Determinants of Health Service Utilization of Urban Health Center (도시 보건소 보건의료서비스 이용의 결정요인)

  • This study was carried out to assess the utilization of urban health center and its related factors among the urban residents. The survey was carried out for 2,394 households in Taegu and Kyongju by the structured questionnaire from 28 March to 4 April and from 2 July to 9 July, 1994. Well trained interviewers visited 2,630 households in Taegu and Kyongju, and interviewed with housewives. Of the target households, 91.0%(2,394 households) were responded through three-time visiting. The major results were summarized as follows : The fourth and fifth decad utilized the health center more frequently than any other age groups.. The lower income group showed higher rate of health center utilization than those in higher income group in Taegu City. The mean length of residence among residents of Kyongju City is longer than those of Taegu City, and the longer length of residence, the higher rate of the health center utilization. Those who are living together with neonate and infant or elderly people showed higher rate of health center utilization than those who are living without neonate and infant or elderly people in both Taegu and Kyongju. The most common reason for visiting the health center was 'low cost'. The major reasons for not visiting the health center were 'not regular customer', 'poor health center facility', and 'low quality of care'. Vaccination, communicable disease control, outpatient care, public hygiene, maternal and child health program were well recognized as health center activities. In logistic regression for the utilization of health center, the significant independent variables were length of residence and recognize the site of health center in both Taegu and Kyongju. The improvement of quality of health service, physical environment of health center and public relations on health center's activities shoulod be considered for reactivation and reingorcement of health center functions.

The evaluation of the appropriateness of resource allocation in a community health center (보건소 기능의 중요도에 따른 자원배분의 적절성 평가)

  • This study was performed to evaluate the appropriateness of resource allocation based on the ranking of health center function. Through the Delphi processes, health center functions were ranked in order of importance as follows; planning and research, followed by health education, health promotion, management of chronic diseases, health screening tests, welfare activities, mental health services, medical personnel management, medical services, prevention of communicable diseases, maternal and infant health services, housekeeping, management of oral hygiene, nutrition services, surveillance for community health services, family planning, and administration of the health center. In relation to the above priorities, the allocation of manpower was not appropriate. Even though the expert groups emphasized on functions such as planning and research, health education, and health promotion, they inputted more personnel for administration of a health center, maternal and infant health services, and medical services which were evaluated with lower importance. The budget allocation showed the same trends as the above. Although the functions such as planning and research, health education, and health promotion, and management of chronic diseases were evaluated highly, the budget was allocated accordingo to the the results of the former fiscal year rather than on the importance of function. However the budget for nutrition services, surveillance for the community health services, family planning, and administration of a health center was allocated according to priority. Based upon the above findings, community health center should be given the opportunity to make their own ranking of health center function and to allocate their resources including personnel and budget in order to improve the responsibilities and roles of the community health center.

Opinion Survey of Health Center Officers on Rural Health Service Improvement Project (농어촌의료(農漁村醫療)서비스 개선사업(改善事業)에 대한 보건소(保健所) 공무원(公務員)의 인식도(認識度))

  • This study was performed investigate the opinion of civil servants in Health center about Rural Health Service Improvement Project The survey by mail was carried out for 447 servants of 25 health centers in Kyungsangbuk-do and the data were collected through self-administered questionnaires to servants about need, participation, concern, and comprehension for the project and satisfied with current facility and equipment of health center. The results were as follows. Generally considered, 48.2% of the improved health center servants was satisfied with health center building and 14.0% or 24.1% of the improving or unimproved center was. About the location of health center, 37.7% of the improved health center servants was satisfied, 25.9% of the unimproved center was. Of the improved health center servants, 43% was satisfied with the medical equipment but in unimproved place, the dissatisfaction was appeared higher than any other place. 49.7% of respondents was participated in making out the Rural Health Service Improvement Project. 50.6% was interested in this project. In the improved area. 65.5% of health center servants replied that the mayor's or county executive's concern about this project was high and 46.5% in councilors but in the unimproved area. their concern was low. About the contents of the project. 24,6% of the servants in the improved center, only 15.2% in unimproved center replied that they had known well. After making out the plan, 13.6% of respondents was unsatisfied with this plan and 17.1% replied that the estimating method of selecting the project area was not good. After the improvement of institution and equipment, 86.1% of health center servants answered that the medical service provided by health center would increase but 59.2% replied that the residents' utilization rate of private medical facility would decrease. The servants of the improved health center replied that the recognition about the developing will of health service(91.2%), the efficiency(91.2%), the quality of health and medical service(93.0%), the amount of health project(91.2%) were improved. In health center which had already improved the institution and equipment, 88.5% of servants replied that the residents' utilization for health center was increased. So, this project should be continuously carried out for health center and health center must develope new project to fit region condition.

  • Currently, there is no facility standard for a city public health center. The facility standard of rural public health center is referred to architectural plan of city public health center. This study is about architectural plan of the city public health center and have been in 7 public health centers in Daegu investigated. Conclusions can be summarized as follows: 1) The Mother and child hygienic section, area ratio which is presented by the facility standard of rural public health center must be increased in case of city public health center since function-reinforcement of the mother and child hygienic section is required. 2) Business section, the area ratio which is presented by the facility standard of rural public health center must be diminished for city public health center because it has been excessively allocated.
