The Study on Pattern Differentiations of Primary Headache in Korean Medicine according to the International Classification of Headache Disorders (ICHD 분류에 따른 원발 두통의 한의학적 변증 연구)

  • This study draws pattern differentiations of headache disorders on the ground of modern clinical applications and Korean medical literature. Categorization and symptoms of headache disorders are based on International Classification of Headache Disorders 3rd edition(beta version). And clinical papers are searched in China Academic Journals(CAJ) of China National Knowledge Infrastructure(CNKI). In the aspect of eight principle pattern identification, primary headache occurs due to lots of yang qi and has more inner pattern rather than exterior pattern, heat pattern rather than cold pattern, excess pattern rather than deficiency pattern. And primary headache is related with liver in the aspect of visceral pattern identification and blood stasis, wind and phlegm are relevant mechanisms. Migraine without aura is associated with ascendant hyperactivity of liver yang, phlegm turbidity, sunken spleen qi, wind-heat, blood deficiency or yin deficiency. Migraine with aura is mainly related with wind and it's major mechanisms are ascendant hyperactivity of liver yang, liver fire, yin deficiency of liver and kidney, blood deficiency or liver depression and qi stagnation. High repetition rate of tension-type headache can be identified as heat pattern or excess pattern. And trigeminal autonomic cephalalgias can also be accepted as heat pattern or excess pattern when the occurrence frequency is high and is relevant to combined pattern with excess pattern of external contraction and deficiency pattern of internal damage based on facial symptoms by external contraction and nervous and anxious status by liver deficiency. This study can be expected to be Korean medical basis of clinical practice guidelines on headache by proposing pattern identifications corresponding to the western classifications of headache disorders.

  • Tension headache is one of the most common form of headache. It may occur at any age, but is most common in adult and adolescents. One cause of this muscle contraction is a response to stress that induces changes and imbalance on autonomic nerve system. To cure tension headache, getting rid of psychological stress and anxiety have to do first. In this case, we described a 42 years old woman who diagnosed as tension headache. She complained headache with scruff pain, dyspepsia, nausea and coldness of toes. through oriental medical treatment(for example, acupuncture, herbal medicine ect.), her condition was improved.

Case Report on the Effectiveness of Chengsimyeonjatang-gamibang, focusing on nine patients with headache (청심연자탕가미방의 효과에 대한 증례군 보고 - 9명의 두통 환자를 중심으로 -)

  • Objectives : The purpose of this case series was to investigate the effectiveness of Chengsimyeonjatang-gamibang for headache. Methods : The case subjects were nine patients who were diagnosed as Taeumin from the Department of Sasang Constitutional Medicine. They were selected among outpatients who visited Sangji Oriental Medical Hospital with complaint of headache from October 2007 to January 2008 through chart reviews. They were treated with Chengsimyeonjatang-gamibang without other treatments. Relative seriousness of headache was determined by Headache Impact $Test^{TM}$(HIT-$6^{TM}$) questionnaire. Results of before and after therapy were evaluated and scored using Visual Analogue Scale (VAS). The results were analyzed using statistical methods. such as Friedman test, Wilcoxon signed-rank test, and Mann-Whitney U Test. Results : The study showed that the VAS score significantly decreased after treatment (P<0.05). Conclusions : In summary, this case report suggests that Chengsimyeonjatang-gamibang appears to be effective in the treatment of headache.

  • Bacteremia is the transient presence of bacteria (or other microorganisms) in the blood. Bacteremia may result no symptom to the healthy persons, but it may result symptoms such as pylexia, headache and even septic shock to the patient in immunosuppression. In this case, a 75 years old male patient had a dental caries and cerebrovascular accident ago, therefore was in a bad condition and often caught cough. Suddenly be bad a headache with a high fever caused by Streptococcus Mutans Bateremia. After defervescence, a slight fever and a headache remained. It may the symptoms of Yolgwol headache(熱厥頭痛) , so Cbeougsaugsabwa-taug(淸上瀉火湯), the symptoms such as a slight fever and a headache are on the mend.

  • A clinical analysis was carried out for 37 patients who was treated for functional headache with HWANGRYUNHAEDOKTANG(黃連解毒湯) herb-acupuncture in Seoul Red-Cross Oriental Hospital. The results were as follows; 1. The effect of HWANGRYUNHAEDOKTANG(黃連解毒湯) herb-acupuncture for functional headache was turned out to be better effective the young and middle age than the old. 2. Man was better effective than woman 3. There were effective at tention headache and migraine. 4. There were effective within 6 monthes from onset. 5. There were availability in according with increasing treatment. Through the clinical consequences of therapy mentioned above, we came to confirm regular therapeutic effects for functional headache and hereafter, it is considered that a clinical comparative study on oriental cure and herb-acupuncture therapy for headache has to be more progressed than now.

  • Headache is symptom of many people suffered. It caused simply by fatigue, stress, but it also caused by inflammation, mechanical or chemical stimulus, circulation disorder, brain damage. Modern societies develop rapidly by traffic system. But lately it suffering with many traffic problem. Traffic accident is most important of many traffic problem. We experienced a 35 year-old female who complained headache, caused by a traffic accident that happened 22 month ago. We treated the patient by the way of Eo-hyeol (瘀血) type of headache with Herbal medication and Acupuncture. After being treated, the patient show that symptoms(headache, insomnia, anorexia) was improved considerably. This result suggests that oriental medicine treatment has good effect on headache due to traffic accident.

  • Objectives: Since we found patients with dyspepsia as their major symptom had high rate of headache, would like to find out a rational diagnosis ground by using EAV correlation between headache and gastrointestinal disorder Methods: We did survey and EAV inspection on 128 patients came by gastrointestinal disorder who quoted 4 and 5 degree for their headache symptoms) Results & Conclusion The results of this study were as follows: 1. From dyspepsia, we saw higher rate by order of epigastric discomfort, bloating, stomach trouble, bleching, etc. 2. By EAV analysis, found functional deterioration by order of stomach, nerval degeneration vessel, gall bladder/bile duct, liver. 3. By EAV analysis, functional deterioration of stomach, gall bladder/bile duct, liver represent dyspepsia and nerval degeneration vessel represent headache. We found the correlation between dyspepsia and headache by observation at a time.

Assessing the Public's Interest in Orofacial Pain Specialists: A Google Trends Analysis

  • Purpose: To assess Google Trends (GT) search behavior regarding orofacial pain (OFP) and headaches. Methods: GT scores for OFP and headache specialists between February 2013 and December 2022 were analyzed. Statistical tests such as Poisson regression analyses, mean differences, and Cohen's D were used to assess the score change over time. Results: The top three search words for OFP specialists were "temporomandibular joint (TMJ) specialist," "TMJ doctor," and "TMJ dentist," whereas the top three search words for headache specialists were "Headache specialist," "Headache doctor," and "Migraine specialist." Here, TMJ is temporomandibular joint. The GT scores for OFP specialists increased significantly (p<0.05) for all years except 2017, with the highest mean difference in 2020. The scores for headache specialists showed similar trends but gradually. Conclusions: The interest in OFP and headache specialists expressed by Google searches has increased over the years. More awareness is needed regarding the OFP scope of practice, and the use of GT may serve as an indicator.

  • Cognitive-behavioral approach to two cases with chronic headache was presented. Cognitive-behavioral interventions focus on indirectly altering symptom-related physiological activity by changing the way patients cope with headache-eliciting stressors. This treatment focuses directly on the patients' cognitive and behavioral changes. Cognitive-behavioral treatment can be divided into three phases Education, self-monitoring, and problem-solving or coping-skills training. Literature reviews on the follow-up evaluation of therapeutic effectiveness revealed that cognitive-behavioral treatment is effective in the management of chronic headache.
