A Case of Gouty Arthritis Patient Treated with Collaborative Oriental and Western Medicine with Acute Inflammation and Liver Injury

  • Objectives : To examine the effects of the collaborative Oriental and Western medicine, we treated a gouty arthritis patient with acute inflammation and liver injury with a combination of Oriental and Western treatments. Methods : Acupuncture, Bangphungtongsung-San(Fangfengtongsheng-san)', 'Kangwhaljetong-Um(Qianghuochutong-yin)', and 'Sosiho-Tang(Xiaochaihu-tang) were offered to an acute gouty arthritis patient with NSAIDs, Corticosteroids and allopurinol. Laboratory data were observed for the duration of hospital days. Results : In spite of Oriental treatments, NSAIDs administration caused liver injury, but continuous Oriental treatments with small amount of Corticosteroids and allopurinol brought recovery of liver function and gouty arthritis. Confusion : Collaborative treatments of Oriental and Western medicine are better than independent Western treatment for gouty arthritis with acute inflammation and liver injury. Further studies will be required to ascertain the collaborative treatment with Oriental and Western medicine for gouty arthritis and other diseases.

  • Recently arthroscopic procedure was introduced into an effective method when chronic gouty arthritis associated with acute inflammation is unresponsive to conservative treatment. The purpose of this study is to cvaluate the efficacy of tophi excision during this procedure. We tried to excise tophi which were known as one of causative materials of acute inflammation as much as possible. We report one case of chronic tophaceous gouty arthritis of the right knee which was satisfactorily treated without recurrence during more than one year after this arthroscopic procedure.

Suppressive effect of Spirulina fusiformis in relation to lysosomal acid hydrolases, lipid peroxidation, antioxidant status, and inflammatory mediator TNF-alpha on experimental gouty arthritis in mice

  • The anti-inflammatory effect of Spirulina fusiformis on monosodium urate crystal-induced inflammation in mice has been investigated and compared with the non-steroidal anti-inflammatory drug Indomethacin. The paw volume, lysosomal enzyme activities, lipid peroxidation, anti-oxidant status and inflammatory mediator tumour necrosis factor-$\alpha$ were studied in control and monosodium urate crystal-induced mice after oral administration of Spirulina platensis in an experimental model for gouty arthritis. In the induced mice, the levels of lysosomal enzymes, inflammatory mediator tumour necrosis factor-$\alpha$, lipid peroxidation and the paw volume increased significantly, whereas the antioxidant status decreased when compared to control mice. $\beta$-glucuronidase and lactate dehydrogenase level were also found to be increased in untreated monosodium urate crystal-incubated polymorphonuclear leucocytes. After the oral administration of Spirulina fusiformis, the physical and biochemical changes observed in monosodium urate crystal-induced animals were significantly restored to near normal levels. The results clearly indicated the anti-inflammatory role of Spirulina fusiformis, a promising drug for gouty arthritis.

  • Colchicine is widely used in the treatment of gouty arthritis, and chronic use of colchicine can cause vacuolar myopathy. We report colchicine-induced myopathy with electrophysiological myotonia in a patient with gouty arthritis. A 86-year-old man with gouty arthritis presented with progressive proximal weakness and gait disturbance. Electrophysiological finding showed myotonic myopathy and muscle biopsy revealed a vacuolar myopathy. His symptoms soon resolved with the discontinuation of colchicine.

  • Osteochondroma, which is an osteocartilaginous exostosis, is essentially the most common primary bone tumor. These benign neoplasms are generally asymptomatic and have a relatively small potential for adverse effects. Calcaneal osteochondroma is rare, furthermore osteochondroma accompanying with gouty arthritis is very rare. Also, the subtalar joint is not a classic site of acute gout. In this report, we report the case of a patient who experienced an unusual calcaneal osteochondroma with undiagnosed gouty arthritis of the subtalar joint.

  • Gouty arthritis is a common crystal arthropathy, but gout tophus in the bipartite patella is a rare condition. This report presented a traumatic separation of bipartite patellar fragment caused by mild trauma in a patient with comorbid gout. When a patient with bipartite patella and underlying gouty arthritis complains of pain after trauma, clinical suspicion is needed about fragment separation of the bipartite patella.

  • Objectives The aim of this study is to find the future perspective through analyzing the trends of acupuncture treatment for gouty arthritis (GA) by searching the China National Knowledge Infrastructure (CNKI) and PubMed. Methods We searched clinical studies about acupuncture treatment for GA through the searching the electronic database of CNKI and PubMed. We analyzed the characteristics of selected studies according to research design, sample size, publication year, treatment methods, evaluation criteria and adverse events. Results Twenty-nine studies published from January 1, 2015 to July 1, 2022 were selected. They consisted of 25 randomized controlled trials, 3 non-randomized controlled trials, and 1 case reports. In these studies, several types of acupuncture treatments were performed and the most treatment was conventional acupuncture treatment with fine needles. The most commonly used evaluation outcome was effective rate. Conclusions Through this result, the Chinese literature positively reported the effect of acupuncture on GA. However, high-quality evaluation by rigorous research using well-designed and more objective outcomes is required.

  • Colchicine is a drug used for the treatment of acute gouty arthritis or various autoimmune diseases. Gastrointestinal adverse effects such as abdominal pain and vomiting are the common side effects of the drug, but rarely myopathy has been reported to occur particularly in renal recipients who were treated concomitantly with immunosuppressants. Herein, we report a case who presented with acute myopathy after treated with colchicine for acute gouty arthritis.

  • Gout occurs mainly in monoarthritis and is found in more than 50% of cases in hallux of the foot. In addition, symptoms sometimes begin in the hand, wrist, and elbow, but they are rarely observed in the spine. The patient was referred for tuberculous polyarthritis due to antituberculosis drug failure. Inflammatory findings were observed in the lumbar, elbow, wrist, hand and foot areas. Surgery was performed on the foot area and a pathology diagnosis revealed gouty arthritis. We report this case with a review of the relevant literature.

  • Colchicine is a drug well known for its use in the therapy of acute attacks of gouty arthritis. Two death caused by oral ingestion of colchicine was reported. Two persons were all women. The one was 34 years old woman and with his boyfriend in a inn and the other was a housewife. The first one was a alcoholic but his boyfriend was afflicted with gout. So her boy friend carried over about 60 tablets of colchicine, 4 tablets of naltrexone and 22 tablets of fluoxetine. After her death there were remained colchicine, 2 tabs, naltraxone 4 tabs and fluoxetine 22 tabs. (omitted)
