH program effectively lowers BP and may be useful in achieving prevention of hypertension.

Elevation of heart-femoral pulse wave velocity by short-term low sodium diet followed by high sodium diet in hypertensive patients with sodium sensitivity

  • BACKGROUND/OBJECTIVES: We compared changes in heart-femoral pulse wave velocity (hfPWV) in response to low sodium and high sodium diet between individuals with sodium sensitivity (SS) and resistance (SR) to evaluate the influence of sodium intake on arterial stiffness. SUBJECTS/METHODS: Thirty-one hypertensive and 70 normotensive individuals were given 7 days of low sodium dietary approach to stop hypertension (DASH) diet (LSD, 100 mmol NaCl/day) followed by 7 days of high sodium DASH diet (HSD, 300 mmol NaCl/day) during 2 weeks of hospitalization. The hfPWV was measured and compared after the LSD and HSD. RESULTS: The hfPWV was significantly elevated from LSD to HSD in individuals with SS (P = 0.001) independently of changes in mean arterial pressure (P = 0.037). Conversely, there was no significant elevation of hfPWV from LSD to HSD in individuals with SR. The percent change in hfPWV from the LSD to the HSD in individuals with SS was higher than that in individuals with SR. Subgroup analysis revealed that individuals with both SS and hypertension showed significant elevation of hfPWV from LSD to HSD upon adjusted analysis using changes of the means arterial pressure (P = 0.040). However, there was no significant elevation of hfPWV in individuals with SS and normotension. CONCLUSION: High sodium intake elevated hfPWV in hypertensive individuals with SS, suggesting that high sodium intake increases aortic stiffness, and may contribute to enhanced cardiovascular risk in hypertensive individuals with SS.

Effects of Dietary Education on Low-sodium Diet Adaptation (식생활교육이 저나트륨식 적응에 미치는 영향)

  • Korean style DASH (Dietary Approaches to Stop Hypertension) and a dietary education program for sodium reduction were developed. Reduced sodium diets (15 and 30% reductions) were developed from general diets for 3 consecutive weeks from Monday through Saturday. Subjects (19 total) were classified into two groups according to dietary education. Experimental period was from June 24 to July 23, 2012. Total sum of adaptation scores for low sodium diets significantly increased in the group that underwent dietary education compared to that without (p<0.05). After the experiment, both groups showed significantly increased values in terms of food group balance, sodium-related nutrition knowledge, attitude, and practice by paired t-test. Especially, group that underwent dietary education showed significantly higher values for attitudes by ANCOVA pre-test as a variation (p<0.01). For the results of the nutrient intake survey, group that underwent dietary education showed significantly increased values for dietary fiber (p<0.01), vitamin A (p<0.001), vitamin K (p<0.001), vitamin C (p<0.01), Folic acid (p<0.001), vitamin B12 (p<0.01), calcium (p<0.01), iron (p<0.05), and zinc (p<0.05) and significantly decreased values for sodium (p<0.05) and chloride (p<0.005). Subjects adapted to reduced sodium diets showed apparent improvements in sodium-related knowledge, attitude, practice and intake of nutrient, and these improvements were even higher in the group that underwent dietary education compared to that without. Thus, adaptation to low sodium diet combined with dietary education can improve dietary habits.

The effects of the DASH diet education program with omega-3 fatty acid supplementation on metabolic syndrome parameters in elderly women with abdominal obesity

  • BACKGROUND/OBJECTIVES: The purpose of this study was to investigate the overall effects of a tailored Dietary Approaches to Stop Hypertension (DASH) nutritional intervention program which included omega-3 fatty acids supplementation, on dietary self-efficacy, dietary knowledge, and dietary behaviors in Korean elderly women with abdominal obesity. Furthermore, we investigated the effects of the program on metabolic syndrome parameters including the antioxidant capacities in these subjects. SUBJECTS/METHODS: A randomized, controlled trial was conducted for 8 weeks. The experimental group (n = 21) received a weekly tailored nutritional program for 8 weeks and the control group (n = 18) received only one educational session. The clinical survey was conducted before and after the intervention period. RESULTS: After the intervention, dietary self-efficacy (P = 0.023), frequency of fruit intake (P = 0.019), and dietary fiber intake (P = 0.044) were higher in the experimental group than in the control group. The oxidative stress (P < 0.001) was lower in the experimental group than in the control group. Moreover, low density lipoprotein (LDL) cholesterol (P = 0.023) had significantly decreased in the experimental group but not in the control group after the intervention. CONCLUSIONS: The intervention program including omega-3 fatty acid supplementation had a positive effect on dietary self-efficacy, dietary behaviors, and oxidative stress among aged women with abdominal obesity.

The Effect of Dementia Prevention Nutrition Program Using MIND Diet on the Changes in Cognitive Function of the Elderly with High-Dementia Risks (MIND 식단교육을 이용한 치매예방 영양프로그램이 고위험 치매노인의 인지기능 변화에 미치는 영향)

  • This study examined the effect of the Mediterranean-DASH Intervention for Neurodegenerative Delay (MIND) diet of the Korean multi-domain dementia prevention program on the cognitive functions of the elderly with dementia risk factors. We developed the program including nutrition, exercise, cognitive training, vascular disease prevention, and motivation. One- hundred and fifty-three participants aged 60~79 years with at least 1 dementia risk factor were randomly assigned in a 1:1:1 ratio to the facility-based intervention (FMI), home-based intervention (HMI), and the control group. The nutrition education program consisted of 10 classes over 24 weeks: the FMI received 7 group sessions and three 1:1 sessions, the HMI received 4 group sessions and three 1:1 sessions with 3 homework sessions. The Nutrition Quotient for Elderly (NQ-E) and the Mini Nutritional Assessment (MNA) were used to evaluate nutritional status. The Repeatable Battery for the Assessment Neuropsychological Status (RBANS), Korean Mini-Mental State Examination (K-MMSE), and the Cognitive Complaint Interview (CCI) were used to evaluate cognitive functions. A total of 136 people completed the program with an 11.1% dropout rate. The NQ-E (P=0.009) and RBANS (P=0.001) scores significantly increased in the FMI (N=45) and HMI (N=49) groups compared to the control group (N=42) after the study. The changes in the score of MNA and CCI did not differ significantly between groups. In conclusion, the nutritional intervention which focused on the MIND diet as a part of a multi-domain intervention program had a positive effect on the improvement of healthy eating habits and cognitive function scores in the high-risk dementia group.
