Dental caries is biofilm induced disease throughout life and is recognized significant oral health problem. This article reviewed new trends in dental caries management by risk assessment, including history, protocol/guideline, and collaborated model. Dental caries prevention and treatment according to caries management by risk assessment (CAMBRA) model is patient-centered, risk-based, evidence-based practice. Team approach is necessary and clinician need to integrate science, practice and product. Dental hygienist take a important role in implementing CAMBRA. CAMBRA model could be incorporated into clinical dental hygiene education based on dental hygiene process of care as standard of dental hygiene practice and education. Dentist and dental hygienist able to provide scientific and ethical care managing dental caries by risk assessment.
Dental caries has been widely prevalent with presence of cavitation on teeth. For the last several decades, the prevalence of dental caries in developed countries has rapidly decreased so there has been needed a new and detailed diagnostic guideline to differentiate the severity of dental caries, especially for early status of caries. The cariology specifically requires the development of an integrated definition of dental caries and uniform systems for measuring the caries process in the fields of clinical diagnosis and treatment, epidemiological researches, and dental education and so forth. The international Caries Detection and Assessment System (ICDAS) optically measures the enamel surface changes and potential histological depth of carious lesions by relying on surface characteristics of teeth. ICDAS is a visual classification system that was developed to diagnose the subtle changes of enamel surface, predict the progress direction of early caries, allow standardized data collection in relation to caries in different settings, and to enable better comparison of oral health between countries worldwide and research studies.
Objectives : The purpose of the study is to investigate the risk factors of dental caries in early childhood. Methods : Subjects were 197 mothers having 5 year old children. They completed the questionnaire from June 1 to July 14, 2012. Results : At birth, the age of mother and the number of teeth experiencing dental caries showed high correlation(r=0.163, p<0.05). Early beginning(r=-0.441, p<0.05) and long period(r=0.441, p<0.05) of mix feeding were closely related to dental caries outbreak. Long period of supplementary food caused deciduous dental caries (r=0.643, p<0.001) and dental caries experience (r=0.723, p<0.001). Long period of breast feeding (r=0.261, p<0.001) also caused deciduous dental caries. Low BMI of mother was colsely related to high dental caries(r=-0.206, p<0.01). Deciduous dental caries accounted for 61.8% and the number of teeth experiencing dental caries was 68.5%. Conclusions : Appropriate breast feeding habit is the most important behavior of preventing deciduous dental caries in 5 year old children.
Objectives: The study aimed to isolate the abundant bacteria in dental caries in children and to investigate the bacterial species involved in addition to those that have been previously reported. Methods: The specimens were collected from the supragingival plaques of each dental caries area, pit and fissure caries, deep dentinal caries, smooth surface caries, and dental caries, and from healthy subjects in the control group. Bacteria were cultured from these specimens, DNA was extracted from the isolated bacteria, and the 16S rRNA gene sequences were analyzed and identified. Results: Based on the results of the 16S rRNA gene sequence analysis for the 90 strains of dominant bacteria from the 45 specimens, 5, 7, 8, 7, and 13 species were identified from the supragingival plaques from healthy teeth, pit and fissure caries, deep dentinal caries, smooth surface caries, and dental caries, respectively. In healthy teeth, Actinomyces naeslundii dominated. Corynebacterium durum, Ralstonia pickettii, and Streptococcus intermedius showed equal distribution. The dominant bacterial species in dental caries, S. sanguinis, showed the greatest difference in prevalence in pit and fissure caries. In deep dentinal caries, S. mutans and Lactobacillus rhamnosus were dominant; in smooth surface caries, S. mutans and S. sanguinis were dominant; and in the supragingival plaques of dental caries, S. sanguinis and S. mutans were dominant. Conclusions: The bacterial species isolated from dental caries encompassed four phyla, eight genera, and 22 species. In addition, the SS1-2 strain, belonging to the genus Neisseria, was identified as a new species from among the isolated strains.
Objectives : In most developed countries there has been a substantial decrease in the prevalence of dental caries in children of 12 years old over the past 40 years. This change in epidemiology highlights the importance of preventive management in dentistry. For successful management of early caries lesions, it is essential to detect and assess the pathological changes of dental caries. Therefore, the purpose of this study was to investigate the overview of International Caries Detection and Assessment System(ICDAS) criteria for the detection and assessment of early caries lesions by literature reviews. Methods : The author reviewed the MEDLINE search by the key words of 'caries assessment', 'caries detection', 'caries measurement', and 'ICDAS'. Results : This literature review provided an overview of International Caries Detection and Assessment System(ICDAS) criteria for the detection and assessment of early caries lesions. This article showed the principles, concept, and primary caries detection criteria of the ICDAS. The ICDAS can macroscopically measure the enamel surface changes and potential histological depth of caries lesions by the surface characteristics of the teeth. The ICDAS criteria allow researchers and clinicians to choose the stage of dental caries and characteristics for assessment. Conclusions : ICDAS can be an adjunct method to clinical decision making and preventive treatment planning.
The new paradigm of dentistry require the detection of caries in their earlier stages. To achieve this, a high technology detection device and systematic and organized caries management system are needed. Caries management by risk assessment (CAMBRA) model is representative caries management system that satisfied new paradigm. Dental caries prevention and treatment according to CAMBRA model is patient-centered, risk-based, evidence-based practice. Therefore, individual caries management such as CAMBRA should be performed through accurate assessment of caries disease indicators and comprehensive assessment of caries risk factors and protective factors. Based on the CAMBRA better effectiveness of comprehensive dental caries management including non-surgical treatment will be accomplished.
As a part of host factors of dental caries, saliva has been well known for its important role in relation to dental caries. The studies on its physical and chemical characteristics on development and progress of dental caries has been conducted. Recently, various comparisons between saliva of caries-susceptable individuals and caries-free individuals has been done and the efforts to understand the mechanisms of salivary intervention of development and progress of dental caries is actively in progress. In this study, 15 children with rampant dental caries and 15 caries free children without any systemic diseases from the ages of 2 to 5 were chosen for the experiment and the whole saliva and parotid saliva from each individuals were collected and protein compositions were compared using polyacrylamide gel electrophoresis (PAGE). As results of this study, in parotid saliva, there was no difference in protein compositions between the rampant dental caries and the caries free children. While electrophoresis was done with the whole saliva, protein with 120 KDa was found in children with rampant dental caries. However, this protein was not found or unclear, if any for the caries free group. (Exceptionally, clear protein band was present for one person.) Protein compositions of whole saliva of rampant dental caries group was compared before and after the caries control and thick and clear protein bands of about 120 KDa were found in both cases. Protein compostions of caries free children and adults were identical. Quantitative analysis of protein was done for the rampant dental caries group and the control group and no significant difference was found. Taken all together, protein with molecular weight of 120 KDa, found in rampant dental caries group, was still present when the treatment for the dental caries was done so it can be assumed that this protein has no interrelation with the presence of active carious lesions during saliva collecting. It can also be presumed that this specific salivary protein with the molecular weight of 120 KDa found in rampant dental caries group has effect on development and progress of dental caries. Identification on this protein with the molecular-weight of 120 KDa and the role of this protein against dental caries remain to be solved.
This study was conducted to provide basic information on toothbrushing habits, snack intake frequency, and current dental caries in elementary school students. A total of 188 students (105 boys and 83 girls) in Changwon city participated in this study. Dental examination was conducted by two dental hygienists in July, 2010. The results were as follows. Exactly 86.2% of students were shown to have dental caries, which includes a sealant and removed teeth due to decay. The average number of teeth with dental caries per student was 3.8. Exactly 60.6% of students were shown to maintain correct toothbrushing habits to prevent dental caries, but only 12.8% brushed their teeth immediately after a meal or snack. Exactly 82.4% percent of participants received toothbrushing education, 47.1% of them received it from an expert, 43.9% received from their parents, and 28.2% used tooth paste with fluorine. Regarding snack intake frequency, ice cream was the most consumed cariogenic food and white milk was the most consumed protective food. The >=7 teeth with dental caries group consumed higher frequency of carbonated beverages intake than the 'no dental caries' group (P<0.05), and the '5~6 teeth with dental caries' group consumed higher frequency of chocolate intake than 'no dental caries' group (P<0.05).
Xylitol is a sweetening agent used to prevent dental caries. It is formulated and consumed in various forms, and its use is recommended in particular for oral health. However, dental specialists are not familiar with xylitol or well-acquainted with the appropriate method to use it. Furthermore, the caries-prevention effect of xylitol has been controversial. Thus, in this study, we aimed to analyze previously published studies on the dental caries prevention effect of xylitol to verify its true effects and establish an appropriate method to use it. We analyzed 94 clinical research manuscripts obtained from domestic and international online databases and found that although 11 manuscripts reported no significant dental caries prevention effect of xylitol, most manuscripts reported dental caries prevention with xylitol. Compared to the manuscripts that reported dental caries prevention with xylitol, those are assessed lower amounts of xylitol or shorted consumption periods, and had dosage form that could not retain xylitol sufficiently long in the mouth, similar to milk. Thus, consumption of 5-11 g of xylitol in the form of gums, candies, or tablets over a long time period could effectively reduce the risk of dental caries. It has the advantage of ease of consumption, regardless of the gender, age, or disability of the person.
BACKGROUND/OBJECTIVES: Few studies have provided evidence of the association between diet quality and dental caries. This study aimed to examine the association between diet quality and untreated dental caries in a Korean representative population. SUBJECTS/METHODS: The study population included a sample of 13,815 participants, aged ≥ 19 from the Korea National Health and Nutrition Examination Survey during 2013-2015. The explanatory variable was diet quality and the outcome variable was untreated dental caries. Untreated dental caries were defined by the number of decayed teeth recorded according to the criteria established by the World Health Organization. Diet quality was defined by using the Korean Healthy Eating Index (KHEI) through the 24-h recall methods. We assessed the association between diet quality and untreated dental caries while adjusting for age, sex, education, income, smoking status, dental visits, toothbrushing frequencies, obesity, and diabetes mellitus. RESULTS: The mean overall KHEI scores in the untreated dental caries group were significantly lower than those in the group without untreated dental caries. Significant differences were observed in the untreated dental caries group based on the KHEI quartiles (P < 0.001). After adjusting for potential confounders, the quartiles of KHEI scores showed an association with untreated dental caries, demonstrating a dose-effect trend (odds ratio [OR], 1.57; 95% confidence interval [CI], 1.35-1.84 for 1st quartile; OR, 1.38; 95% CI, 1.19-1.59 for 2nd quartile; OR, 1.32; 95% CI, 1.14-1.53 for 3rd quartile; reference quartile highest]). CONCLUSIONS: The findings indicated an inverse association between diet quality and untreated dental caries in Korean adults. Healthcare providers should take into account the significant role of diet quality in preventing and managing oral health.
