Pattern of Decrease of Prostate Specific Antigen after Radical Radiotherapy for the Prostate Cancer (전립선암 환자에서 방사선치료 루 전립선특이항원 농도 변화 양상)

  • Purpose : Prostate specific antigen (PSA) is a useful tumor marker, which is widely used as a diagnostic index and predictor of both treatment and follow-up result in prostate cancer. A prospective analysis was carried out to obtain the period of PSA normalization and the half life of PSA and to analyze the factors influencing the period of PSA normalization. The PSA level was checked before and serially after radical radiotherapy. Materials and Method : Twen쇼 patients with clinically localized prostate cancer who underwent radical external beam radiotherapy were enrolled in this study. Accrual period was from April 1993 to May 1998. Median follow-up period was 20 months. Radiotherapy was given to whole pelvis followed by a boost to prostate. Dose range for the whole pelvis was from 45 Gy to 50 Gy and boost dose to prostate, from 14 Gy to 20 Gy. The post-irradiation PSA normal value was under 3.0 ng/ml. The physical examination and serum PSA level evaluation were performed at 3 month interval in the first one year, and then at every 4 to 6 months. Results : PSA value was normalized in nineteen patients (95%) within 12 months. The mean period of PSA normalization was 5.3 (${\pm}$2.7) months. The half life of PSA Of the nonfailing patients was 2.1 (${\pm}$0.9) month. The nadir PSA level Of the nonfailing Patients waS 0.8 (${\pm}$0.5) ng/ml. The period of PSA normalization had the positive correlation with pretreatment PSA level (R$^{2}$=0.468). The nadir PSA level had no definite positive correlation with the pretreatment PSA level (R$^{2}$=0.075). The half life of serum PSA level also had no definite correlation with pretreatment PSA level (R$^{2}$=0.029). Conclusion :The PSA level was mostly normalized within 8 months (85%). If it has not normalized within 12 months, we should consider the residual disease in prostate or distant metastasis. In 2 patients, the PSA level increased 6 months or 20 months before clinical disease was detected. So the serum PSA level can be used as early diagnostic indicator of treatment failure.

Systematic Review of Selection of Acupuncture Points for Lower Back Pain (요통 침치료를 위한 경혈 선정에 관한 체계적 문헌 고찰)

  • Objectives : The purpose of this study is to figure out which acupoints are selected to treat low back pain and attempt to analyze the effectiveness of those acupoints in current clinical trials. Methods : We searched the three electronic databases(PUBMED, RISS, KISS) and manually checked related Korean journals and reference lists up to April 2012. We included randomized controlled trials, clinical controlled trials, and case reports/series using needle type acupuncture( manual acupuncture, electronic acupuncture) to treat low back pain in English and Korean. We investigated the frequency of selected acupoints, change of visual analogue scale of low back pain and statistical significance in each study among trials. Results : We included 37 articles(Domestic 17, International 20) 53 studies(Domestic 28, International 25) in this study. The most frequently adopted acupoints were BL23, BL24, BL25, GB30, BL26, BL60, BL32, BL40, KI3, GV3, ST36 in domestic studies and BL23, BL25, BL40, BL60, GB30, GB34, BL32, BL26 in international studies. There were differential effectiveness of acupoints between domestic and the international studies using statistical significance of visual analog scale for low back pain. Conclusions : These results suggest that both proximal and distal acupoints based on meridian theory were used in clinical trials to treat low back pain. It would be helpful to provide clinical guideline, evaluate the results of clinical trials appropriately, and reveal the effectiveness of acupoints.

  • These experiments were carried out to investigate the effects of cumulus cells for in vitro fertilization and development of bovine follicular oocytes matured in vitro. The bovine ovaries were obtained at a slaughter house and the follicular oocytes surrounded by cumulus cells were collected by puncturing follicles with 2~6 mm of diameter. Bovine oocytes were matured in vitro for 24~26 hours in a CO2 incubator with 5% CO2 in air at 39$^{\circ}C$. The medium used for maturation was TCM-199 supplemented with hormones, pyruvate, FCS and antibiotics. Epididymal spermatozoa were capacitated by in vitro culture for 2~3 hours in BO solution containing BSA(5mg/ml) and caffeine(2.5mM). Insemination was made by introducing about 10~15 matured oocytes into the suspension of capacitated spermatozoa. Six hour after insemination the eggs were transferred to TCM-199 supplemented with FCS(10%) and HEPES(25mM), cultured for 7~8 days with 10~15 eggs/well in 4-well multidishes(Nunc Co.) forming cumulus cell monolayer. The results obtained in these experiments were summarized as follows ; 1. The majority of the follicular oocytes with compacted cumulus cells existed in GV stage while those with dispersed or denuded cumulus cells existed GVBD and M II stage. 2. After 24~26 hours maturation, the maturation rates of the follicular oocytes cultured in TCM-199 containing hormones were slightly higher than those of oocytes cultured in medium without hormones, and the frequency of cumulus compacted or denuded oocytes reaching M II stage cultured in medium containing hormones was 75.7% or 51.7%, respectively(P<0.05). 3. After 20 hours in vitro insemination, percentages of ova fertilized were 61.4% or 51.4%, respectively, for cumulus oophorus intacted or removed, and increased frequency of ova with both male and female pronuclei was found when cumuli were present(P<0.05). 4. The rates of embryos developed to 2-, 4-, 8-, 16-cell and morula or blastocyst stage after cocultured with cumulus cells were 65.0%, 45.3%, 34.7%, 28.0% and 22.7%, respectively. The results for momla or blastocyst stage were significantly higher than those of the embryos cultured in the basic medium(P<0.05).

A Review Study on the Treatment of Postpartum Depression in Traditional Chinese Medicine (산후 우울증 치료에 대한 중의학의 최근 임상연구 동향)

  • Objectives: This study was performed to review the research trends regarding the treatment of Postpartum Depression (P.P.D) in the Traditional Chinese Medicine (TCM). Methods: We searched articles in CNKI (China National Knowledge Infrastructure) from 2004 to 2013. Search key words were "产后抑郁症", "Postpartum Depression", and we selected 27 studies except for non-clinical studies, insufficient number of studies, unrelated studies and tests on animals. Results: We selected 27 articles with following results: 1) One study of Jadad Quality Assessment Scale score showed 3 points, 4 studies showed 2 points and 22 studies showed less than 1 point. Overall, the quality of the studies was low. 2) DSM - IV was frequently used as a diagnostic criteria and HAMD was mostly used as evaluation criteria. Diagnostic criteria and evaluation criteria were the same in HAMD and EPDS. 3) In TCM, there are various ways, such as herbal medicine, acupuncture, electro acupuncture, auricular acupuncture and/or massage to treat the postpartum depression. 4) The roots of Bupleurum falcatum Linne, Glycyrrhiza uralensis Fisch, Angelica gigas Nakai and Paeonia lactiflora Pall, the rhizome of Atractylodis macrocephala Koidz, the semen of Zizyphus jujuba Mill, the radix of Curcuma aromatica Salisb, the sclerotium of Poria cocos Wolf and the rhizomas of Cyperus rotundus L and of Cnidium officinale Makino are mainly used in the herbal medicine. BaekHoi (GV20) and Yindang (Ex-HN3) are mainly used in acupuncture points. 5) Most of the studies showed effective results. Some of the results were similar to those of the control groups which were cured by western medicine. Other results were better than those of control groups. Conclusions: Research on postpartum depression has been actively conducted in TCM and all studies showed effective results in the study group patients. Therefore, with this study, we hope to activate more clinical research on the treatment of patients in Korean traditional medicine.

Analysis of Clinical Research Trends on Traditional Chinese Medicine Treatment for Depression Syndrome Similar to Hwabyung: Focusing on CNKI (화병(火病) 유사 병증의 한의학적 치료에 대한 임상연구 동향분석 - CNKI를 중심으로)

  • Objectives: To analyze whether a concept similar to Korean 'Hwabyung' exists in China. We investigated the status of clinical studies conducted in China for relevant diseases and trends of the traditional Chinese medicinal (TCM) treatment adopted in clinical studies. Methods: To explore the concept of a condition similar to Korean Hwabyung in China, we searched for the existence of concept and pattern that were parallel or similar to those of Hwabyung in TCM text-books and diagnostic guidelines. We searched and analyzed clinical studies of TCM treatment for depression syndrome similar to Hwabyung from CNKI using terms 'depression' and 'qi stagnation transforming into fire'. Using extracted data, characteristics of clinical research, herbal medicine, and acupuncture treatment used in the clinical research and their effects were systematically reviewed. Results: Symptoms of 'qi stagnation transforming into fire' were most similar to those of Hwabyung. Nine articles were selected from a total 258 articles. Most of them used DSM-IV or CCMD-3 for depression diagnosis. They applied 'diagnostic and efficacy guidelines for TCM diseases and syndromes' for pattern diagnosis of 'qi stagnation transforming into fire'. Danzhixiaoyao-san and Jiaweixiaoyao-san were found to be effective when they were used alone or in combination with antidepressants. Acupuncture treatment also showed remarkable effect on LR3, HT7, LI4, PC6, GV20 when it was used alone or in combination with antidepressants. However, careful interpretation is required because a small number of studies are included.Conclusions: 'Qi stagnation transforming into fire' seemed to have symptoms similar to Hwabyung. However, further research is needed to determine its diseases and pattern types compared to Hwabyung. According to included studies of 'depression with Qi stagnation transforming into fire', herbal medicine and acupuncture treatment tended to be effective in relieving depressive symptoms. However, more discussion is required for future application of herbal medicine and acupuncture for treating Hwabyung.

A Case Report of a Patient with Subarachnoid Hemorrhage and Deep Vein Thrombosis Treated with a Combination of East-West Medicine (지주막하 출혈 후 발생한 심부정맥 혈전증 환자에 대한 한양방 병용치료 증례 보고 1례)

  • Purpose: To report on a patient with subarachnoid hemorrhage and deep vein thrombosis treated with a combination of Korean and Western medicine. Methods: A patient diagnosed with subarachnoid hemorrhage and deep vein thrombosis was treated with herbal medicine (加減淸熱瀉濕湯 (Gagam-cheongyeolsaseub-tang)), acupuncture (GV2, GB20, LI11, LI4, TE5, GB34, ST36, SP9, ST38, GB39, SP6, LR3), moxibustion (CV6, CV9), and Western medicine for deep vein thrombosis and subarachnoid hemorrhage. To evaluate edema, one of the classic symptoms of deep vein thrombosis, we measured leg circumference at a specified time. Results: After taking Gagam-cheongyeolsaseub-tang and being treated with acupuncture, moxibustion, and Western medicine, the patient showed improvement in leg edema. The left calf circumference decreased from 37.5 cm to 34.5 cm. Conclusions: Korean medicine, acupuncture, moxibustion, and Western medicine may provide effective treatment for deep vein thrombosis in patients with subarachnoid hemorrhage, but further study is needed.

Review of Recent Clinical Trials for Depression in Traditional Chinese Medicine-Based on Randomized Controlled Trials and Systematic Reviews- (우울증 치료의 중의학 최신 임상연구 동향-무작위 대조군 임상연구와 체계적 문헌 고찰을 중심으로-)

  • The purpose of this study was to review the research on treating depression in Traditional Chinese Medicine(TCM) over the last 5 years to set the foundation for further studies. We searched for and analyzed articles about depression in CNKI(China National Knowledge Infrastructure) from January 2010 to December 2014. The results were as follows : The most frequently used diagnostic criteria was CCMD-3(The Chinese Classification of Mental Disorders-3), and the most used evaluation criteria was HAMD(Hamilton Depression Rating Scale). Prescription of decoction of medical herbs was most frequently used as a therapeutic method. Acupuncture, traditional Chinese psychotherapy, and music therapy were also used for some studies. The most frequent TCM Syndrome Differentiation Type was stagnation of liver-QI. For decoction of Chinese herbs, Soyo-san(Xiaoyao-san) and Sihosogan-san(Chaihushugan-san) were most often prescribed, and Bupleuri Radix, Paeoniae Radix Alba, Curcumae Radix, Poria cocos wolf, Angelicae Gigantis Radix, Atractylodis Rhizoma Alba were mainly used for medical herbs. BaekHoi(GV20) and Yindang(Ex-HN3) were often used as acupuncture sites. Post-Stroke Depression was the most common case of intercurrent or secondary depression. According to the Jadad Quality Assessment Scale, the quality of the reports was not high as most of the reports had a score of 3 or below. Most systematic reviews on depression were conducted by Chinese researchers. The problem with Clinical research on depression, according to those reviews, was that there were no standardized criteria for the diagnosis and treatment and the trials were usually not randomized nor controlled. We found out there are various clinical methods for treating depression in TCM, and hope that this research could provide the preliminary data for designing and conducting clinical trials for depression.

Report on the 4th WHO Informal Consultation on Development of International Standard Acupuncture Points Locations (제 4차 WHO 경혈 위치 국제표준화 회의 보고)

  • Objective: To inform of the 4th WHO informal consultation meeting on the development of international standard acupuncture points locations, held in Daejeon, Korea, on April $23{\sim}26$, 2005. Results and Conclusions: 12 experts from Korea, China and Japan discussed the locations of 18 controversial points that were not agreed at the previous meeting, and 16 non-controversial points that all 3 nations agreed on the locations but still needed more discussion for their expressions, as well as other non-controversial points that were agreed on both the locations and the expressions but were suggested by Chinese party to be discussed again. Also, the guidelines for English translation of the standard acupuncture points locations and the development of the standard acupuncture points charts, diagrams, and dolls were discussed in this meeting. Through this meeting the locations of ST30, SP11, SP12, TE18, GB9, GB10, LR4, LR5, LR6, BL6, BL7, BL9, GB19 were agreed, but it was suggested that more research would be needed for the locations of LI20, GV26, PC8, PC9, LR7, GB30, LR12, and the expressions of ST36, ST38, ST39, BL1. Due to the time limitation, 16 acupuncture points out of those Chinese party suggested to have more discussion about were not discussed at this meeting and remained to be discussed at the next meeting.

A literatual study on the acupuncture and moxibustion for hemiparesis of stroke in Euibujipsung (중풍 후 운동 장애에 대한 『의부집성(醫部集成)』의 침구치료 고찰)

  • Objectives and methods : The Euibujipsung is the one of the huge-scale encyclopedias about Oriental Medicine. To investigate the most frequently used acupoints for hemiparesis after stroke, we used Euibujipsung CR-ROM database with several key words concerned with motor weakness (半身不遂 不遂不隨 癱瘓 中臟 中腑 風痱, etc.). Results : In the result, we found five popular acupoints (GV20, LI11, LI15, ST36 and GB39), and four meridians (Stomach, Gall bladder, Large intestine and Small intestine). We also found that the Yang meridians were cited more frequently than the Yin. Conclusion : Therefore we think that these findings can give further ideas to clinical practice and research fields for stroke rehabilitation in Oriental medicine.

A clinical study of East-West Pain Treatment on Chronic Headach Patients (만성두통환자에 대한 동서협진의 임상적 연구)

  • Objective : Because the cause and etiology of chronic headache are not yet fully explained, the treatment of this symptom is not simple. In order to study the effectiveness of East-West pain treatment on chronic headache, we evaluated its effect of pain alleviation and quality of life improvement on chronic headache patients who were treated with nerve block and acupuncture, and only acupuncture for eight weeks. Methods : 92 clinical experiment participants were gathered and through a questionnaire patients who experienced headache for more than 4 hours a day and more than 15 days per month were qualified as chronic headache patients. The qualified patients were classified into two groups, nerve block and acupuncture group(EW group, n=49) and acupuncture group(E group, n=43). Treatment was applied 2 times a weeks for 8 weeks. The acupoints, GV20, HN23, ST8, HN46, TE17, GB20, LI20O, LI11, LI14, ST36, and LR3 were stimulated for 20 minutes. The effects of two groups were analyzed using VAS scores and BPI(Brief Pain Inventory).
