A Clinical Report on 35 Inpatients with Facial Paralysis Effects of Early Oriental Medicine Treatment on House-Brackmann Grade in Bell's Palsy (벨마비 입원환자 35례에 대한 임상보고 조기 한방치료가 벨마비에서 House-Brackmann grade에 미치는 영향을 중심으로)

  • Objective : This study was designed to evaluate the influence of time from onset of Bell's palsy until commenced Oriental medicine therapy on Bell's palsy. Methods : We investigated 35 cases of patients with peripheral facial palsy who visited Wonkwang University Oriental Hospital from 10th, December 2010 to 31st August, 2011 and classified them as time from onset of Bell's palsy until commenced Oriental medicine therapy. Time from onset of Bell's palsy until commenced Oriental medicine therapy of Group A was within 3 days. Group B was from 4 days to 8 days. We evaluate the treatment effect of each group by House-Brackmann grade before treatment and 1,3 week treatment. Results : 1. There was a statistical difference in period from onset of Bell's palsy until commenced Oriental medicine therapy between Group A and Group B. 2. There was a statistical difference after 1 week treatment in Group B. 3. There was a statistical difference after 3 weeks treatment in both Group A and B.3. After 3 week treatment, there was statistical significant difference in decreased value of House-Brackmann grade between Group A and Group B. Group A showed more recovery compared with Group B. Conclusions : 1. These results suggest that early oriental medicine treatment could be more effective in recovery of Bell's palsy. 2. Further studies will be required to identify the beneficial effect of early oriental medicine treatment in bell's palsy.

Comparison of Efficacy between Acupuncture Treatment and Collaborative Treatment with Prednisolone on Acute Bell's Palsy (급성 Bell's Palsy의 침 치료와 Prednisolone을 병행한 협진 치료의 효과 비교)

  • Objectives The purpose of this study was to compare the effects of acupuncture treatment with those of collaborative treatment regarding acute Bell's palsy. Methods Twenty-six patients who received outpatient treatment between March 2012 and February 2013 were divided into 2 groups. The East-West treatment group (EW group, n=12) received prednisolone and then acupuncture afterwards. The Eastern treatment group (E group, n=14) received just acupuncture. Acupuncture was administered 3 to 4 times a week, and both groups were educated to practice facial muscle exercises at home. Evaluations were made before treatment, after 1 week, 2 weeks, and 3 weeks using the House-Brackmann facial nerve grading system. Results Compared to before treatment, the House-Brackmann grades of both EW and E groups after 1, 2 and 3 weeks of treatment significantly decreased (p<0.05). Regarding group comparison, the House-Brackmann grade of the EW group was significantly lower than the E group at 1 week (p=0.043), but there was no significant difference at 2 and 3 weeks. Regarding improvement of House-Brackmann grades, there was no significant difference between the two groups at 1, 2 and 3 weeks (p>0.05). Conclusions Compared to just acupuncture treatment, collaborative treatment with prednisolone significantly improved the House-Brackmann grade after 1 week, but there was no difference as time passed. Because early intervention determines the prognosis of Bell's palsy, collaborative treatment at the early stage will be clinically helpful to patients.

  • Background and Objective : The facial nerve grading system proposed by House and Brackmann is most widely accepted for the clinical assessment of facial nerve injury. Because of the limitations and subjectivity of the House-Brackmann scale, several new scales of varying degrees of objectivity and ease of use have been introduced. To assess methods of evaluating the function of the facial nerve that have been introduced over the past 20 years, We compared with the House-Brackmann scale. Method : We referred to the information through Entrez Pubmed and Korean studies information(KSI) from 1985 to 2006 about methods of evaluating facial nerve function. We choose 7 scales that focused on objective and easy of use. Result and conclusion : Sunnybrook scale is a weighted, subjective scale with incorporation of secondary defects into a single composite score. Sunnybrook scale can be recommended over House-Brackmann scale.

  • Objectives The purpose of this study is to report a pediatric patient with bilateral facial nerve palsy. Methods The patient was treated by acupuncture, moxa, herb(Cheongungkyeoji-tang, Palmulgunja-tang) for 1 month. We described his progression and measured the degree by House-Brackmann scale and Yanagihara's scale. Results He started getting better from 7th day of the onset. During admission, the facial palsy had improved from Grade V/V(Rt/Lt) to Grade II/II(Rt/Lt) by House-Brackmann scale and from 0/0(Rt/Lt)score to 36/36(Rt/Lt)score by Yanagihara's scale. Conclusions Compared with unilateral facial nerve palsy cases, this case was not different from the time when it got better, and how much it improved. However, biased degree had increased and then decreased because of the time gap between the right facial improvements and left facial one.

  • Objectives : This study was designed to evaluate the influence of the period of disease on Bell's palsy patients Methods : We investigated 40 cases of patients with peripheral facial palsy who visited Yumin Oriental Hospital from 1st January to 31st August, 2008 with both oriental and western medical therapy and classified them as period of disease. The period of disease of Group A was within 3 days. The period of disease of Group B was from 4 to 10days. We evaluated the treatment effect of each group by House-Brackmann grade before treatment and 1, 2 week treatment. Results: 1. There was a statistical significant difference in period of disease between Group A and Group B. 2. After 1 week treatment, Group A showed more recovery compared with Group B. 3. After 2 week treatment, there was no statistical significant difference in House-Brackmann grade between Group A and Group B Conclusions : These results suggest that early treatment could be more effective in recovery of Bell's palsy.

  • Objectives : This study was designed to evaluate the effects of oriental medical treatments on the facial nerve palsy in Herpes Zoster Oticus. Methods : Subjects were 1 patients with the facial nerve palsy in Herpes Zoster Oticus. She had improved slightly still she had be gotten the medical services on inpatient states for 2 month. We gave Kamiligigepung-san to the patient 90 minutes after each meal and tried acupuncture 2 times every day. And we had evaluated the conditions of the patient using VAS, House-Brackmann Grade and Yanagihara scale. Results : After the comparison between an admission and discharge date at VAS, House-Brackmann Grade and Yanagihara scale, the facial nerve palsy of the patient had improved. Conclusions : It is effective the Kamiligigepung-san and Acupuncture on the facial nerve palsy in Herpes Zoster Oticus.

  • This study was conducted to investigate whether treatment with Korean medicine might help to improve symptoms of patients diagnosed with Ramsay Hunt Syndrome. A patient diagnosed with Ramsay Hunt Syndrome was hospitalized for 15 days from October 4, 2018 to October 18, 2018 and treated using Korean medicine (Sinbaro3 pharmacopuncture, acupuncture, electroacupuncture, and herbal medicine). Therapeutic effects were evaluated using the Numerical Rating Scale (NRS), House-Brackmann grade, Yanagihara's score survey, and Digital Infrared Thermographic Imaging (DITI). After treatment, the Numerical Rating Scale score (6 at admission down to 2 points at discharge), House-Brackmann grade (Grade V down to Grade II), Yanagihara's score (6.0 up to 22.0), and Digital Infrared Thermographic Imaging showed improvement from admission to discharge. Treatment with traditional Korean medicine, may help improve symptoms of Ramsay Hunt Syndrome such as facial nerve paralysis, neuralgia after paraplegia, hearing difficulties, and dizziness.

  • Objectives : This study investigated the effect of imbalance of autonomic nerve system on the idiopathic facial palsy by the comparison Heart Rate Variability results of Facial palsy group and healthy control group, and to clarify correlation between House-Brackmann Grade and Heart Rate Variability results. Methods : 119 idiopathic facial palsy patients and 88 health subjects who underwent HRV test were retrospectively reviewed based on medical records. We compared between the HRV results of facial palsy group and that of normal control group, and also compared the HRV results of facial palsy subgroup classified by House-Brackmann Grade. Results 1. All HRV results-Mean Heart Rat(MHRT), Standard Deviation of all the Normal RR intervals(SDNN), Total Power(TP), Very Low Frequency(VLF), Low Frequency(LF), High Frequency(HF), ratio betwween the Low Frequency and High Frequency power(LF/HF ratio) of facial palsy group are decreased compared to that of normal control group, especially SDNN, TP, VLF, LF, LF/HF ratio showed significant difference(p<0.05). 2. HRV results showed no significant correlation in House-Brackmann Grade. Conclusions : This study showed that lower HRV results of facial palsy group than normal control group and suggests that imbalance of autonomic nerve system related with facial palsy. HRV could be a objective tool to reflect condition of idiopathic facial palsy patients.

25 Cases of Facial Paralysis Inpatients by Korean Medical Treatment with Hwangryunhaedok-tang Pharmacopuncture (구안와사 입원환자 25명에 대한 황련해독탕 약침을 병행한 한방치료 증례 보고)

  • Objectives : The purpose of this report is to show the effectiveness of traditional korean medical treatment with Hwangryunhaedok-tang pharmacopuncture. Methods : Twenty five patients were treated by korean medical treatment including Hwangryunhaedok-tang pharmacopuncture. We measured valuation standards such as House-Brackmann Grade(H-B Grade) and Numeric Rating Scale(NRS) to assess the effectiveness of treatments. These valuation standards were measured at admisson and discharge. Results : The results were obtained as below. 1. In House-Brackmann Grade, 60.0% of patients were improved and 36.0% were maintained at the same level. 4.0% of patients were worsen. 2. Average of Numeric Rating Scale was changed from 8.5 to 3.0 after whole treatment. Conclusions : Korean medical treatments including Hwangryunhaedok-tang pharmacopunture can be effective for improving symptoms of facial paralysis.

Five Cases Study on Treating Sequelae of Peripheral Facial palsy Using Jung-an Acupuncture (정안침요법 복합치료를 이용한 말초성 안면마비 후유증 치료 5례)

  • Objectives : The purpose of this study is to report the effect of Jung-an acupuncture on the sequelae of peripheral facial palsy.Methods : Five patients suffering from sequelae of peripheral facial palsy were treated with Jung-an acupuncture. House-Brackmann grade system, the scale of Peitersen grades and picture, before and after the Jung-an acupuncture treatment were compared to evaluate its treatment efficacy.Results : House-Brackmann total grade of all subjects were improvement and Peitersen grades of all subjects were improvement. In two cases, the patients had improvement in synkinesisConclusions : This study showed that Jung-an Acupuncture treatment can be effective in improving functional and psychological outcome even for sequelae of peripheralfacial palsy. Randomized-controlled trial for further evaluation is necessary to verify the results and findings of this study.
