OBJECTIVE : To study the management (diagnostic and therapeutic) of esophageal foreign bodies with rigid esophagoscopy. MATERIAL AND METHODS : All 100 patients admitted to the Dankook University Hospital for ingestion of foreign bodies between May 1994 and July 1999. The outcome for each patient was determined by examining hospital records of demographic information, identification of the foreign body and the removal procedure used. RESULTS : Rigid esophagoscopy was performed for suspected foreign bodies in 100 cases an impacted. The most frequent location was the upper third of the esophagus (68%). The most common type of foreign body was fish bones in adult(61%) and coins in children(70%). In 12 patients. flexible endoscopy had failed previously to remove the foreign body and 4 cases were migrated esophageal metal stent in esophageal stenosis. CONCLUSIONS : The rigid esophagoscopy is appropriate techniques for managing esophageal foreign bodies.

The development of office-based, unsedated transnasal esophagoscopy (TNE) has proven to be a major technological advance and with time and experience the application of this technology is becoming more widespread. TNE has allowed otolaryngologists to perform a variety of diagnostic and therapeutic procedures in the office setting. Studies consistently demonstrate that the image quality and diagnostic capability of TNE is equivalent to conventional esophagoscopy. The modern TNE endoscopes offer high quality optics, air-insufflation, and irrigation capability through a 2-mm working channel, and the ability to perform biopsies and select procedures. In general, the role of TNE in the head and neck patient is three-fold : to screen for synchronous and metachronous esophageal squamous cell carcinoma (ESCC) ; to differentiate post-treatment changes/symptoms from malignancy ; and to perform certain office-based procedures. TNE offers many specific advantages to the head and neck patient that are not afforded by conventional esophagoscopy. Because of surgical and postirradiation changes, many HNSCC patients have trismus or neck stiffness preventing completion of conventional transoral esophagoscopy. Perhaps most importantly, TNE provides enhanced patient safety, increased tolerability, better practice efficiency, and cost savings. For these reasons, TNE has become a particularly useful tool in the otolaryngologist's armamentarium.

It is well tolerated by patient with topical anesthesia alone and no sedation or patient preparation is required. The technique is easily learned and performed in the otolaryngologist and highly cost efficient. It is a useful tool for accurate diagnosis and can be used in a variety of office-based procedures.

Objectives : The purpose of this study was to investigate the endoscopic evidence of esophagitis in laryngopharyngeal reflux disease (LPRD) patients using transnasal esophagoscopy (TNE) and to correlate these findings with treatment response. Methods : Fifty patients underwent TNE at Korea University Anam Hospital from July 2007 to Feb 2009. Participants were selected from patients that presented with various laryngeal symptoms. One experienced otolaryngologist assessed esophagitis according to the Los Angeles classification system using the TNE findings. Results : Fifteen of 50 LPR patients (30%) were found to have esophagitis (12 patients with Grade A, 3 patients with Grade B, no patients with grade C/ D esophagitis). Among the 15 patients positive for esophagitis based on the endoscopic findings, 12 (80%) showed symptom improvement after pharmacological therapy. Symptom improvement was correlated with evidence of esophagitis (p=0.002) but not with RFS (p=0.749). Conclusion : Endoscopic evaluation of esophagitis using TNE is a potentially valuable tool for predicting treatment response in LPRD patients.

식도이물의 합병증은 희소하나, 간혹 예리한 골편, 예리한 금속성리물 및 이물의 장기체류에 의해 식도손상을 일으키는 이물자체에 의한 경우, 식도질환 환자의 진단, 치료 목적으로 식도경검사 때의 조그마한 부주의에 의한 경우 및 식도의 위치관계 등의 원인으로 합병증을 일으키어 자칫 잘못하면 치명적인 상태까지 야기시키는 수도 있다. 그러므로 여기에 대한 조기진단과 신속한 치료을 가하지 않으면 높은 사망률과 병변을 초래하기도 한다. 이러한 의미에서 저자는 식도이물자체에 의한 합병도 및 식도경검사시의 부주의에 의한 합병증 27례를 경험하였기에 문헌적고찰과 함께 보고하는 바이다.

진단이나 치료 목적으로 내시경술이 빈번하게 사용됨으로써 오늘날 식도천공의 위험은 증가하는 경향을 보인다. 식도벽에 경미한 상처를 주더라도 치명적인 경우까지 야기시킬 수 있으며 조기진단과 신속한 치료를 하지 않으면 늪은 사망률 병3변(病變)을 일으킨다. 본 연자들은 식도 이물 제거시 식도경술로 인한 식도천공 및 식도주위농양을 동반한 2례에 대한 치료를 하여 좋은 결과를 얻었기에 문헌고찰과 함께 보고 하는 바이다.
