Factors Affecting Dental Utilization and Dental Expenses in the Economically Active Population: Based on the 2010~2014 Korea Health Panel Data

  • Background: The health of the economically active population contributes to increased corporate productivity by reducing the productivity loss caused by disease and increasing job efficiency, which in turn is a national benefit. Since the economically active population is a concept encompassing workers and a source of economic development for a country, that population's health should be treated with importance not only from a personal standpoint but also at a national level. Methods: In this study, data of 11,007 adults aged 20 years and older who participate in economic activities were analyzed in the five-year Korea Health Panel Study from 2010 to 2014 including the number of dental visits and dental medical expenses. Results: Factors related to "gender," "education level," "age," "duty category," "income level," "employment type," "national health insurance," and "chronic disease status" of the economically active population are affected in relation to the number of visits and dental medical expenses. The number of dental visits increased with higher education levels (p<0.001), and the number of visits to the dentist increased with older age (p <0.001). Dental medical expenses were 91,806 Korean won (KRW) more for "white-collar workers" than for "blue-collar workers" (p<0.03), and 127,674 KRW more for "regular workers" than for "atypical workers" (p<0.02). Conclusion: When it is necessary to improve policies to enhance the efficiency of the distribution of health and medical resources in the overall balance of the dental health sector, we should try to identify various factors of oral health disorder due to income inequality among the classes according to the country's employment type in order to find ways to reduce the health gap among the social classes.

코로나 바이러스 감염증-19가 한국 소아청소년의 치과 이용에 미치는 영향 (Impact of Coronavirus Disease 2019 on Dental Service Utilization of Korean Children and Adolescents)

  • 이 연구의 목적은 코로나 바이러스 감염증-2019 팬데믹 이후 소아청소년의 치과 이용 실태를 조사하고 성인과 비교하여 파악하는 것이다. 건강보험심사평가원에서 제공하는 2017년 1월부터 2020년 12월까지 20세 미만의 소아청소년 및 20세 이상의 성인의 주상병과 치료코드별 치과 내원 횟수 자료를 사용했다. 연도별 인구수 변화가 내원 횟수에 미치는 영향을 줄이기 위해 인구수 1000명당 치과 내원 횟수를 이용해 분석하였다. 치수 및 근단주위조직의 질환 주상병으로 인한 치과 내원 횟수가 팬데믹 이후 모든 연령에서 감소했고, 치수치료에 해당하는 치료 코드로 인한 치과 내원 횟수는 0 - 4세군을 제외하고 감소했다. 수복치료에 해당하는 치료코드로 인한 치과 내원 횟수는 팬데믹 이후 0 - 19세군에서 감소했다. 2020년 신규확진자 수와 치료 코드의 치과 내원 횟수 변화율을 월별로 비교했을 때, 우리나라 대유행이 있던 2월과 8월 이후 치수치료 및 수복치료는 점차 감소했다. 치은염 및 치주질환 주상병으로 인한 치과 내원 횟수는 팬데믹 이후 모든 연령군에서 증가했다. 소아청소년의 수복치료로 인한 내원 횟수 감소율은 성인보다 컸다. 팬데믹 이후 Multi-visit 치수치료로 인한 치과 내원 횟수 감소율은 1-visit 치수치료보다 컸다. 이 연구를 통해 코로나 바이러스 감염증-2019 전파 이후 소아청소년들의 치과 내원 횟수를 파악하고 성인과 비교해볼 수 있다.

  • Objectives: The purpose of this study was to determine the importance of brushing before sleeping by comparing the use of oral care products, frequency of dental clinic visits, and oral conditions between individuals who brush and do not brush their teeth before sleeping using data from the 5th and 6th Korean National Health and Nutrition Survey (KNHANES). Methods: Statistical analysis was performed using SPSS 21.0 on data files obtained according to a complex sampling design, and a significance level of <0.05 was set. General characteristics of the participants and year were analyzed using chi-square analysis. Data on the oral conditions, use of oral care products, and frequency of dental clinic visits were analyzed using logistic regression and linear regression. Results: Individuals who brushed their teeth before sleeping showed greater use of oral care products and more frequent dental clinic visits, oral examinations, preventive treatment, and treatment for simple caries (p<0.05) than individuals who did not. Individuals who did not brush their teeth before sleeping showed higher prevalence of permanent teeth caries and periodontal disease, as well as chewing and speaking problems (p<0.05), than those who bushed before sleeping. Conclusions: Individuals who do not brush before sleeping exhibit poorer oral health and lower use of oral care products and frequency of recent dental treatment than those who brush before sleeping. Therefore, it is necessary to alter the government's active policy and improve education about the importance of brushing before sleeping to improve oral health.

치과환자의 치과위생사에 대한 의료 커뮤니케이션과 신뢰, 만족과의 관계 (A study on the relationship between patient's medical communication, reliance and satisfaction to dental hygienist)

  • Objectives : The purpose of this study was to help form treatment relationship with patient through more effective communication by defining the relationship between dental hygienist's medical communication and outpatient's reliance satisfaction. Methods : The study researched 273 male and female patients who visited dental clinics and hospitals of Busan from March 12 to March 26, 2012 and its results are as follows. Results : 1. The linguistic communication of dental hygienist was $3.72{\pm}0.63$ and non-linguistic communication was $3.48{\pm}0.58$. 2. For the dental hygienist's reliance satisfaction, the reliance was $3.62{\pm}0.65$ and the satisfaction was $3.74{\pm}0.65$. 3. The dental hygienist's communication degree depending on general characteristic was statistically significant when the job of patient was housewife(p<0.001) and the number of dental clinic visits was more than 10 times (p<0.000). The dental hygienist's non-linguistic communication was statistically significant depending on patient's gender(p<0.000), age(p<0.002), job(p<0.001) and number of dental clinic visits (p<0.000). 4. The dental hygienist's reliance and satisfaction showed statistically significant difference depending on patient's gender(p<0.000), age(p<0.002), job(p<0.001) and number of dental clinic visits (p<0.000). 5. The dental hygienist's non-linguistic communication showed a positive correlation with reliance and satisfaction(p=0.000). Conclusions : When considering the result above, it is necessary to develop the teaching method and material to educate the communication ability of dental manpower. It is necessary to reinforce the curriculum of dental hygienics and the education of dental hygienist to perform effective, smooth communication between dental hygienists.

  • Of 42 children, 21 in male and 21 in female, whose ages ranged from 2 to 5 years old, the response of young children to their initial series of dental visits was examined. The results were as follows; 1. With continued experience, the child's response improved, indicating desensitization to dental stress. 2. Experience may reduce the general amount of negative response by allowing the child to accurately distinguish between stressful and non-stressful procedures.

Full mouth rehabilitation of a patient with Sturge-Weber syndrome using a mixture of general and sedative anesthesia

  • Issues related to the control of seizures and bleeding, as well as behavioral management due to mental retardation, render dental treatment less accessible or impossible for patients with Sturge-Weber syndrome (SWS). A 41-year-old man with SWS visited a dental clinic for rehabilitation of missing dentition. A bilateral port-wine facial nevus and intraoral hemangiomatous swollen lesion of the left maxillary and mandibular gingivae, mucosa, and lips were noted. The patient exhibited extreme anxiety immediately after injection of a local anesthetic and required various dental treatments to be performed over multiple visits. Therefore, full-mouth rehabilitation over two visits with general anesthesia and two visits with target-controlled intravenous infusion of a sedative anesthesia were planned. Despite concerns regarding seizure control, bleeding control, and airway management, no specific complications occurred during the treatments, and the patient was satisfied with the results.

Retrospective Study on the Flow and Characteristics of Dental Emergency Patients in Chosun University Hospital

  • Purpose: The aim of the present study is to assess the importance of proper treatment timing for dental emergency patients by characterizing current patient care in the emergency room. Materials and Methods: A retrospective chart review of 3,211 patients who visited the Chosun University Hospital's dental emergency department (Gwangju, Korea) was conducted from January 1, 2011 to May 31, 2014. Information regarding age, gender, onset date, main causes, and diagnoses were collected and analyzed. The main causes were divided into six categories: assault, household/play, sports, traffic, work, and others. Result: Emergency visits were more common for men (69%), and the ratio of males to females was 2.2:1 On average, the major cause was household/play (49.8%), followed by others (18.9%), traffic (16.6%), assault (9.1%), sports (2.9%), and work (2.6%). The most frequent diagnosis on average was dental trauma with 82.4%, followed by infection (10.7%), others (4.7%), and bleeding (2.2%). Conclusion: The main reasons for visits to the dental emergency department are dental trauma, dental infection, bleeding, and others. The most frequent reason for dental emergency patients to visit the emergency department was dental trauma (82.4%).

동아시아 국내 이주여성의 치과방문여부와 구강건강교육유무가 구강건강상태에 미치는 영향 (Influence on Oral Health Condition by Visits to Dental Clinics and Dental Education Experience of East Asian Immigrant Women in Korea)

  • Objectives : The purpose of this study is to figure out and establish the basic data that can help improve management of oral healthcare for the international marriage immigrant women. Methods : In this sense, we surveyed questions on 237 women out of 1,300 immigrant women, who have participated in the program by multi-cultural household supporting center, in a fashion of face-to-face investigation and on-the-spot direct cavity inspection simultaneously from May 1 to October 31, 2010. Collected data were electro-statistically computerized under SPSS 17.0 program and analyzed with frequency analysis, recurrence analysis and logistic regression analysis respectively. Results : The number of filled teeth got lower when they had lower number of dental visits. Women from the Philippines had the highest number of defect teeth, and the number of fixed prosthetic appliance became less when they had less number of dental visits. When they had no experience of oral health education, they had less fixed prosthetic appliances. There was higher percentage of periodontal disease when they did not have dental visit compared with the dental visit cases. Conclusions : Preparation of a policy for east Asian immigrant women for them to visit dentist comfortably and expansion of systematic oral health education are highly recommended.

한국의료패널의 치과외래 이용 및 의료비 지출에 대한 4년간(2008-2011)의 변화추이 (Dental utilization and expenditures in Korea Health Panel Survey, 2008-2011)

  • Korea Health Panel Survey (KHPS) has been conducted to provide nationally representative estimates of health care use and expenditures for non-institutionalized population in Korea since 2008. The aim of this study was to present estimates from KHPS on dental utilization and expenditures from 2008 to 2011. While weighted estimates were presented for each year from 2008 to 2011, the focus of this study was on the 2011 estimates and changes in dental utilization and expenditures between 2008 and 2011. Weighted estimates were obtained using population weights and statistical significances were analysed by independent t-test or ANOVA test using SPSS version 20 for Windows. In 2011, 21.1% of Korean population obtained outpatient dental services from dentists. From 2008 to 2011, the percent of people with a dental visit in each year increased slightly. In 2011, the mean number of dental visits was 4.46. While the mean number of dental visits remained stable by 2010, it increased slightly in 2011. The total annual dental expense in 2011 for people with a visit was about 460,000 Korean Won and 10% higher in 2008. In summary, this study results highlight low rates of dental utilization and high dental expenses in Korean population.

빅데이터를 이용한 소아청소년에서의 코로나 바이러스 감염증-19와 치과 방문의 연관성 분석 (Analysis of the Association between COVID-19 and Dental Visits in Children and Adolescents through Big Data)

  • 이 연구는 코로나 바이러스 감염증-19로 인한 소아청소년의 치과 방문에 대한 변화를 알아보기 위해 수행됐다. 한국 건강보험심사평가원에서 제공하는 자료를 기준으로 2019년 1월부터 2020년 8월까지 소아청소년의 치과 방문 횟수 및 치과주상병 변화를 월별, 지역별 분석하였다. 2020년 1월부터 8월까지의 소아청소년의 치과의원 및 치과병원 총 방문 횟수는 2019년 같은 기간과 비교시 0 - 9세 연령군에서는 642,202회(16.3%), 10 - 19세 연령군은 313,488회(9.2%) 감소하여 0 - 9세 연령군이 10 - 19세 연령군보다 더 큰 감소를 보였다. 같은 기간 상병별로는 Z29(기타 예방적 조치의 필요)로 인한 감소가 0 - 9세 연령군에서는 118,219회(34%), 10 - 19세 연령군에서는 83,944회(31%) 감소하여 가장 큰 변화를 보였다. 코로나 바이러스 감염증-19로 인한 전반적인 소아청소년의 치과 이용이 감소된 것으로 분석되며 이는 추후 소아청소년들의 구강 보건 악화로 이어질 가능성이 있으며, 이 연구는 이후 코로나 바이러스 감염증-19와 같은 전염병 발생 시 참고 자료가 될 수 있을 것이다.
