A Case Report of Eczema treated by Injinho-tang (양명병(陽明病) 인진호탕(茵蔯蒿湯)으로 호전된 유두 습진 1례)

  • Objective : This study aims to explain the effect of Injinho-tang on Eczema which a Yangmyung-byeong patient has. Methods : This is a case report on a female patient who has been suffering from Eczema. According to Diagnostic system based on Shanghanlun provisions, Injinho-tang was provided for six months. Results : After a series of Injinho-tang treatments, the patient's symptoms and result improved. Conclusions : This case study showed an effectiveness of using Injinho-tang according to Diagnostic system based on Shanghanlun provisions on Eczema.

Effect of Injinho-tang on the Liver and Hyperlipidemia (인진호탕(茵蔯蒿湯)이 손상간(損傷肝) 및 고지혈증(高脂血症)에 미치는 영향(影響))

  • In order to investigate the effect of Injinho-tang on liver treated with CCl4, galactosamine, thioacetamide and hyperlipidemia induced by fructose, triton wr-1339, this experiment was performed. The results are as followings. 1. Injinho-tang decreased the serum TG and TC level in normal rats. 2. Injinho-tang decreased significantly the serum TG level induced by 75% Fructose. 3. Injinho-tang depressed significantly the serum TG and TC level in hyperlipidemia induced by Triton WR-1339. 4. The silymarin-Injinhotang group decreased more significantly the serum GOT, GPT, TC, TG level in comparison to the Injinho-tang treated group in CCl4, galactosamine and thioacetamide treated mice, rats.

Study on Validity of Using Injinho-tang in Non Alcohoic Fatty Liver Disease (비알콜성 지방간의 약물개발 중 인진호탕(茵蔯蒿湯)의 타당성 연구)

  • Now a days, number of non alcoholic fatty liver patients are increasing more rapidly compare to past rate, and the average age of patients is getting younger, but there are no appropriate therapeutics in non alcoholic fatty liver disease. This study was aimed to analyze relationship between non alcoholic fatty liver disease and Injinho-tang. The papers were collected and analysed from domestic and international journals. The effects of Injinho-tang and constituent-herb were researched. Non-alcoholic fatty liver disease was induced complex causes of the metabolic syndrome. Medications that can be used in non-alcoholic fatty liver disease, it should be have many effects such as anti-hepatic fibrosis, hepatocyte protection, liver cancer inhibitory effect, inflammatory cytokine regulation, improving hyperlipidemia, weight control, decrease the toxicity of the drug, antioxidant. Injinho-tang (Artemisia capillaris Thunb, Gardenia fructus, Rhei rhizome) has been widely used in disease that causes jaundice and liver biliary disease. Drugs for standardization of Injinho-tang index components(6,7-Dimethylesculetin, geniposide, rhein) have been presented. And Injinho-tang has been proven reliability in the administration of single dose toxicity. Also clinical stability in the administration of four years was reported. Injinho-tang has been reported some effects which anti-hepatic fibrosis, hepatocyte protection, liver cancer inhibitor, inflammatory cytokine regulation, improving hyperlipidemia, weight control, decrease the toxicity of the drug, and antioxidant. Therefore, Injinho-tang can be used in Non alcoholic fatty liver disease without Syndrome Differentiation.

Preventive effects of Injinho-tang and Haninjinho-tang on hyperlipidemia and liver damage induced by alcohol (인진호탕(茵蔯蒿湯)과 한인진호탕(韓茵蔯蒿湯)이 알코올 투여(投與)로 유발(誘發)된 흰쥐의 고지혈증(高脂血症)과 간(肝) 손상(損傷)의 예방(豫防)에 미치는 영향(影響))

  • Objectives : This study was performed to compare the preventive effect of water extracts from Injinho-tang and Haninjinho-tang on hyperlipidemia and liver damage induced by alcohol. Methods : Except for the normal group, we fed rat on 25% alcohol for 55 days. And each extract was administrated for the same period. We measured the serum component in rat's blood, body weight and weight of liver. Results : 1. At first, we observed effects of Injinho-tang and Haninjinho-tang on hyperlipidemia induced by alcohol. Injinho-tang group showed significant decrease of triglyceride, body weight in comparison with those of the control group. Haninjinho-tang group showed significant decrease of total cholesterol, triglyceride in comparison with those of the control group. I 2. At second, we observed effects of Injinho-tang and Haninjinho-tang on liver damage induced by alcohol. lnjinho-tang group showed significant decrease of AST (aspartate aminotransferase), ALT (alanine aminotransferase), ALP (alkaline phosphatase), liver weight in comparison with those of the control group. Haninjinho-tang group showed significant decrease of AST (aspartate aminotransferase), ALT (alanine aminotransferase) in comparison with those of the control group. Conclusion : Reviewing these experimental results, it appears that water extracts from Injinho-tang and Haninjinho-tang have pharmaceutical preventive efficacy on hyperlipidemia and liver damage induced by alcohol. Therefore further additional study should be conducted to elucidate in depth the pharmaceutical efficacy of these.

  • Objectives: Lately, non alcoholic fatty liver patients are increasing rapidly and the average age of patients are getting younger. Non alcoholic fatty liver often represents asymptomatic and korean pattern diagnosis is difficult to implement without symptoms. This study aimed to analyze interrelationship between non alcoholic fatty liver disease and Injinho-tang. Methods The AHP is a systematic procedure for analyzing the elements of any problem hierarchically. Based on survey of expertise, as series of pairwise comparison judgements is performed to evaluate the various elements in the hierarchy. We are expected to use the AHP analysis that would apply to oriental doctor's diagnostic process analysis. Results and conclusion Non-alcoholic fatty liver disease induced complex causes of the metabolic syndrome. Injinho-tang has been widely used disease that causes jaundice and liver biliary disease. According to AHP analysis, LFT is the most important facor in choosing Injinho-tang on non alcoholic fatty liver disease. BMI and body composition by bioelectrical impedance analysis are the next important factors. Personal hot temper also gives a big impact on choicing Injinho-tang on non alcoholic fatty liver disease.

Clinical Effect of Injinho-tang on Hyperbilirubinemia in Hepatobiliary Disorders: A Systematic Review (간담도 질환에서의 고빌리루빈혈증에 대한 인진호탕의 임상 효과 : 체계적 고찰)

  • Purpose: This systematic review was conducted to evaluate the clinical effects of Injinho-tang on hyperbilirubinemia in hepatobiliary disorders. Methods: We searched for randomized controlled clinical trials that had administered Injinho-tang as an intervention in the following medical databases: Public/Publisher MEDLINE (PubMed), Excerpta Medica dataBASE (EMBASE), Cochrane library, Research Information Sharing Service (RISS), ScienceON, Oriental Medicine Advanced Searching Integrated System (OASIS), and China National Knowledge Infrastructure (CNKI). Among the retrieved studies, only trials that met the inclusion criteria were selected, and serum total bilirubin values were extracted and analyzed from the finally selected trials. Results: The serum total bilirubin values of 1,302 patients with various hepatobiliary diseases were synthesized through a meta-analysis, which confirmed a decrease in serum total bilirubin of 21.03 𝜇mol/L (95% CI -29.58~-12.49, p<0.01) in the group administered with Injinho-tang compared with the control group. Conclusions: Injinho-tang is effective in alleviating hyperbilirubinemia in hepatobiliary diseases when administered with conventional treatment. However, the potential risk of bias, high heterogeneity among the included trials, and differences in herbal composition are limitations of the results of this meta-analysis.

Research about application of Shanghanlun disease pattern identification diagnostic system by analyzing 2 cases treated by Injinho-tang (인진호탕(茵蔯蒿湯)을 활용한 2개 증례 분석을 통한 『상한론(傷寒論)』 변병진단체계(辨病診斷體系) 응용에 관한 연구)

  • Objective : The objective of this study is to define the conceptional meaning of Shanghanlun provision while applying Shanghanlun disease pattern identification diagnostic system. Methods : 2 clinical cases, whose patients were treated by Injinho-tang, which was selected according to Shanghanlun provisions dualized with Je-Gang(提綱) and Jo-Moon(條文), were analyzed. Results : According to the results of analysis of 2 cases, the patients' diseases were improved when the treatment was applied according to Shanghanlun disease pattern identification diagnostic system and the interpretation of Shanghanlun provisions according to the etymology of Chinese character. Conclusions : These results suggests that Shanghanlun provisions be applied according to etymological interpretation of Chinese character in Shanghanlun disease pattern identification diagnostic system.

Antioxidative and Hepatoprotective Effects of Injinho-Tang and Osumogwa-Tang (인진호탕(茵蔯蒿湯)과 오수목과탕(吳茱木瓜湯)의 항산화(抗酸化) 및 간세포(肝細胞) 보호효과(保護效果))

  • Korean traditional medicine has been used for the treatment of the various diseases based on both oriental medicinal theory and clinical trials. Thus, the prescriptions of Korean traditional medicine would be useful for the development of new therapeutics. This research focuses on the fundamental study in Korean traditional prescriptions for the development of new hepatoprotective agents. We found two prescriptions. Injinho-Tang and Osumogwa-Tang, showed the significant DPPH free radical scavenging and hepatoprotective effect, respectively. It is well-known that free radical scavenging effect is related to the prevention of various pathological events including liver injury. This paper deals with hepatoprotective effects on tacrine-induced cytotoxicity in Hep G2 cells, free radicals scavenging on both DPPH and superoxide of above two prescriptions. Hot water extract of Injinho-Tang did not show the significant hepatoprotective effect on tacrine-induced cytotoxicity in Hep G2 cells, however, it shows the significant scavenging effects for both DPPH and superoxide radicals. On the other hand, all of the hot water extracts of constituent herbal drugs in Injinho-Tang exhibited the promising protective effect on tacrine-induced cytotoxicity in Hep G2 cells. Of these, water extract of Rhei Rhizoma showed the most prominent effect on tacrine-induced cytotoxicity in Hep G2 cells. Bioassay-guided fractionation of Rhei Rhizoma extract has furnished four compounds, and their chemical structures have been identified by comparison of their spectral data with those of literature as chrysophanol (1), emodin (2), 3,5-dihydroxy-4'- methoxystilbene (3), and rhapontigenin (4), respectively. Among the isolated compounds, compounds 2-4 revealed the significant hepatoprotective effect in vitro when their $EC_{50}$ values compare with that of silybin, as a positive control. It also exhibited that emodin possessed the most hepatoprotective effect among these active compounds. In case of Osumogwa-Tang, its hot water extract showed the moderate protective effect on tacrine-induced cytotoxicity in Hep G2 cells. Hot water extract of Chaenomelis Fructus, one of the constituent herbal drug of this prescription, exhibited the significant hepatoprotective effect with $EC_{50}$ value of $7.8{\pm}0.1\;{\mu}g/ml$, however, it showed strong cytotoxicity in Hep G2 cells above the concentration of $25\;{\mu}g/ml$. It was revealed that both hot water extract of Evodiae Fructus and its butanol soluble fraction showed the moderate hepatoprotective effect but concentration-dependent activity in Hep G2 assay system. Two quinolone alkaloids, evocarpine and dihydroevocarpine, also tested for their hepatoprotective effects on tacrine-induced cytotoxicity in Hep G2 cells, however, these two compounds derived from the Evodiae Fructus as the major constituents did not show in vitro hepatoprotective effect. From these results, it would be necessary to further isolation of its hepatoprotective compounds from the butanol soluble fraction of the hot water extract of Evodiae Fructus.

A Case Report of UDCA and Injinho-tang Co-administration for Liver Damage Caused by Rifampin (Rifampin으로 인한 간손상에 UDCA와 인진호탕 병용투여 증례 보고)

  • We have experienced a case in which herbal medicine was administered to treat drug-induced liver damage and would like to introduce it. A 49-year-old man exhibited a positive result in the interferon-gamma release assay. He had never suffered from tuberculosis in the past, and the route and time of infection could not be confirmed. He had no respiratory or systemic symptoms suggestive of active tuberculosis, and a chest X-ray examination showed no active lung lesions, so he was diagnosed with latent tuberculosis infection. He was confirmed to be within the normal range in the liver function test, renal function test, and complete blood cell count test, and started taking rifampin (600 mg qd). In the screening test performed on the 19th day of taking the drug, other test items were normal, but alanine aminotransferase (ALT) increased to 50 U/L (reference value: 4-40 U/L). In a test performed on the 29th day of taking the drug, ALT was clearly elevated to 102 U/L. Ursodeoxycholic acid and Injinho-tang were taken together with rifampin, and the patient's progress was observed. In a test performed 14 days later, ALT decreased to 26 U/L, within the normal range. It is presumed that Injinho-tang may have partially contributed to alleviating liver damage in this case.

A Case Report of Injinho-Tang on Lacquer Tree Allergy Unresponsive to Oral Steroid Therapy (경구 스테로이드 요법에 반응하지 않는 옻닭 알레르기에 대한 인진호탕 치험 1례)

  • Objective : The purpose of this study is to report a case of lacquer allergy improved by Korean medicine treatment. Methods : The patient visited our hospital complaining of rash, and pruritus which lasted for 10days after eating lacquer chicken. Considering that patient's symptoms occurred due to food allergen, we prescribed Injinho-tang(Yinchenhao-tang) and acupuncture at Daejang-Junggyeok for 7days. Results : On the 4th day of treatment, pruritus decreased to less than half of the initial treatment, and skin symptoms also improved to a mild level. About a week later, most of the skin symptoms disappeared (Total Objective Symptom score decreased 10 to 3), and subjective symptoms such as pruritus improved to about 30% of the time of the first visit (Total Subjective Symptom score decreased 12 to 3). Conclusions : After 7days of Korean medicine treatment, lacquer tree allergy with skin lesions and pruritus has improved considerably.
