일부지역 119 구급대와 사설이송기관의 이송 서비스 이용 실태와 만족도에 관한 연구 (A Study on the Use Realities and Satisfaction with Transport Services in 119 Emergency Medical Service System and Private Transport Agent in Some Areas)

  • Purpose : Centering on users(patients) who are offered the patient transport service by the emergency medical service system in our country, the use and satisfaction are analyzed with the transport service in 119 Emergency Medical Service System and Private transport agent. Results : 1. As for personnel in ambulance cars, 119 emergency staff showed a higher boarding rate of the first-class emergency medical technicians than private transportation centers : 48.4% and 17.7%, respectively. 2. Private transportation centers showed higher user satisfaction with transportation service than 119 emergency staff, which was not statistically significant. 3. As for the case that needs to receive the transport service in the future, the ratio with the will to use 119 Emergency Medical Service System(70.9%) was indicated to be much higher than the ratio of the private transport agent(29.1%). Suggestions : First, Centers transporting first-aid patients should essentially secure a sufficient number of first-class emergency medical technicians as professional emergency medical staff to reinforce qualitative improvement in emergency medical service. Second, It is necessary to establish a systematic monitoring system and develop educational programs in order to enhance satisfaction with the use of 119 emergency staff. Third, the government or the local government needs to positively support and guide the private transport agent, which is in charge of the public medical service.

GIS를 이용한 응급의료 진료권 분석: 서울시를 사례로 하여 (Analysis of the Emergency Medical Service Area Using GIS: the Case of Seoul)

  • 본 연구는 응급환자의 의료기관 이용 현황자료를 토대로 GIS를 이용하여 응급의료 진료권을 분석하는데 목적을 두었다. 진료권을 분석하기 위해 6개 응급의료센터를 사례로 선정하여 2003년 9-11월까지 내원한 환자의 주소 자료를 수집하였다. 해당 응급의료기관을 중심으로 하여 거리에 따른 내원환자의 분포 비율을 산출하여 진료권을 분석한 결과 해당 응급의료센터와 인접한 지역에 거주하고 있는 환자의 비율이 가장 높게 나타났다. 그러나 6개 응급의료센터의 1차 진료권의 범위는 상당한 차이를 나타내었는데, 이는 병상 규모 뿐만 아니라 인지도와 환자들의 선호도 등에 따라 응급의료센터의 진료권역이 달라짐을 말해준다. 본 연구에서는 서울시 응급의료 서비스의 공급 표면도를 구축하는 방법을 모색하였다. 32개 응급의료센터로 부터 거리에 따른 내원환자의 분포 비율을 가중치로 부여하여 거리조락에 따른 공급력 수준을 나타내는 개개의 커버리지를 구축한 후, 이를 지도 대수 연산을 통해 합성하였다. 이렇게 구축된 응급의료 서비스의 공급 표면도를 통해 응급의료 서비스 공급 수준은 공간상에서 상당한 차이가 나타나는 것으로 파악되었다.

Analysis of Factors Affecting Satisfaction of 119 Emergency Service Users in Korea

  • This study attempted to identify the service satisfaction with the 119 emergency service and the factors that affect it based on the responses of subjects who used the service. Data collection was conducted from February 11 to March 11, 2021, using convenience sampling. A total of 1842 subjects who used emergency medical services using 119 ambulance in 2020 participated. For the collected data, frequency, percentage, mean, and standard deviation were calculated, and t-test, analysis of variance, and multiple regression analysis were performed using SPSS 23.0. Based on these results, to further enhance the satisfaction of users of the 119 emergency service and the quality of emergency medical services, it is important to improve the quality of paramedics through education and systems. In addition, to increase user satisfaction, efficient and systematic communication education is required. The quality of emergency medical services will increase only when communication skills required for explanations and promoting understanding are improved.

응급의료기관의 공간분포와 응급의료 서비스 수급의 공간적 격차 (Spatial Distribution of the Emergency Medical Facilities and Spatial Disparity of the Demand-Supply Level for the Emergency Medical Service)

  • 응급의료 서비스는 국가가 형평성있게 적절한 서비스를 제공하여야 하는 공공성이 놀은 영역이라고 될 수 있다. 우리나라의 응급의료기관은 매우 불균등하게 분포되어 있을 뿐만 아니라 인력, 장비, 시설 면에서도 지역간에 상당한 차이를 보이고 있다. 된 연구에서는 응급의료 서비스의 수급 현황을 공간적 관점에서 분석하기 위해 서울시를 대상으로 하여 응급의료 서비스에 대한 잠재적 수요 표면도와 공급 표면도를 구축하였다. 이렇게 구축된 응급의료 서비스 수요 표면도와 공급 표면도를 연산 기능을 이용하여 응급의료 서비스의 수급 표면도를 생성하였다. 그 결과 응급의료 서비스의 공간적 격차가 매우 두드러지게 나타났다. 수요력이 공급력보다 크게 나타난 지역은 응급의료센터의 추가 지정이 잠재적으로 요구되며, 수요력이 공급력보다 작게 나타난 지역은 응급의료 서비스의 과잉 공급으로 인해 병원운영의 어려움을 잠재적으로 안고 있다고 풀이할 수 있다. 본 연구 결과는 바람직한 응급의료 서비스의 제공과 적정 수준의 진료권의 범위를 설정하는데 필요한 정보로 활용될 수 있을 것이다.

병원 전 응급의료체계에서 간호사의 업무범위에 대한 소고(小考) (A Study on the Scope of Practice of Nurses in the Prehospital Emergency Medical Service System)

  • 간호사는 의료법 상 의료인으로서 의료기관에서 진료의 보조 등의 업무를 수행한다. 의료인인 간호사는 119 구조·구급에 관한 법률에 근거 구급대원으로서 병원 전 단계 응급의료체계에서 응급환자를 대상으로 응급의료를 제공한다. 병원 전 응급의료체계에서 업무를 수행하는 간호사의 업무범위는 의료법 상 포괄적으로 규정되어 있는 업무를 판례 등을 통해 구체화되어 왔다. 반대로 응급구조사의 업무범위는 응급의료법에 의해 구체적 행위를 열거하는 방식으로 규정되어 있다. 병원 전 단계 응급의료체계에서 응급의료종사자로서 업무를 수행하는 간호사의 업무범위는 대상 환자의 긴급성, 응급의료기관으로의 이송 중이라는 점, 통신 상의 의료지도라는 특수성을 고려하여 간호사의 업무범위를 의료기관 내에서와 달리 적용하여 해석되어야 한다. 따라서 상대적으로 병원 전단계에서 간호사가 응급구조사보다 넓은 업무범위가 인정되고 이것이 정책목표에도 부합된다.

응급환자의 전원과 의사의 설명의무 (Interhospital Transfer of Emergency Patients and Informed Consent)

  • Inter-hospital transfer, depending on its medical and legal appropriateness, affect the prognosis of patients and can even lead to legal disputes. As Emergency Medical Service Act, any physician shall, in case where deemed that pertinent medical service is unavailable for such patient with the capacities of the relevant medical institution, transfer without delay such patient to another medical institution where a pertinent medical service is available. For medico-legally appropriate inter-hospital transfer, the head of a medical institution shall, in case where he transfers an emergency patient provide medical instruments and manpower required for a safe transfer of the emergency patient, and furnish the medical records necessary for a medical examination at the medical institution in receipt of such patient. And transfer process must comply with the requirements prescribed by executive rule such as attachment of the referral, provision of ambulance, fellow riders and informed consent of transfer. Those engaged in emergency medical service shall explain an emergency medical service to an emergency patient and secure his consent. In addition to the duty to inform about emergency medical service to the patient and his or her legally representative, there is also a duty for doctors to sufficiently explain to the patient and his or her legally representative during inter-hospital transfer that the need for the transfer, the medical conditions of the patient to be transferred and emergency treatment that will be provided by the hospital from which the patient is going to transferred. Likewise, the hospital to which the patient is transferred must be thoroughly informed about matters such as the patient's conditions, the treatment the patient was given and reasons for transfer by transferring doctors.

소방 구급활동에 필요한 지도의사와 1급응급구조사의 수는? (How Many Doctors and Paramedics Does Fire Service Need for Medical Direction in Korea?)

  • Purpose : It was to improve medical direction system through presenting need of doctor and paramedic in Korean Fire Service. Methods : This study was conducted by applying demand coefficients(4 for doctor, 3 or 4.5 for paramedic) to some data on medical director, paramedic, ambulance from National Emergency Management Agency. Results : Number of medical director & paramedic were 4 & 1,217. Number of necessary doctor for medical direction was 64 or 28(in case of direct medical direction) & 16 or 7(in case of indirect medical direction). Number of necessary paramedic for direct medical direction was 492(in case of 35% ALS ambulance) & 1,062(in case of 50% ALS ambulance). Conclusions : To improve quality & efficiency of medical direction brought up need of amendment of the Emergency Medical Services Act to apply indirect medical control such as standing orders, protocol, case review.

1급 응급구조사의 자격에 대한 입법론적 고찰 (Legislative study on the qualification of level 1 emergency medical technicians)

  • Purpose: To provide legislative data for the amendment of Article 36 of the Emergency Medical Service Act on the qualifications of emergency medical technicians. Methods: The study was drafted based on the Emergency Medical Service Act; data on the emergency medical technicians (EMT) system and curriculum in Korea, United States. Japan, and Taiwan; and previous studies on the EMT system in Korea. Results: The length of education. work scope. amd role of level 1 EMTs vary significantly depending on the type of qualifications they have as emergency medical professionals, while the supply of level 1 EMTs has already reached a point of saturation. Accordingly, the current regulation on allowing level 2 EMTs with at least three years of experience to take the level 1 EMT test presents serious inequity for students just graduating from their emergency medical services program. It is also a factor that degrades the professionalism of level 1 EMTs. Conclusion: Article 36, paragraph 2. subparagraph 3 of the Emergency Medical Services Act pertaining to regulations on "EMTs who have worked as level 2 EMTs for at least 3 years" needs to be removed.

  • Purpose: To discuss the records and legal standards of emergency medical technician training institutions in Korea and abroad, to identify the problems, and to provide the basic resources for improving the EMT-basic training institutions. Methods: We received advice through an advisory meeting of experts (professors of department of emergency medical technicians) and interested parties (Korean Association of Emergency Medical Technicians, officials of emergency medical technician training institutions) and referred to various reports published by governments, official institutions, and other trustworthy organizations. Also, we communicated with the related experts abroad (3 countries) on the phone or by email for surveys. Results: Compared to the abroad, it is necessary to categorize the standards and procedures of designating the emergency medical technician training institutions in Korea and improve the management of training institutions to train competent emergency medical technicians. Conclusion: It is necessary to designate and manage continuously the emergency medical technician-basic training programs for the systematic primary healthcare service.

노년기 연령그룹에 따른 응급의료이용 영향 요인 (Factors Affecting the Utilization of Emergency Medical Services by Age Group among Elderly Individuals)

  • Objectives: The purpose of this study was to analyze the factors affecting the utilization of emergency medical services and characteristics of emergency medical services according to age group among elderly individuals. Methods: This study conducted t-test and linear regression analysis on data of 1,960,575 participants to achieve the objective. Results: Analysis of the factors affecting the use of emergency medical services showed statistically significant correlation in all age groups. As the age of elderly people increased, the use of emergency medical service increased. Conclusions: Emergency medical policies are needed, such as coordinators with expert knowledge of medical and health administration and specialist emergency room operations that can provide specialized medical service for older patients.
