Chronic Desquamative Gingivitis (만성 박리성 치은염)

  • Desquamative gingivitis is characterized by a diffuse erythema of the free and attached gingiva associated with areas of vesiculation, erosion, and desquamation. Desquamative gingivitis is not a distinct disease entity but represents a reaction pattern of the gingiva to various stimuli. Pemphigus vulgaris, cicatricial pemphigoid, and lichen planus may presents as desquamative gingivitis. We observed 3 patients whose disease was limited to the gingiva, and studied them by light and direct immunofluorescence microscope. We classified them according to clinical, histologic, and immunopathologic observations. Identification of the underlying causes of desquamative gingivitis is of utmost importance and is dependent upon clinical, histologic, and immunologic criteria.

Effects of coconut oil pulling and professional toothbrushing on gingivitis (치은염 환자에게 적용한 오일풀링과 전문가 칫솔질법의 효과)

  • Objectives: The purpose of this study was to provide basic data for gingivitis management programs by dental hygienists by evaluating the effect of oral health education, oil pulling, and professional toothbrushing on gingivitis. Methods: A total of 38 subjects were divided into three groups: control group (12 subjects), experimental group 1 (13 subjects), and experimental group 2 (13 subjects). The control and experimental groups were instructed to brush using the rotating method. Distilled water was provided to the control group after training. Coconut oil was provided every morning for about 10 minutes. In the experimental group 2, a professional brushing method was used at each visit. Results: There were no significant differences in oral health among the three groups, and there was homology between patient hygiene performance (PHP) index (p=0.144) and bleeding rate (p=0.213). The PHP index showed a significant interaction between the group and measurement time. The control and experimental groups showed changes in the PHP index with time (F=3.711; p=0.013). The bleeding rate showed a significant interaction between the group and measurement period. The control and experimental groups showed changes in the bleeding rate with time (F=6.707; p<0.001). Conclusions: Oral health education, professional toothbrushing, and oil pulling specialists in oral care of gingivitis were effective in managing gingivitis. It is necessary to educate people on self-management methods for oral health promotion using gingivitis management programs by dental hygienists.

Analysis of Correlation between Smoking and Gingivitis an Actual Condition of Oral Health Care in Some Vocational High School Boy Students (일부지역 실업계 고등학교 남학생의 구강관리실태 및 흡연과 치은염과의 상관성 분석)

  • The purpose of this study was to examine correlation between smoking and gingivitis in boy students targeting 281 students in the 2nd grade for vocational boys' high schools where are located in Cheongju city. As a result of carrying out questionnaire research and oral examination, the following conclusions were obtained. 1. The smoking-experience rate in subjects of this study accounted for 59.4%. The current smoking rate was indicated to be 41.3%. Students' average gingivitis prevalence accounted for 61.5%. Out of those students, the students with experience of smoking accounted for 44.8%. The average gingivitis index was $0.20{\pm}0.27$. Out of those students, the students with experience of smoking were indicated to be $0.29{\pm}0.31$. 2. In case of having experience of smoking, the probability of having gingivitis was high(p = 0.000). The more drinking experience led to the higher gingivitis prevalence(p = 0.010). 3. In the multi-variate analysis, a case of gingivitis prevalence was higher in the group with experience of smoking by 5.5 times(p = 0.000) compared to the non-smoking group. A case that the smoking volume for a day is over 6 pieces was higher by 8.4 times(p = 0.000) compared to the non-smoking group. A case of being over 25 months in the smoking period was higher 13.2 times(p = 0.000) compared to the non-smoking group.

The effect of dentifrice containing garlic extract on dental plaque and gingivitis (마늘추출물 함유 치약이 치면세균막 감소 및 치은염 완화에 미치는 영향)

  • The purpose of this study was to determine the effect of dentifrice containing 0.1% extracts of garlic on dental plaque and gingivitis in a double blind and crossover clinical studies in 33 healthy adults aged from 20 to 22 years who provided a consent for their participation. Oral examination was performed through clinical periods and on day of baseline, 6, 13, 19, 25 days plaque index and gingival index were scored by Turesky' modified index and L$\ddot{o}$e & Silness index. After 12, 19, 25 days use of their respective dentifrices, statistically decreases of plaque index, gingival index were shown in both the experimental and the control group, respectively, Experimental group exhibited significantly the lower plaque levels and the higher levels of gingival health by the use of the dentifrices contained extract of garlic from 12 days compare with control group(p<0.05). The degree of decrease was more significant on gingivitis level of the experimental group than the control group(p <0.05). This result indicate that the use of dentifrice containing extract of garlic has a positve effect in preventing plaque and gingivitis and treating periodontal diseases.

  • Prevotella intermedia is a Gram-negative, obligately anaerobic, nonsporeforming, and nonmotile rod. P. intermedia is associated with periodontitis, pregnancy gingivitis, acute necrotic ulcerative gingivitis, endodontic infection, and rheumatoid arthritis. P. intermedia KCOM 1107 (= ChDC KB29) was isolated from a human subgingival dental plaque of gingivitis lesion. Here, we present the draft genome sequence of P. intermedia KCOM 1107.

  • Objectives: This study is to compare measuring gingival and peridontal indices and changes in dental plaque per period using a three mix types of dentifrice and to investigate dental diseases preventive effects depending on gingivitis reducing effect of dentifrice through a clinical experiment. Methods: This study targeted adult females and males with mild to moderate gingivitis from age 20 to 60. The Calculus index, Papillary Marginal Attached Gingival (PMA) index, Gingival index, Patient Hygiene Performance (PHP) index, and Plaque index were measured at pre-experiment and at 1, 2, 4 weeks post experiment. Results: The PMA, Gingival index, PHP index, plaque index of experimentla group decreased after 4 weeks (p<0.05). Conclusions: A three mix types of dentifrice for relieving tooth sensitivity was verified to be effective in removing dental plaque and reducing gingivitis.

THE EFFECT OF SUPEROXIDE DISMUTASE ON EXPERIMENTAL GINGIVITIS AND ACTIVITY OF 3T3 FIBROBLAST (Superoxide Dismutase가 백서의 실험적 치은염과 3T3 섬유모 세포의 활성에 미치는 영향)

  • Inflammatory cells may produce active species of oxygen in antimicrobial defense. While such species can directly damage surrounding tissue, their major secondary role may be to mediate important components of the inflammatory response. Superoxide dismutase, antioxidant, have significant anti-inflammatory properties in rheumatoid arthritis, ischemic tissue injury and gastrointestinal disease. Increased oxidative product formation diseases. And superoxide dismutase produced by Porphyromonas Gingivalis is resistant to killing by polymorphonuclear leukocyte. The purpose of this study was to investigate on the effects of superoxide dismutase in 3T3 fibroblast and in experimental gingivitis in the rats. The effect of superoxide dismutase(SOD) to cell morphology and cell activity was measured in cultured mouse 3T3 fibroblast. After experimental gingivitis were induced by lipopolysaccharide(LPb) and bovine serum albumin(BSA), injection of SOD were done. WBC count and histologic findings were observed at 1, 2, 3, and 7 days. The results were as follows; 1. There was a little difference between LPS treated groups and SOD treated groups in 3T3 fibroblast morpholoy. 2. There was no difference between only SOD treated groups (except SOD 150U at 3days) and control in 3T3 fibroblast activity. 3. LPS $0.5{\mu}g/ml$ and SOD treated groups (except 150U) had decreased 3T3 fibroblast activity and no significant difference at 3 days. 4. LPS $5.0{\mu}g/ml$ and SOD treated groups were significantly increased cell activity of 3T3 fibroblast than control group at 1 day(P<0.05). 5. In LPS induced gingivitis, the number of leukocytes in SOD treated was significantly decreased than in saline treated at 1 day(P<0.05). 6. In histopathologic findings of LPS or BSA induced gingivitis, inflammatorycell infiltration in SOD treated groups were less than in saline treated group at 1, 2 and 3 days.

THE EFFECT OF GINGIVAL GEL ON PERIODONTIUM IN MANDIBULAR FRACTURE PATIENTS APPLIED BY ARCH BAR (선부자를 적용한 하악골 골절환자의 치주조직에 기능성 치약이 미치는 영향)

  • For many years, intermaxillary fixation using arch bar has been operated in treatment of mandibular fracture patients. But it has many complications including injury of operators and assistants cause by wire, inflammation of periodontium. For that reasons alternatives are required; osteosynthesis technique using mini plate, intermaxillary fixation using IMF screws have been available. Treatment by arch bar fixation, however, is still valuable to treat craniomaxillary fracture patients. The purpose of this study is to know effect arch bar on periodontium and influence gingival gel on periodontium applied by arch bar. 40 mandibular fracture patients are monitored. 30 patients were applied by arch bar, 10 patients were not. And the former were classified by 3 categories; Nano vitamin and Mastic gel were applied to 10 patients respectively and any gingival gel was not used to 10 patients. Clinical attachment level, bleeding on probing and periodontal depth of each group were measured and compared before operation and on 2 weeks and 6 weeks after operation. Mann-Whitney U test was used to analyze result which leads to this conclusion. 1. Whether arch bar is applied or not, treatment of mandlbular fracture gave rise to gingivitis, but 6 weeks after operation, gingivitis is restored to the same level as the state before operation. 2. More severe gingivitis appeared when arch bar is applied to mandibular fracture than when it is not. 3. Both gingival gel used in this study can reduce gingivitis which can be caused by arch bar. 4. In this study, Mastic gel is more effective for prevent gingival inflammation cause by arch bar than nano vitamin. In regard to this result, gingivitis is considered to be available because it is reversible and does not induce periodontal disease. Gingival gel is regarded to be helpful for patients applied by arch bar to feel less discomfort.

The Effects of the Mixture of Herbal Extract on Developing Plaque and Gingivitis (생약복합제재에 의한 구강양치가 치태 및 치은염에 미치는 영향)

  • This double-blind controlled clinical and microbiological study was carried out to determine the effects of mouthwash preparation containing the mixture of herbal extract on developing plaque and gingivitis in the experimental gingivitis model. Following a 2-week normalization period, 34 dental students were distributed randomly into 1 of 3 treatment groups. They rinsed, under supervision, two times daily for 3 weeks with either normal saline(CT), 0.1% chlorhexidine(CH), or the mixture of herbal extract (HT), but refrained from any oral hygiene measures. The Plaque Index(PlI), the Gingival Index(GI), and the amount of Gingival Crevicular Fluid(GCF) were measured at week 0,1,2, and 3 of the experimental period, while the assessment of total wet weight of plaque and the phase contrast microscopic examination of plaque were performed at the end of experimental period(3 weeks). Subjects using mouthrinse preparation containing the mixture of herbal extract demonstrated negligible, if any, changes in the accumulation and microbial composition of plaque compared to those using normal saline, while the reduction of gingival inflammation by this mixture was highly significant and comparable to that of chlorhexidine. The results of this study indicate that the preparation containing the mixture of herbal extract do not provide any antiplaque benefits but is very effective in inhibiting the development of and in reducing existing experimental gingivitis when used as mouthrinse. Further research is needed to determine whether a significant reduction of gingival inflammation without a concomitant decrease in plaque accumulation is of clinical importance.

Effective Management of Acute Necrotizing Ulcerative Gingivitis with Proper Diagnosis and Immediate Treatment

  • Necrotizing periodontal diseases, especially acute necrotizing ulcerative gingivitis (ANUG), it should be noted, occur abruptly and progress rapidly, eventually causing severe soft-tissue and alveolar bone loss. This report presents the cases of two ANUG patients and provides a brief treatment protocol for easy and effective clinical management. After proper diagnosis, sequential treatment with cessation of mechanical brushing, along with a prescription of systemic antibiotics and chlorhexidine as a mouth rinse, scaling, root planing, and supportive periodontal therapy, was utilized. In all cases discussed in this report, there was marked improvement in a few days. ANUG, though an uncommon disease, can be efficiently managed with proper diagnosis and immediate treatment.
