Effect of xylitol on dental caries prevention : a literature review

  • Xylitol is a sweetening agent used to prevent dental caries. It is formulated and consumed in various forms, and its use is recommended in particular for oral health. However, dental specialists are not familiar with xylitol or well-acquainted with the appropriate method to use it. Furthermore, the caries-prevention effect of xylitol has been controversial. Thus, in this study, we aimed to analyze previously published studies on the dental caries prevention effect of xylitol to verify its true effects and establish an appropriate method to use it. We analyzed 94 clinical research manuscripts obtained from domestic and international online databases and found that although 11 manuscripts reported no significant dental caries prevention effect of xylitol, most manuscripts reported dental caries prevention with xylitol. Compared to the manuscripts that reported dental caries prevention with xylitol, those are assessed lower amounts of xylitol or shorted consumption periods, and had dosage form that could not retain xylitol sufficiently long in the mouth, similar to milk. Thus, consumption of 5-11 g of xylitol in the form of gums, candies, or tablets over a long time period could effectively reduce the risk of dental caries. It has the advantage of ease of consumption, regardless of the gender, age, or disability of the person.

  • Dental caries is a biofilm-related oral disease, and continues to afflict the majority of the world's population. Although fluoride, delivered in various modalities, remains the mainstay for the prevention of caries, additional approaches are required to enhance its effectiveness. Natural products have been used as a major source of innovative and effective therapeutic agents throughout human history, and have shown promise as a source of components for the development of new drugs. In addition. studies using natural products to prevent or treat oral diseases such as dental caries have received a great deal of attention. A number of compounds, such as epicatechin, allicin and sanguinarine, isolated from natural products, have also been investigated for their efficacy against oral microbial pathogens. However, the use of natural products as an anti-caries agent in clinical practice was controversial because of inadequate knowledge concerning their mechanisms of action and chemical characterization. This study focuses on the current knowledge of natural products in dental caries prevention and suggests natural products are importance sources for the prevention of dental caries.

Application of Fluoride for Dental Caries Prevention in Older Adults with Dry Mouth: a Clinical Review

  • Dry mouth is common among older adults and significantly affects the oral health-related quality of life. It is a significant risk factor for dental caries, particularly root caries, in older adults due to concurrent periodontal disease and age-related comorbidities. Clinicians managing patients with dry mouth must be aware of preventive measures against dental caries. This clinical review aims to update our knowledge on the use of fluoride for caries prevention in order to establish better strategies for the management of dry mouth in older adults.

  • Dental caries is biofilm induced disease throughout life and is recognized significant oral health problem. This article reviewed new trends in dental caries management by risk assessment, including history, protocol/guideline, and collaborated model. Dental caries prevention and treatment according to caries management by risk assessment (CAMBRA) model is patient-centered, risk-based, evidence-based practice. Team approach is necessary and clinician need to integrate science, practice and product. Dental hygienist take a important role in implementing CAMBRA. CAMBRA model could be incorporated into clinical dental hygiene education based on dental hygiene process of care as standard of dental hygiene practice and education. Dentist and dental hygienist able to provide scientific and ethical care managing dental caries by risk assessment.

A study on the patient's awareness of oral health in public health center (보건소 내원 환자들의 구강보건 인식도에 관한 조사)

  • There are two purposes in this study. The first one is to collect some oral health information through the investigations upon the oral health care of Korean adults. Referring to the result of the investigation. I wanted to furnish fundamental data with oral health education for each age and with a publicity booklet editing. That is the second purpose of this research. Among the people who visited Seo-gu Public Health Center in Kwang-ju, 207 people answered the questionnaires. There were 86 men and 121 women from the 20-aged to the 59- aged. The questionnaires covers knowledge aquisition path for dental caries prevention, the cognition degrees for the causes of dental caries and its prevention, the cognition degrees for the causes of periodontal disease and its prevention, the degrees for oral health methods, and the importance of oral health. The conclusions are as follows; 1. The cognition degree for dental caries prevention: The cognition for pit and fissure sealant was appeared most highly in thirties by 85.3%. but 62.1 % in fifties was answered, "Never heard". The cognition degree for fluoride application was appeared most highly in thirties by 73.5%. and the cognition degree for water fluoridation was most highly in forties by 54.2%. 2. The knowledge for pit and fissure sealant was acquired mostly through dental hospital in every age by 54.2%. 3. The knowledge for fluoride application was learned mostly through dental hospitals in twenties and thirties by 32.7%. and mostly through TV or radio health programs in forties and fiftieseach by 35.7% and 50.0%. 4. The knowledge for water fluoridation was acquired through TV or radio programs in all ages such as twenties. thirties. forties and fifties. Its rate was 57.8%. 5. The cognition degree of the cause of dental caries: 53.1 % of all ages think that dental caries can most frequently be caused by being lack of toothbrushing. 6. The cognition degree of the cause of periodontal disease: 58.5% people of all ages think that both dental plaque and calculus might be the main cause of periodontal disease. 7. The cognition degree of dental caries prevention: 72.8% people think that dental caries can be prevented by right tooth brushing method. and 8.7% people think that they can be prevented by scaling. However, 10.7% people of them were not interested in dental caries prevention. 8. The importance of oral health: 35.3% people think that teeth health is the most important, and 63.8% people think oral health is one of the most important health problems. Forties answered that tooth health was the most important thing, and fifties. thirties and twenties followed in the order. 9. The cognition for oral health maintenance: all ages(twenties. thirties. forties. and fifties) answered that right toothbrushing method was the best way to keep oral health by 69.1%.

  • The new paradigm of dentistry require the detection of caries in their earlier stages. To achieve this, a high technology detection device and systematic and organized caries management system are needed. Caries management by risk assessment (CAMBRA) model is representative caries management system that satisfied new paradigm. Dental caries prevention and treatment according to CAMBRA model is patient-centered, risk-based, evidence-based practice. Therefore, individual caries management such as CAMBRA should be performed through accurate assessment of caries disease indicators and comprehensive assessment of caries risk factors and protective factors. Based on the CAMBRA better effectiveness of comprehensive dental caries management including non-surgical treatment will be accomplished.

The effect of Silver Diamine Fluoride in preventing dental caries (Silver Diamine Fluoride의 치아우식 예방 효과)

  • Silver diamine fluoride (SDF) is an alkaline topical solution and it derives from the conjunction of silver nitrate and fluoride. It reduces the growth of cariogenic bacteria, inhibits degradation of dentinal collagen, impedes demineralization and enhances remineralization. It is inexpensive due to the low cost of materials and its application to dental surface is very simple and requires relatively short chair time. Previous studies have shown that the dental caries prevention effect of SDF is superior or similar to topical fluoride application. The main disadvantage of SDF is its esthetic result, and it permanently blacken carious enamel and dentin. The use of SDF has not yet been approved in Korea, but it may be helpful to prevent and treat dental caries in patients with special health care needs and uncooperative young patients.

Association between sealants and caries assessed using the Seventh Korea National Health and Nutrition Examination Survey (KNHANES) (치아홈메우기와 치아우식과의 연관성 분석: 제7기 국민건강영양자료조사 이용)

  • Objectives: This study aimed to evaluate the relationship between pit and fissure sealants of the molars, and caries prevalence and experience to assess the effect of sealants on caries prevention. Methods: Data from 16,119 people were extracted from the seventh Korean national health and nutrition examination survey, which was conducted from 2016 to 2018. Demographic, socioeconomic, oral health-related behavioral, and oral examination survey data were collected as independent variables. SAS statistics for complex samples and multiple logistic regression analysis (Windows ver. 9.4) was used for the statistical analyses. Results: Caries prevalence and experience were significantly associated with age, sex, daily toothbrushing frequency, interdental cleaning, and having molar sealant. Multiple logistic regression analysis revealed a significant association between having molar sealants and the caries prevalence and experience among participants under 30 years of age after adjusting socio-economic variables and toothbrushing-related factors. Conclusions: This study confirmed the effect of molar sealants on caries prevention in Korea. Our results support that the application of molar sealants in children, adolescents, and young individuals is effective in preventing caries.

  • Dental caries is the first disease to cause the pathological extraction of teeth in children. The complete prevention of dental caries is not achieved by toothbrushing alone. The use of fluoride and pit and fissure sealant is regarded as key tools to prevent dental caries. Fluoride-containing tablets or multi-vitamins and community water fluoridation can be used as systemic application techniques. Professional fluoride application, fluoride iontophoresis on teeth, fluoride mouth rinsing and fluoride-containing toothpaste can be used as local application techniques. Pit and fissure sealant is mainly used to prevent dental caries on occlusal surfaces of premolars and molars. Sweeteners not to occur dental caries has been developed to substitute sucrose. Dental erosion increases according to the high consumption of acidic soft drink or beverages. The appropriate use of fluoride and pit and fissure sealant are recommended to prevent dental caries effectively and the education is required to reduce the consumption of acidic soft drinks or beverages to decrease dental erosion.

Study of necessity of teenagers' drinking, mouth knowledge estimation by smoking and dental health education (청소년의 음주·흡연에 따른 구강지식평가 및 구강보건교육의 필요성에 관한 연구)

  • This study is to enforce to ready basic data of effective dental education program and no smoking and no drinking program for high school students who do smoking and drinking, executed investigation by own recording way using smoking and questionnaire consisted of actual conditions and caries prevention awareness and dental health knowledge degree of drinking to some man high school. Analyzes result that total 487 people respond in SPSS WIN 13.0 programs and got following sequence. Result of this study is as following. 1. Mans high school students had more students who do not than a drinking student and main current received most soju. Third-year student appeared by thing which receive a lot of soju than 1 or 2 class student but different other difference was not seen by class. 2. Mans high school students had more students who do not than a smoking student and a student who smoke more than 3 years was the most. I can know was old since third-year student smokes than 1 or 2 class student. 3. It was not difference that keep in mind by drinking, smoking availability although there were many students who recognize that all of the on-time dental medical examination, sugar intake limitation, fluorine inclusion toothpaste, teeth fluorine application are seldom effective on caries prevention. 4. Recognized that right brushing is effective relatively on caries prevention. I was shown difference that recognized that righter brushing is effective on bad tooth courtesy call than a student who a student who do not smoke does, and keeps in mind according to smoking availability ($x^2$=6.78, p<0.05). 5. I knew best about question of 'A caries may not treat if is painless' among that question ask knowledge for a caries, and appeared by thing which do not know best about question of 'Know that method that supply fluorine in water'. 6. A student who do not drink general dental know-how for a caries was high knowledge degree for a caries than a drunk student but it was no difference that keep in mind by drinking availability. Also, a student who do not smoke by smoking availability was high knowledge degree for a caries than a done student but difference that keep in mind statistically was not seen. 7. It was no difference that keep in mind by drinking availability, but a student who do not smoke by smoking availability was high relation knowledge degree with food and a caries than a smoked student, and I was shown difference that keep in mind according to smoking availability(t=-2.03, p<0.05). 8. Awareness and synthetic estimation degree of knowledge degree a student who do not smoke than a smoked student mouth knowledge degree high, and I was shown difference that keep in mind according to smoking availability(t=-2.03, p<0.05). When see as result such as singularity, family, school, all social institutions may have to help by national business coming and try so that can become national business based on special know-how for development of effective and epoch-making education program as quickly as possible.
