Effects of Silver Spike Point Electrical Stimulation on Glomerular Filtration Rate in Volunteer (은침점전기자극의 인체적용이 신사구체여과율(Glomerular Filtration Rate)에 미치는 효과)

  • The purpose of the present study was to investigate the effectiveness of silver spike point (SSP) low frequency electrical stimulation on glomerular filtration rate (GFR), specifically, such as diuretic action in 24 hour urine and in plasma analysis from normal volunteer. The current of 1 Hz continue type (CT) of SSP low frequency electrical stimulation significantly decreased in plasma creatine from normal volunteer. However, the urine creatinine clearance (Ccr) was significantly increased by SSP low frequency electrical stimulation in normal volunteer. These results suggest that the SSP low frequency electrical stimulation, especially current of 1 Hz continue type, significantly regulates urine creatinine clearance and glomerular filtration rate from normal volunteer. Therefore, the SSP low frequency electrical stimulation is a good regulator through a diuretic action of hypertension.

The Value of Serum Concentration of Cystatin C as a Marker for Glomerular Filtration Rate in Children and Adolescents (소아 및 청소년에서 사구체 여과율의 지표로서 혈청 Cystatin C 농도의 유용성)

  • Purpose : Cystatin C has been proposed for the assessment of glomerular filtration rate, being more accurate than creatinine determination. We measured serum cystatin C concentrations in the pediatric population, and analysed the correlation between cystatin C and glomerular filtration rate. Methods : Cystatin C and creatinine were measured by the particle enhanced nephelometric immunoassay and Jaffé method, respectively, in 276 children and adolescents without evidence of kidney disease. The glomerular filtration rate was estimated by Schwartz's formula. Results : The mean serum cystatin C concentration was significantly higher in infants under the age of 12 months than in older population. There was a negative correlation between cystatin C and age under 12 months, but not a significant change of cystatin C with age in children older than 12 months. For children older than 12 months, the reference range of cystatin C was 0.46-1.05 mg/L. On the other hand, there was a positive correlation between creatinine and age in the whole population. We also observed a positive correlation between estimated glomerular filtration rate and 1/cystatin C. Conclusion : The measurement of cystatin C is more practical than creatinine for estimating glomerular filtration rate in the pediatric population.

Influence of Bethanidine on the Renal Function of the Dog (개의 신장 기능에 미치는 Bethanidine의 영향)

  • Bethanidine, which is known as a sympathetic blocking agent, was injected into the vein of a dog in an attempt to investigate the influence on renal funciton. Bethanidine resulted in an increased urine flow and glomerular filtration rate, and it produced an increase of urinary sodium and potassium excretion and a decrease of reabsorption rate of sodium and potassium in renal tubules, whereas renal plasma flow showed no significant changes. After pretreatment of phentolamine, a specific alpha adrenergic blocking agent, bethanidine did not significantly increase glomerular filtration rate and diuresis, significantly increased urinary sodium and potassium excretion although the magnitudes were reduced when compared with that of bethanidine alone. In conclusion, bethanidine-induced diuresis appears to be the result of an inhibited tubuler reabsorpting of electrolytes within the kidney through its sympathetic blockade of renal nerves and of an increased glomerular filtration rate, which was caused by the constriction of vas efferense in the glomeruli.

Effect of Atrial Natriuretic Factor on the Renal Function and Renin Release in Unanesthetized Rabbit (무마취 가토 신장기능에 미치는 Atrial Natriuretic Factor의 영향)

  • Since it has been suggested that atrial receptor may be involved in the mechanism of extracellular volume regulation, it was shown that the granularity of atrial cardiocytes can be changed by water and salt depletion, and that an extract of cardiac atrial tissue, when injected intravenously into anesthetized rats, was shown to cause a large and rapid increase in renal excretion of sodium. Various natriuretic peptides were isolated and synthetized, and the effects were investigated by many workers. Most studies, however, have been carried out under anesthesia and there have teen some controversies over direct effect of the factor on the renal function. Therefore, it was attempted in this study to access the effects of an atrial extract and a synthetic natriuretic factor in unanesthetized rabbits. Intrarenal arterial infusion of atrial extract caused a rapid increase of urinary volume and excretion of sodium. Glomerular filtration rate and renal plasma flow were both increased with no change in filtration fraction. The ventricular extract produced no change in urinary excretion of electrolytes, nor in renal hemodynamics. Intrarenal infusion of synthetic atrial natriuretic factor caused increases of renal excretory rate of sodium, chloride and potassium, and $FE_{Na}$. Glomerular filtration rate, renal plasma flow increased. And free water clearance also increased. Accentuated excretory function correlated well with increased glomerular filtration rate and renal plasma flow during infusion and for 10 minutes following the cessation of the infusion. Renin secretion rate decreased during constant infusion of atrial natriuretic factor. However, no correlation was found with the changes in glomerular filtration rate, renal plasma flow, or urinary excretion of sodium. These results suggest that atrial extract or atrial natriuretic factor induces changes in renal hemodynamics, as in excretion of electrolytes either indirectly through hemodynamic changes or directly by inhibiting tubular reabsorption. At the same time, renin secretory function is affected by the factor possibly through an unknown mechanism.

Correlation between glomerular filtration rate and urinary N acetyl-beta-D glucosaminidase in children with persistent proteinuria in chronic glomerular disease

  • Purpose: Urinary excretion of N acetyl-beta-D glucosaminidase (NAG) and ${\beta}_2$-microglobulin (${\beta}_2$-M) was increased in the presence of proximal tubular damage. Based on these urinary materials, we investigated the ability of expecting renal function in chronic glomerular diseases. In this study, we evaluated the relationship between glomerular filtration rate (GFR) urinary NAG, and urinary ${\beta}_2$-M. Methods: We evaluated 52 children with chronic kidney disease at the Chung-Ang University Hospital between January 2003 and August 2009. We investigated the 24-hour urinalysis and hematologic values in all 52 patients. Serum creatinine, creatinine clearance (Ccr), serum cystatin C, urinary ${\beta}_2$-M and urinary NAG were measured. Results: Out of 52 patients, there were 13 children with minimal change in disease, 3 children with focal segmental glomerulosclerosis, 17 children with immunoglobulin A nephropathy, 15 children with Henoch-Sch$\ddot{o}$nlein purpua nephritis, 3 children with poststreptococcal glomerulonephritis, and 1 child with thin glomerular basement membrane disease. In these patients, there were significant correlation between the Ccr and urinary NAG (r=-0.817; $P$ <0.01), and between the GFR (as determined by Schwartz method) and urinary NAG (r=-0.821; $P$ <0.01). In addition, there was a significant correlation between the GFR (as determined by Bokencamp method) and urinary NAG (r=-0.858; $P$ <0.01). Conclusion: In our study, there was a significant correlation between the GFR and urinary NAG, but there was no correlation between the GFR and urinary ${\beta}_2$-M, suggesting that the GFR can be predicted by urinary NAG in patients with chronic glomerular disease.

Effect of Nifedipine on Renal Function in Dogs (Nifedipine의 개 신장기능에 미치는 영향)

  • This study was performed in order to investigate the effect of nifedipine, a vasodilating drug which acts through calcium antagonism, on renal function using mongrel dog. Nifedipine, when given interavenously in doses ranging from 1.5 to 5.0$\mu\textrm{g}$/kg, elicited diuresis along with less changes of glomerular filtration rate and significant increases of renal plasma flow, so that the filtration fraction(FF) decreased significantly, at the same time both osmolar and free water clearances increased, and amount of sodium, potassium and calcium excreted in urine increased significantly. Nifedipine, when infused into a renal artery in doses from 0.05 to 0.15$\mu\textrm{g}$/kg/min, exhibited identical responses to the actions of intraveneous nifedipine except significant increase of glomerular filtration rate and no change of FF, which was confined only to the infused kidney. The renal action of nifedipine into a renal artery were not influenced by renal denervation, decreased significantly by ouabain, Na$^+$-K$^+$-ATPase inhibitor, which was given into a renal artery. Nifedipine infused into a renal artery in dog pretreated with propranolol i.v. produced diuresis associated with the increase of electrolytes excretion by reduction of electrolyte reabsorption and with no changes of glomerular filtration rate and renal plasma flow. Thus, it is concluded that nifedipine infused into a renal aretery produces diuretic action along with both improvement of hemodynamics and inhibition of electrolytes reabsorption, which may be related to sympathetic $\beta$-receptor or Na$^+$-K$^+$-ATPase activity because the action of nifedipine in kidney is blocked by propranolol or ouabain.

Comparative analysis of Glomerular Filtration Rate measurement and estimated glomerular filtration rate using 99mTc-DTPA in kidney transplant donors. (신장이식 공여자에서 99mTc-DTPA를 이용한 Glomerular Filtration Rate 측정과 추정사구체여과율의 비교분석)

  • Purpose Glomerular filtration rate(GFR) is an important indicator for the diagnosis, treatment, and follow-up of kidney disease and is also used by healthy individuals for drug use and evaluating kidney function in donors. The gold standard method of the GFR test is to measure by continuously injecting the inulin which is extrinsic marker, but it takes a long time and the test method is complicated. so, the method of measuring the serum concentration of creatinine is used. Estimated glomerular filtration rate (eGFR) is used instead. However, creatinine is known to be affected by age, gender, muscle mass, etc. eGFR formulas that are currently used include the Cockroft-Gault formula, the modification of diet in renal disease (MDRD) formula, and the chronic kidney disease epidemilogy collaboration (CKD-EPI) formula for adults. For children, the Schwartz formula is used. Measurement of GFR using 51Cr-EDTA (diethylenetriamine tetraacetic acid), 99mTc-DTPA (diethylenetriamine pentaacetic acid) can replace inulin and is currently in use. Therefore, We compared the GFR measured using 99mTc-DTPA with the eGFR using CKD-EPI formula. Materials and Methods For 200 kidney transplant donors who visited Asan medical center.(96 males, 104 females, 47.3 years ± 12.7 years old) GFR was measured using plasma(Two-plasma-sample-method, TPSM) obtained by intravenous administration of 99mTc-DTPA(0.5mCi, 18.5 MBq). eGFR was derived using CKD-EPI formula based on serum creatinine concentration. Results GFR average measured using 99mTc-DTPA for 200 kidney transplant donors is 97.27±19.46(ml/min/1.73m2), and the eGFR average value using the CKD-EPI formula is 96.84±17.74(ml/min/1.73m2), The concentration of serum creatinine is 0.84±0.39(mg/dL). Regression formula of 99mTc-DTPA GFR for serum creatinine-based eGFR was Y = 0.5073X + 48.186, and the correlation coefficient was 0.698 (P<0.01). Difference (%) was 1.52±18.28. Conclusion The correlation coefficient between the 99mTc-DTPA and the eGFR derived on serum creatinine concentration was confirmed to be moderate. This is estimated that eGFR is affected by external factors such as age, gender, and muscle mass and use of formulas made for kidney disease patients. By using 99mTc-DTPA, we can provide reliable GFR results, which is used for diagnosis, treatment and observation of kidney disease, and kidney evaluation of kidney transplant patients.

Influence of Dopamine on Intrarenal Blood Flow in Dog (개의 신내 혈류에 미치는 Dopamine의 영향)

  • In order to certify the diuretic mechanism of dopamine, this study was performed in dog. The following results were obtained. Dopamine, when given intravenously, produced diuresis, and increased glomerular filtration rate (GFR), renal plasma flow (RPF), and amount of sodium excreted in urine. When infused directly into a renal artery, dopamine elicited a marked diuresis confined only to the infused side, with concomitant rises in osmolar clearance and sodium excretion as well as a slight increase in free water clearance. Simultaneously total renal plasma flow and medullary plasma flow increased markedly with a increase of glomerular filtration rate and renal plasma flow. Medullary concentration gradient of sodium also markedly lowered in the infused kidney. These changes were not observed during mannitol diuresis and renal action of dopamine were not apparent in dog pretreated with haloperidol. From the above experimental results, it is thought that dopamine, when given into a vien or infused directly into a renal artery, induces diuresis, and the mechanism of its action is due to dual actions which are hemodynamic effect along with glomerular filtraction rate, and the increased response in the medullary blood flow.

Estimation of Glomerular Filtration Rate using Chromium-51 EDTA (Cr-51 EDTA GFR 검사 결과의 분석 및 의의)

  • Purpose: Correct estimation of Glomerular filtration rate (GFR) is very important for an accurate clinical assessment of the kidney function. This study compares four GFR markers, a serum creatinine-based estimation using MDRD formula, Cystatin-C, Cr-51 EDTA 2 samples and 6 samples. Materials and Methods: Serum creatinine concentrations, Cystatin-C serum concentrations and Cr-51 EDTA clearance are measured in 43 patients who received or donated kidney. Results: The correlation coefficient between serum based estimated GFR (MDRD) and Cr-51 EDTA 6 samples was 0.817 (p<0.01). The correlation coefficient between Cystatin-C based GFR and EDTA 6 samples was 0.7322 (p<0.01). Regression analysis showed a statistically significant correlation between Cr-51 EDTA 2 samples and 6 samples (r=0.971, p<0.01). Mean value and ${\pm}2SD$ for the difference between Cr-51 EDTA 2 samples and 6 samples were 4.7 mL/min and ${\pm}9.3$ respectively. Conclusions: The estimation of two samples Cr-51 EDTA showed that the method can be simplified by reducing blood samples without losing its high accuracy.

Pharmacological Studies of Plantaginis Semen (차전자(車前子)(질경이)의 약리학적(藥理學的) 연구(硏究))

  • The pharmacological actions to methanol extract(PME) obtained from Plantaginis semen were examined in the rabbit. 1) PME, when administered into the vein of rabbit, produced the fall of blood pressure and stimulation of respiration. The former action was inhibited by atropine but the latter not affected by atropine. 2) PME caused contraction in both isolated intestinal and uterus strips, atropine blocked the contraction of intestinal strips while did not the uterus contraction. 3) PME decreased the heart rate of rabbit anesthetized with urethane. 4) PME elicited antidiuresis with doses ranging from 10mg/kg. The antidiuresis appeared to be related to the hemodynamic changes decreases in the renal plasma flow and glomerular filtration rate. Urinary sodium and potassium decrease in the renal plasma flow and glomerular filtration rate. Urinary sodium and potassium decrease in relation to the diminished filtration.
