Objectives : The purpose of this study is to find out the relation between the height of intercrestal line, and HIVD(Herniated Intervertebral Disc)of Lumbar spine. Methods : We investigated 445 cases (208 male, 237 female) of patients who were diagnosed as HIVD of L-spine at either L4/5 or L5/S1 level. We analysed the relation between the height of intercrestal line and the level of HIVD. Results : 1. Among 445 cases, the level of intercrestal line with HIVD of L-spine at the specific location was different between male and female. In male, the intercrestal line is more likely to be located at L4 level, while in female more likely to be located at L5 level. 2. Among 445 cases, L5/S1 HIVD patients(272, 61.1%) were more than L4/5 HIVD patients(173, 38.9%). At L4 body area, L4/5 HIVD patients were more than L5/S1 HIVD patients, however, at L4/5 intervertebral area and L5 body area, L5/S1 HIVD patients were more than L4/5 HIVD patients. (p<0.01) 3. When the line is located at higher level, HIVD of L-spine tends to be occurred at L4/5 level. Contrastly, when the line is located at lower level, HIVD of L-spine tends to be occurred at L5/S1 level. (p<0.01) Conclusions : High intercrestal line leads to L4/5 HIVD, while low intercrestal line leads to L5/S1 HIVD. Possibly, it is caused by different length and thickness of the iliolumbar ligament.

Objectives : This research was to evaluate the MST effectiveness on iliopsoas muscle of herniated inter-vertebral disc in lumbar spine. Methods : This research was carried out with the 58 inpatients who received treatment for their HIVD in lumbar spine from January 1 to April 12, 2014 in Daejeon Jaseng hospital of oriental medicine. We randomly divided into two groups: Group A=common treatment on HIVD of L-spine without MST(acupuncture, pharmacopuncture, herb medication, chuna treating and physiotherapy) and Group B=common treatment on HIVD of L-spine with MST. To verify the MST effect, tenderness(checked by algometer pressure), NRS(numeric rating scale), and ODI(Oswestry disability index) were measured before and after treatment. The statistically significance was evaluated by SPSS 18.0 for Windows. Results : In the Group B, the tenderness was significantly decreased compared with Group A. However, on the NRS and ODI, it did not produce the meaningful results as these two values decreased in all groups. Conclusions : From this research, when patients who were diagnosed HIVD of L-spine with iliopsoas muscles tenderness receive MST additionally, the satisfaction of patients as well as physician confidence will increase.

Objective : This study was designed to analyze the of correlation between narrowing disc space of lumbar X-ray and HIVD of L-spine CT in lumbosacral strain patients. Methods : 63 cases of lumbosacral strain patients who visited Bul-kyooh Oriental Hospital from June 13, 2000 to August 9, 2001 were selected. We performed the radiography by lumbar X-ray and L-spine CT for all cases. Results : 28 of 63 cases revealed narrowing disc space in Lumbar X-ray and these 28 cases were diagnosed as HIVD in L-spine CT at all. 35 of 63 cases revealed normal in Lumbar X-ray and 13 of these 35 cases were diagnosed as HIVD in L-spine CT. Conclusions : We found that narrowing disc space in lumbar X-ray has significant relevance to HIVD in L-spine CT(P-value = 0.001). For accurate diagnosis and treatment of lumbosacral strain patients. combination of clinical symptoms, physical examination and radiography of X-ray and HIVD is needed.

Objectives : This study reported 3 cases of L-spine HIVD which was treated with spine decompression, as a new medical treatment alleviates lumbago by non-surgical treatment, and oriental medicine treatment. Methods : Each patient has been treated with spine decompression and oriental medicine treatment, and the degree of improvement has been evaluated by VAS and Rating scale for LBP. Results : Through the result, the medical treatment proved to have valid effect for L-spine HIVD, but patients did not become healthy enough restored to their job before. Conclusions : At the result of this, every case can be certified to give appropriate effect, and there needs more research into synergy between spine decompression and oriental medicine treatment.

Objectives: The propose of this study is to find out the clinical application of conservative treatment to 3 patients who has Disc Extrusion on L-spine MRI Methods: We examined 3 patients with Lumbar Intervertebral Disc Herniation (HIVD of L-spine) with Disc Extrusion who showed changes on MRI images before/after the treatment among HIVD of L-spine patients who visited Jaseng Hospital of Oriental Medicine. Results: In this study, the first MRI examination of HIVD of L-spine patients was performed at the first visit and re-examination of MRI was done after the treatment. In each case, the size of the extruded disc was considerably reduced in MRI image. Low back and leg pain was also reduced significantly after conervative oriental medical treatment. Conclusions: Conservative oriental medical treatment can be effective for improving symptoms of HIVD, decreasing pain, also decreasing the volume of herniated disc.

Objectives : The purpose of this study is to report the effect of acupotomy for patients with Lumbar intervertebral disc Herniation. Methods : We treated Four patients who have HIVD of L-spine with acupotomy. Visual analog scores(VAS) and Oswestry Low-back pain Disabiliby Index(ODI) were compared before-treatment with after-tretment. When the patients is discharged, the satisfaction of acupotomy were graded by Five-point Likert scale. Results : VAS and ODI were decreased at all case. Likert scale point were cheked as grade 4 at all case. Conclusions : This study shows acupotomy has useful effect on HIVD of L-spine.

Objective : To report cases of HIVD of C&L-Spine successfully treated with conservative Korean Traditional Medicine treatment with Megadose Pharmacoacupuncture. Methods : Four patients diagnosed with HIVD of C&L-Spine were treated with Megadose Pharmacopuncture, acupuncture and herbal medicine. Numeric Rating Scale(NRS), Neck Disablility Index(NDI) and Oswestry low back pain Disablility Index(ODI) scores were collected before and after treatment for comparison. Results : There was a significant decrease in NRS, NDI and ODI scores. The mean NRS score decresed from 9 to 2.25, NDI from 45 to 14 and ODI from 46.66 to 11.33. Conclusion : The result suggests Megadose Pharmacoacupuncture can be an effective treatment on patients with HIVD of C&L-Spine.

Objectives : To report cases of HIVD of L-Spine successfully treated with Korean medical treatment including Samul-tang(Siwu-tang) Gagam. and Chuna manual treatment. Methods : Three patients diagnosed with HIVD of L-Spine were treated with Korean medical treatment including Samul-tang(Siwu-tang) Gagam and Chuna manual treatment. Numeric Rating Scale(NRS), Oswestry low back pain Disablility Index(ODI) scores and EQ-5D were collected before and after treatment for comparison. Results : There was a significant decrease in NRS, ODI scores.And, there was a significant increase EQ-5D. The mean NRS score decresed from 10 to 1.33, ODI from 66.66 to 8.88. Tne mean EQ-5D score incresed from 0.225 to 0.856. Conclusion : The result suggests Korean medical treatment including Samul-tang(Siwu-tang) Gagam and Chuna manual treatment can be an effective treatment on patients with HIVD of L-Spine.

Objectives: This study was designed to correlation between the axial section area in psoas major muscles and uncleus discharge pat of HIVD(herniated intervertebral disc) of lumbar(L)-spine. Methods: We measured the axial section area in psoas muscles. Then through the medical charts, we investigated nucleus discharge pat of HIVD of L-spine. We analyzed the relationship between them. Results: There was no significant correlation nucleus discharge part and psoas major muscle's axial section area. There was significant correlation aging and psoas major muscle's axial section area with control the sex. Conclusions: According to above results, there was no significant the area of axial section in psoas major muscles and nucleus discharge part of HIVD of L-spine.

Objective: The purpose of this study is to report chronic sciatica paitient with HIVD of L-spine who improveded by orthotics Methods: once we treated the patient with acupuncture, nagatives, chu-na after that added with orthotics, We compared each effect. The effect was evaluated by progress of symtoms and Visual Anlogue Scale Results & Conclusion :After treatment of orthotics sciatica was reduced in the score of VAS from 7to3.
