The Effects of Discharge Planning for the Elderly with Pulmonary Disease in the Emergency Room (퇴원계획중재가 응급실 내원 호흡기 질환 노인의 퇴원준비도, 간호서비스 만족도, 재입원율에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of discharge planning on patient satisfaction, the readmission rate and preparedness for discharge in the elderly admitted to the emergency room (ER) for pulmonary disease. Methods: A quasi-experimental intervention study design was used. Older adults with pulmonary health problems in the ER in one general hospital were randomly allocated to either an experimental (n=21, 74.2 years) or control group (n=19, 70.7 years). The experimental group participated in a discharge planning program by a geriatric nurse practitioner. Data were collected from medical records, physical measurements and structured questionnaires including information on demographics, patient satisfaction, readmission, and preparedness for discharge. Results: Participants in the experimental group had significantly better outcomes with regard to patient satisfaction with nursing services (p=.003) and preparedness for discharge (p=.034). However, there was no significant effect on the readmission rate (p=.392) Conclusion: The results suggested that a discharge planning program could bolster nursing service satisfaction and preparedness for discharge in older patients admitted to the ER for pulmonary health problems. To clarify the effects of discharge planning on older patients admitted to the ER, a larger sample population, better instruments for various measures, a new manual on discharge planning and frequent follow-up will be necessary.

  • Objective : The purpose of this study was to investigate the necessity of occupational therapist's involvement in patient discharge planning, the areas that should be considered for discharge screening and planning, and to provide the basic data required for the development of a discharge assessment tool. Methods : We conducted an online questionnaire survey of 60 occupational therapists who were working at medical institutions and had agreed to participate in the study. The questionnaire was composed of 36 questions regarding the general characteristics of the current discharge planning process and the necessity of discharge assessment and planning. Descriptive statistics, an independent t-test, and a one-way ANOVA were conducted using SPSS 20.0. As for the post-hoc test, Scheffe's test was used. Results : The awareness of occupational therapist's role in discharge planning and the necessity of a discharge assessment tool were high, but the occupational therapist's awareness of discharge-related knowledge was low. The difficulties in discharge planning showed high response rate in the absence of adequate fee-for-service in the patient interview and assessment and the lack of team approach and appropriate assessment tools for discharge planning. The high-needs areas for evaluation during discharge were fall risk and BADL, and the low-needs areas were well-being and functional level prior to onset. Conclusion : This study is expected to provide preliminary information necessary for the development of a discharge assessment tool for effective discharge planning.

  • I. Background The problem of discharging patients from hospital have been well documented in the literature over the last 20 years. They included poor communication between hospital and community, inadequate notice of discharge, over-reliance on informal support and lack of statutory support, inattention to patients needs before leaving hospital, and wasted or duplicated visits by community nurses. Most patients discharged from hospital are able to return home with little or no support, while others will require a 'package of care' to support them back to good health. Patient with complex care needs, including the frail elderly and those with mental health problems, may require continuing care in special housing, residential, or nursing homes. With this population,effective discharge arrangement is needed and the study on this problem is urgent in Korea because the Medical Reform Project is on suspension of success. II. Results of the Study: 1. Discharge service needs assessed on 360 elderly patients who were hospitalized during the survey period at four university hospitals. Patients want to know the information on disease management after discharge. Follow-up telephone service is the most frequently checked service. 2. Multidisciplinary Discharge Planning is recommended at the hospital level to reduce the readmission and decrease the length of stay. 3. Further research is needed to validate and test the assumption of the solution which is developed in this research.

A Case-control Study of unexpected Readmission in a University Hospital (비예정 재입원의 위험요인에 대한 환자-대조군 연구)

  • Objectives: This study describes the risk factors affecting the unexpected readmission of 261 patients who were discharged from a university hospital in Seoul. Methods: This case-control study reviewed medical records of inpatients who had been discharged from a hospital between 1 August 1995 and 31 October 1995 after the treatment for general diseases. The cases were 68 patients who were readmitted unexpectedly within 28 days of discharge from an index stay, and the controls were 193 Patients who were discharged without readmission during the study period. Results: Logistic regression analysis results were as follows; Patients who had no operation during their hospital stay were more likely to be readmitted unexpectedly than patients who had operation. Patients who had 1 or 2 parts of their body being involved in treatment were more likely to be readmitted unexpectedly than patients who hand more than 3 parts of their body being involved in treatment. Patients who had complications after surgery were more likely to be readmitted unexpectedly than patients who had no complications. Insufficient discharge planning caused unexpected readmissions. Conclusions: Discharge planning education should be extended to health care providers. And the assessment of discharge planning should be evaluated. Readmission is often necessary for the treatment of related problems of originating from initial hospitalization, which causes cost problems. Unexpected readmission is preventable and the models for readmission can serve as a valuable clinical tool for high risk patients.

Association Between Unplanned and Planned Readmissions in an University Hospital (비예정과 예정된 재입원 환자들간의 관련 요인 분석)

  • This study describes associated factors of readmission of 213 inpatients from an university hospital in Seoul. This retrospective study reviewed medical records of patients who discharged from a hospital stay for general diseases between 1 August 1995 and 31 October 1995, Cases were 68 discharge patients with an unplanned readmission within 30 days of discharge from an index stay. And the other cases are 145 patients who had more than two discharges and didn't have an unplanned readmission within 30 days. Logistic regression model was analyzed and the results were as follows; 1. duration of readmission, rate of unpayed, room, path, and risk of disease were more likely to be readmitted unexpectedly than the expected readmission patients. 2. early readmission, low risk condition group, and inadquateness of discharge plann for patients had unplanned radmissions rather than planned readmissions. Therefore, discharge planning education to health care provider is required and assessement of discharge planning should be evaluated. Readmissions are usually for related problems that arose during the original hopitaliztion and caused cost problems. Especially the unplanned readmissions are frequently preventable. Ultimately, models for readmissions can serve as a valuable clinical tool for target high-risk patients and older patients and with this kind of tools we can reduce hospital readmissions and maintain high-quality of inpatient care.

Monitoring of Outlet Discharge and Water Quality from a Small Agricultural Reservoir (소규모 농업용 저수지의 통관 방류량 및 수질 측정)

  • This study was carried out to measure the amount of outlet discharge and analyse the water quality from an agricultural reservoir. Joongpyong reservoir was selected as the monitoring site. Daily discharge was determined by measured water level data of delivery canal and the stage-discharge relation curve. The measured water discharge through culvert outlet of Joongpyong reservoir was 593,200m3 which was equivalent to irrigation depth 1,186mm during irrigation period in 1999. And water samples were taken from the surface water of reservoir and delivery canal, periodically. Temporal variation of water quality constituents such as water temperature, pH, EC, total nitrogen, total phosphorus were investigated. The result showed that pH was ranged 6.9 7.8, total nitrogen 1.39 4.11mg/L, total phosphorus 0.007 0.036 mg/L, respectively.

  • This study was conducted to seek the effective water management method of the irrigation reservoirs. Joongpyong reservoir was selected for the hydrologic monitoring, and investigated from May in 1999 to December in 2001. The water level and amount of outlet discharge were measured, the stage discharge equation as a rating curve was induced, and which were compared to the irrigation water requirements calculated by a daily simulation model. The water balance of Joongpyong reservoir was analyzed, mainly on the reservoir storage ratio during irrigation period. Comparing the observed storage and simulation data, the results of the simulation were well agreed with the measured data.

  • This study is concern with the design of experiment planning for quality improvement in flow shop manufacturing system. In this study, the procedure of two stage experiment planning are proposed and applied to the manufacturing process of Li/$MnO_2$ batteries. The result of in this paper is that, compared with the current process conditions, 52% of the insulation inspection process, 81.6% of the first premier discharge process, 98.5% of the second premier discharge process, 84.7% of O.C.V./C.C.V. process, and 86.2% of voltage inspection process are decreased. After a given period of time, the life of the batteries extends to 75 hours, which means the 15% improvement in capacity. In case that the proposed methods are applied to the process Improvements of the flow shop manufacturing system, the much effected in experimental cost- saving and quality improvement.

A Study on Estimation by Depth Integrating Method of Sediment Discharge (수심적분법에 의한 유사량 추정연구)

  • In Korea, total sediment discharge of a river has been estimated simply by using certain sediment transport formulas including, among others, Einstein's formula. Those formular, however, are known not to be reliable enough for the result calculated by them to be used directly to river planning and management. Therefore, the study used the Modified Einstein Procedure to the estimation of total sediment discharge, because this method is reliable estimated by measurement. Here, measurement of sediment discharge used depth integrating method. The major results obtained from the study for estimation by depth integrating method of sediment discharge in Naeseong stream are as follow; 1 The sedeiment characteristics of Naeseong stream are; The distribution of sediment grain size shows that silt and clay are 55% and sand is 45%. and the bed load sediment grain size is constituted that sand contained with the grain size from O.062mm to 2.0mm is 80% 2. The sediment rating formulas derived from the regression analysis between the sediment discharge and flow discharge are; Seogpo-Gyo : Qs=$0.017 \times 10^{-4} Q^{2.352}$, where discharge is l0cms $0.074 \times 10^{-4} Q^{2.066}$, where discharge is l0cms

Analysis and Comparison of Flow Rate Measurements Using Various Discharge Measuring Instrument and ADCP (다양한 유량 측정기기와 ADCP를 이용한 유량 비교 분석)

  • Discharge data examine the process of hydrologic cycle and used significantly in water resource planning and irrigation and flood control planning. It makes high quality discharge data, they carry out research on standard and method of discharge measurement, and equipment improvement. Now various flow meters are utilized to make discharge data in Korea. However, accuracy of equipment and exprerimental research data from measurement are not enough. ADCP(Acoustic Doppler Current Profiler) have been introduced and utilized for flow measurements since the end of 1980's. ADCP flow method is a formal method for flow measurement can easily applyd to relatively large rivers gradually recognized. This equipment can measure the non-contact three-dimensional velocity and water depth data very quickly and efficiently. Also, spatial and temporal resolution of the data is more accurate than any other flow measurement methods which measure flow rate by velocity - area measurement method. In this paper, the velocity is measured using various flow meter and verified the effectiveness by applying from the ADCP in Geum-river. Various flow meters which are med for discharge measurements are VALEPORT002, FLOW TRACKER, PRICE AA and ADCP. The average of five times flow measurement result by ADCP was $10.412m^3/s$, with a standard deviation of 0.68. The repeat test by ADCP and comparison between ADCP and other flow devices to verify the most import factor, flow measurement accuracy. In the result, repeat test of the ADCP showed similar values, flow values were similar to other velocity device results and the average error is 7.7%.
