A Systematic Review of Chuna Manual Therapy for Adolescent Idiopathic Scoliosis

  • This study analyzed randomized controlled trials (RCTs) and case studies investigating Chuna manual therapy and variations of this term, for adolescent idiopathic scoliosis. On June $15^{th}$, 2019, 6 online databases were used to retrieve studies. A total of 527 articles were retrieved, and 14 RCTs and 20 case studies were selected for review. Typically, the frequency of Chuna therapy was 1-2 times/week. The most common period of treatment was 12 months in RCTs and 3-6 months in case studies. Cobb's angle was the most frequent evaluation index used (11 RCTs and 20 case studies). In control groups, brace treatment was used in 8 RCTs. In 6 RCTs and 20 case studies, Cobb's angle significantly decreased after Chuna therapy, and in 4 RCTs, Chuna therapy was as effective as brace treatment, with no significant difference between groups. Adverse events were not reported except for minor reactions in only 3 case studies. This review suggested that Chuna therapy for adolescent idiopathic scoliosis was more advantageous than and as effective as brace treatment in most cases, although the risk of bias in 13 RCTs was unclear.

Korean Medical Treatment Including Daoyin Exercise Therapy in Patient with Lower Back Pain Who Suffered from Idiopathic Scoliosis: A Case Report (특발성 척추측만증의 과거력을 가진 요통 환자에서 도인운동요법을 병행한 한방치료의 효과: 증례 보고)

  • This retrospective study reports the effectiveness of Daoyin exercise therapy in a patient with lower back pain who suffered from idiopathic scoliosis. A patient was treated with Korean medicine containing Daoyin exercise therapy for 4 weeks. The patient was assessed for the numeral rating scale (NRS), Cobb's angle, correctability, and coronal balance. After treatment, the NRS of low back pain decreased from 9 to 1. The Cobb's angle of the thoracic curve decreased from 27.31° to 17.66°. The Cobb's angle of the lumbar curve decreased from 21.86° to 9.05°. Correctabililty was 35.34% in the thoracic curve and 58.60% in the lumbar curve. And coronal balance decreased from positive 32.80 mm to negative 3.20 mm. This study suggests that Daoyin exercise therapy could be effective therapeutic choice for lower back pain with idiopathic scoliosis.

Effect of Sling Lmubar Stabilization Exercise Program on the Balance of Adolescent Idiopathic Scoliosis Patients (슬링 요부안정화 운동프로그램이 청소년기 특발성 척추측만증 환자의 균형에 미치는 영향)

  • The purpose of this study lies in examining how sling lumbar stabilization exercise affects static standing balance of adolescent idiopathic scoliosis patients. The subjects were forty patients who were diagnosed with idiopathic scoliosis. They were randomly assigned either to a experimental group (n=20) that received sling lmubar stabilization exercise program or to a control group (n=20) that received only education. Data were obtained for each patient from total sway path and sway velocity in standing on both feet with eyes closed and standing on both feet with eyes open were measured, using balance performance monitor. All variables were measured at pre- and post-exercise. There were significantly differences between the both groups for all variables at post-exercise (p<.05). The experimental group showed a significantly decreases in all variables at pre- and post-exercise (p<.05). But the control group showed a no significantly differences. These results revealed that sling lumbar stabilization exercise improved static standing balance, by reducing COP(center of pressure) sway path and sway velocity of idiopathic scoliosis patients, and it is expected that sling lumbar stabilization exercise will be used as balance enhancement exercise intervention for the treatment of adolescent idiopathic scoliosis patients.

Cardiopulmonary function and scoliosis severity in idiopathic scoliosis children

  • Purpose: Idiopathic scoliosis is a structural lateral curvature of the spine of unknown etiology. The relationship between degree of spine curvature and cardiopulmonary function has not yet been investigated. The purpose of this study was to determine the association between scoliosis and cardiopulmonary characteristics. Methods: Ninety children who underwent preoperative pulmonary or cardiac evaluation at a single spine institution over 41 months were included. They were divided into the thoracic-dominant scoliosis (group A, n=78) and lumbar-dominant scoliosis (group B, n=12) groups. Scoliosis severity was evaluated using the Cobb method. In each group, relationships between Cobb angles and cardiopulmonary markers such as forced vital capacity (FVC), forced expiratory volume in one second ($FEV_1$), $FEV_1$/FVC, left ventricular ejection fraction, pulmonary artery flow velocity, and tissue Doppler velocities (E/E', E'/A') were analyzed by correlation analysis linear regression. Results: In group A, 72 patients (92.3%) underwent pulmonary function tests (PFTs), and 41 (52.6%) underwent echocardiography. In group B, 9 patients (75.0%) underwent PFT and 8 (66.7%) underwent echocardiography. Cobb angles showed a significant negative correlation with FVC and $FEV_1$ in group A (both P<0.05), but no such correlation in group B, and a significant negative correlation with mitral E/A ratio (P<0.05) and tissue Doppler E'/A' (P<0.05) in group A, with a positive correlation with mitral E/A ratio (P<0.05) in group B. Conclusion: Pulmonary and cardiac function was significantly correlated with the degree of scoliosis in patients with thoracic-dominant scoliosis. Myocardial diastolic function might be impaired in patients with the most severe scoliosis.

Effects of Sling Exercise on Spinal Flexibility and Dynamic Balance in Adolescent Scoliosis Patients (청소년기 척추측만증 환자에 대한 슬링운동이 척추유연성과 동적 균형에 미치는 영향)

  • Background: The purpose of this study was compare and analyze the flexibility and dynamic balance in adolescents with idiopathic scoliosis and provide scientific basis for effectively treating idiopathic scoliosis. Methods: The subjects of this study were 20 members of adolescences. To measure the flexibility of the spine, a left buckling instrument was used, and it was measured using a modified sit and reach test. They were randomly divided into core exercise with sling program group (n=10) and general exercise program group (n=10), each consisting 50 minutes each for 4 times per week for 8 weeks. Results: The results of the study are as follows. First, Both groups were increased statistically significantly in spinal flexibility but no significant differences have been found between the two groups after 8 weeks of exercise. Second, Both groups were increased statistically significantly in balance ability of dynamic foot pleasure ratio but no significant differences have been found between the two groups after 8 weeks of exercise. Conclusion: In summarizing the results of this study, sling exercise with core exercise was effective in the decrease of flexibility and stability than general scoliosis exercise in adolescents with idiopathic scoliosis. It is also believed to be applicable to spinal diseases caused by muscular weakness since it is effective in strengthening core muscle strength.

The Study of Correlationship of the Fukuda Stepping Test to Determine Type of Idiopathic Scoliosis Curve (척주측만증 환자의 만곡과 후쿠다 검사의 상관성에 관한 연구)

  • PURPOSE: The Fukuda test can be used at home and in school to diagnose scoliosis at an early stage and prevent serious curvature of the spine. This study aimed to use the Fukuda test to detect scoliosis. An additional aim was to invoke the national interest in imbalanced postures and habits, which result in scoliosis, by providing data obtained in periodic assessments. METHODS: The study consisted of 35 idiopathic scoliosis patients (22 in right lumbar spinal region and 13 in the left lumbar spinal region). The distance of displacement and angle of displacement were measured following the Fukuda test. A correlation analysis was then used to examine the distance of displacement and angles of displacement and rotation with regard to the direction of the curve in scoliosis. RESULTS: There was a significant negative correlation (p<0.00) between the direction of the curve in scoliosis and the angle of displacement, but there was no correlation between the cobb's angle and distance of displacement or between the cobb's angle and angle of rotation. CONCLUSION: The Fukuda test did not capture changes in spinal curvature such as the cobb's angle, or subsequent changes in the muscles. Thus, the Fukuda test is not suited to examining the direction or status of the thoracic curve in scoliosis patients. Simple methods to objectively measure scoliosis need to be developed.

Effects of Schroth 3-Dimensional Exercise on Cubb's Angle, Abdominal Endurance, Flexibility and Balance in Adolescents with Idiopathic Scoliosis (특발성 측만증 청소년의 Schroth 3차원 운동이 콥스각과 복부근지구력 및 유연성, 평형성에 미치는 영향)

  • The purpose of this study was to investigate the effects of 8-week schroth 3-dimensional exercise and ball-sling complex exercise treatment on cobb's angle, abdominal muscle endurance, flexibility and balance in adolescents with idiopathic scoliosis. Thirty subjects with scoliosis were random assignment into one of two experimental groups, either schroth treatment(n=15) or ball-sling complex exercise treatment(n=15). Exercise program was to perform for 8-week and 90min and 3 times per week. The results of this study were as follows; First, cobb's angle was significantly decreased after 8-weeks in schroth treatment(p<.001). And also abdominal muscle endurance(p<.05), flexibility(p<.001) and balance(p<.001) were significantly improve in pre vs. post treatment. Second, the ratio of cobb's angle change was significantly higher in schroth treatment compared to ball-sling complex exercise treatment following 8-weeks(p<.05). These results suggest that although both the schroth treatment and bal-sling complex exercise treatment in adolescents with idiopathic scoliosis can improve on cobb's angle, abdominal muscle endurance, flexibility and balance, but schroth treatment was better than ball-sling complex exercise treatment improving effect of cobb's angle. Therefore, we consider that schroth treatment has more effect of prevention and therapy in idiopathic scoliosis in adolescents.

A Case report of Idiopathic Scoliosis Patient : Comparison of X-ray and Digital Infrared Thermographic Imaging (특발성 척추측만증 환자의 X-ray와 적외선 체열촬영의 비교 1례)

  • Purpose : The purpose of this study is to report the comparison of X-ray and Digital Infrared Thermographic Imaging (DITI) of a patient with idiopathic scoliosis. Method: The patient in this case was a 25-year-old female. Her chief complaint was a pain in lumbar, scapular and shoulder regions. We examined her with DITI and X-ray. And then We compared DITI and X-ray. Results: Cobb's angle of thoracic spine was $24.78^{\circ}$ as a primary curve. Cobb's angle of thoracolumbar spine was $17.63^{\circ}$. Temperature on convex side of the thoracic spine was $0.3^{\circ}C$ higher than the other side. Temperature on convex side of cervical spine was $1.4^{\circ}C$ higher than the other side. There was no correlation curvature degree with temperature difference. Conclusion : There was a tendency that the temperature on convex side of the spine is higher than the other side of the spine. DITI is a useful assessment tool when it is used with X-ray as a diagnostic tool of idiopathic scoliosis. Further studies are needed.

  • Objectives : The purpose of this study was to report clinical effects of Lumbar flexion-distraction manipulation and Sitting position thoracic and lumbar vertebra adjustment manipulation on patient with Juvenile idiopathic scoliosis Methods : The patients have been treated with the above two Chuna manual manipulations. We measured Cobb's angle, VNRS before and after treatment Results : After treatment, we confirmed these transition : In first case, Cobb's angle of T-spine changed from 27 to 23 : Cobb's angle of L-spine changed from 17 to 10 : VNRS changed from 6 to 2. In second case Cobb's angle of T-spine changed from 30 to 21 : Cobb's angle of L-spine changed from 16 to 16 : VNRS chagned from 6 to 3 Conclusion : The result suggests that these two Chuna manual manipulations were effective treatment on patients with Juvenile idiopathic scoliosis.

A Survey Study of the Juvenile Idiopathic Scoliosis Using Radiation Indirect Examination (방사선 간접검사를 이용한 청소년의 척추 측만증에 관한 연구)

  • The purpose of this Study was to investigate the prevalence rate of idiopathic scoliosis to the students from the elementary to the university in S city of Chung-Nam using 100 mm Mirror Camera radiation indirect examination units, with on age range of between eight and thirteen (1.526 subjects), fourteen and sixteen (462 subjects), seventeen and eighteen (291 subjects), nineteen and twenty four(508 subjects) and total of 2,787 participants with the 590 male subjects and 2,197 female subjects. The results of this study can be summarized and compared the primary examination with the secondary test of greater than $10^{\circ}$of Cobb's Angle were obtained as follows; 1. Indirect Examination were conducted to find idiopathic scoliosis amomg total 2.787 subjects, 257 subjects (9.2%) who showed positive sign greater than $10^{\circ}$in the Cobb's Angle ; below age of thirteen (132 subjects), between fourteen and sixteen (52 subjects), seventeen and eighteen (35 subjects), nineteen and twenty four (38 subjects). The $x^2-test$ analysis of Indirect Examination showed no statistical significant difference association between the age range and $10^{\circ}$Cobb's Angle of spinal curve(P>0.059). 2. The numbers of idiopathic scoliosis of greater than $10^{\circ}$Cobb's Angle of spinal curve in the primary examination were observed in 147 subjects (57.2%) at the Thoracolumbar region, 81subjects (31.5%) at the Thoracic region, 20 subjects (7.8%) at the Cervicothoracic region, 7 subjects (2.7%) at the Lumbar region, 2 subjects (0.8%) at the Cervical region. So, the large numbers were Thoracolumbar region, 183 participants (71.2%) showed the right side curve of scoliosis and 74 participant (28.8%) showed the left side curve of scoliosis. 3. The main region of the pain in one's own self more than $10^{\circ}$ Cobb's angle of spinal curve were no pain 219 subjects (85.2%), 18 subjects (7.0%) at the Lumbar region, 9 subjects (3.5%) at the Cervical region, 7 subjects (2.7%) at the Thoracic region, 2 subjects (0.7%) at the shoulder girdle region, 1 subjects (0.39%) at the pelvis and whole body region. There was statistical significant difference association between the Cobb's Angle of spinal curve and the main pain region of one's (P<0.006). This study may be significant to an early stage investigate of the prevalence rate of idiopathic scoliosis in the juveniles using 100 mm Mirror Camera radiation indirect examination units. The results of this study help that the students in a stage on growth the basis of data early discovery and therapy of idiopathic scoliosis.
