요양병원 간병 인력의 손 위생 이행의도에 영향을 미치는 요인 (Factors Influencing Care workers' Intention of Implementing Hand Hygiene in Long-term Care Hospitals)

  • Purpose: The purpose of this study was to investigate factors influencing care workers' intention of hand hygiene implementation in long-term care hospitals. Methods: A total of 180 care workers working at long-term care hospitals were recruited. Data collection was done from July 22 to September 7, 2018. Results: The significant TPB variables influencing the intention of hand hygiene implementation were perceived behavior control (β=.41, p<.001), normative belief (β=.28, p<.001) and attitude toward behavior (β=.15, p=.014). These factors explain 39% of care workers' intension of implementing hand hygiene in long-term care hospitals. Conclusion: In order to strengthen the commitment of hand hygiene, it is necessary to have a positive attitude toward hand hygiene by eliminating the obstacles to hand hygiene.

A Study of Care Workers Knowledge on Pressure Ulcer

  • The purpose of this study was to examine the degree of knowledge of care workers working at long term care hospitals and nursing homes on pressure ulcer. A total of 81 care workers including 34 at long term care hospitals and 47 at nursing homes were surveyed. 24 questions were used to evaluate their degree of knowledge on pressure ulcer. Their knowledge on pressure ulcer scored 12.84 out of the total score of 24 points(SD=3.40), which was equal to 53.50 (SD=24.23) out of 100 points. Their knowledge on the prevention of pressure ulcer was highest among the subareas of evaluation. Their knowledge on pressure ulcer statistically significantly differed according to education on pressure ulcer(P<.05). A pressure ulcer is a skin disorder that may be prevented and cured. At this point when long term care facilities are rapidly increasing, care workers highlevel knowledge on and good management of pressure ulcer is very important. Practically educating them on pressure ulcer including the provision of recent, updated relevant knowledge will be necessary.

노인요양병원 간호종사자의 이직의도에 영향을 미치는 요인 (Factors to Affect Turnover Intention of Nursing Workers at Long-Term care Hospitals )

  • 본 연구는 요양병원 간호종사자인 간호사 및 간호조무사를 대상으로 이직의도에 미치는 영향요인을 파악하기 위한 서술적 조사연구이다. 2022년 8월 12일부터 16일까지 G시 소재 4개 요양병원 간호종사자 146명을 대상으로 실시되었다. 연구결과, 이직의도는 일반적 특성 중 연령, 종교, 결혼상태, 학력정도, 월 평균 급여, 직종, 근무형태 및 이직경험에 대해 유의한 차이가 있었다(p<.05). 이직의도는 직무스트레스(r=.51, p<.001) 및 소진(r=.62, p<.001)과는 유의한 양의 상관관계가 있었으며 조직몰입과는 유의한 음의 상관관계가 있었다(r=.-56, p<.001). 조직몰입은 직무스트레스(r=-.25, p=.002) 및 소진(r=.-.67, p<.001)과 유의한 음의 상관관계가 있었으며 소진은 직무스트레스(r=.56, p<.001)와 유의한 양의 상관관계가 있었다. 이직의도에 영향을 미치는 요인으로는 직무스트레스(β=.32, p<.001), 조직몰입(β=-.30, p=.001), 학력정도(β=.17, p=.022)로 나타났으며 설명력은 49.6%였다. 본 연구결과를 토대로 요양병원 간호종사자의 직무스트레스 감소 및 조직몰입 향상을 위한 근무환경 개선 등을 통해 이직의도 감소를 위한 노력이 필요하리라 생각된다.

코로나19 대유행 시 지역사회 요양병원 종사자의 근무경험: 혼합연구방법 (Working Experience of the Community-based Long-term Care Hospital Workers during the COVID-19 Pandemic: Mixed Methods Research)

  • Purpose: This study is a mixed methods research that was conducted to verify factors affecting the working experience of community-based long-term care hospital workers during the COVID-19 pandemic. Methods: The study was carried out from July 19 to November 3, 2021 for 340 nurses who worked at 10 long-term care hospitals located in G city. Results: As the study results, factors that affected job stress of the workers working at community-based long-term care hospitals included job satisfaction (β=-.27, p<.001), work demand (β=-.25, p<.001), fatigue (β=.19, p=.001), and cooperation and leadership (β=-.12, p=.049). It was found that the participants were struggling with physical and mental stress caused by the increased workload due to the preventative measures taken to stop the infection and spread of COVID-19. Despite this, they accepted their situation as necessary to overcome the pandemic and shared the quarantine guidelines of the government and community health centers while actively responding to prevent the spread of COVID-19 under the leadership of their supervisors. However, they were experiencing psychological and emotional burnout in the prolonged pandemic situation. Conclusion: It is considered necessary to help relieve their stress and provide psychological and mental support by adopting a policy to develop and apply comprehensive programs.

요양병원평가가 요양병원 종사자의 근무환경, 직무만족과 서비스 질에 미치는 영향 (Effects of National Evaluation of Long-Term Care Hospitals on Hospital Workers' Work Environment, Job Satisfaction, and Quality of Services)

  • Purpose: The purpose of this study is to evaluate the work environment and job satisfaction of hospital workers and to assess their effects on quality of services under the national long-term care hospital evaluation (hereafter, national evaluation), which has implemented since 2008. Methods: A self-administered survey was conducted on 178 hospital workers' at 18 hospitals in Chungcheong province, Korea. Survey questionnaires include questions about respondent' awareness of the national evaluation and any change in work environment and job satisfaction under the national evaluation. We used a path analysis to assess the effects of work environment and job satisfaction on quality of services. Results: Results showed that the effects of the national evaluation on work environment and job satisfaction were positive, which, in turn, leads to better quality of services. Improvements in the work environment under the national evaluation have resulted in increased job satisfaction. High scores for job satisfaction showed a significant association with the quality of services provided in long-term care hospitals. In addition, the national evaluation itself had a positive effect on improving quality of services. Conclusions: In order to facilitate quality improvement activities under the national evaluation, it is suggested that workers be provided with education and training. Continuous efforts to improve work environment and to enhance job satisfaction would lead to provision of better quality of services in long-term care hospitals.

요양병원 환자분류체계 개발 (Development of Patient Classification System in Long-term Care Hospitals)

  • Purpose: To develop the patient classification system based on the resource utilization for reimbursement of long-term care hospitals in Korea. Method: Health Insurance Review & Assessment Service (HIRA) conducted a survey in July 2006 that included 2,899 patients from 35 long-term care hospitals. To calculate resource utilization, we measured care time of direct care staff (physicians, nursing personnel, physical and occupational therapists, social workers). The survey of patient characteristics included ADL, cognitive and behavioral status, diseases and treatments. Major category criteria was developed by modified delphi method from 9 experts. Each category was divided into 2-3 groups by ADL using tree regression. Relative resource use was expressed as a case mix index (CMI) calculated as a proportion of mean resource use. Result: This patient classification system composed of 6 major categories (ultra high medical care, high medical care, medium medical care, behavioral problem, impaired cognition and reduced physical function) and 11 subgroups by ADL score. The differences of CMI between groups were statistically significant (p<.0001). Homogeneity of groups was examined by total coefficient of variation (CV) of CMI. The range of CV was 29.68-40.77%. Conclusions: This patient classification system is feasible for reimbursement of long-term care hospitals.

요양병원 요양보호사의 감정노동, 직무스트레스 및 소진이 이직의도에 미치는 영향 (The Influence of Emotional Labor, Job Stress, and Burnout on Turnover Intention of Care Worker's at Long-Term Care Hospitals)

  • 본 연구는 요양병원 요양보호사의 감정노동, 직무스트레스, 소진과 이직의도의 정도와 그 관계를 파악하며 이직의도에의 영향요인을 규명하기 위하여 시도되었다. K대학교 기관생명윤리위원에서 승인을 받았으며 자료수집 기간은 2015년 8월 12일부터 9월 30일까지이며, 자료는 SPSS/WIN 18.0 프로그램을 이용하여 분석하였다. 대상자는 B광역시와 G도 소재 10개 요양병원에 근무하는 요양보호사 200명으로 구조화된 자기기입형 설문지를 통해 자료를 수집하였다. 자료는 기술적 통계, t-test, ANOVA, Pearson's correlation coefficients, Stepwise multiple regression으로 분석하였다. 연구결과, 요양보호사의 이직의도는 감정노동(r=.372, p<.001), 직무스트레스(r=.471, p<.001), 소진(r=.411, p<.001)과 통계적으로 유의한 양의 상관관계를 보였으며, 단계적 회귀분석 결과, 이직의도에 영향을 미치는 요인은 직무스트레스(${\beta}=.29$, p<.001), 소진(${\beta}=.24$, p<.001), 취업형태(${\beta}=.19$, p<.001), 감정노동(${\beta}=.16$, p=.017)으로 확인되었고, 이 변인들은 요양병원 요양보호사의 이직의도를 총 32.6% 설명하는 것으로 나타났다. 본 연구결과를 토대로 요양병원 요양보호사의 이직의도를 감소시키기 위해서는 직무스트레스를 감소시킬 수 있는 적절한 근무환경 개선 및 명확한 직무기술이 요구되며, 소진 및 감정노동의 경감을 위한 적극적인 심리사회적 중재 개발 및 적용이 필요하다고 하겠다.

요양병원 간호보조인력의 환자안전에 대한 지식, 태도 및 환자 안전활동 (A Study on Knowledge, Attitude, toward Patient Safety and Patient Safety Care Activities among Long-Term Care Hospitals' Nursing Assistants)

  • 본 연구에서는 요양병원 간호보조인력 즉 간호조무사, 요양보호사의 환자안전에 대한 지식, 태도 및 환자안전활동 정도를 파악하여, 요양병원 환자안전관리 역량증진 및 간호보조인력을 위한 교육과정 개발의 기초자료로 활용하고자 시도된 서술적 조사연구이다. 자료 수집은 2017년 3월 2일부터 3월 26일까지 B광역시 소재 요양병원 중 2013년 적정성평가 2등급 이상, 200병상 이상의 6곳에서 근무하는 간호보조인력 230명을 대상으로 구조화된 설문지를 이용하여 수집하였다. 수집된 자료는 SPSS/WIN 20.0 프로그램을 이용하여 실수와 백분율, 평균, 표준편차, t-test, ANOVA, $Scheff{\acute{e}}$ test, 상관관계분석 및 다중회귀분석을 실시하였다. 본 연구의 결과, 요양보호사보다 간호조무사가, 고학력일수록, 환자안전교육을 받은 횟수가 많을수록 환자안전활동 정도가 높은 것으로 나타났다. 또한 환자안전에 대한 지식이 태도에 영향을 미치고, 태도는 환자안전활동에 영향을 미치는 것으로 나타났다. 따라서 바람직한 태도 개선을 위해 직종별 차별화된 교육과 연간 5회 이상의 실제적이고 지속적인 교육 프로그램의 제공이 필요하고 적절한 근무시간 등의 제도개선 또한 필요할 것으로 사료된다. 이 결과를 토대로 요양병원 간호보조인력의 환자안전관리 역량증진 및 환자안전에 대한 지식 향상과 바람직한 태도개선을 위한 교육과정 개발의 기초 자료로 활용할 수 있을 것이다.

요양병원 요양보호사의 작업환경, 심리적 업무요구, 피로가 근골격계 증상에 미치는 영향 (Factors Influencing on Musculoskeletal Symptoms of Care Workers in Long-Term Care Hospitals for Job Environment, Psychological Work Demand, Fatigue)

  • 본 연구는 요양병원 요양보호사의 작업환경, 심리적 업무요구, 피로도, 근골격계 증상의 관계를 파악하고 근골격계 증상에 미치는 영향요인을 규명하기 위해 시도되었다. 연구방법은 S시에 소재한 3곳의 요양병원에 근무하는 요양보호사 128명을 대상으로 2020년 4~5월까지 설문 조사하였다. 연구결과 요양병원 요양보호사의 근골격계 증상은 어깨, 손/손목/손가락, 허리 세 부분에 대한 증상발생빈도, 증상지속기간, 통증의 강도를 합한 값으로 42점 만점에 평균 11.41±9.50점으로 나타났다. 요양병원 요양보호사의 근골격계 증상에 미치는 영향요인을 규명하기 위해 회귀분석을 실시한 결과 근무병동(β=.18, p=.034)과 심리적 업무요구(β=.21, p=.013), 피로도(β=.25, p=.003)가 근골격계 증상에 유의한 영향을 미치는 것으로 나타났으며, 이들 3개 변수는 근골격계 증상에 대하여 21.6%의 설명력을 보였다(F=6.87, p<.001). 본 연구결과를 바탕으로 요양병원 요양보호사들의 근골격계 증상을 감소시키기 위해서는 담당하는 환자의 중증도를 고려하여 심리적 업무요구와 피로도를 줄일 수 있는 중재프로그램 개발 및 적용이 필요하다.

장기요양환자에서 환자 특징 및 기능상태와 환자돌봄 시간과의 관련성 (A Relationship of Care Time with Functional Status and Patients Characteristics among Patients in Long-term Care Hospitals)

  • Objectives : The aim of this study was to investigate the functional status variables related to the care time of health professionals for patients in long-term care facilities. Methods : The functional stati of 1001 patients in 8 long-term care hospitals were examined by the Resident Assessment Instrument for Long-term Care Facility Version 2.0. The care time of health professionals for patients was calculated using data from a self-reported task survey by nurses, auxiliary nurses, private aides, doctors, physiotherapists and social workers. Results : The average care time per diem was 240.6 minutes. The care time by doctors, nurses and private aides were 11.0, 71.0 and 139.5 minutes, respectively. The lower the function of activities of daily living (ADL) and the greater the symptoms of extensive services, special care and clinical complexity, the more care time was served. On the contrary, the greater the symptoms of nursing rehabilitation, depression, cognitive disorder, behavior problem and psychiatry/mood disorder, the less care time was served. Age and gender were not significantly related to the care time. Conclusions : Developing a case mix classification system for elderly long term care patients may be helpful for both of patients and health care providers. The ADL, extensive services, special care and clinical complexity of variables should be considered in the development of a case mix system for the long term care of patients in Korea.
